Description of the Problem Vehicle: 2014 Toyota Prius v (Mileage: 70,000) Problem Description: The vehicle is experiencing a critical failure of the Electronically Controlled Braking (ECB) system, specifically the Brake Actuator and Booster Pump assembly. The dashboard currently displays multiple w
... arning indicators: ABS light, Traction Control light, and the Brake System Warning light (Master Warning "!"). Symptoms: Mechanical Failure: The internal high-pressure accumulator is failing to hold pressure. The electric brake booster pump is heard cycling (straining) every 10–20 seconds even when the vehicle is stationary and the brake pedal is not depressed. Loss of Braking Safety Features: The system has disabled regenerative braking and Anti-lock Braking (ABS), causing significantly increased stopping distances and a "wooden," unresponsive brake pedal feel. Predictability of Failure: This failure matches the exact symptoms described in Toyota’s Customer Support Program 20TE07 (and previously ZJB), which acknowledges an internal defect in the brake actuator. Safety Risk: The sudden transition from power-assisted braking to manual hydraulic braking during a drive creates an immediate safety hazard. As the pump motor continues to cycle excessively to compensate for the internal leak, it is at risk of total thermal failure, which would lead to a near-total loss of braking assist while the vehicle is in motion. Additional Notes: Despite the vehicle having only 70,000 miles (well under the 150,000-mile limit acknowledged in Toyota’s support programs), Toyota has declined to provide assistance because the vehicle has surpassed the 10-year age mark. I am filing this report because a primary safety component (brakes) should not experience catastrophic internal failure at 70,000 miles, regardless of the vehicle's age.Read more

I purchased my vehicle 2021 Mercedes-Benz GLE 350 from Mercedes Dealership in Ontario, and had it for only about 4 months before it catching on fire while parked at the doctors office. I purchased it [XXX], and it caught fire on [XXX] while at the doctors. I didn't drive much since I work from home.
... Primary reason for purchase was to have a working and safe vehicle to transport family to their doctor appointments. Mileage at purchase was 59982. Last mileage 60,292. Insurance currently has the vehicle at Copart in Colton while doing their investigation for the claim I filed. I have the police report and fire report as well. At the time of purchase, I asked if there was anything wrong or if any know issues with vehicle, I was reassured no issues. Reviewing the insurance CCC report, I come to find out the vehicle has 3 recalls with possibility of fire incidents. The carfax provided doesn't say that either recall issue was performed as another recall issue with the fuel pump was performed and disclosed. Beyond devastated that I have no vehicle now and that I expected this one to last much much longer. There was no indication of anything wrong with the vehicle, no warning lights nor check engine. Fire was indicated to start in engine compartment on the passengers side by the fire investigator. This incident affected 3 families. As a teacher, I find everyone's life precious and its sickening to know that my life and other lives were in danger. Dear NHTSA, I am writing to formally report a critical safety issue involving our 2020 Ford Explorer that I believe demands immediate investigation and consideration for a recall. While my wife was driving the vehicle on the freeway with our two young children, the transmission began shifting errati
... cally and aggressively at various speeds. The vehicle experienced sudden power loss, delayed gear engagement, and violent jerking motions that nearly resulted in a rear-end collision due to the abrupt deceleration. This malfunction placed my family and others on the road in immediate danger. This is not an isolated incident. I am aware that there is already a Technical Service Bulletin (TSB) issued for this transmission issue, as well as an active class action lawsuit related to the same defect. These facts indicate that Ford is aware of the problem, yet the risk to consumers remains unaddressed through a formal recall. This is a serious safety hazard that must be escalated. I am urging your team to take immediate action to investigate this matter and initiate a recall if warranted. The safety of families like mine should not be compromised by known mechanical failures. Please confirm receipt of this message and advise on the next steps for inspection, documentation, and escalation. I am prepared to provide additional details, including service records and diagnostic reports, if needed. I trust that Ford and the appropriate regulatory bodies will act swiftly and responsibly to protect consumers and uphold safety standards.Read more

