The vehicle has experienced a pattern of recurring electrical failures beginning shortly after purchase, involving driver door controls (window, lock, mirror, and interior lighting functions), which have required four separate repair attempts over approximately three years, including replacement of
... a door module, a wiring harness, and a door switch assembly. Despite these repairs, the same driver door electrical failure has recurred multiple times. The vehicle has also experienced a seat memory module failure (module was replaced), and a persistent rear hatch malfunction that has never been resolved. The most serious concern is that the vehicle has, on multiple occasions, unexpectedly shut down while being driven, including instrument cluster and infotainment screens going dark, occurring when the automatic engine stop/start feature engages at intersections. The vehicle has restarted on its own after acceleration in some instances. Power steering assist has also been affected during at least one of these events. Most recently, the vehicle failed to start at all on one occasion, requiring roadside assistance to jump the battery; the dealer diagnosed unstable battery voltage but did not identify a root cause connecting this event to the prior shutdown incidents. This creates a significant safety risk due to potential loss of vehicle control or loss of power while operating in traffic. The problem has been reproduced and confirmed by the dealer on multiple occasions (door module and wiring harness failures were diagnosed and repaired), though the shutdown-while-driving events specifically have not been duplicated by the dealer despite multi-day testing. The vehicle has been inspected exclusively by the authorized dealer; it has not been inspected by police or independent parties. Warning symptoms (flickering door lights, dead door controls) preceded the shutdown/stall issue by roughly a year before the stalling symptom began.Read more

The contact stated that the vehicle was previously taken to the dealer for a recall repair under NHTSA Campaign Number: 26V051000 (Suspension), but the dealer informed the contact that the vehicle passed inspection and no repairs were needed. ... dated June 4th 2026. Three times people walked right behind my car when I was backing up. I almost hit them! I decided to video tape it. The back up camera also doesn't always work and did not go off when I was backing into the curb until I hit it without an alarm going off. The lighting alert system was also messed up because the light was on green until I almost hit the curb. This also happened on June 4th and I have a video. I could have hit a small animal or child or damaged my car. I have brought this to the attention of my Subaru Service department and they claim they can not replicate this problem.This is of great concern for me and I have filed a Lemon Law Case as I am too fearful I will hurt someone or myself because of these issues. In addition to this, my headlights are aimed down at the ground and I live on curvy country roads. Hard to see at night. My service department said Subaru of America would have to approve the adjustment. They never got back to me on this issue. The tech specialist told the service department manager that I had electrical problems with my vehicle as she witnessed the HVAC working improperly. My radio, navigations, phone and sound systems also cause distracted driving. The Radio would blast on high volume randomly . The navigation, phone and radio volume would be blasting without being able to turn this down. There is no way to turn off the radio. If you do, the whole navigations screen won't work. My navigation system is not functioning properly and has blacked out . Subaru has seen my vehicle 6 times for these issues . They claim they can't replicate the problem. My car is sitting in the garage. It is too unsafe to drive.Read more

Tesla Corte Madera submitted a buyback request on my behalf after witnessing all these issues, but Tesla corporate denied it. ... from 2011 through 2019 were recalled for engine problems including excessive oil consumption and rod bearing failure possibly resulting in engine fires. If engines were not replaced an extended warranty was given for 150,000 miles or 15 years. Hyundai will not remedy our excess oil consumption problem locally or nationally. An oil consumption test was done at the Hyundai dealer. Conclusion: by dealer and national level office: it's not bad enough to remedy. There were no engine warning lights activated because we check the oil regularly and add as needed. This post-2019 2.4 liter engine continues to be a hazard as some owners from previous year's models had engine seizures in traffic which is dangerous. Some owners reported engine fires. We have not experienced those tragedies but possibly post-2019 Tucson owners should be alerted to potential dangers.Read more

... peration. The most serious event occurred while towing an approximately 15,000-pound loaded dump trailer on a public county road. While climbing a hill, the truck entered reduced-power/limp mode associated with the emissions/DPF system. Power was reduced so severely that the truck could no longer continue uphill and came to a stop in the travel lane. This created a safety hazard because the road was narrow and winding, with no safe place to pull off. I had to back the truck and loaded trailer several hundred yards downhill while traffic was present behind me. Before the event, the truck displayed “Continue Driving, Cleaning Exhaust Filter.” The warning did not state that severe power reduction or loss of towing capability was imminent. I did continue operating the vehicle, but the truck was being used for normal commercial towing/work operations where immediate highway-speed driving was not practical. The issue has been confirmed by Chevrolet dealers through DTC P2463 on multiple visits. The only correction performed has been forced regeneration. I was advised that no failed component was identified and no permanent repair has been performed. The vehicle is available for inspection, and repair records are available. My concern is that the emissions-related derate strategy can cause sudden and severe loss of motive power while towing on public roads, leaving the vehicle unable to maintain forward movement or safely clear the travel lane. This has occurred repeatedly during normal work use, not as a one-time event, and the warning provided did not give a clear indication that the truck could soon become unable to tow or maintain forward movement.Read more

