On March 14 2026 i was driving from [XXX] . To [XXX] to have my tires checked because i felt a strong vibration on my steering column and truck itself, it was getting worse so i took it to my mechanic who does my regular maintenance on truck. After inspecting tire ( [XXX] 55 R20) was called over a
... nd shown that my front passenger side tire had a flat spot, it looked deformed mechanic informed me that the tire had separated from radial. And i need to get another tire right away, since there was non in stock we did the next best thing we took the front passenger tire the affected tire put it in the back and put the rear good tire up front. The next day i was on my way to purchase the replacement tire when the tire blew out. I was on a heavily traffic road [XXX] . I managed to take control of truck doing about 40 mph at the time and drove it off the road to a safer spot to look at damage. Well the passenger rear tire witch was originally on the front that had the separated radial blew a chunk out of the inner sidewall. I put on the spare and went and replaced the tire with another goodyear new tire. On march 28 as i was coming home from [XXX] late at night the driver side rear tire blew as well again had to put on spare and next day took to local mechanics in [XXX] , and checked tire and again i was told that the tire separated from radial and that was the cause of blew out, they inspected all the tires at this point since this was the second tire from the same DOT set with less then 12000 miles that had separated. Well they found a third tire that had aldo separated and had a flat spot on driver side front tire, now there are 3 tires from the same set, coincidence, i dont think so i replaced the second and third tire. One the third day April 1, 2026 the passenger side front tire blew as well, now i have 4 tires that had radial separation with less then 12000 miles and less then a year old, in a mater of a month. I am filing a safety complaint regarding a sudden and complete loss of power event while driving my 2021 Land Rover Discovery. The vehicle experienced a DC-DC converter failure with no warning, resulting in a sudden shutdown. The engine lost power, critical systems went offline, and I was unable to
... shift the vehicle into neutral. The vehicle became immobilized in active traffic, creating a dangerous situation. This incident occurred without warning and left me with no ability to safely move the vehicle out of traffic or rely on normal emergency functions. This presents a serious safety hazard to both the driver and others on the road. The vehicle has been at the dealership since mid-January 2026. The dealer has stated that the DC-DC failure caused additional component failures. However, no root cause has been identified beyond “internal failure,” and I have not been provided confirmation that the system has been properly tested or that the issue will not recur. I am concerned that this failure can cause cascading electrical issues, including loss of the 12V system, impacting multiple critical vehicle functions at once. The lack of a clear diagnosis and verified repair raises concerns about reliability and safety. Given the sudden loss of power, inability to move the vehicle, and absence of a confirmed root cause or validated repair, I am formally requesting that the National Highway Traffic Safety Administration open a defect investigation into the MHEV/DC-DC system. This failure creates a significant safety risk, including vehicles becoming immobilized in active traffic without warning. The risk of recurrence without a defined cause is especially concerning and may indicate a broader issue.Read more

To Whom It May Concern, I am submitting this complaint to report a serious mechanical issue involving the transfer case on my 2022 Toyota Highlander. This problem raises concerns regarding the safety, reliability, and overall performance of the vehicle. While driving, I began hearing a grinding no
... ise coming from the vehicle. Concerned, I brought the vehicle to an authorized dealership for a diagnostic inspection. I was informed that there is a transmission-related issue and that the vehicle requires a transfer case replacement. At the time of diagnosis, the vehicle had approximately 77,000 miles and has been primarily driven locally under normal conditions. This type of major drivetrain failure at such mileage is unexpected and concerning, especially given that the vehicle has been properly maintained. The failure of a critical component such as the transfer case can significantly affect vehicle control and drivability, posing a potential safety risk. This raises concerns about a possible defect in the drivetrain system or a broader issue affecting similar vehicles. I respectfully request that this matter be investigated to determine whether this is an isolated incident or part of a wider pattern. If necessary, appropriate corrective actions should be taken to ensure consumer safety. Thank you for your attention to this matter. I am available to provide any additional documentation, including service records and diagnostic reports, upon request.Read more