Vehicle Make/Model/Year: Ford Transit Passenger Van, 2021 VIN: [XXX] 1. Component/System That Failed: The automatic sliding door on the passenger side of the van has malfunctioned repeatedly. The door does not open properly, often stopping at approximately 6 inches, and frequently fails to close
... fully. In multiple instances, the door has opened while the vehicle was in motion, which presents a serious safety hazard. The affected component is still available and can be inspected upon request. 2. Safety Risk Description: This malfunction puts occupants’ safety at serious risk. If the door opens while driving, passengers (especially children or individuals with limited mobility) are vulnerable to falling out. Additionally, in an emergency situation, Emergency Medical Services (EMS) or other first responders would not be able to easily access the van through the sliding door, delaying urgent medical treatment or evacuation. This could result in preventable injuries or fatalities. 3. Repair History and Reproduction of the Problem: This issue has already been addressed by a Ford-certified dealership approximately six months ago; however, the problem has recurred without warning. The dealership had previously claimed to have fixed the issue, but it is now presenting the same symptoms, suggesting the repair was either temporary or the root problem was not resolved. The problem can be reproduced and is ongoing. 4. Inspections: The vehicle has been inspected and serviced by an authorized Ford dealership, but the repair has not held. To my knowledge, the vehicle has not yet been inspected by the manufacturer, police, or insurance representatives, but I am willing to make the vehicle available for any required inspection. 5. Warning Signs or Messages: There were no warning lamps, messages, or indicators prior to the recurrence of this malfunction. This failure occurred well before reaching the expected life of the vehicle, yet outside the warranty window and having NO symptoms before the sudden failure. After doing some research on Pilot owner online forums, as well as, the NHTSA website, it seems similar complaints have been well reported and a recall has been issued for some vehicles in the same year and model under NHTSA campaign number 23V75100. This instant volume switch to max output with static while driving almost caused an accident as I was distracted trying to turn down my radio as quickly as possible with pain in my ears on the interstate. While stationary even after turning down the radio I still have lingering pain from how loud it was. From what I can tell this is something with the radio unit or amp within the car.Read more

I am submitting this report to document a persistent ticking noise in my 2022 Mazda CX-5, which appears to be the same issue outlined in Mazda Service Bulletin 01-002/21 for previous model years. This issue is commonly associated with Hydraulic Lash Adjusters (HLAs), which maintain proper valve clea
... rance. The noise occurs most prominently during cold starts and resembles the previously reported HLA ticking sound. Owners of earlier CX-5 models have attributed this issue to air bubbles or insufficient oil supply within the HLAs, with cold weather exacerbating the problem due to restricted oil circulation. Despite Mazda's claim that revisions to the engine components have resolved this issue in newer models, my experience suggests it remains unresolved in the 2022 model year. I am concerned about the long-term reliability of these engine components and the potential repair costs if the issue leads to premature wear or failure. While Mazda has not issued a Technical Service Bulletin (TSB) for the 2022 CX-5, multiple owners have reported similar ticking noises online, indicating that the problem may persist. I have a video recording of the noise but am unable to attach it to this submission. If further investigation is warranted, I would be happy to provide the footage upon request. Additionally, similar reports and video evidence from other owners can be found on online forums, including the CX-5 subreddit, where searching for “clicking” will yield multiple examples. I urge the NHTSA to review this issue, as it may indicate an unresolved mechanical defect affecting the reliability of the 2022 Mazda CX-5’s engine. Please advise on any potential investigations or next steps regarding this matter.Read more

My 2017 CR-V (~$80k miles) will randomly fire every system or warning alert on the dashboard. This includes vital or important alerts like Brakes, Check engine light, Tire pressure, etc. There has been no way to reset it, and it persists through car restarts, system reboots, and driving. The first t
... ime this happened was [XXX], and I was able to reset it. However, it has returned a month and a half later ([XXX]) and there has been no remedy or reset that has worked. This issue is extremely dangerous, as it completely mitigates the safety features put in place for this vehicle. With all of the system warnings misfiring and no way to reset or check which are legitimate versus a complication, I am putting myself and others at risk if I drive. I will not know if there is a true issue with a tire, my engine, my brakes... I need my car but am wary of driving it now to keep myself and others safe. It also makes some features inaccessible (i.e., cruise control). I conducted further research prior to taking it to the dealership to better understand the potential cause and what could be done. After conducting that research, it seems this is a VERY common and fairly well-known issue. I have linked one of several forums demonstrating the scope of this issue. Example forum: [XXX] The system misfires are being diagnosed as issues with the fuel injectors, and owners are getting told to replace those fuel injectors to remedy the issue (cost of which equals around $1,400-2,200). Few also report that following installation of new fuel injectors, the issue has come back. Not everyone has ~$2k to fix an issue that is so common it should have already been addressed by Honda. NHTSA Safety Investigations, CHRYSLER TOWN AND COUNTRY
Investigations opened by NHTSA into potential safety defects. These are separate from recalls and may or may not result in a recall.
PE, Closed
, Model Year: 2016
Sliding Doors Inoperative
The Office of Defects Investigation (ODI) opened PE21-016 on July 7, 2021, to investigate 364 complaints alleging that one or both sliding doors on 2016 Model Year (MY) Grand Caravan or Town and Country vehicles could not be opened. The door(s) could not be opened using the mechanical handle or elec
... tronic remote switches. Many of the complaints described having to remove passengers seated in the rear of vehicles through the front doors, the second sliding door (if it was operational), the window, or the car?s rear hatch. The complainants have expressed concerns that if the sliding door(s) cannot be opened in the event of an emergency or crash, it could trap passengers or delay their egress. On June 4, FCA published TSB# 23-017-20 instructing dealers to replace the door lock actuator instead of the entire door latch assembly on the subject vehicles when consumers complained about either one or both of the sliding door locks not functioning and/or emitting a loud buzzing noise during lock/unlock operation. On June 16, 2021, FCA approved a Quality Warranty Extension for fifteen years/150,000 miles from the vehicle's in-service date for sliding door lock actuators on vehicles built between May 1, 2015, through January 9, 2017. FCA has started notifying owners about the new program. FCA will provide reimbursement for owners who previously paid for sliding door repairs.Consumers whose sliding doors are inoperative will have the opportunity to have their vehicle diagnosed, and if a fault is identified, have the sliding door actuator repaired at no charge.Analysis of the FCA IR response and ODI complaint data found that the majority (96%) of sliding door failures occurred prior to the calendar year 2020 when the subject vehicles had approximately 4 years in service. At the time of the closing of this investigation, 99% of the subject vehicles are passed the age where failures typically occur. The number of new reports has been decreasing since 2020 and further reduced following the FCA Warranty Extension program. ODI found that prior to a failure of the sliding door(s) there is normally a loud buzzing noise/sound generated that alerts the operator to the actuator starting to wear out.ODI has not identified any crashes or injury allegations due to the sliding door being inoperable on the subject vehicles following nearly six years of exposure. ODI will continue to monitor complaints received for the incidence of sliding doors becoming inoperative.Accordingly, the investigation is closed. The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist, and the agency reserves the right to take additional action if warranted by new circumstances. For a complete summary of this investigation and related consumer complaints, please see attached closing resume document.Read more

