Operator of this vehicle was commuting and truck started to slow down and chug. Vehicle hesitated when operator applied gas pedal. -Transmission cooler inlet/outlet pipe as per Chevrolet known issues. -Safety of driver at risk as failure happened in heavy traffic, unable to pull to safety -Yes confirmed by authorized repair facility -Part inspected by authorized mechanic. -No warnings, symptoms, prior to failure.
The rear window in my 2021 Silverado 2500 leaks when it rains, water is getting in the carpet/seats creating a bad odor and mold. The dealership will not assist in fixing this known issue.
Similar to the issues that were on the gas version of this engine and the recall associated with it. The engine seized on the expressway. When it was towed to the dealership, they said it was catastrophic engine failure. They found a lot metal shavings in the oil pan. They did not diagnose any further due to cost. At the time it happened, there were no warning lights or check engine lights on.
While traveling at highway speed on the interstate in my 2021 Chevrolet Silverado 2500 HD (approx. 120,000 miles), the vehicle experienced a sudden hard jolt followed by a momentary rear wheel lockup. This occurred without warning. Immediately after, the transmission defaulted into a fail-safe condition and became locked in 4th gear. The vehicle would move but could not shift properly, resulting in abnormally high RPM and engine load at highway speeds. I was forced to continue driving several miles in this condition to reach a safe exit. The unexpected drivetrain behavior and temporary wheel lockup created a serious safety risk, including potential loss of control and risk of collision. The dealership identified transmission diagnostic trouble codes P2715 and P2733 (pressure control solenoid performance). Transmission fluid was found to be burnt and contaminated with metal debris. The vehicle is subject to GM Special Coverage N242454441 for a known control valve body defect that can cause harsh shifting and reduced engine power. Although code P0747 (referenced in the bulletin) was not present, the failure involved transmission hydraulic pressure control. The dealership is replacing the valve body under the special coverage but is not addressing the internal transmission damage, stating they cannot determine whether the damage resulted from the covered defect. The sudden rear wheel lockup and transmission fail-safe condition at highway speed present a significant safety concern involving unintended drivetrain behavior.
The vehicle experienced a sudden and complete transmission failure at approximately 77,800 miles. There was no prior abuse or modification. The failure resulted in loss of normal drivability and required full transmission replacement. The transmission was replaced with a remanufactured unit due to catastrophic internal failure. Repair costs were approximately $7,000. I am aware of similar GM transmission issues and recalls affecting comparable model years and components. Although my VIN is not currently listed under a recall, the failure appears consistent with known GM transmission defect patterns. The failure occurred while the vehicle was in the middle of an intersection in heavy traffic, resulting in sudden loss of vehicle functionality. A child passenger was onboard at the time. The incident created a significant safety hazard due to loss of drivability in an active traffic environment.
On 01/16/2026, Quirk Chevrolet completed product safety recall N242482680 for the Brake Pressure Switch Connector Fire on my vehicle. Shortly after, my vehicle would come to an abrupt stop on its own while driving. This happened twice on my way home and then the third time at home at which point, it would no longer move forward or backward. The vehicle needed to be towed to the dealership that did the recall. After researching online, I discovered this is a known issue after the recall because the recall component is faulty. One source reports the cause is an improperly manufactured jumper harness which is missing protective tape or a faulty internal C-clip in the cylinder. Another source states the post-service emergency brake issues often stem from a software update meant to enhance safety or an incomplete repair of the brake pressure switch wiring harness. The update automatically engages the parking brake. I absolutely know that the catastrophic failure of my vehicle's braking system was a direct result of the recall work; however, the dealership made me pay for the repairs out of pocket until such time as GM or Chevy makes an "official" recall on the faulty recall. My safety and the safety of others was put at risk by way of my vehicle stopping abruptly on its own. The problem has been reproduced by many sources online - just do an online search "2021 Chevy 5500 emergency brake issue after brake recall completed" and see all the results that come up corroborating my personal experience. Quirks Chevrolet in Manchester, NH inspected the vehicle. No warning lamps or messages appeared on the dash. Please contact me if you want any additional information or copies of receipts, etc. My vehicle is available for inspection upon request.
first issue was around April-May 2025 when my engine started knocking and burning fuel. Warning light came on and it went right in to the dealer where I have all my maintenance done. They determined that it due to (2) faulty fuel injectors and I had them replaced at that time. In December of 2025 my engine started doing the same thing as before with the knocking and burning fuel and reduced speeds when on the highway so again it went into the dealer. It has since been in there and first was told it was the fuel injector again. They priced out the one was bad and I asked them to replace the remaining 6 fuel injectors so I didn't have this issue again in the near future. They did and the vehicle had the same issues going on. During the next inspection it was determined that the engine needed new rocker arms. These were replaced but this also did not solve the problem and the vehicle would not pass the regeneration on the engine. During the next inspection it was determined that we needed to replace a cylinder and one of the valves. After this was complete they were still having issues and then I was told that the Piston was bad and there are (2) bad valves so I need an entire engine replacement. My truck only has 130,000 miles and has been in the shop for almost 2 months last year from engine issues and it has had all maintenance completed at he same dealership with Records.