I’m writing to report a recurring and increasingly dangerous issue with the CMBS (Collision Mitigation Braking System) in my vehicle. The problem began shortly after I purchased the car. The system would intermittently flash the word “BRAKE” and apply the brakes without any apparent reason. In
... itially, this was more of an annoyance, but it has since escalated into a serious safety concern. Most recently, while driving through an intersection with no vehicles in front or behind me, the system suddenly applied the brakes with significant force. This caused everything in the front seat to be thrown to the floor and forced me into the steering wheel. The weather was clear—no rain or sun glare—and there were no visible obstructions. Had there been a vehicle behind me, I believe a rear-end collision would have been likely due to the abrupt deceleration. I have brought the car to the dealership where I purchased it multiple times, but they were unable to replicate the issue and eventually asked me to stop bringing the vehicle in. I then tried a different dealership, but they also refused to investigate further since they couldn’t duplicate the problem. Even after disabling the CMBS system about a year after purchase, the issue has persisted. I’ve since learned that this issue is under investigation, and I want to formally express that I no longer feel safe driving this vehicle. I am requesting a resolution to this matter as soon as possible. Please let me know the next steps or if any additional information is needed. We purchased a brand-new 2021 Kia Sedona in 2021. Since January, we’ve been experiencing an intermittent but serious issue where the vehicle refuses to accelerate or shift beyond a certain point. This issue has put us in dangerous situations multiple times. One incident occurred while crossing a

On another occasion, while merging onto a highway, the car failed to accelerate, and we feared being rear-ended due to our inability to keep up with traffic. There are no warning lights or error messages to indicate a problem when this occurs. The only temporary solution has been to pull over, turn the car off, and restart it. After doing this, the vehicle functions normally for a few days, weeks, or even months before the issue reoccurs. We have taken the vehicle to our local Kia dealership, but they were unable to replicate the problem or identify any issues. We also brought it to a second Kia dealership with the same result—no diagnosis and no resolution. We are extremely concerned for our safety and would appreciate guidance on how to escalate this issue or pursue a resolution. I also have a video of an incident.Read more

I took it to my neighborhood repair shop when I got back and they said two spark plugs are shot, they replaced them and it ran fine. But now it’s stuttering again, just a few months later. I called the dealership again and they said I could bring it it for a diagnostic (not free) and then I got a letter about a recall. Not sure if it’s related to my issue because the dealer said probably not and said they’d call me when they knew more and my car had the computer issue not this one. No one has called and the car is still stuttering. I took it to the mechanic again and he said spark plugs look fine, but again, it is losing power in 4th gear, just around 45 mph only when I’ve been driving for more than two hours. I would just like the dealership to take a look and fix it.Read more

Intermittent ABS light and Service Electronic Braking System lights coming on and off. Appears from previous reports this is a known issue that has been going on for years now with this vehicle and should be fixed. I purchased a new hydraulic and electronic module from dealer at around 35000 miles
... and replaced. Issue was fixed or so I thought, at around 70000 miles lights are intermittently on and off again. No known issue was contacting ram direct, dealer I bought vehicle from stated the electronic module is backordered and has already been replaced with another part number, I believe it was a faulty replacement from the get go. Hydraulic module was said to be under engineered testing again per the dealer. Extremely frustrating from the customer end, no cruise, no abs braking capabilities. No known deaths to this point linked to this specific issue to my knowledge but I wonder if Ram will move a little faster on the fix or recall once they have something fatal on their backs for this.Read more

I am writing in reference to a new Toyota Highlander that I purchased in December of 2016. The reason I chose Toyota over a Ford or GM SUV was Toyota’s long history of producing reliable vehicles. I researched and relied on Consumer Reports which gave the Highlander and Honda Pilot the highest rat
... ing for customer satisfaction and reliability. About two weeks ago I started hearing a high-pitched whining coming from the transmission, difficulty shifting, and lurching forward when engaging the gear. Last Wednesday my wife was driving on a busy freeway in Atlanta when the vehicle lost acceleration. She was nearly hit while stopped on the freeway. Fortunately, kind motorists got out of their cars and pushed her to the side of the road. The car was towed to a Toyota service shop, and we were advised they could not do the repairs. It was a known internal Toyota issue. I did some research online and found hundreds of posts from Highlander owners of 2017-18 vehicles complaining about issues with the transmission failing. I found Toyota Customer Support Bulletin ,[XXXXXXXX] REPAIR COVERAGE FOR UA80 TRANSMISSION ON CERTAIN 2017-2018 MY SIENNA AND CERTAIN 2017-2018 MY HIGHLANDER VEHICLES. We called Toyota support on 10/9/23 and spoke with a customer service representative named Amber who then transferred us to Vena. They advised based on the VIN number our vehicle was not covered. Vena advised that since all our service was not at a Toyota dealership, they could not honor the repairs. The bulletin stated “after the Primary Coverage, the Secondary Coverage is applicable for 10 years from the vehicles date of first use, regardless of mileage”. The cost of this repair is over $6,000. The bulletin list Highlander 2WD and 4WD SUVs. The representative could not explain how the VIN number exclusions are determined and applied to our SUV. The contact owns a 2017 Ford Edge. The contact stated that the check engine warning light was illuminated. The vehicle was taken to an independent mechanic, where it was diagnosed that an engine cylinder was misfiring. The mechanic replaced the cylinder; however, the failure reoccurred. Additionally
... , the contact stated while driving 65-70 MPH, the vehicle made abnormal beeping sounds and decelerated unintendedly to 35 MPH. The contact was able to veer to the side of the road. The vehicle was towed to an independent mechanic, where the contact was informed that there was no coolant in the vehicle. The mechanic added coolant in the vehicle to determine whether coolant was leaking. The vehicle was not repaired and was returned to the contact. The contact stated that the failure recurred while driving. The contact veered to the side of the road and turned off the vehicle for 5 minutes. The contact restarted the vehicle and drove to the nearest independent mechanic, where it was diagnosed that coolant was leaking into the head gasket and the engine needed to be replaced. The vehicle was not repaired. The dealer was notified of the failure and referred the contact to the manufacturer for assistance. The contact related the failure to TSB Number: 22-2133. The manufacturer was notified of the failure, but no assistance was provided. The failure mileage was approximately 45,500.Read more

