Defect Description
Chrysler (FCA US, LLC) is recalling certain 2014-2020 Jeep Grand Cherokee, and 2014-2019 Ram 1500 vehicles equipped with 3.0L diesel engines. A high pressure fuel pump (HPFP) failure may introduce internal debris into the fuel system, potentially resulting in fuel starvation.
Consequence
Fuel starvation may result in an unexpected loss of drive power, increasing the risk of a crash.
Remedy

Defect Description
Chrysler (FCA US LLC) is recalling certain 2014-2019 Jeep Grand Cherokee vehicles equipped with the 3.0L EcoDiesel engines. The Exhaust Gas Recirculation (EGR) cooler may crack and allow pre-heated vaporized coolant to enter the EGR system. This mixture may combust inside the intake manifold.
Consequence
Combustion in the intake manifold can increase the risk of a fire.
Remedy
FCA US LLC will notify owners, and dealers will replace the EGR cooler, free of charge. Dealers will also inspect and, as necessary, replace the intake manifold. Parts are not currently available. Owners were mailed an interim notification on December 21, 2021. A second notification will be mail

Chrysler (FCA US LLC) is recalling certain model year 2017 Jeep Grand Cherokee manufactured August 8, 2016 through August 16, 2016. In the affected vehicles, the rear tow hook bracket or tow eye bracket may be loose.
Consequence
If the tow bracket is loose, the bracket may detach from the vehicle while being it is being driven, possibly becoming a road hazard. If the bracket detaches during a vehicle recovery, there may be a loss of control of the towed vehicle. Either scenario increases the risk of a crash.
Remedy
Chrysler will notify owners, and dealers will confirm both tow hook bracket bolts are properly tightened, free of charge. The recall began on October 31, 2016. Owners may contact Chrysler customer service at 1-800-853-1403. Chrysler's number for this recall is S73.

When the bank received the title documentation, the mileage listed on the title was 101,985 miles. This indicates a discrepancy of nearly 40,000 miles between the mileage represented at the time of sale and the mileage recorded on the title. Key details: * Odometer at purchase: approximately 64,000 miles * Mileage listed on dealership paperwork: approximately 64,000 miles * Mileage listed on title received by lender: 101,985 miles * Current odometer reading: approximately 73,000 miles This discrepancy suggests the possibility that the vehicle was sold with inaccurate mileage disclosure or that the odometer may have been rolled back before the sale. There have been several issues that have occured over the course of the last 3-4 years . Just this winter alone , i have had my 4wd serv needed maintenance , the air conditioning doest get cold the heater doesnt heat up , i have checkee fluids && cleaned necessary components , i have checked all fu

Power loss , battery drainage , headlight harness wont allow any bulb to light up , . I have done all necessary procedures in order to verify all issues , scanner wont reset or delete any old codes . Battery removal has no effeft in change . All these mechnical , electrical && suspensiom issues , have occured making vehicle unoperable through out majority of the year . About 6-8 months out of 12 . Way too many issues for me to rememeber , let alone list on here . The whole vehicle has unfortunately been nothing but a big maintainence issue .Read more

... Jeep sideways in a snowbank for two weeks to prevent further rollaways, creating added risks for my family in winter conditions. The vehicle remains at the dealership. During the first two days of the evaluation, the dealer confirmed brake shoe thickness (4mm), verified the EPB software was current, and found no diagnostic codes. They could not recreate the rollaway or identify a cause. No mechanical or electronic faults were found. FCA communication has been inconsistent. I experienced dropped calls, long holds, contradictory statements, and repeated refusals to provide written documentation of the inspection authorization. FCA said they could not escalate the safety case unless the vehicle was already at the dealership, yet still refused written confirmation of the purpose of the inspection, and I was not willing to bring the vehicle in without documentation. After the evaluation, when I attempted to relay the required findings per the RO instructions, FCA informed me the safety case had already been closed, leaving no record of the defect. FCA said only the rental authorization remained open and they could no longer escalate my case. I then had to re-explain the entire rollaway incident. FCA and the dealer now disagree on whether the RO is open or closed, and FCA has not explained who is responsible for the ongoing loaner or what the next steps are. The defect remains unresolved and the Jeep is unsafe to operate or park at my home.Read more