The contact owns a 2021 Chevrolet Silverado 2500. The contact stated that the connector for the heater system was causing the vehicle to overheat, subsequently enabling the vehicle to go faster than 50 MPH. If the vehicle reaches 75 miles, it would immediately decrease in speed to 5 MPH. The vehicle was taken to a certified mechanic who diagnosed a P20C1 code, indicating reductant heater failure had occurred. The vehicle was not repaired. The contact called the dealer, who recommended calling the NHTSA Hotline to file a safety complaint. The manufacturer was informed of the failure they also referred the contact to the NHTSA Hotline to file a safety complaint. The failure mileage was approximately 68,000.

Driving about 45 mph and unexpectedly both roof rail air bags deploy stoping vehicle on the road.
I own a 2021 Chevrolet Silverado 2500HD Duramax diesel with approximately 100,300 miles. The truck began displaying a “Service Emissions System” warning and a countdown message that in a certain number of miles the vehicle will be limited in speed (for example to 65 mph). GM’s own Duramax diesel owner’s supplement explains that when a “Service Emission System” message appears it is accompanied by a distance-until-speed-limit countdown and that the truck will progressively limit speed first to about 65 mph and then to about 55 mph if the condition is not corrected, telling the owner to see the dealer. This creates a safety risk because the derate can occur while driving on highways or rural roads, where an unexpected limit on maximum speed can interfere with merging, passing, and keeping up with traffic. This is our only vehicle, and we must use it for important medical appointments, including cardiology and surgery-related visits. The condition started only about 300 miles beyond GM’s 5-year / 100,000-mile powertrain warranty. I opened a case with Chevrolet / GM Corporate (Service Request No. XXX) and took the truck to Sons Chevrolet in Columbus, Georgia on November 17, 2025 for this issue and a rear wheel lockup recall. The dealer kept the truck from about 9:00 a.m. to 3:00 p.m. and indicated they might need to keep it for at least two more days, but I was not given a clear written explanation of the diagnostic trouble codes or a firm repair plan. Because GM’s own manuals describe this “Service Emission System” message together with speed-limit countdowns, and because the vehicle can actively limit speed on the road if the fault is not resolved, I am concerned this represents a broader emissions/derate defect on late-model Silverado HD Duramax trucks. I request that NHTSA review similar complaints and consider whether further investigation or a safety recall is warranted. Took truck to dealer with concern of higher than normal braking force needed. Couldn’t verify concern. Looked today under the steering column and the brake booster is leaking fluid inside the cabin of the truck when the brake pedal goes through the firewall. There was Almost no brake fluid inside the reservoir causing the brakes to fail with no warning. Looked at similar trucks and all have brake fluid leaking from the same location. With as little as 10,000 miles. Mine has 22,000 miles and it’s failed requiring complet replacement of the brake booster. Taking to the dealership to replace the complete brake booster assembly when part is available. This is a 21 Chevy 2500 truck that pulls up to 18,000 pounds. higher than normal braking application needed, can be deadly or lead to complete brake failure whether loaded towing a trailer at a weigh of 25,000lbs or even the 7,000 pounds empty the truck weighs would reasonably result in the loss of someone’s life. Only thing that drew me to this concern was a harder application of the brake pedal and noticing brake fluid under the dashboard. Would gladly allow anyone to inspect my truck.