Interior Vehicle Fire
On August 6th, 2020, the Office of Defects Investigation (ODI) opened this Preliminary Evaluation (PE20-012) to investigate alleged thermal events, and/or vehicle fires originating at the charge hub in 2014 Chrysler Town and Country minivans manufactured by Fiat Chrysler Automobiles (FCA). While the
... investigation was focused on the 2014 Chrysler Town and Country vehicles, the scope of the investigation was expanded to include the 2013-2020 Chrysler Town and Country, and Dodge Grand Caravan vehicles (the RT Platform).During the investigation ODI sent two separate Information Request letters to the manufacturer. One on August 17th, 2020 and one on March 31st, 2021.ODI identified a subject 2014 Chrysler Town and Country van on a vehicle salvage website that appeared to have experienced a thermal event where the charge hub is located. With the help of the NHTSA's Vehicle Research and Test Center (VRTC), ODI purchased the vehicle from the salvage company to inspect the damage and determine if more information about the failure could be obtained. A joint inspection with the manufacturer was conducted. Following this comprehensive inspection, a cause of the fire could not be determined.FCA initiated a parts collection initiative to obtain parts from the field for testing. The test results from the FCA field parts collection were inconclusive as they could not identify a common cause for the alleged thermal events. FCA looked at multiple liquid solutions that could have entered the charge hub circuit and caused it to short out, potentially leading to a fire. FCA also performed testing where the charge hub circuit was shorted out using a wire to observe if a flame or fire was produced. FCA provided their assessment to ODI concluding that none of the testing methods performed lead to a flame or fire being propagated.Given the low rate of thermal incidents at the subject vehicle charge hub, no reports of serious injury, zero reports of crash or death, and a lack of common failure mode, this investigation is closed. The closing of this investigation does not constitute a finding by NHTSA that no safety-related defect exists. The agency reserves the right to take further action if warranted by the circumstances.Read more