This morning I was driving to work going approximately 45 mph and the engine in my 2021 Ford Bronco shut off/stalled. Thank GOD there was no one behind me at that time. The dash board lit up and a message appeared saying "put the car in park and restart the engine" There was nothing touched on th

NHTSA Safety Investigations, CHEVROLET MALIBU
Investigations opened by NHTSA into potential safety defects. These are separate from recalls and may or may not result in a recall.

DTN Air Bag Inflator Rupture
The Office of Defects Investigation (ODI) has become aware of eight vehicle crashes in which a rupture of a Jilin Province Detiannuo Automobile Safety System Co., Ltd. (DTN) air bag inflator occurred during the deployment of the driver side air bag. All eight drivers in these crashes sustained serio
... us or fatal injuries that appear linked to the ruptures. Six of those drivers sustained fatal injuries and two sustained severe injuries. In all eight incidents, the subject inflators had been installed as replacement equipment after the vehicle was involved in a previous crash. Information gathered during NHTSA’s initial analysis of these incidents indicates that the inflators were potentially imported illegally into the United States. In conjunction with this safety investigation, NHTSA is also working with appropriate authorities to address any illegal activities associated with the importation of these inflators. As a result of the significant safety concerns relating to these inflators, as well as extensive information already obtained by ODI relating to their field ruptures, ODI is moving directly to the Engineering Analysis (EA) stage of its investigative process rather than first opening a Preliminary Evaluation. NHTSA has taken a variety of actions to address the risks posed by the importation of substandard or counterfeit air bag components. However, until recently, these risks appeared spread across a variety of components and manufacturers. As part of ODI’s continuous monitoring of this issue, the agency has obtained information to suggest that at least eight incidents involve ruptures of inflators manufactured by DTN. In parallel with this dedicated investigation into DTN inflators, ODI will continue to monitor reports of field incidents to evaluate risks from other substandard imported air bag components. In 2012, ODI first became aware of substandard or counterfeit air bag modules sold through online marketplaces, such as auction sites. NHTSA obtained examples of these air bag modules and conducted testing. None of the inflators equipped in these modules were manufactured by DTN. This testing revealed that the inflators consistently malfunctioned during deployments, such as by failing to deploy the air bag or expelling metal shrapnel due to a rupture. This led NHTSA to release a consumer advisory in October 2012 to alert the public and repair professionals to the potential dangers of substandard or counterfeit air bag modules. ODI also worked with major vehicle manufacturers, who released advisories and notifications concerning the use of such air bag modules. Since then, NHTSA has continuously monitored for reports of malfunctioning air bags that may be associated with substandard or counterfeit components. These monitoring efforts have been supported by the increased visibility the advisories have brought to the issue, and by several standing general orders in place as part of separate investigations into Takata and ARC inflators, which require certain manufacturers to report any alleged or suspected inflator field ruptures to NHTSA. Until 2017, ODI was not aware of any field incidents involving an abnormal deployment of an imported substandard or counterfeit air bag module. In September 2017, ODI became aware of a crash involving a model year (MY) 2009 Honda Civic where the air bag ruptured during deployment, killing the driver. After inspecting the vehicle and air bag parts, ODI determined that the air bag inflator was not original equipment and that the air bag module was likely a substandard import. Insufficient information existed to determine the manufacturer of the inflator in this incident, however, it does not appear to resemble those manufactured by DTN. On June 16, 2023, ODI received a Vehicle Owner Questionnaire (VOQ #11527380) alleging that the rupture of a driver side air bag caused fatal injuries to the driver of a MY 2020 Chevrolet Malibu. ODI’s assessment of the rupture indicated that the air bag inflator was not original equipment but was instead manufactured by DTN, a Chinese company (also known as Yoshibayashi Toku theanol Automotive Safety Systems Co. Ltd.). From June 2023 to July 2024, ODI became aware of four additional ruptures involving substandard air bag modules