I was enjoying holiday driving with family in my 2013 Chrysler Town & Country Limited when suddenly a very loud pop was heard, with a forceful strike to the back of my head being felt. Alarmed & dazed for a moment, it was observed that my driver’s side active headrest had just exploded forward s
... triking me on the back of my head. Once off the highway and stopped, I discovered that the active headrest had actually come apart. From the damage inside the exposed headrest, this catastrophic failure seemed to be mechanical in nature with root cause thought to be STRESS FATIGUE for the plastic casement. In other words, this active headrest did not just randomly “go off” from an errant SRS command or an electronically manipulated trigger short. I observed the headrest’s metal latch pin to still be in the locked or reset position. Instead, the plastic assembly holding two (2) heavy coil springs ruptured unprovoked. When this happened, the front part of the headrest assembly rapidly propelled forward. Though the 2010-2016 Chrysler Town & Country active headrests are an integral part of the air bag or Supplemental Restraint System, THIS particular failure was NOT due to a SRS Control nor electrical malfunction. Rather, the plastic retainer struts surrounding the metal latch pin failed. Being that my head and neck were static in position with a small amount of clearance from the front face of the headrest, the propulsion of the headrest forward caused quite a sharp smack to the back of my head. This led to some residual neck strain for the balance of the day. I have not experienced any repercussions since that day of the incident, but I do believe this to be a SAFETY ISSUE for others who may be more age OR physically compromised. I’ve found through MOPAR tech forum and dealership research that the 2010 - 2016 active headrest FAILURE is NOT rare, but more and more COMMON to the Chrysler make. Please advise. Though the 2010-2016 Chrysler Town & Country active headrests are an integral part of the air bag or Supplemental Restraint System, THIS particular failure was NOT due to a SRS Control nor electrical malfunction. Rather, the plastic retainer struts surrounding the metal latch pin failed. Being that my head and neck were static in position with a small amount of clearance from the front face of the headrest, the propulsion of the headrest forward caused quite a sharp smack to the back of my head. This led to some residual neck strain for the balance of the day. I have not experienced any repercussions since that day of the incident, but I do believe this to be a SAFETY ISSUE for others who may be more age OR physically compromised. I’ve found through MOPAR tech forum and dealership research that the 2010 - 2016 active headrest FAILURE is NOT rare, but more and more COMMON to the Chrysler make. Please advise. This is extremely dangerous. It happened to us previously, while we were on the interstate driving approx 65 mph, and it almost caused us to crash the vehicle. Another problem to also note, is this same problem has caused the A/C unit to stop working. Blows out hot air, but will not produce cold air. A/C was completely replaced. Still not working. Mechanic says yet again, it's a failure of electrical sources due to the TIPM module. This should be an immediate recall from Chrysler, for 2013 Town N Country Minivan. Over 200 logged complaints of the exact same problem. They manufactured a broken TIPM module in these make & model vehicles. Now, my husband is stranded 200 miles from home, on the side of the road, and it's 98 degrees out. This is 4th time, weve experienced a total power failure. So it doesn't matter if you replace the fuel pump, it will still DIE. We are having this POS towed to our local mechanic, AGAIN. Its available for inspection. On [XXX], 2022 I brought my 2013 Chrysler Town & Country mini-van to Schumacher Chrysler of Delray Beach, FL. After 2 days (on [XXX]) the service advisor informed me that their parts department didn’t have a matching headrest in stock and they would have to order one. At that time the vehicle had 68,530 miles. I followed up monthly, then bi-monthly, then quarterly until October 2023. At that point (XXX) I called your office where it was assigned case [XXX]. Your office told me to call Stellantis, which I did the next day. It became their case [XXX]. They told me that they’d contact the dealer, Schumacher, and I’d hear from the dealer “soon." I never did and I finally called Stellantis on [XXX] as followup; 4 1/2 months is a long time for “soon." Stellantis claimed they tried calling me in December 3 times. I never received any such calls or phone messages (and in checking caller id on my phone — which I basically never erase — had no record of any phone calls from either Stellantis or Schumacher. You’d think someone would have the courtesy to leave a message.) Stellantis told me yesterday that I needed to start-over; make the 25 mile round-trip drive to Schumacher (the closest Chrysler dealership) to “diagnose” the problem. On multiple occasions, and without warning, the car died while driving creating extremely dangerous and stressful situations. Additionally, the defective fuel pump relay caused the fuel pump to run even when the car was not running. This then caused damage to the fuel pump and required replacing

A recall was issues and executed for the same issue on Jeep and Durango models. I also see that this was brought up previously for this T&C model but was determined not to warrant a recall. A known, dangerous, costly, and preventable issue that was corrected elsewhere but not in this model. How is that possible? ... ehicle. The puddle also smelled of gasoline. I had the vehicle towed to a repair shop. The issue was determined to be a cracked oil filter housing assembly and the clip on the fuel return line had failed causing fuel to be sprayed onto the engine. The repair cost will be just over $800. It is fortunate that the engine was still cold and that the loss of oil caused the engine to shut off. If the engine had been hot enough to ignite the fuel, this could have had tragic results with a potential loss of my family's life. The oil filter housing assembly is a known issue with the Pentastar V6 3.6L engine. Chrysler so far has failed to take responsibility for this issue. I recently had to have repairs due to a faulty fuel pump relay. There appears to be related Chrysler TIPM recalls on almost every other model but the 2013 Town and Country. This issue appears to cause the vehicle to not start or allow the fuel pump to remain running after the vehicle is shut down. My Town and Country would not start and it produced strong fuel fumes. As noted, there are multiple safety issues noted with this model of vehicle that remain unresolved. It is a 2013 model but the issues are ones that have existed since the production of the vehicle and Chrysler has yet to assume responsibility. Sliding Doors Inoperative