Totally Integrated Power Module Failure
In a letter dated August 21, 2014, the Center for Auto Safety (CAS) petitioned the National Highway Traffic Safety Administration (NHTSA) to initiate a defect investigation of alleged failures associated with the Totally Integrated Power Module (TIPM) installed in Chrysler SUV?s, trucks, and vans be
... ginning in the 2007 model year. The petitioner alleges that TIPM defects may result in engine stall, airbag non-deployment, failure of fuel pump shutoff resulting in unintended acceleration, fire, and other symptoms. On September 25, 2014, the Office of Defects Investigation (ODI) opened DP14-004 to evaluate the petition for a grant or deny decision. ODI analyzed complaint data provided by CAS as well as complaints submitted to ODI from consumers. In total, there were 296 complaints submitted by the petitioner in the original petition and five supplements, including 271 complaints related to the subject vehicles equipped with TIPM-7. Approximately 3 percent of CAS complaints are related to vehicles equipped with TIPM-6 and ODI?s review of these complaints did not identify any safety defect trends. ODI conducted a detailed review of complaints narratives submitted by CAS and consumers including careful analysis of vehicle repair histories, warranty claims obtained from the manufacturer and any available Customer Assistance Inquiry reports (CAIR). Analysis of the field data indicated that MY2011-2013 Jeep Grand Cherokee and Dodge Durango vehicles exhibited significantly higher complaint rates related to fuel pump relay (FPR) failures than other subject vehicles. In a September 3, 2014 letter to NHTSA, Chrysler submitted a Defect Information Report (DIR) identifying a defect in the FPR within the TIPM-7 which can result in a no start or stall condition in approximately 188,723 model year (MY) 2011 Jeep Grand Cherokee (WK) and Dodge Durango (WD) vehicles manufactured from January 5, 2010 through July 20, 2011 (14V-530).In a February 24, 2015 letter, Chrysler submitted a second DIR expanding the scope of the FPR defect condition to include an additional 338,216 MY 2012 through 2013 Jeep Grand Cherokee vehicles manufactured from September 17, 2010 through August 19, 2013 and MY 2012 through 2013 Dodge Durango vehicles manufactured from January 18, 2011 through August 19, 2013 (15V-115).ODI analysis of the CAS allegations of TIPM defects resulting in stall while driving, airbag non-deployment, unintended acceleration, fire and other faults identified a single defect condition related to 1 of over 60 different circuits in the TIPM assembly. The most common effect of this defect condition, related to the fuel pump relay, was a no-start concern, but it could also result in stall while driving (76 complaints). No valid evidence was presented in support of claims related to airbag non-deployment, unintended acceleration or fire resulting from TIPM faults and these claims were found to be wholly without merit based on review of the field data and design of the relevant systems and components.Except insofar as the petitioner's contentions relate to the defect condition addressed by the Chrysler recalls, the factual bases of the petitioner's contentions that any further investigation is necessary are unsupported. In our view, additional investigation is unlikely to result in a finding that a defect related to motor vehicle safety exists or a NHTSA order for the notification and remedy of a safety-related defect as alleged by the petitioner at the conclusion of the requested investigation. Therefore, in view of the need to allocate and prioritize NHTSA's limited resources to best accomplish the agency's safety mission, the petition is denRead more

Engine Stall After Refueling
This defect petition evaluates July 7, 2014 allegations from an owner of a model year (MY) 2007 Dodge Grand Caravan minivan that engine stall occurs in these vehicles without warning after refueling. The petitioner's vehicle is a Chrysler RS platform minivan. The RS platform includes MY 2003 through 2
... 007 Dodge Grand Caravan, Dodge Caravan, Chrysler Town and Country and Chrysler Voyager minivans.NHTSA evaluated the petition by analyzing consumer complaints submitted to the Agency, analyzing field data, reviewing technical information provided in Chrysler's response to an Agency information request and testing an RS minivan identified by its owner as experiencing post-refuel engine stalls.After completing this evaluation, NHTSA has concluded that further investigation is unlikely to result in a finding that a defect related to motor vehicle safety exists.The stalls occurring in these vehicles only occur immediately after filling up the fuel tank and happen at low engine speeds. The failure rate is low, even after 8 to 13 years of the vehicles being in service, and there are no reports of crashes or injuries resulting from the stall condition.In the Agency's view, additional investigation is unlikely to result in finding the existence of a safety-related defect in the subject vehicles.Given the need to allocate and prioritize NHTSA's limited resources to best accomplish the Agency's safety mission, the petition is denied.This action does not constitute a finding by NHTSA that a safety-related defect does not exist. The Agency will take further action if warranted by future circumstances. More details concerning our review can be found in the Federal Register Notice for this action.The following 161 VOQs are included in the petition evaluation: 10209242, 10229468, 10240708, 10260923, 10275939, 10276532, 10296487, 10298175, 10328669, 10330144, 10346517, 10349789, 10354758, 10363689, 10372112, 10372368, 10372514, 10378031, 10379764, 10382938, 10387448, 10394063, 10398877, 10413918, 10418652, 10420924, 10427063, 10437525, 10445392, 10446902, 10470028, 10471249, 10473733, 10502231, 10503114, 10504260, 10509862, 10511988, 10512120, 10512385, 10512650, 10513147, 10538154, 10549841, 10551763, 10555188, 10555418, 10556754, 10565367, 10574012, 10574398, 10586555, 10587212, 10587559, 10593433, 10607841, 10607913, 10608922, 10615810, 10622468, 10628141, 10632112, 10632128, 10632172, 10632175, 10632231, 10632262, 10632266, 10632270, 10632305, 10632316, 10632326, 10632370, 10632385, 10632399, 10632492, 10632542, 10632765, 10632833, 10632928, 10633197, 10633281, 10633409, 10633421, 10633483, 10633507, 10637752, 10637804, 10637950, 10637975, 10638032, 10638289, 10638290, 10638401, 10638968, 10639218, 10639623, 10641157, 10641603, 10641614, 10641836, 10644339, 10644705, 10649713, 10649895, 10649932, 10650313, 10652590, 10653147, 10653783, 10654533, 10655226, 10658900, 10659679, 10660320, 10662588, 10662645, 10662662, 10663473, 10667056, 10667437, 10668081, 10668094, 10668131, 10668612, 10668969, 10668970, 10669584, 10669678, 10669729, 10669775, 10669866, 10669895, 10669992, 10670422, 10670506, 10676348, 10676732, 10677120, 10679795, 10679934, 10680057, 10680290, 10680342, 10680714, 10681512, 10682584, 10682906, 10683022, 10683114, 10683549, 10683620, 10683697, 10683743, 10689898, 10690761, 10691284, 10691556, 10691751, 10693061, 10694300Read more