equipped on Chevrolet Malibu vehicles. At the time, insufficient information existed for these crashes to determine who manufactured the ruptured inflators. As a result, NHTSA released a consumer advisory on July 10, 2024, alerting car buyers and owners to be aware of cheap, substandard replacement air bag inflators that can cause death or serious injury in a crash. The advisory urged consumers who own or are considering the purchase of a used vehicle to learn the vehicle’s history and ensure the vehicle has genuine air bag inflators. In March 2025, NHTSA learned about another rupture involving a suspected substandard, aftermarket inflator that was equipped in a MY 2017 Hyundai Sonata. The driver of this vehicle sustained fatal injuries that appeared related to the rupture. In August 2025, NHTSA received a similar report of a fatal air bag rupture in a MY 2019 Hyundai Sonata. Most recently, in October 2025, ODI received a report of a fatal air bag rupture in a MY 2020 Chevrolet Malibu. Photographs of the air bag components in these three crashes indicated that the ruptures all involved inflators manufactured by DTN. Further investigation of the inflator fragments in three of the prior incidents confirmed that they also involved DTN inflators. Photographs of the components involved in one of the other incidents also strongly suggest that the ruptured inflator was manufactured by DTN. Turn Signal Lamps
GM designed the MY 2004 and 2005 Chevrolet Malibu and Malibu Maxx vehicles with a daytime running lamp (DRL) system that uses the same filament (the high power filament of a dual filament bulb) as the front turn signals. The high power filament operates at full battery voltage when functioning as a D
... RL.The DRL is illuminated whenever the headlamps are off, increasing the duty cycle of the front turn signal bulb. The MY 2003 and earlier Malibu vehicles (Malibu Maxx was not manufactured for sale in the U.S. prior to MY 2004) used a headlamp based design, operated at a reduced voltage, for the DRL function; GM reverted back to this design on the MY 2006 and later Malibu and Malibu Maxx vehicles. Filament based bulbs have a finite life (hours of illumination) for any given operating voltage. As a result, increasing the duty cycle of a bulb typically decreases the life span of the bulb.GM identified and addressed two front turn signal/DRL and rear turn signal bulb quality issues early in MY 2004 production, however vehicle warranty claims for failed bulbs continued to occur. To address the high warranty levels of bulb failures (90% of the warranty claims submitted to ODI by GM were bulb failures and 10% were socket failures), GM issued a technical service bulletin in March 2006 advising dealers to use a new specification of bulb when addressing service failures, and to also inspect the electrical sockets for thermal damage and replace if necessary. In June 2006 (after subject vehicle production ended) GM changed the turn signal bulb specification to that of the TSB bulb in vehicle production also.To date, ODI has not identified any allegations of crash, injury, or fire related to the alleged defect even though the subject vehicles have experienced 5 - 6 years of field exposure.ODI's analysis of consumer complaints, both VOQ and manufacturer, shows a declining trend. The warranty rate for turn signal bulb replacement is high however statistical analysis of the warranty data suggests that the majority of the original equipment bulbs have already been replaced.GM conducted a safety recall (06V-263) for MY 2004-2005 Cadillac XLR to replace the front turn signal/DRL bulbs and the electrical sockets. However GM advised that the turn signal/DRL bulbs/sockets on the subject vehicles differ from those on the recalled XLRs in several respects, including using bulb sockets with four electrical terminals as opposed to three, which improves the electrical connection, allows the bulb to operate at a lower temperature, and adds stability to the bulb in the socket; operating at a lower voltage, which increases vehicle bulb life; using vented headlamps, which allows bulbs to run cooler than those installed in a non-vented XLR headlamp; and using a clear bulb, which runs cooler than the amber bulbs installed on the XLR.Additionally, the warranty claim level related to subject vehicle sockets is unremarkable compared to other defect investigations into defective socket assemblies.A safety-related defect trend has not been identified at this time.Further use of agency resources does not appear to be warranted.Accordingly, this investigation is closed. The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist. The agency will take further action if warranted by the circumstances.See attached report for further information.Read more