The Office of Defects Investigation (ODI) opened PE21-016 on July 7, 2021, to investigate 364 complaints alleging that one or both sliding doors on 2016 Model Year (MY) Grand Caravan or Town and Country vehicles could not be opened. The door(s) could not be opened using the mechanical handle or elec
... tronic remote switches. Many of the complaints described having to remove passengers seated in the rear of vehicles through the front doors, the second sliding door (if it was operational), the window, or the car?s rear hatch. The complainants have expressed concerns that if the sliding door(s) cannot be opened in the event of an emergency or crash, it could trap passengers or delay their egress.On June 4, FCA published TSB# 23-017-20 instructing dealers to replace the door lock actuator instead of the entire door latch assembly on the subject vehicles when consumers complained about either one or both of the sliding door locks not functioning and/or emitting a loud buzzing noise during lock/unlock operation. On June 16, 2021, FCA approved a Quality Warranty Extension for fifteen years/150,000 miles from the vehicle's in-service date for sliding door lock actuators on vehicles built between May 1, 2015, through January 9, 2017. FCA has started notifying owners about the new program. FCA will provide reimbursement for owners who previously paid for sliding door repairs.Consumers whose sliding doors are inoperative will have the opportunity to have their vehicle diagnosed, and if a fault is identified, have the sliding door actuator repaired at no charge.Analysis of the FCA IR response and ODI complaint data found that the majority (96%) of sliding door failures occurred prior to the calendar year 2020 when the subject vehicles had approximately 4 years in service. At the time of the closing of this investigation, 99% of the subject vehicles are passed the age where failures typically occur. The number of new reports has been decreasing since 2020 and further reduced following the FCA Warranty Extension program. ODI found that prior to a failure of the sliding door(s) there is normally a loud buzzing noise/sound generated that alerts the operator to the actuator starting to wear out.ODI has not identified any crashes or injury allegations due to the sliding door being inoperable on the subject vehicles following nearly six years of exposure. ODI will continue to monitor complaints received for the incidence of sliding doors becoming inoperative.Accordingly, the investigation is closed. Interior Vehicle Fire
On August 6th, 2020, the Office of Defects Investigation (ODI) opened this Preliminary Evaluation (PE20-012) to investigate alleged thermal events, and/or vehicle fires originating at the charge hub in 2014 Chrysler Town and Country minivans manufactured by Fiat Chrysler Automobiles (FCA). While the
... investigation was focused on the 2014 Chrysler Town and Country vehicles, the scope of the investigation was expanded to include the 2013-2020 Chrysler Town and Country, and Dodge Grand Caravan vehicles (the RT Platform).During the investigation ODI sent two separate Information Request letters to the manufacturer. One on August 17th, 2020 and one on March 31st, 2021.ODI identified a subject 2014 Chrysler Town and Country van on a vehicle salvage website that appeared to have experienced a thermal event where the charge hub is located. With the help of the NHTSA's Vehicle Research and Test Center (VRTC), ODI purchased the vehicle from the salvage company to inspect the damage and determine if more information about the failure could be obtained. A joint inspection with the manufacturer was conducted. Following this comprehensive inspection, a cause of the fire could not be determined.FCA initiated a parts collection initiative to obtain parts from the field for testing. The test results from the FCA field parts collection were inconclusive as they could not identify a common cause for the alleged thermal events. FCA looked at multiple liquid solutions that could have entered the charge hub circuit and caused it to short out, potentially leading to a fire. FCA also performed testing where the charge hub circuit was shorted out using a wire to observe if a flame or fire was produced. FCA provided their assessment to ODI concluding that none of the testing methods performed lead to a flame or fire being propagated.Given the low rate of thermal incidents at the subject vehicle charge hub, no reports of serious injury, zero reports of crash or death, and a lack of common failure mode, this investigation is closed.The closing of this investigation does not constitute a finding by NHTSA that no safety-related defect exists. NT CRASH SENSORS ARE DESIGNED TO OPTIMIZE DETECTION OF FRONTAL OFFSET AND ANGULAR CRASHES.THE ADVANCED AIR BAG SYSTEM OFFERS DIFFERENT LEVELS OF AIR BAG INFLATION BASED ON INPUT FROM THE CRASH SENSORS.THIS INVESTIGATION REVEALED THAT THE FRONT CRASH SENSORS CAN CORRODE FROM WATER ENTERING THE SENSOR ASSEMBLY.DC IS STILL INVESTIGATING ALL POTENTIAL ROOT CAUSES OF SENSOR FAILURES.A CORRODED SENSOR CAN SET A FAULT CODE, ILLUMINATE THE AIR BAG WARNING LAMP, AND BECOME DISABLED.IT APPEARS THAT FAILURE OF ONE OR BOTH FRONT CRASH SENSORS CAN POTENTIALLY RESULT IN NO/LATE DEPLOYMENT OF FRONTAL AIR BAGS AND NON-DEPLOYMENT OF DRIVER'S HIGHER INFLATION LEVELS IN CERTAIN FRONTAL CRASHES.IT SHOULD BE NOTED THAT OCCUPANTS OF VEHICLES INVOLVED IN A CRASH MAY NOT BE AWARE OF LATE OR IMPROPERLY REDUCED LEVEL OF AIR BAG INFLATION AND THUS FIELD REPORTING OF SUCH EVENTS MAY BE SUPPRESSED.THIS PRELIMINARY EVALUATION HAS BEEN UPGRADED TO AN ENGINEERING ANALYSIS (EA06-003) FOR FURTHER INVESTIGATION OF MY 2005 (AND 2006) DC MINIVANS.NOTE: TOTAL NUMBER OF ODI AND MANUFACTURER COMPLAINTS INCLUDES SOME DUPLICATE REPORTS.Read more