They could not recreate the rollaway or identify a cause. No mechanical or electronic faults were found. FCA communication has been inconsistent. I experienced dropped calls, long holds, contradictory statements, and repeated refusals to provide written documentation of the inspection authorization. FCA said they could not escalate the safety case unless the vehicle was already at the dealership, yet still refused written confirmation of the purpose of the inspection, and I was not willing to bring the vehicle in without documentation. After the evaluation, when I attempted to relay the required findings per the RO instructions, FCA informed me the safety case had already been closed, leaving no record of the defect. FCA said only the rental authorization remained open and they could no longer escalate my case. I then had to re-explain the entire rollaway incident. FCA and the dealer now disagree on whether the RO is open or closed, and FCA has not explained who is responsible for the ongoing loaner or what the next steps are. The defect remains unresolved and the Jeep is unsafe to operate or park at my home.Read more

I was told the entire engine needs to be replaced. The vehicle has low mileage. Our safety was at risk because I had no warning to get the car to the shop before a major malfunction. I was very fortunate my car was stopped at the time. If I was waiting at a left turn in an intersection we could have had a terrible collision. The exact problem has not been reproduced. ... quipped with Monostable electronic (E-shift) gearshift assemblies supplied by ZF Group (ZF). The E-shift system operates electronically and the gear requested by the driver is transmitted from the shifter via the CAN Bus to the Transmission Control Module which makes the requested shift. The Monostable gearshift does not move into a detent but springs back to a centered/neutral position after the driver selects a gear and releases the shifter. A button on the shift knob must be depressed to shift out of Park, shift out of Neutral, and to shift from Drive to Reverse or Park.The gear selected is shown on a display in the dash and illuminated letters on the shifter. If the driver's door is opened when the gearshift is not in Park, a chime sounds and a message is displayed on the instrument panel to warn the driver.In addition, the engine Start/Stop push-button control logic does not permit normal engine shut-off when the transmission is not in Park. This logic may provide feedback to drivers who attempt to turn the engine off when the transmission is not in Park. However, this function does not protect drivers who intentionally leave the engine running or drivers who do not recognize that the engine is still running.NHTSA testing during PE15-030 indicates that operation of the Monostable shifter is not intuitive and provides poor tactile and visual feedback to the driver, increasing the potential for unintended gear selection.ODI?s analysis of the PE15-030 complaint and field report data identified 306 incidents of vehicle rollaway followingintended shifts to Park in the 2014-2015 Grand Cherokee. These resulted in 117 alleged crashes. Twenty-eight of the crashes reportedly caused injuries, including 3 with a fractured pelvis and 4 others requiring some degree of hospitalization (a ruptured bladder, fractured kneecap, broken ribs, damaged to right leg). Other injuries include reports of a broken nose, facial lacerations requiring stitches, sprained knees, severe bruising, and trauma to legs.An Engineering Analysis (EA16-002) has been opened to assess the scope, frequency, and safety-related consequences of the alleged defect. ... clude MY 2015 Grand Cherokee vehicles and identified a total of 176 incidents in complaints provided by FCA or submitted to ODI by consumers.ODI conducted the following work to assess the alleged defect in the subject vehicles:1) evaluated the design of the subject Forward Collision Warning (FCW) system, including the maximum braking authority during automated braking; 2) analyzed the frequency and severity of field incidents of unintended braking; 3) analyzed warranty data for evidence of system faults that may be related to the alleged defect; and 4) reviewed the performance of the subject system in testing conducted as part of NHTSA?s 2014 Automatic Emergency Braking (AEB) test program.ODI?s analysis found that the maximum brake activations that can be commanded by the subject system are limited in both braking force and duration.The system is not capable of achieving full ABS braking, as alleged in some complaints, unless driver brake application triggers the Advanced Brake Assist (ABA) mode.Analysis of field data found that unintended braking events occur randomly for a variety of causes and are not driven by any single environmental factor, traffic condition, or vehicle fault.The incidents involve short duration moderate braking events with minimal changes in vehicle speed.None of the incidents resulted in crash or injuries.The subject vehicles performed as well or better than peer vehicles in testing conducted as part of NHTSA?s AEB test program.This preliminary evaluation is closed. ... okee vehicles manufactured by Fiat Chrysler Automobiles (FCA).The recall was in response to the Office of Defects Investigation (ODI) investigation EA12-001 during which data provided by FCA indicate that the air bag circuitry inside the Occupant Restraint Control (ORC) module can degrade. ORC degradation can result in an inadvertent air bag deployment, without a crash or impact, while the vehicle is being operated.An IABD may involve