I have a 2015 ford transit 150 and I was driving home on the highway and out of nowhere my van downshift into I believe 1st or 2nd gear causing the engine to redline and the rear tire to lock up and cause me to lose control of the vehicle! For about 3-7 seconds in that time the cars behind me had to slam on there breaks almost rear ending me due to the abrupt deceleration from my vans rear tire lo
... cking up and as that happened the back of my van began to sway out from behind me and right at the time I believed I would have rolled over possibly killing me and injuring other drivers the van randomly went back into the proper gear allowing me to gain control and pull over. After I had time to process what just happened the check engine light came on and with my scan tool I read the code and it said that the speed sensor for the transmission had malfunctioned and after some research I found that similar year ford f150s with that same transmission and that same part number had recalled that part for the same issue! And I ask that if you would please save life’s by recalling this part it is not a debate it is a necessity please please recall this partRead more

I am reporting a recurring structural failure of the Sky One-Touch power top assembly on my 2024 Jeep Wrangler. Since August 28, 2024, the vehicle has been out of service for a cumulative total of 62 business days across seven separate repair attempts. Safety Risk: The failure involves the tensioning system and internal springs of the power top. The defect has progressed to the point that visibl
... e daylight is present through the gaps between the roof assembly and the vehicle body while the top is fully closed. At highway speeds (55–70 mph), this gap causes the roof to lift and oscillate significantly. This creates a severe driver distraction and poses an unreasonable risk of the assembly further destabilizing or detaching from the vehicle while in motion, endangering occupants and trailing traffic. Dealer Misconduct & Repair History: During the third repair attempt, a service associate confirmed the loss of tension in the straps and springs. However, the technician subsequently refused to document these findings. The dealership omitted from their report to Stellantis that they performed unauthorized 'readjustments' and 'screwed the assembly back in' to temporarily mask the defect. This deceptive reporting prevents an accurate warranty history and deliberately avoids Lemon Law accountability. Despite these 'fixes,' the structural seal remains compromised. The vehicle is currently unsafe for highway operation."Read more

Vehicle: 2021 Jeep Grand Cherokee L Issue Type: Electrical System / Loss of Power / Braking System While operating my 2021 Jeep Grand Cherokee L on Interstate 40 at approximately 70 miles per hour, with two minor children in the vehicle, the vehicle suddenly and without any prior warning began to rapidly decelerate, despite continued pressure on the accelerator pedal. At the time the decelerati
... on began, there were no warning lights, alerts, or indications of any malfunction. The vehicle exhibited a significant loss of power, and it felt as though it was being mechanically restricted or “dragging,” severely limiting its ability to maintain highway speed. Only after the vehicle had already begun to decelerate did a brake system warning indicator appear on the dashboard. This sequence of events indicates that the warning system did not provide advance notice of the failure. This sudden and uncommanded deceleration created an immediate and serious safety hazard, particularly given the vehicle’s speed and surrounding interstate traffic. The inability of the vehicle to respond appropriately to acceleration input placed occupants at risk of a rear-end collision or other serious traffic incident. Although the warning indicator later turned off, the vehicle continued to operate abnormally, with ongoing reduced acceleration and diminished responsiveness. This incident occurred without prior warning, and the delayed activation of the brake warning indicator raises serious concerns regarding a potential defect involving the vehicle’s electrical, braking, and/or powertrain control systems, including possible failure of system monitoring or alert mechanisms. Given the circumstances including highway speed, sudden loss of propulsion, delayed warning notification, and the presence of child passengers. I respectfully request that this issue be formally investigated as a safety-related defect.Read more

December 2025: While driving, multiple warning lights (check engine, blind spot, lane departure, forward collision, accident avoidance)
... flashed repeatedly. Battery died and was replaced by AAA. Vehicle taken to dealership due to safety concerns; dealership found no issues. March 2026: Same warnings occurred again while driving. Vehicle lost all power while stopped at a red light. Hazard lights did not work. Gear selector knob rotated freely and could not be put in park. After opening the hood, vehicle restarted but windows, powered seats, and infotainment stopped working. Dealership replaced fuse array. April 2026 (~3 weeks later): Vehicle would not start. After accessing battery under passenger seat, vehicle turned on with alarm and lights flashing. Same warning lights reappeared while driving. Interior electronics (windows, seats, infotainment) remained non-functional. Safety Risk: Complete loss of power in traffic, inability to use hazard lights, failure of driver-assistance safety systems, and inability to shift into park create significant risk of crash or injury. Inspection/Repair: Vehicle inspected multiple times by dealership and AAA. Batteries and fuse array replaced. Issue persists and root cause is UNKNOWN. Hi! This is my 2nd input here about my 2022 Nissan Rogue. Recaping that whike driving home, smoke began to emerge from under the hood and multiple warning signs popped up (please see previous input for more information). Cureentky, my vehicle was diagnosed with a converter issue. The recent recall was related to the engine. The converter and my spark plugs (2 spark plugs replaced already) were all
... damaged by the engine. This makes sense why there are numerous issues. While the extended warranty covers the engine and not the converter, under the law of consequential damage, anything damaged by this engine is covered due to the cause from the engine. I currently have a rental and am leaving my Rogue at the dealership due to it being unsafe to drive and that the dealership has not provided a remedy for this defect, which is covered by law under this recall on the Rogue due to consequential damage. My Rogue will continue to have issues until all related/noncoverd parts are fixed under this. This vehicle is now a lemon due to consistent issue with the engine it damaging other parts. I am hoping for a good will pay and buyback.Read more

