When we google that, we see that there is an existing GM-issued service bulletin for certain units of the same year, make and model of our car. We therefore would like it to be investigated to have this service bulletin expanded to include our vehicle. It is unacceptable for a three-year old car not to unlock, lock, or start reliably and isn't normal wear and tear and there have not been any accidents.Read more

My battery health was checked by the repair shop at the dealership within the last 5 months. The malfunction could have caused a vehicle or house fire and endangered nearby occupants and property. The vehicle is currently at King Windward Nissan in Hawai‘i for inspection and repairs, and a Nissan Consumer Affairs case has been opened. Anecdotally, I have heard from 4 other Nissan Ariya owners in Hawai'i who have experienced similar battery failures with their Nissan Ariyas within the first two-years of purchase.Read more

On Sunday, April 12, 2026, at approximately 5:00 PM, I experienced a sudden and complete engine failure while driving my 2019 Chevy Silverado equipped with the 6.2L V8 engine. Without warning, the vehicle lost all ability to accelerate, creating an immediate and dangerous situation for myself, my fa

I was forced to rely on the vehicle’s remaining momentum to maneuver to the side of the road, all while other drivers honked as I attempted to merge with no engine power. This incident caused significant stress and posed a serious safety risk. My vehicle had to be towed home and then to [XXX] Casa Chevrolet at [XXX] . After inspection, the dealership informed me that the engine requires a full rebuild, despite the vehicle having only 115,697 miles. According to the dealership, the engine shows **no compression on all cylinders**, **crankshaft and camshaft compression failure**, **pistons coated in oil and fuel with visible damage**, and a **non‑functioning timing chain likely due to a broken guide**. These findings mirror the exact symptoms currently under investigation in GM’s **EA25007** inquiry involving the 6.2L V8 (L87) engines in 2021–2025 GM trucks and SUVs GM has already issued a major recall affecting nearly **600,000 vehicles** due to defective connecting rods and crankshafts that can cause sudden engine failure, stalling, and potential fire hazards. Although my 2019 Silverado is not included in the recall, it is experiencing the same catastrophic engine failure associated with the defective components identified in the investigation. The dealership has quoted me $10,000 for an engine rebuild and has refused to assist with the cost. I have also filed a complaint directly with GM, as this failure presents the same safety risks and mechanical defects acknowledged in the recalled models. A seized engine can cause sudden and complete loss of vehicle control which puts not only the driver but other drivers and pedestrians at risk. This failure occurred without any warning while the vehicle was in regular use. The oil leak was brought to the selling dealership, Auto Merchants Inc. in Plano Texas, for repair on January 6th 2026. They kept the vehicle overnight and confirmed it was fixed. The problem was not resolved as the vehicle continued losing oil. On March 3rd 2026 I discovered the engine was completely empty and added 6 full quarts of oil with no warning light appearing. I also contacted Hyundai consumer affairs and the Hyundai dealership directly who confirmed the manufacturer warranty had already expired at the time of purchase on December 16th 2024 and declined to cover the repair. The vehicle has not yet been formally inspected by the manufacturer or insurance representatives. The dealership serviced it on January 6th 2026 and claimed the leak was repaired however the engine seized on April 20th 2026 due to continued oil loss. There were zero warning lamps or indicators of any kind prior to the engine seizing completely on April 20th 2026. After this I attempted for 3 months for GM to cover the repair as the vehicle was less than 5 years old and less than 60,000 miles under the 5 yr/60K warranty but denied it. On March 26th, 2026 while driving on the parkway at about 60 mph, my vehicle experienced loss of power (slow deceleration, shaking/hesitation) and I immediately took it to the repair shop where it was diagnosed as a P0308 code (misfire on cylinder 8) which caused a repair of the #8 spark plug, ignition coil and wire resulting in a $380. repair. Two days later, I experienced the same issue and then took it to an engine diagnostic specialty shop where I have been told that this issue will run about $4,000 due to valve and comprehension issues on an engine that has 60, 680 miles. I have seen numerous L87 engine complaints on Chevy Forums with no assurances that repairs or engine replacement will rectify the issue and feel that this is an issue that GM should be addressing for safety issues and an economic loss to its customers. Upon the failure, the tempered glass shattered completely, the majority of the glass was forced into the passenger cabin, landing primarily in the backseat. This created an immediate safety hazard, distraction, and risk of injury from glass debris while maintaining vehicle control at high speeds. There is no "impact crater" or point of origin suggesting a rock chip as the front portion of the roof shattered completely; the glass appears to have failed due to internal tension or thermal stress. It is my understanding that there is a federal law stating that a manufacturer can not deny reimbursement for using a 3rd party auto shop for repairs and as far as the truck being too old for reimbursement, the repairs were done well within the 10 years allowed by federal law so I don't understand what the problem is . DTN Air Bag Inflator Rupture