frontal, side and belt pretensioner devices, and may result in injuries consisting of burns, cuts and bruises to the upper body region, and or the potential for a loss of control and or crash.The remedy FCA developed for 12V-527 consisted of installing an in-line jumper harness with an integrated electrical filter for the circuits that connect to the air bag modules (squib circuits).This filter is intended to eliminate transient electrical spikes to the ORC which FCA asserted are responsible for the IABDs.ODI has identified a total of 9 NHTSA complaints across all affected vehicles indicating an IABD event occurring after the vehicle had previously been remedied under 12V-527.In addition, FCA reported 12 additional complaints of post-remedy IABD events. The IABD events took place both while driving and while the vehicle was stationary. Six customers incurred injuries consistent with air bag deployments however no crashes were reported.As a response to this investigation, FCA has issued 2 new recalls involving the IABD.On January 27, 2015 FCA notified ODI that it would conduct a safety recall (15V-046) to remedy potential IABD events occurring in 753,176 MY02-03 Jeep Liberty vehicles built through 03/28/03 and MY 2002-2004 Jeep Grand Cherokee vehicles built through 5/23/03.Note that this recall, which was scoped based on field IABD events does not include all MY 2003 Liberty or MY 2004 Grand Cherokee manufactured with the same level ORC module.According to FCA, the ORC degradation and subsequent electrical overstress condition that causes the air bags to deploy inadvertently is not fully remedied with the filter installed during the 12V-527 campaign.To remedy IABDs under 15V-046, FCA will install a new design ORC module using a next generation electrical architecture.The next generation architecture has been used in other non-subject vehicles for many years without evidence of IABD events occurring.In addition to this recall, on October 15, 2015 FCA advised NHTSA that it would conduct safety recall 15V-673 to remedy potential IABD events in the remaining 284,089 MY 2003 Jeep Liberty and MY 2004 Jeep Grand Cherokee vehicles manufactured with the earlier generation ORC module that were not previously recalled.FCA initially did not recall these populations due to the lack of known IABD events.However, FCA is now aware of 7 IABD events occurring in this population. ... red between January 5, 2010 and December 11, 2013 (see NHTSA Safety Recall 14V-391), with notifications to begin in August 2014. According to Chrysler, the fire is caused by an electrical short in the vanity lamp wiring for either one of the sun visors mounted on the vehicle. The sun visors are mounted to the roof of the vehicle through the headliner with three metal screws. It is possible for the sun visor wiring to be pierced by one of these screws either during initial vehicle assembly or later headliner area repairs which may cause an electrical short potentially resulting in fire.ODI opened an investigation involving MY 2011-2013 Jeep Grand Cherokee and Dodge Durango vehicles based on consumer reports of headliners catching fire at the front of the passenger's compartment near the sun visors. Complainants reported failure symptoms that ranged from discoloration to open flame burning of the headliner and/or sun visor material. Complainants reported a burning plastic smell and smoke from the headliner, often occurring while the vehicle was in motion. Damage ranged from a small burn spot on the headliner to a near complete burn of the vehicle resulting in a total loss; sunroof damage/glass shattering occurred in some cases. Incidents are more likely to occur when the ignition key is in the on position.However, several vehicles continued to burn after the key was turned off, and key-off fires, while highly unlikely, cannot be fully eliminated as a potential problem. The earliest incidents chronologically were often associated with vehicle assembly or in-plant repair procedures. Chrysler revised assembly and in-plant repair procedures for the headliner which reduced the number of fields incidents.However, anytime a repair is made to the headliner of the vehicle there could still be a risk of shorting the wiring and creating a hazard.Dealership technicians will inspect the vehicles for damaged wiring caused by an improper service procedure and repair any damage. Also, all vehicles will have a plastic guideway installed on each sun visor that routes the wiring away from the attachment screws preventing the wiring from being shorted. All MY 2011-2014 Grand Cherokee and Dodge Durango built before December 11, 2013 are affected by the recall; vehicles manufactured after this date had the guideway installed during vehicle production.ODI received a total of 6 complaints as of June 2014, all of which reported a fire and two of which alleged related injuries. All 6 incidents occurred while the vehicle was being driven. In each case the driver smelled smoke and witnessed the headliner burning on either the driver or passenger side. Two complaints indicate smoke inhalation injuries to the driver. In response to ODI's information request letters of 8/28/2013 and 2/26/2014, Chrysler provided consumer and warranty data which was received by ODI on 10/7/2013 and 3/19/2014. Headliner Fires
The Office of Defects Investigation (ODI) has identified 5 reports of vehicle headliners catching fire at the front of the vehicle near the sun visors on Jeep Grand Cherokees.Customers reported a range of fire conditions ranging from minor overheating to an open flame at the headliner and/or sun vis