Vehicle: 2015 Subaru Forester Mileage at Failure: 85,000 miles Component: Powertrain – CVT Transmission My 2015 Subaru Forester is experiencing a premature safety‑related CVT transmission failure at approximately 85,000 miles. The vehicle developed a pronounced whining noise that increases with vehicle speed, consistent with internal CVT bearing or pulley failure. This is a known defect patter
... n in Subaru Forester CVTs and is not related to maintenance or wear. The noise is continually progressing and the risk of sudden catastrophic failure makes the vehicle unsafe to operate. An authorized Subaru dealer and the District Parts & Service Manager (DPSM) both confirmed that the CVT has failed and requires full replacement. This failure occurred well within Subaru’s extended CVT warranty mileage limit of 100,000 miles, and matches the defect pattern Subaru acknowledged when it extended CVT coverage nationwide for this model. However, Subaru is refusing to replace the CVT due to the 10 calendar year extension expiration. Subaru’s maintenance schedule for this vehicle does not require any CVT fluid replacement interval, so there is no missed maintenance that contributed to the failure. A failing CVT creates a safety hazard due to to the risk of sudden loss of power, hesitation, or inability to accelerate. I am filing this complaint so NHTSA is aware of this premature CVT failure and the associated safety concerns of SOA's refusal to replace the CVT under their extended warranty.Read more

I am reporting an ongoing defect with the infotainment system in my 2020 Mazda CX-5, involving ghost-touch inputs, system freezing, and random reboots. These issues began around the second year of ownership and have progressively worsened. The system constantly registers inputs that are not being made (ghost-touch), changes settings on its own; and occasionally- reboots while operating the vehicl
... e. This makes it difficult or impossible to reliably use essential vehicle functions such as navigation, communication, and audio controls. The issue is particularly concerning because it creates a driver distraction and potential safety risk, as the system behaves unpredictably and requires attention while driving to manage or disable unintended inputs. I reported these issues multiple times to an authorized Mazda dealership while the vehicle was still under warranty. However, I was told no issue could be found or that software updates would resolve the problem. These updates did not fix the issue. On August 9, 2025, another system update was performed under the Mazda Connect Class Action Settlement, but the problem persists. The dealership is now requiring a paid diagnostic fee before proceeding further, despite the known nature of this defect. Based on my research and widespread owner reports, this issue is commonly associated with infotainment system defects, including screen delamination and/or failure of the Connectivity Master Unit (CMU). I am submitting this complaint to document the defect and request further review, as it affects safe operation and driver attention.Read more

I am submitting a formal safety complaint regarding my 2017 Honda Civic, which is experiencing a serious steering issue that appears to be consistent with a known defect currently under recall for similar vehicles. When driving at speeds above approximately 40 mph, the steering wheel becomes noticeably “sticky” or resistant, making it difficult to maintain smooth and controlled steering. This
... condition creates a safety concern, particularly at highway speeds, as it affects the vehicle’s responsiveness and handling. I understand that there is an existing recall related to steering issues affecting certain Honda Civic models. However, my vehicle’s VIN is not currently included in the recall population, despite exhibiting the same symptoms described in those cases. Given that this appears to be a widespread and documented issue, I believe my vehicle may also be affected by the same underlying defect. I am concerned that I have to pay out of pocket for a repair that is directly related to a known safety issue already acknowledged in other vehicles of the same make and model. I respectfully request that NHTSA review this matter, investigate whether the recall scope should be expanded, and take appropriate action to ensure all affected vehicles are covered. I am happy to provide additional details, including my VIN, maintenance records, or further documentation upon request. Thank you for your time and attention to this matter.