The Office of Defects Investigation (ODI) has become aware of eight vehicle crashes in which a rupture of a Jilin Province Detiannuo Automobile Safety System Co., Ltd. (DTN) air bag inflator occurred during the deployment of the driver side air bag. All eight drivers in these crashes sustained serio
... us or fatal injuries that appear linked to the ruptures. Six of those drivers sustained fatal injuries and two sustained severe injuries. In all eight incidents, the subject inflators had been installed as replacement equipment after the vehicle was involved in a previous crash. Information gathered during NHTSA’s initial analysis of these incidents indicates that the inflators were potentially imported illegally into the United States. In conjunction with this safety investigation, NHTSA is also working with appropriate authorities to address any illegal activities associated with the importation of these inflators. As a result of the significant safety concerns relating to these inflators, as well as extensive information already obtained by ODI relating to their field ruptures, ODI is moving directly to the Engineering Analysis (EA) stage of its investigative process rather than first opening a Preliminary Evaluation. NHTSA has taken a variety of actions to address the risks posed by the importation of substandard or counterfeit air bag components. However, until recently, these risks appeared spread across a variety of components and manufacturers. As part of ODI’s continuous monitoring of this issue, the agency has obtained information to suggest that at least eight incidents involve ruptures of inflators manufactured by DTN. In parallel with this dedicated investigation into DTN inflators, ODI will continue to monitor reports of field incidents to evaluate risks from other substandard imported air bag components. In 2012, ODI first became aware of substandard or counterfeit air bag modules sold through online marketplaces, such as auction sites. NHTSA obtained examples of these air bag modules and conducted testing. None of the inflators equipped in these modules were manufactured by DTN. This testing revealed that the inflators consistently malfunctioned during deployments, such as by failing to deploy the air bag or expelling metal shrapnel due to a rupture. This led NHTSA to release a consumer advisory in October 2012 to alert the public and repair professionals to the potential dangers of substandard or counterfeit air bag modules. ODI also worked with major vehicle manufacturers, who released advisories and notifications concerning the use of such air bag modules. Since then, NHTSA has continuously monitored for reports of malfunctioning air bags that may be associated with substandard or counterfeit components. These monitoring efforts have been supported by the increased visibility the advisories have brought to the issue, and by several standing general orders in place as part of separate investigations into Takata and ARC inflators, which require certain manufacturers to report any alleged or suspected inflator field ruptures to NHTSA. Until 2017, ODI was not aware of any field incidents involving an abnormal deployment of an imported substandard or counterfeit air bag module. In September 2017, ODI became aware of a crash involving a model year (MY) 2009 Honda Civic where the air bag ruptured during deployment, killing the driver. After inspecting the vehicle and air bag parts, ODI determined that the air bag inflator was not original equipment and that the air bag module was likely a substandard import. Insufficient information existed to determine the manufacturer of the inflator in this incident, however, it does not appear to resemble those manufactured by DTN. On June 16, 2023, ODI received a Vehicle Owner Questionnaire (VOQ #11527380) alleging that the rupture of a driver side air bag caused fatal injuries to the driver of a MY 2020 Chevrolet Malibu. ODI’s assessment of the rupture indicated that the air bag inflator was not original equipment but was instead manufactured by DTN, a Chinese company (also known as Yoshibayashi Toku theanol Automotive Safety Systems Co. Ltd.). From June 2023 to July 2024, ODI became aware of four additional ruptures involving substandard air bag modules equipped on Chevrolet Malibu vehicles. At the time, insufficient information existed for these crashes to determine who manufactured the ruptured inflators. As a result, NHTSA released a consumer advisory on July 10, 2024, alerting car buyers and owners to be aware of cheap, substandard replacement air bag inflators that can cause death or serious injury in a crash. The advisory urged consumers who own or are considering the purchase of a used vehicle to learn the vehicle’s history and ensure the vehicle has genuine air bag inflators. In March 2025, NHTSA learned about another rupture involving a suspected substandard, aftermarket inflator that was equipped in a MY 2017 Hyundai Sonata. The driver of this vehicle sustained fatal injuries that appeared related to the rupture. In August 2025, NHTSA received a similar report of a fatal air bag rupture in a MY 2019 Hyundai Sonata. Most recently, in October 2025, ODI received a report of a fatal air bag rupture in a MY 2020 Chevrolet Malibu. Photographs of the air bag components in these three crashes indicated that the ruptures all involved inflators manufactured by DTN. Further investigation of the inflator fragments in three of the prior incidents confirmed that they also involved DTN inflators. Photographs of the components involved in one of the other incidents also strongly suggest that the ruptured inflator was manufactured by DTN. Turn Signal Lamps