In some reports the fire spread to the front seats and/or door panels of the vehicle.In one report, the sunroof was damaged causing the glass to shatter.The cause of these fires is an electrical short in the vanity lighting wiring circuit that is routed to either one of the sun visors.The sun visors are mounted to the roof of the vehicle through the headliner with three screws.The sun visor wiring may be penetrated, or pierced by one of these screws either during initial vehicle assembly or subsequent headliner area repairs.The piercing causes an electrical short that could result in a fire.There is no dedicated fuse for the affected circuit so the electrical short can continue until the short clears or the vehicle is keyed off.The Dodge Durango uses the same headliner assembly, and Chrysler indicates there are similar headliner fire incidents affecting this model. Power steering hose failure
The Office of Defects Investigation (ODI) analyzed complaint data provided by Chrysler as well as complaints submitted to ODI from consumers. ODI's analysis identified a total of 24 unique reports indicating power steering hose blow-offs resulting from a dislodged power steering cooler turbulator fi
... n in some early production vehicles.According to Chrysler, a dislodged fin can occur when the turbulator is inserted into the power steering cooler or loaded into the assembly machine.A dislodged fin may block the flow of power steering fluid creating excess back-pressure in the power steering system leading to the possibility of a return hose blow-off.Chrysler and its power steering cooler supplier, Dana, modified the cooler assembly process to address factors that may contribute to hose blow-offs. The redesigned turbulator assembly stations were completely implemented in production by April 2012.During PE12-020, Chrysler identified occurrences where the power steering return hose may blow-off at the power steering cooler.This is a condition that typically occurs early in the life of the vehicle with 65% occurring at less than 2,000 miles and 96% occurring with less than 8,000 miles. The data also shows that 88% of the conditions occurred with less than 4 months in service. Over 50% of the blow-off complaints occurred on vehicles with build dates between November 22, 2011 and December 23, 2011. None of the conditions occurred with more than 12,000 miles or 7 months in service. Chrysler?s complaint analysis indicates up-to 14 additional vehicles may experience the hose blow off condition.None of the 24 unique reports with the hose blow-off condition alleged smoke or fire in the engine compartment. In the event of a blow-off, it is highly unlikely for power steering fluid to reach any of the ignition sources in the engine compartment and result in an engine fire. Complaint analysis identified five reports alleging engine compartment fires that resulted from other or unknown causes, including one attributed to an overheated transmission fluid. Two fires of unknown cause cited in the opening resume for PE12-020 involved vehicles equipped with V8 SRT 6.4L engines which are not equipped with the steering system affected by the turbulator fin concern. ODI will continue to monitor field experience in Jeep Grand Cherokee vehicles equipped with that engine.There have been no reports of loss of steering control, crashes or injuries as result of this condition.Additionally, all affected vehicles that may have experienced a hose blow-off condition were repaired under Chrysler?s 3 year/36,000 mile manufacturer?s warranty at no cost to the owner and supplier?s process improvements put in place have eliminated the likelihood of power steering hose blow offs. There is no indication of loss of motive power or unreasonable safety risk associated with the alleged defect in the subject vehicles. Crash Related Fuel Tank Fires
ODI has completed an extensive analysis of rear crash fuel tank system integrity data for the subject Jeep Cherokee, Grand Cherokee and Liberty vehicles (SVs).As of June 2013, ODI knew of 56 post-collision fatal fires, 28 non-fatal fires, and 6 fuel leak incidents (totaling 90 incidents, 75 fataliti
... es and 58 injuries) involving the SVs.Based on this analysis, ODI issued a Recall Request Letter (RRL) on June 3, 2013 requesting that Chrysler recall the Grand Cherokee and Liberty vehicles due to the rate of fatal, non-fatal fire, and fuel leak incidents when compared to peer vehicles (compact and medium SUVs) built during the same time period.In the RRL, ODI described how the location of the aft-mounted fuel tank made the SVs vulnerable to rear impact crash fuel tank failures.During the analysis, besides the high speed/high severity crashes present in both the SV and peer vehicles, ODI observed a significant number of low and moderate speed rear impact crash related fires and leaks, particularly in the Liberty.The data also demonstrated that SVs originally equipped with towing hitch receivers (trailer hitches) appeared to be under-represented in rear-impact related fuel tank failures.In the RRL, ODI requested that Chrysler provide its response by June 18, 2013, otherwise NHTSA might proceed to an Initial Decision that the SVs contain a safety-related defect.Chrysler responded to the RRL on June 4, 2013 and vigorously disputed the tentative findings of the RRL.Among other things, Chrysler argued that the SVs had an overall safety record superior to their peers, met or exceeded all applicable federal motor vehicle safety standards (FMVSS) and only experienced fires in severe high energy rear impacts.Nonetheless, while continuing to maintain that the SVs did not contain a safety-related defect, Chrysler proposed a recall (13V-252) of approximately 2.5M (1.6M currently registered) model year (MY) 1993 - 1998 Grand Cherokee (ZJ) and MY 2002-2007 Liberty (KJ) vehicles on June 18, 2013.The recall remedy submitted by Chrysler would employ a hitch receiver assembly consisting of a steel cross-member behind the fuel tank with forward-projecting arms bolted to the frame rails on either side of the fuel tank.For the MY 1999 - 2004 Grand Cherokee, Chrysler indicated it would conduct a customer satisfaction campaign and inspect vehicles equipped with aftermarket tow hitch receivers, and if necessary, replace any such hitch receivers whose conditionmay increase the risk of fuel system failure in rear crashes.Examination of the available data established that the MY 1999 - 2004 Grand Cherokee did not pose the same magnitude of safety risk as the MY 1993 - 1998 Grand Cherokee and MY 2002-2007 Liberty, particularly in low and moderate speed rear impacts.Because the agency has concluded that the vehicles do not present an unreasonable risk to safety, ODI is closing its investigation of the MY 1999 - 2004 Grand Cherokee.Although it was not within the scope of the Petition initiating this investigation or the Preliminary Evaluation preceding this EA, ODI also examined the performance of the MY 1993 - 2001 Cherokee as part of this investigation.NHTSA's assessment of the available data for the Cherokee did not establish an unreasonable risk in comparison to peer vehicles.ODI does not approve proposed defect remedies.While offering to install hitch receiver assemblies on the MY 1993-1998 Grand Cherokees and 2002-2007 Liberty, Chrysler did not, in ODI's view, provide enough evidence demonstrating the effectiveness of the proposed remedy in rear impacts.The Complete Summary for this Closing Resume (EA12-005) can be seen byRead more