We believe that it is the park sensor in the column . When you place the truck into park, the screen displays the words shift to park period and the dinger.Inside the truck continues to ding for a long time, even with opening and closing the door ... The truck is actually in park, so it does not move. But the display says it's not.And apparently it is not registering because it keeps dinging
Unable to remove key from ignition for a period of time ie. 30 sec. to a minute or more. After driving the vehicle for a either a long or short period of time and you park it, you can't turn the ignition lock all the way off.
In December of 2024 Safety Recall N2424440-01 was applied. On the morning of Saturday August 23, 2025 while driving on a two (2) lane highway with the cruise control set to 60 MPH the vehicle “shuttered” then the shifted into a lower gear locking the rear wheels bringing the vehicle to an almost complete stop in very short distance. I was able to pull off the road out of traffic. Since it was a light traffic morning, no one else was close limiting the risk to me. Had there been anyone behind me they would have been caught off guard by the sudden stop with no signal of brake lights showing. The vehicle is in “limp mode” and will not exceed 35 or 40 MPH. The vehicle will shift in to park and neutral. It will not move in reverse forward. The vehicle has been transported to the nearest Chevrolet dealership for diagnostics and service.
The contact owns a 2021 Chevrolet Silverado 2500. The contact stated while the vehicle was turned off and shifted into park, the tailgate opened independently. The failure occurred frequently, and the contact removed the fuse to prevent the tailgate from unlatching independently. The contact stated that the failure only occurred while the vehicle was not in drive. The dealer had been contacted and notified of the failure. After researching the failure, the contact became aware of NHTSA Campaign Number: 24V060000 (Latches/Locks/Linkages); and related the failure to the recall. The dealer confirmed that the VIN was not included in NHTSA Campaign Number: 24V060000 (Latches/Locks/Linkages). The vehicle was not repaired. The manufacturer was informed of the failure, and a case was opened. The manufacturer referred the contact to the NHTSA Hotline to report the failure. The failure mileage was unknown.
The tailgate on my truck is electrically opened & closed. I've been having trouble with the tailgate opening randomly, even while driving. This has happened multiple times and can create a very dangerous situation, especially if i'm hauling something in the bed of the truck and it could possibly fall out if the tailgate were to open. Once i was hauling a bunk bed and the tailgate opened without me knowing it. Fortunately, we were driving through town on a flat road when it happened & nothing fell out. Someone behind me pulled up beside me to let me know my tailgate just opened. I recently took my truck to my local Chevy dealer & they said there was no recall on it & they wanted $170 for them just to look at it. I looked online and see recall# N232426400 for it but i've never received a notice & the dealer says there is no recall on it for my truck. (I have a video of it opening & closing by itself but it's too large to send you)

GM has acknowledged the problem and has issued a recall on transmissions effecting 2020 – 2022 Chevrolet Silverado 2500/3500 Heavy Duty trucks equipped with diesel engines manufactured between January 28, 2019 – January 11, 2022. Our truck, a 2021 Chevrolet Silverado 2500HD, was taken in to D’arcy Chevrolet in Morris, IL to have Recall N242454440 – Momentary rear wheel lock-up – Transmission and Engine Control Module Programming with SP. Under this recall dealers will install new transmission control module software, free of charge. GM will provide a special coverage program to cover the repair of transmissions that are identified by the remedy software as containing a defective control valve. After the recall service was performed we were told by the dealership that the transmission needed to be replaced. It clearly states under the recall performed that GM is to provide special coverage to cover repairs that are identified by the remedy software as containing a defective control valve. Also, by law, dealers are never to charge for recall repairs. But GM and the D’Arcy Chevrolet are refusing to cover the repair costs for the transmission under special coverage program.