The most egregious of cases have been when driving through Texas, New Mexico, and Arizona where the speed limit is often 75 mph on Interstate highways (e.g., [XXX]). On multiple occasions the vehicle has ignored construction speed limit changes to 55 mph, and instead has tried to maintain 75-80 mph through the construction zone. Not only could that get me a nasty speeding ticket, but it could also put construction workers lives at risk. It has also happened in non-construction zones, like when on a non-interstate highway that passes through towns and reduces speed when doing so. It doesn’t fail 100% of the time, but it has been an issue more than 50% of the time I enter a construction zone. The Around View Monitor (AVM) system failed. The rear backup camera image no longer functions properly due to a failed AVM control unit. The vehicle is a 2023 Nissan Pathfinder Platinum equipped with Nissan’s camera-based surround view and backup camera system. The vehicle has been diagnosed by an
... authorized Nissan dealership, and the failed component remains installed in the vehicle and is available for inspection. The failure disabled the vehicle’s backup camera and related camera-based visibility systems. This significantly reduces rearward visibility while backing up and increases the risk of striking pedestrians, children, pets, vehicles, or objects that may not be visible through mirrors alone. Drivers have come to rely on the backup camera system as an important safety feature, and its sudden failure creates a safety hazard during parking and low-speed maneuvers. The problem was diagnosed and confirmed by an authorized Nissan dealership. The dealership determined that the Around View Monitor (AVM) control unit had failed and advised that the module requires replacement. The vehicle has not been inspected by police or insurance representatives. Nissan corporate has not yet physically inspected the vehicle to my knowledge. The backup camera system functioned normally until it suddenly stopped working. There were no warning lights, messages, prior malfunctions, collisions, or damage that would have indicated a problem. The failure occurred without warning. I purchased this 2023 Nissan Pathfinder only one month ago, and the AVM system suddenly failed after just a few weeks of ownership. The dealership diagnosed a failed AVM control unit and quoted approximately $3,400 ($2800 parts and $600 labor) for repair. A critical rear-visibility safety system should not fail unexpectedly in a vehicle this new. The repair cost is prohibitive, leaving us owners without an important safety feature. ... ion on the 2026 Lexus TX500h would shudder during acceleration or deacceleration. I noticed it happening around 25-40mph mostly and it has happened more than once. I have demonstrated the issue to the dealer on Wed, May 20th around 10:15am Pacific but they didn't know how to address the issue. I did a ride along with the service tech from Lexus of Pleasanton and it happened again during acceleration. The dealer noted that there were no error code when they scanned the system and no observed leaks. I have a video clip but was unable to upload it. The issue with transmission shift thump when this issue happen feels like the transmission locked up. It would shake the vehicle and lose power while accelerating before it starts to gain power back in 1-2 seconds. This causes a safety issue since that pause in acceleration thus this can be dangerous while merging onto a freeway or areas with lots of traffic. I noticed the same issue on some occasions, it would shake during deceleration as well. This issue is intermittent. Maybe Lexus/Toyota can address this with the transmission shifting update. IMO, It's a safety issue that needs to be looked and fixed by Lexus/Toyota. Dealer asked me to come back only. DTN Air Bag Inflator Rupture
The Office of Defects Investigation (ODI) has become aware of eight vehicle crashes in which a rupture of a Jilin Province Detiannuo Automobile Safety System Co., Ltd. (DTN) air bag inflator occurred during the deployment of the driver side air bag. All eight drivers in these crashes sustained serio
... us or fatal injuries that appear linked to the ruptures. Six of those drivers sustained fatal injuries and two sustained severe injuries. In all eight incidents, the subject inflators had been installed as replacement equipment after the vehicle was involved in a previous crash. Information gathered during NHTSA’s initial analysis of these incidents indicates that the inflators were potentially imported illegally into the United States. In conjunction with this safety investigation, NHTSA is also working with appropriate authorities to address any illegal activities associated with the importation of these inflators. As a result of the significant safety concerns relating to these inflators, as well as extensive information already obtained by ODI relating to their field ruptures, ODI is moving directly to the Engineering Analysis (EA) stage of its investigative process rather than first opening a Preliminary Evaluation. NHTSA has taken a variety of actions to address the risks posed by the importation of substandard or counterfeit air bag components. However, until recently, these risks appeared spread across a variety of components and manufacturers. As part of ODI’s continuous monitoring of this issue, the agency has obtained information to suggest that at least eight incidents involve ruptures of inflators manufactured by DTN. In parallel with this dedicated investigation into DTN inflators, ODI will continue to monitor reports of field incidents to evaluate risks from other substandard imported air bag components. In 2012, ODI first became aware of substandard or counterfeit air bag modules sold through online marketplaces, such as auction sites. NHTSA obtained examples of these air bag modules and conducted testing. None of the inflators equipped in these modules were manufactured by DTN. This testing revealed that the inflators consistently malfunctioned during deployments, such as by failing to deploy the air bag or expelling metal shrapnel due to a rupture. This led NHTSA to release a consumer advisory in October 2012 to alert the public and repair professionals to the potential dangers of substandard or counterfeit air bag modules. ODI also worked with major vehicle manufacturers, who released advisories and notifications concerning the use of such air bag modules. Since then, NHTSA has continuously monitored for reports of malfunctioning air bags that may be associated with substandard or counterfeit components. These monitoring efforts have been supported by the increased visibility the advisories have brought to the issue, and by several standing general orders in place as part of separate investigations into Takata and ARC inflators, which require certain manufacturers to report any alleged or suspected inflator field ruptures to NHTSA. Until 2017, ODI was not aware of any field incidents involving an abnormal deployment of an imported substandard or counterfeit air bag module. In September 2017, ODI became aware of a crash involving a model year (MY) 2009 Honda Civic where the air bag ruptured during deployment, killing the driver. After inspecting the vehicle and air bag parts, ODI determined that the air bag inflator was not original equipment and that the air bag module was likely a substandard import. Insufficient information existed to determine the manufacturer of the inflator in this incident, however, it does not appear to resemble those manufactured by DTN. On June 16, 2023, ODI received a Vehicle Owner Questionnaire (VOQ #11527380) alleging that the rupture of a driver side air bag caused fatal injuries to the driver of a MY 2020 Chevrolet Malibu. ODI’s assessment of the rupture indicated that the air bag inflator was not original equipment but was instead manufactured by DTN, a Chinese company (also known as Yoshibayashi Toku theanol Automotive Safety Systems Co. Ltd.). From June 2023 to July 2024, ODI became aware of four additional ruptures involving substandard air bag modules equipped on Chevrolet Malibu vehicles. At the time, insufficient information existed for these crashes to determine who manufactured the ruptured inflators. As a result, NHTSA released a consumer advisory on July 10, 2024, alerting car buyers and owners to be aware of cheap, substandard replacement air bag inflators that can cause death or serious injury in a crash. The advisory urged consumers who own or are considering the purchase of a used vehicle to learn the vehicle’s history and ensure the vehicle has genuine air bag inflators. In March 2025, NHTSA learned about another rupture involving a suspected substandard, aftermarket inflator that was equipped in a MY 2017 Hyundai Sonata. The driver of this vehicle sustained fatal injuries that appeared related to the rupture. In August 2025, NHTSA received a similar report of a fatal air bag rupture in a MY 2019 Hyundai Sonata. Most recently, in October 2025, ODI received a report of a fatal air bag rupture in a MY 2020 Chevrolet Malibu. Photographs of the air bag components in these three crashes indicated that the ruptures all involved inflators manufactured by DTN. Further investigation of the inflator fragments in three of the prior incidents confirmed that they also involved DTN inflators. Photographs of the components involved in one of the other incidents also strongly suggest that the ruptured inflator was manufactured by DTN. Turn Signal Lamps