Air Bag Non-Deployment/Engine Stall
In March 2011 Chrysler submitted a defect information report for NHTSA Recall No. 11V139 declaring a defect condition that could cause the ignition key to inadvertently move from the RUN to the ACC position on model year (MY) 2010 Dodge Journey, Grand Caravan, and Chrysler Town and Country. The repor
... t identified engine stalling as the defect consequence and cited customer complaints and warranty claims, as well as two rear-end collisions, as leading to the recall action.Chrysler determined that a defective WIN module, which is effectively the equivalent of the ignition switch, was the cause of the inadvertent key rotation, and that a new design WIN module would be installed as a remedy. In Spring 2014 ODI conducted outreach to major light vehicle manufacturers regarding ignition key position and its effect on air bag system availability. The outreach was conducted in connection with NHTSA Recall No. 14V047, a recent recall involving inadvertent ignition key rotation and subsequent air bag disablement in certain GM products.During discussions held in April 2014, Chrysler advised ODI that Chrysler air bag systems of this vintage, including the subject vehicles, revert to energy reserve when the ignition key moves to the ACC or OFF position, and that under energy reserve the air bags would only be available for a short period, approximately two tenths of a second (ODI notes this consequence was not identified in the 11V139 recall report).A review of the ODI consumer database conducted after the April 2014 discussions identified complaints alleging that MY 2008-2009 vehicles may have a similar defect as the MY 2010 vehicles, and that some MY 2010 consumers reported experiencing ignition key position concerns after the 11V139 recall remedy was applied.Accordingly this investigation was opened. In response to ODI?s information request letter, Chrysler reported that MY 2008-2009 subject vehicles used the same design WIN module as the recalled vehicles, and also identified 66 consumer reports and one non-injury crash incident potentially related to inadvertent ignition key rotation.Chrysler?s review of the MY 2010 remedy used in 11V139, which involved installation of a Trim Ring as opposed to replacement of the WIN module, showed the remedy did not adequately address ignition key position concerns, and in some use-cases could cause other systems to operate improperly (radio, instrument panel lighting, HVAC fan motor, etc.).Chrysler subsequently filed multiple defect information reports to 1) add the MY 2008-2009 Dodge Grand Caravan and Chrysler Town and Country, and the MY 2009 Dodge Journey to the original recall scope, and 2) stating that the complete WIN module would be replaced with a new design component in all recalled vehicles (including those that had the Trim Ring installed during 11V139).Chrysler?s report also advises of constrained remedy parts availability, see NHTSA Recall No. 14V373 for further details.Accordingly, the investigation is closed based on the recall action Chrysler is undertaking. The ODI reports cited above can be reviewed online at http://www-odi.nhtsa.dot.gov/owners/SearchNHTSAID under the following identification numbers: 10584873, 10583399, 10573295, 10566365, 10546805, 10513060, 10509284, 10503121, 10496599, 10475027, 10456099, 10450629, 10443680, 10422766, 10410447, 10409269, 10402947, 10402751, 10402660, 10396943, 10394957, 10356632, 10315972Read more