The contact owns a 2021 Chevrolet Silverado 2500. The contact stated while driving 45 MPH and depressing the accelerator pedal, the vehicle failed to respond. There was no warning light illuminated. The contact was able to pull over to the side of the road and the vehicle was towed to the residence. The vehicle was then towed to the dealer, where it remained for two months, but was not diagnosed or repaired. The vehicle was then towed to John Elway Chevrolet (5200 S Broadway, Englewood, CO 80113), to be repaired under NHTSA Campaign Number: 24V797000(Power Train); however, after three months, the dealer informed the contact that the recall repair would not be honored because the emission system had been removed from the vehicle. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The vehicle was towed to Patterson Custom Diesel, where the vehicle was being repaired. The manufacturer was not notified of the failure. The failure mileage was approximately 27,000.
The tailgate opens by randomly. I was in a CVS parking lot walking towards my car and saw the tailgate open without any commands. A young kid running towards his mother's car was just able to avoid running into it and getting 'clotheslined'. Another vehicle found it necessary to drive around the tailgate protruding from the parking spot. I have been finding the tailgate down multiple times, and thought I may have left it open even when I was sure I didn't. I also shut my tailgate recently and noticed I have a large dent in the tailgate itself when I closed it. It may have opened on another vehicle, or it was open when I was backing up assuming it was closed. After reading online I came across many owners having the exact same problems with the exact same truck. GM has told me the problem is VIN specific, and my VIN was not listed; therefore, no recall notice ever sent out to me.
My truck has been having the "Shift To Park" warning for quite a while now. Truck has roughly 72000 miles. Before it would happen intermediately, now it happens all the time. Often I have to leave my truck unlocked for long periods of time due to this malfunction. Hoping my battery will still have power to restart the next time. Reading and researching this issue online, it is a known fact that this is a widespread problem and GM needs to step up and issue a recall.
The contact owns a 2021 Chevrolet Silverado 2500. The contact stated that upon reaching his destination and shifting the vehicle into park, the vehicle failed to recognize that the gear shift lever had been moved to park (P). The contact stated that the "Please Shift to Park" message was displayed and the doors were unable to be locked. The vehicle returned to normal functionality, but the failure became a regular occurrence. No warning lights were illuminated. The vehicle was not taken to the dealer or an independent mechanic. The vehicle was not repaired. The manufacturer was notified of the failure and referred the contact to the NHTSA Hotline for assistance. The failure mileage was 25,000.
The tailgate on our truck keeps automatically coming down and up on its own. It doesn’t matter if the truck is in park or drive. It doesn’t matter if it’s locked or unlocked. It will do it several times in a row. It will go half way back up and then just slam down. Unfortunately we didn’t know this was an issue as we just bought the truck a few days before we noticed it happening and it fell down and hit something in our garage causing a dent and multiple scratches as it happened overnight without us being aware of the issue. It happens when I’m going down the road, which means I cannot use the bed of my truck as things could fall out. Also, I’m limited on where I can park it. We called GM and a dealership and there’s nothing they can do since it’s not recalled unless we pay to have it fixed. So now we have a new to us truck that’s now dented because of this issue. There is no warning it’s going to happen. After the tailgate comes down I get a notification saying tailgate down when I’m driving. This happens everyday, several times a day and we’re having to avoid driving it when possible due to this issue.
RECEIVING SERVICE TRAILER BRAKE SYSTEM. SEEMS THIS IS A KNOWN ISSUE WITH TRUCK AND SHOULD BE A RECALL THAT GM FIX AND NOT PUT ONTO CUSTOMER

DEF system failure causing truck to enter limp mode being reduced to 65 MPH, then 55 MPH, and then restricted to 5 MPH. Error in def system led to DEF heater to go bad causing the incident to occur.
I was driving with family on the highway when the rear wheels locked up causing seatbelts to lock up and abruptly stop violently.