NHTSA's assessment of the data collected during this investigation indicates that rear-impact-related tank failures and vehicle fires are more prevalent in the JGC than in the non-Jeep peer vehicles. ... hat the plastic fuel tank's placement behind the rear axle and below the rear bumper, and the lack of adequate shielding, make it more vulnerable to rupture or leakage from a rear-impact by another vehicle (including damage from other components located on the Grand Cherokee), or in the case of rollover crashes, from other external objects.CAS also alleged that the fuel filler neck tears off in crashes.In its petition, CAS cites data from NHTSA's Fatality Analysis Reporting System (FARS) showing 172 fatal fire crashes with 254 fatalities involving the subject vehicles from calendar years 1992 through 2008.CAS stated that there have been at least 44 crashes with 64 total fatalities (subject and non-subject vehicles) where fire was entered as the Most Harmful Event (MHE) in the FARS database.In response to the CAS petition, ODI made a preliminary examination of available data.FARS data showed 2,988 occupants of the subject vehicles have died in crashes since 1992.Of those, 55 died in 44 crashes where fire was listed as the Most Harmful Event.Identifying crashes most likely associated with the alleged defect as described by CAS (defined as the subject vehicle being struck at the 5, 6 or 7 o'clock positions) isolated 10 crashes with 13 occupant fatalities.ODI also reviewed the Vehicle Owner Questionnaires (VOQ) database and identified 12 reports alleging A) a post-crash fuel tank leak and/or B) a post-crash fire potentially related to a fuel tank leak.Of the 12 reports, 10 involved fires (two involved fuel leaks only) with 9 alleged injuries and 1 alleged fatality.The existence of these post-crash fires does not, by itself, establish a defect trend.Further review and investigation into these incidents is needed to determine the existence of any relationship between the alleged defect and each fire or leak.It should be noted that ODI also conducted a preliminary review of the Early Warning Reporting (EWR) data that did not find the subject vehicles to be over-represented for post-crash fires.ODI's initial review neither supports nor excludes the possibility that a defect exists in the subject vehicles.However, ODI has always taken the position that vehicle fires pose a significant safety risk.Accordingly, ODI has granted the petition to further investigate the conditions associated with post-crash fires in these vehicles.Read more