Loss of headlight illumination
The subject vehicle (SV) population includes MY 2005-2006 vehicles built between March 2004 and December 2005 manufactured with a TRW supplied headlight switch assembly (HSA).The HSA was the primary focus of the investigation; earlier and later vehicles utilized different HSAs.Consumers complain tha
... t the headlights in the subject vehicles do not illuminate when the HSA is turned on, or that the headlights extinguish while on (subject failure). No other driving lights are affected. The front marker, tail, and instrument lamps operate. Also, the high beams can be illuminated by holding the turn signal stalk in the up position. As reported by consumers, turning the HSA off and on typically corrects the failure.Initially the failures occur infrequently but they become progressively more frequent with age.Chrysler has indicated its intention to offer of an extended lifetime warranty on the HSA, so that consumers with this problem can have their HSA replaced at no charge when they begin experiencing the subject failure.Commencing sometime in early 2013, Chrysler will send letters to SV owners notifying them of the lifetime warranty. The SV headlight system design is essentially the same as that described in EA 05-009, which involved headlight anomalies in earlier model Chrysler minivans; refer to it to for additional information. The subject failure is caused by friction between the plastic cam and metal surfaces of the electrical contacts in the HSA, which results in the production of plastic particles (debris).Analysis shows that debris can collect on the electrical contact that controls the headlights, resulting in an open circuit and preventing the headlights from turning on.Subsequent operation of the HSA may temporarily displace the debris, at which time the headlights operate correctly.Lubricant was applied to the plastic cam beginning in December 2005, reducing debris production.Consequently, subsequent failure levels were lower. The report counts shown above include only those that involve the subject failure.Reports involving other failure modes which were identified have not been included either because they were not safety related, or if they were safety related, because there was no defect trend. The single crash event listed in the Failure Report Summary Table above involved a deer strike allegedly due to loss of headlights in February 2006.No injuries were reported in this crash. An April 2004 non-crash incident (?Other? above) involved a momentary loss of headlights. The consumer drove into a ditch, and one minor injury was alleged. The subject vehicles experienced a 2.2% warranty claim rate. While standing alone this would appear to be significant, a portion of these warranty claims likely involve other problems with the HSA unrelated to the subject failure. In view of the on road experience of drivers in rapidly restoring headlight illumination by turning the headlight switch off and on again, the rate and particularly the recent trend in problems with the switch, and Chrysler?s extended lifetime warranty coupled with the owner notification of that extended warranty, further investigation does not appear to be warranted.Accordingly, the investigation is closed. The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist, and the agency reserves the right to take additional action if warranted by new circumstances.Details on Chrysler?s lifetime HSA warranty extension for the SV population will be available in the public file for this investigation.Read more

Loss of Headlight Illumination
To date, ODI has interviewed several complainants who confirmed that the headlights fully extinguish, and remain extinguished, on an intermittent basis. In most cases this occurred while the vehicle was in motion. The occurrences do not show a repeatable pattern, and the problem occurs at a low freque
... ncy, typically one or more weeks between failures. For this reason most complainants have not attempted diagnosis or repair of the problem, so the cause is unknown, and the problem continues to occur. In the cases where repairs have been attempted, which consisted mainly of headlight switch and body control module replacements, mixed success is reported. In interviews complainants advised ODI that they can usually toggle the headlight switch, or turn on the high beams by actuating the column stalk (pulling it upward), when the headlights unexpectedly turn off.Complainants report that toggling the switch is usually effective, and the high beams have always been available when the column is used.During the course of the interviews ODI was able to obtain a few suspect headlamp switches for further evaluation. No verifiable crashes or injuries have been identified in the vehicle owner complaints (VOQs).Chrysler notes that its complaints also include allegations of dimming or flickering of the headlights in addition to loss of headlamps, and that their analysis of available data indicates a declining trend occurring in the field.Chrysler data reports two alleged crash incidents; one where a consumers vehicle lost forward lighting and struck a deer, and one where a consumer alleged their lighting stopped working, causing them to depart the roadway and drive into a ditch.A minor injury is alleged in this second incident. Both incidents occurred several years ago, and neither allegation has been confirmed by ODI.ODI is upgrading this investigation to an Engineering Analysis (EA11-010) to continue its analysis of the alleged defect and its consequences, which will include additional analysis of consumer reports, their frequency and trend, and Chrysler's technical information for the headlight system and its components.Additionally testing will be conducted to evaluate possible root cause(s) of the alleged defect, and will include analysis of suspect components collected from the field, and/or assessment of subject vehicles that have reportedly experienced the condition.Read more

OVERHEATING OF SEAT HEATERS
ODI OPENED THIS INVESTIGATION BASED ON 24 COMPLAINTS ALLEGING THAT THE ELECTRICALLY HEATED DRIVER OR PASSENGER SEAT SPARKS/OVERHEATS.DAIMLERCHRYSLER BELIEVES THE FAILURE IS DUE TO ONE OF TWO FACTORS:THE STITCHING OPERATION ON THE SEAT COVER MAY CAUSE THE SEWING NEEDLE TO SEVER SOME STRANDS OF THE WI
... RE HEATING GRID IN THE SEAT, OR A POOR SOLDER JOINT/WIRE CONNECTOR FAILS DUE TO IMPROPER PROCESSING OR LONG TERM OVERSTRESSING.THE IGNITION SWITCH AND THE SEAT HEATER SWITCH MUST BOTH BE TURNED ON TO POWER THE SEAT HEATER.COMPLAINANTS REPORT THAT WHEN OVERHEATING OCCURS, SWITCHING THE HEATER OFF ALSO STOPS THE OVERHEATING.THE FAILURE CANNOT OCCUR IN AN UNATTENDED VEHICLE WITH THE IGNITION SWITCH OFF.ODI FINDS THAT THE 33 FIRE ALLEGATIONS ARE REPORTING VERY LOCALIZED OVERHEATING(PHOTOS OF DAMAGED SEATS SHOW HOLES THE SIZE OF A PENCIL OR LESS).THERE ARE NO REPORTS OF SUSTAINED COMBUSTION AND NO REPORTS OF OTHER FIRE DAMAGE TO THE VEHICLE.THE INJURY ALLEGATIONS ARE REPORTING MINOR INJURY.NO CONSUMERS REQUIRED MEDICAL TREATMENT (PHYSICIAN CARE).THERE ARE NO ALLEGATIONS OF LOSS OF VEHICLE CONTROL DUE TO FAILURE OF A HEATED SEAT.A SAFETY-RELATED DEFECT HAS NOT BEEN IDENTIFIED AT THIS TIME AND FURTHER USE OF AGENCY RESOURCES DOES NOT APPEAR TO BE WARRANTED.ACCORDINGLY THIS INVESTIGATION IS CLOSED.THE CLOSING OF THIS INVESTIGATION DOES NOT CONSTITUTE A FINDING BY NHTSA THAT NO SAFETY-RELATED DEFECT EXISTS.THE AGENCY RESERVES THE RIGHT TO TAKE FURTHER ACTION IF WARRANTED BY THE CIRCUMSTANCES.Read more