RL.The DRL is illuminated whenever the headlamps are off, increasing the duty cycle of the front turn signal bulb.The MY 2003 and earlier Malibu vehicles (Malibu Maxx was not manufactured for sale in the U.S. prior to MY 2004) used a headlamp based design, operated at a reduced voltage, for the DRL function; GM reverted back to this design on the MY 2006 and later Malibu and Malibu Maxx vehicles. Filament based bulbs have a finite life (hours of illumination) for any given operating voltage.As a result, increasing the duty cycle of a bulb typically decreases the life span of the bulb.GM identified and addressed two front turn signal/DRL and rear turn signal bulb quality issues early in MY 2004 production, however vehicle warranty claims for failed bulbs continued to occur.To address the high warranty levels of bulb failures (90% of the warranty claims submitted to ODI by GM were bulb failures and 10% were socket failures), GM issued a technical service bulletin in March 2006 advising dealers to use a new specification of bulb when addressing service failures, and to also inspect the electrical sockets for thermal damage and replace if necessary.In June 2006 (after subject vehicle production ended) GM changed the turn signal bulb specification to that of the TSB bulb in vehicle production also.To date, ODI has not identified any allegations of crash, injury, or fire related to the alleged defect even though the subject vehicles have experienced 5 - 6 years of field exposure.ODI's analysis of consumer complaints, both VOQ and manufacturer, shows a declining trend.The warranty rate for turn signal bulb replacement is high however statistical analysis of the warranty data suggests that the majority of the original equipment bulbs have already been replaced.GM conducted a safety recall (06V-263) for MY 2004-2005 Cadillac XLR to replace the front turn signal/DRL bulbs and the electrical sockets.However GM advised that the turn signal/DRL bulbs/sockets on the subject vehicles differ from those on the recalled XLRs in several respects, including using bulb sockets with four electrical terminals as opposed to three, which improves the electrical connection, allows the bulb to operate at a lower temperature, and adds stability to the bulb in the socket; operating at a lower voltage, which increases vehicle bulb life; using vented headlamps, which allows bulbs to run cooler than those installed in a non-vented XLR headlamp; and using a clear bulb, which runs cooler than the amber bulbs installed on the XLR.Additionally, the warranty claim level related to subject vehicle sockets is unremarkable compared to other defect investigations into defective socket assemblies.A safety-related defect trend has not been identified at this time.Further use of agency resources does not appear to be warranted.Accordingly, this investigation is closed.The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist.The agency will take further action if warranted by the circumstances.See attached report for further information.Read more