The current recall for the defective transmission valve does not address the safety issue with the vehicle. This a known issue with the truck and the transmission valve should be replaced rather than simply monitoring it. My safety and the vehicles I share the road may be at risk if the software upgrade limits the drivability while pulling a trailer on interstate highways. The life span of this vehicle is well beyond the allotted 150,000 miles of coverage and to not extend this "special coverage" beyond the original owner makes this a safety hazard to future owners of the vehicle as wellupgrade .GM is calling the software a "service correction" when in reality they are making no correction, just monitoring the problem.
In certain GMC and Chevrolet Models 2019 to 2025 it is common knowledge and easily found on the web that there is a manufacturer defect that does not allow the vehicle to be placed in “park” properly due to defective parts in the steering column that do not allow the vehicle to safely turn off fully. The module does not seat and the vehicle will remain on in auxiliary. https://m.youtube.com/watch?v=RTcX-j3R5OQ . Again the link is an example but this defect is rampant.

Originally Chevrolet offered a 2 phase approach for this recall. First phase was to install monitoring software in the ECU and TCU to monitor for early transmission control valve premature wear. Second phase was to actually replace the suspect valve. I've just received a second correspondence from Chevrolet for this recall, now only offering Phase 1, TCU and ECU monitoring software installation. So if the monitoring software detects premature valve failure, the vehicle will be limited to 5th gear. I don't think limiting a vehicle to 5 our of 10 gears is a safe solution. If I'm on the freeway, towing 10,000 lbs, and my vehicle is limited to 5th gear, I may have to travel a very long time at way below posted speed limits before I can get to a dealer for repair. As we all know, travelling on the freeway at lower than posted speed limits can be just as dangerous as travelling above posted speed limits. And in fact, here in Idaho, if you're travelling below posted speed limits you can be fined just as if you were exceeding speed limits. I feel Chevrolet's original solution is the best. Replace the suspect valve proactively instead of actually waiting for it to fail which can possibly cause an unsafe condition.
Engine Code P10D9. Electrical harness to the DEF tank melted. My truck was not part of the GM recall but clearly effects more vehicles than they believe. See NHTSA report N212342980. Spoke with Chevy customer service today and they stated the wiring harness melting is not covered under warranty. This is a fire hazard that GM should remedy. The recall is clearly larger than 38,000 vehicles. Are we supposed to drive the vehicles and not know if they will catch fire or not? I spoke with a technician at Milnes Chevy in Imlay city today and he stated that he sees so many of these trucks that were not under the recall having the same issue with melted wiring harness.
The DEF heater short circuit and derates the truck. If the government says we have to have this they should cover the cost to fix it.

The contact owns a 2021 Chevrolet Silverado 2500. The contact stated while driving 55-60 MPH, the rear wheels seized. The contact had to release the accelerator pedal to self-correct the failure. The contact stated that the failure occurred twice. The vehicle was taken to the dealer, and the contact was informed that the vehicle was included in NHTSA Campaign Number: 24V797000 (Power Train); but was repaired. The contact stated while driving 25-30 MPH after the repair, the failure reoccurred a third time. The check engine warning light illuminated and the “Engine Power Reduced” message was displayed. The vehicle was taken to another dealer, Chevrolet of Montebello (310 W Whittier Blvd, Montebello, CA 90640); but was not diagnosed or repaired. The manufacturer was made aware of the failure and opened a case. The failure mileage was 122,000.
While the truck is in motion and the transmission shifts gears it has a kick to it also it down shifts on its own and other times the truck loses all power like if it was in neutral
The contact owns a 2021 Chevrolet Silverado 2500. The contact received recall notifications of NHTSA Campaign Number: 24V060000 ( Latches/Locks/Linkages) and would like to be removed from the recall distribution list of NHTSA Campaign Number: 24V060000 ( Latches/Locks/Linkages) because the bed of the vehicle had been replaced and the recall was no longer applicable. The dealer was not contacted. The manufacturer was contacted.