FUEL TANK LEAKS
THIS ENGINEERING ANALYSIS WAS OPENED ON JANUARY 31, 1997 BASED ON INFORMATION GATHERED DURING PRELIMINARY EVALUATION (PE), PE96-051. THE PE WAS OPENED ON AUGUST 19, 1996, TO INVESTIGATE ALLEGATIONS OF FUEL LEAKAGE FROM THE AREA WHERE THE FUEL PUMP/SENDING UNIT JOINS THE FUEL TANK.FOR BREVIDY, THIS A
... REA WILL BE REFERRED TO AS THE PUMP/TANK INTERFACE.THESE ALLEGATIONS WERE FIRST RECEIVED BY ODI ON AUGUST 2, 1996 WHILE INVESTIGATING ALLEGATIONS OF CHRYSLER MINIVAN THROTTLE SHAFT HAND-UP, AND UNCONTROLLED ENGINE SPEED, IN PE96-036.WHILE DISCUSSING THAT ISSUE WITH A FLEET, IT WAS ALLEGED THAT AT LEAST 10(OUT OF 40) 1996 EARLY PRODUCTION CHRYSLER MINIVANS LEAKED FUEL FROM THE PUMP/TANK INTERFACE.ANALYSIS OF THE COMPLAINT AND WARRANTY DATA INDICATE FUEL TANK LEAKAGE INCIDENTS IN THE SUBJECT VEHICLES PREDOMINATLY AFFECTED EARLY PRODUCTION VEHICLES.THE DATA INVOLVING CONFIRMED INCIDENTS OF TANK/PUMP INTERFACE LEAKS SUGGEST THAT THOSE FAILURES HAVE BEEN DECLINING SINCE JULY 1996.ODI HAS RECEIVED NO REPORTS OF THE SUBJECT CONDITION INVOLVING ORIGINAL EQUIPMENT PUMPS AND TANKS SINCE SEPTEMBER, 1997.CURRENTLY THIS ISSSUE DOES NOT REQRESENT A CONTINUING FIELD CONCERN.ODI WILL CONTINUE TO MONITOR THE SUBJECT VEHICLES FOR FUEL LEAKS OF ANY TYPE.A SAFETY RELATED DEFECT TREND HAS NOT BEEN IDENTIFIED AT THIS TIME AND FURTHER USE OF AGENCY RESOURED DOES NOT APPEAR TO BE WARRANTED.THE CLOSING OF THIS INVESTIGATION DOES NOT CONSTITUTE A FINDING BY NHTSA THAT NO SAFETY RELATED DEFECT EXISTS.THE AGENCY RESERVES THE RIGHT TO TAKE FURTHER ACTION IF WARRANTED BY THE CIRCUMSTANCES.Read more

SLIDING DOOR LATCH FAILURE
CHRYSLER MAINTAINS THAT THE COMMON FACTOR CONTRIBUTING TO KNOWN SLIDING DOOR OPENING INCIDENTS IS PRIOR INCORRECT DEALER LATCH ADJUSTMENT.CHRYSLER ATTRIBUTES THIS PROBLEM TO AN INACCURATE SERVICE MANUAL PROCEDURE - SLIDING DOOR LATCH/LOCK CONTROL, REMOVAL AND INSTALLATION.CHRYSLER FURNISHED INFORMAT