RL.The DRL is illuminated whenever the headlamps are off, increasing the duty cycle of the front turn signal bulb.The MY 2003 and earlier Malibu vehicles (Malibu Maxx was not manufactured for sale in the U.S. prior to MY 2004) used a headlamp based design, operated at a reduced voltage, for the DRL function; GM reverted back to this design on the MY 2006 and later Malibu and Malibu Maxx vehicles. Filament based bulbs have a finite life (hours of illumination) for any given operating voltage.As a result, increasing the duty cycle of a bulb typically decreases the life span of the bulb.GM identified and addressed two front turn signal/DRL and rear turn signal bulb quality issues early in MY 2004 production, however vehicle warranty claims for failed bulbs continued to occur.To address the high warranty levels of bulb failures (90% of the warranty claims submitted to ODI by GM were bulb failures and 10% were socket failures), GM issued a technical service bulletin in March 2006 advising dealers to use a new specification of bulb when addressing service failures, and to also inspect the electrical sockets for thermal damage and replace if necessary.In June 2006 (after subject vehicle production ended) GM changed the turn signal bulb specification to that of the TSB bulb in vehicle production also.To date, ODI has not identified any allegations of crash, injury, or fire related to the alleged defect even though the subject vehicles have experienced 5 - 6 years of field exposure.ODI's analysis of consumer complaints, both VOQ and manufacturer, shows a declining trend.The warranty rate for turn signal bulb replacement is high however statistical analysis of the warranty data suggests that the majority of the original equipment bulbs have already been replaced.GM conducted a safety recall (06V-263) for MY 2004-2005 Cadillac XLR to replace the front turn signal/DRL bulbs and the electrical sockets.However GM advised that the turn signal/DRL bulbs/sockets on the subject vehicles differ from those on the recalled XLRs in several respects, including using bulb sockets with four electrical terminals as opposed to three, which improves the electrical connection, allows the bulb to operate at a lower temperature, and adds stability to the bulb in the socket; operating at a lower voltage, which increases vehicle bulb life; using vented headlamps, which allows bulbs to run cooler than those installed in a non-vented XLR headlamp; and using a clear bulb, which runs cooler than the amber bulbs installed on the XLR.Additionally, the warranty claim level related to subject vehicle sockets is unremarkable compared to other defect investigations into defective socket assemblies.A safety-related defect trend has not been identified at this time.Further use of agency resources does not appear to be warranted.Accordingly, this investigation is closed.The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist.The agency will take further action if warranted by the circumstances.See attached report for further information.Read more

I OPENED THIS INVESTIGATION BASED ON 64 REPORTS ALLEGING TURN SIGNAL FAILURE ON MY 1999-2000 ALERO, GRAND AM AND MALIBU VEHICLES NOT COVERED UNDER THE RECALL AND BUILT PRIOR TO FEBRUARY 2000.IN DECEMBER 2004, GM WILL ISSUE A SPECIAL POLICY FOR MY 1999-2000 ALERO, GRAND AM AND MALIBU VEHICLES NOT COVERED UNDER RECALL 03V327.THE POLICY COVERS TURN SIGNAL/HAZARD FLASHER FAILURE FOR A PERIOD OF 10 YEARS OR 150,000 MILES (WHICHEVER OCCURS FIRST) TO PROVIDE A NO COST REPLACEMENT OF THE HAZARD SWITCH.GM WILL SEND LETTERS TO OWNERS STARTING DECEMBER 30, 2004.THE ACTIONS TAKEN BY THE MANUFACTURER IS SUFFICIENT TO RESOLVE THE ISSUES RAISED BY THIS INVESTIGATION.THE CLOSING OF THIS INVESTIGATION DOES NOT CONSTITUTE A FINDING BY NHTSA THAT NO SAFETY-RELATED DEFECT EXISTS.THE AGENCY RESERVES THE RIGHT TO TAKE FUTURE ACTION IF WARRANTED BY THE CIRCUMSTANCES.Read more

NG ASSIST SYSTEM (EPS) THAT PROVIDES VARIABLE STEERING ASSISTANCE BASED ON STEERING WHEEL TORQUE, STEERING WHEEL POSTION, AND VEHICLE SPEED.GENERAL MOTORS DOCUMENTS IDENTIFIED THE FOLLOWING TWO FACTORS THAT CONTRIBUTED TO THE ALLEGED DEFECT IN THE SUBJECT VEHICLES: A.CONTAMINATION OF THE TORQUE AND POSITION SENSOR FROM THESEPARATION OF GREASE APPLIED TO THE STEERING COLUMN ASSEMBLY. B.ELECTRICAL NOISE GENERATED ON THE POWER AND GROUND SLIP RING SURFACES OF THE TORQUE AND POSITION SENSOR.THE NOISE IS GENERATED AS THE SENSOR CONTACT BRUSHES MOVE ALONG THE SLIP RING SURFACES WHEN THE STEERING WHEEL IS ROTATED.ODI'S ANALYSIS SHOWED FAILURERATES THAT ARE OF CONCERN TO ODI AND INDICATED THAT THE ALLEGED DEFECT IS CONTINUING TO OCCUR IN THE SUBJECT VEHICLES.