ON FEBRUARY 22, 2000, THE OFFICE OF DEFECTS INVESTIGATION (ODI) INITIATED PE00-009 BASED ON SEVEN REPORTS OF WINDSHIELD WIPER FAILURE IN CERTAIN MODEL YEAR (MY) 1997 & 1998 CHEVROLET MALIBU & OLDSMOBILE CUTLASS VEHICLES.THE INVESTIGATION WAS UPGRADED TO EA00-019 ON JULY 12, 2000.DURING THE COURSE OF

THE INVESTIGATION, IT WAS FOUND THAT THE FAILURES WERE THE RESULT OF FRACTURE OF THE PASSENGER-SIDE WINDSHIELD WIPER PIVOT HOUSING (SUBJECT COMPONENT).SUCH A FRACTURE LED TO AN IMMEDIATE AND IRRECOVERABLE LOSS OF WINDSHIELD WIPER FUNCTION.GMREPORTED THAT THE MATERIAL COMPOSITION (50% GLASS-REINFORCED ACETAL) OF THE SUBJECT COMPONENT WAS INSUFFICIENT TO WITHSTAND STRESSES IMPOSED BY CERTAIN FROZEN PRECIPITIATION ACCUMULATIONS.THE SUBJECT COMPONENT MATERIAL WAS CHANGED TO THE STRONGER 50% GLASS-REINFORCEDNYLON-6, EFFECTIVE JANUARY 98.OBSOLETE SERVICE PART WERE DISCARDED.GM WILL REPLACE THE SUBJECT COMPONENTS IN 129,427 SUBJECT VEHICLES FOUND IN 28 STATES (AK, CO, CT, DE, ID, IL, IN, IA ME, MD, MA, MI, MN, MT, NE, NH, NJ, NY, ND, OH, PA, RI, SD, UT, VT, WV, WI, WY), THE DISTRICT OF COLUMBIA, AND ALL CANADIAN PROVINCES.THESE STATES ACCOUNTED FOR 91% OF THE CONSUMER COMPLAINTS IDENTIFIED ABOVE.ODI WILL MONITOR THIS COMPAIGN FOR ADEQUATE EFFCTIVENESS AND, IF NECESSARY, TAKE FURTHER ACTION IN THE FUTURE.FURTHER EXPENDITURE OF AGENCY RESOURCES ON THIS MATTER IS UNJUSTIFIED AT THIS TIME.THE ENGINEERING ANALYSIS IS CLOSED.Read more