STEERING WHEEL SEPARATION
AFTER AN EXTENSIVE INVESTIGATION AND ANALYSIS, CHRYSLER HAS IDENTIFIED THE DEFECT AS A FATIGUE FAILURE, ORIGINATION IN THE HEAT AFFECTED REGION OF THE PROJECTION WELDS OF THE STEERING WHEEL HUB TO STEERING WHEEL ARMATURE ATTACHMENT.THE HUB IS THE PORTION OF THE STEERING WHEEL ASSEMBLY THAT CONNECTS
... IT TO THE STEERING COLUMN SHAFT AND IS ATTACHED TO THE UNDERSIDE OF THE STEERING WHEEL ASSEMBLY BY 4 WELDS.IF CRACKS DEVELOP IN THE HEAT AFFECTED REGION OF THE WELDS AND PROPAGATE THROUGH THE STEERING WHEEL ARMATURE TO EACH OF THE WELDS, THE STEERING WHEEL CAN SEPARATE FROM THE HUB.THIS IS A SLOW PROGRESSIVE FAILURE WHICH NORMALLY OCCURS AT HIGH MILEAGE.IN SOME CASES COMPLAINANTS HAVE REPORTED A LOOSENESS OR WOBBLING OF THE STEERING WHEEL PRIOR TO SEPARATION.TO REMEDY THIS DEFECT, CHRYSLER WILL INSPECT THE STEERING WHEELS OF THE AFFECTED VEHICLES FOR CRACKS IN THE HUB AREA.IF ANY CRACKS ARE FOUND, THE STEERING WHEEL WILL BE REPLACED.ALL OTHER VEHICLES WILL HAVE REINFORCING PLATES INSTALLED WHICH WOULD PREVENT A STEERING WHEEL SEPARATION SHOULD CRACKING OCCUR.Read more

LIFTGATE LATCHES
LIFTGATE LATCHES IN THE SUBJECT VEHICLES HAVE FAILED IN LOW AND MODERATE SPEED IMPACTS RESULTING IN OCCUPANT EJECTIONS, INJURIES, AND FATALITIES.THERE ARE TWO FAILURE MODES ASSOCIATED WITH LATCH FAILURE: FORK BOLT-DETENT LEVER BYPASS, AND INERTIAL UNLATCHING.NHTSA ANALYSES OF CHRYSLER AND PEER MINIV
... ANS TOOK THE FORM OF STATIC AND DYNAMIC CRASH TESTING, ENGINEERING EVALUATIONS, COMPLAINT AND FIELD ANALYSES, AND REAL WORLD DATA ANALYSES.A TECHNICAL REPORT DISCUSSING THESE ANALYSES CAN BE FOUND IN THE ENGINEERING ANALYSIS TECHNICAL REPORT IN THE PUBLIC VIEWING FILE.CHRYSLER HAS ADVISED NHTSA THAT IT WILL CONDUCT A CAMPAIGN TO REPLACE THE LATCHES ON ALL THE SUBJECT VEHICLES.THE CHRYSLER CAMPAIGN WILL CONSIST OF NOTIFICATION LETTERS TO ALL OWNERS; FREE REPLACEMENT OF ALL LATCHES WITH STRONGER, SAFER LATCHES REGARDLESS OF THE AGE OF THE MINIVANS; NATIONAL ADVERTISING TO PROMOTE CUSTOMER AWARENESS OF THE CAMPAIGN; QUARTERLY REPORTS REGARDING CAMPAIGN COMPLETION RATES; AS WELL AS ADDITIONAL MEASURES TO ENSURE A HIGH COMPLETION RATE, ABOVE AND BEYOND THE ACTIONS THAT NORMALLY WOULD BE TAKEN IN A FORMAL SAFETY RECALL.THESE INCLUDE FOLLOW UP WRITTEN AND TELEPHONIC NOTIFICATIONS TO OWNERS WHO DO NOT BRING THEIR VEHICLES IN FOR CORRECTIVE ACTION.NHTSA FIRMLY BELIEVES THAT THE CRASH PERFORMANCE OF THE LATCHES ON THESE MINIVANS RAISES SERIOUS SAFETY CONCERNS, AND THAT THE CAMPAIGN ADDRESSES THESE CONCERNS.THE CHRYSLER CORRECTIVE ACTION WILL INCREASE THE STRENGTH OF THE LIFTGATE LATCHES AND DECREASE THE POTENTIAL FOR INERTIAL UNLATCHING, THEREBY REDUCING THE RISK OF CRASH-RELATED OPENINGS OF THE LIFTGATE AND POSSIBLE OCCUPANT EJECTION THROUGH THE LIFTGATE OPENING.OWNERS OF CHRYSLER MINIVANS WILL GET A STRONGER, SAFER LATCH FREE OF CHARGE AT THE EARLIEST POSSIBLE TIME, WITHOUT THE NEED FOR FURTHER TECHNICAL INVESTIGATION AND COMPLEX AND TIME-CONSUMING LITIGATION WITH AN UNCERTAIN OUTCOME.BASED ON THE ABOVE, ODI IS CLOSING THIS INVESTIGATION.Read more
