The overall rating combines three components: health inspections, staffing levels, and quality measures. A 5-star rating indicates quality well above average, while 1 star means quality much below average. This database contains 14,703 facilities across 53 states.
Deficiencies are violations found during state health inspections. They range from minor issues to serious problems that put residents at risk. When deficiencies are severe or repeated, CMS can impose monetary fines on the facility. The number of deficiencies and total fines are indicators of a nursing home's compliance history.
All data comes from the CMS Provider Data Catalog, specifically the Nursing Home Compare dataset. CMS publishes this data through its public API at data.cms.gov.
