Vehicle: 2021 GMC Sierra SLT | VIN: [XXX] Dealership: Carl Black GMC Roswell, GA (11225 Alpharetta Hwy, 30076) | Invoice #[XXX] | Service Date: [XXX] Paragraph 1 – Suspension Hazard: Dealership diagnosed a worn left upper ball joint (popping sound on left turns) and installed Rough Country afterm
... arket upper control arms on my stock-height Sierra SLT. These arms are designed for lifted suspension and alter factory caster/camber geometry, placing extreme stress on ball joints. This can cause sudden ball joint separation, wheel collapse, and total loss of steering control at speed. The dealership admitted on the invoice they "could not find branding" on original parts and chose non-OEM lift components without my consent or any safety disclosure. Paragraph 2 – Brake Failure & No Safety Warning: The same service included a full front/rear brake job (pads + resurfaced rotors) using their certified technicians and parts. After pickup, the brakes failed to operate properly. When I returned, the service manager refused to correct their own workmanship and instead quoted $1,900 to repair the very brakes they just serviced. Most critically, at vehicle pickup they issued no written or verbal safety warning—no "do not drive" instruction—despite the brakes being knowingly compromised, endangering me and the public. Paragraph 3 – Requested Action: The service manager was unprofessional, condescending, and refused accountability. I request an immediate NHTSA investigation into Carl Black GMC's unauthorized use of lift-kit suspension parts on stock vehicles, failure to warranty their own brake service, and reckless omission of safety warnings at delivery. This dealership pattern poses an imminent public risk. Please escalate. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

The vehicle exhibits a severe, structural inability to retain its factory suspension and steering alignment specifications under normal driving conditions, creating a severe tire blowout and safety hazard. At approximately 8,400 miles (January 2025), the dealership documented that the alignment was
... heavily out of specification. At approximately 9,400 miles (February 2025), a certified third-party shop corrected the vehicle back to perfect factory specifications. Less than 9,700 miles of standard city driving later, at approximately 19,100 miles (May 2026), a dealer inspection revealed the vehicle's alignment geometry had completely shifted out of specification across three separate wheels (Right Front Toe, Right Rear Camber, Left Rear Toe). This rapid alignment failure dynamically scrubbed the front steering tires completely bald down to the internal metal structural cords/threads, creating an immediate, severe safety hazard on the road. The vehicle's inability to hold standard parameters points to a defective suspension tracking design or components incapable of supporting the vehicle load. The dealership refused to cover this mechanical failure under warranty, forcing an out-of-pocket tire replacement expense. A formal corporate escalation case has been opened with the manufacturer.Read more

While driving, the check engine light appeared and the vehicle began running rough and hesitating during acceleration. The engine experienced a noticeable loss of power, which created a dangerous situation in traffic and nearly caused an accident when attempting to accelerate with surrounding vehicl
... es. The vehicle was taken to a Honda dealership for diagnosis. The dealership confirmed a failed fuel injector on cylinder number four and a catalyst efficiency fault code (P0420). The dealership explained that the failed injector may have caused improper fuel delivery and contributed to damage to the catalytic converter. The dealer also indicated that similar fuel injector failures are addressed in Honda Service Bulletin 21-010 for this model year. However, my vehicle was not included in the VIN range covered by the bulletin even though it is a 2019 model and uses the same fuel injector part number referenced in the bulletin. The issue was confirmed by an authorized Honda dealership and the recommended repair was replacement of the fuel injector kit and catalytic converter. Honda corporate declined to fully cover the repair costs despite the similarity to the known issue described in the service bulletin. This engine malfunction created a loss of power while driving, which could present a safety hazard in traffic.Read more

There is a strong smell of gas in the passenger compartment and a very strong smell of gas gas on the drivers side near the rear tire. The Chevy dealer has confirmed the the gas smell is due to a know defect where the fuel pump develops cracks in the housing because of the poor quality plastic used.
... The dealer estimates that it will cost between $3,000 to $4,000 to repair. This is a very dangerous problem. I'm afraid with the very strong smell of gas that an electric spark might cause the car to explode or when it's stored inside cause a vehicle and structure fire. Why has the NHTSA not made GM do a recall for this defect. It is a very serious problem. Owners of any C6 corvette are sitting on a time bomb and their life is in danger. GM has already admitted that it is a design defect but has only issued a service bulletin. Do something before people die.Read more

On [XXX, I was driving on the interstate when my engine seized and the car lost all power, leaving me stranded on the side of the road on a busy exit at night. This was very dangerous to myself and others. I purchased this car in July of 2023 with 69,000 miles in good condition. I immediately made a
... n appointment to take care of the recalls and updates to this car, which were performed February 2024 due to this being the first available appointment. Also, at my first oil change (3,000 miles after the previous oil change) I was told by the mechanic that there was absolutely no oil in my car. Since then I have had to put SEVERAL quarts of oil in between each oil change, costing me hundreds of dollars. Then June of 2024, the P1326 code flashed on my car and it went into limp mode also while I was driving on a busy street, putting myself and others in danger. I took it to the dealership and was advised to continue driving it and there was nothing they can do until my engine fails. I also brought up the oil consumption issue and was told that even if I did an oil consumption test (at my cost), there was nothing they could do. I was advised by my service advisor not to do a consumption test. I took his advice and continued to drive my car, adding countless quarts of oil weekly to my car. Fast forward to September 2024, when my engine light came on due to misfires in cylinder 3&4 and I took it to a local mechanic due to the incredibly long wait for an appointment at Hyundai. I was told all 4 spark plugs were oil fouled due to excessive oil consumption. This happened again July of 2025. My mechanic advised me that there was nothing to do about it, but that this would continue to happen due to the oil consumption issue. Now Hyundai is refusing to take responsibility for this engine that has had documented issues resulting in class action lawsuits and safety issues for myself and others and refusing any financial assistance. No warning lamps were on before incident. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATIORead more

I bought my car used when it had around 11,000 miles on it. It didn't take long (maybe a month) before I noticed that the fuel gauge indicator above the steering wheel was malfunctioning. The screen stays off for a period of time and sometimes flickers on with indistinguishable text. There are times
... that it works just fine, but it is becoming less and less common. At first, I thought it was attributed to the cold weather, but it also happens in the heat. I recently had a separate recall repaired and asked the mechanic about this issue. He stated that several people had told him the same story and he said I would have to replace the entire gauge cluster to fix it. This is very unfortunate because I am someone who travels often and I can't be stuck in the middle of nowhere without gas because I was unable to determine how much I had. There should be another way to monitor how much gas you have. I can't see how much oil life I have left either, or the mileage on my car when the screen goes out. My car was at low mileage when I bought it, and it still only has around 30,000 miles on it so I do not understand why this issue is happening.Read more

I am filing this complaint regarding recurring safety, security, and system failures in my Tesla Model Y, concerns about unauthorized access, and Tesla’s refusal to provide complete diagnostic and privacy data. These issues have directly endangered my safety and prevent me from confirming whether
... the vehicle has been compromised. The vehicle’s touchscreen and associated software systems repeatedly failed. An unfamiliar key fob activity was detected on July 27, 2025 and on at least two occasions prior to July 27, 2025, the touchscreen went black and could only be restored via a full hard reboot. Safety Risks: * Loss of touchscreen functionality could disable access to critical driving and emergency controls. * Potential unauthorized access poses risks of theft or malicious interference with the vehicle. * Chemical/gas odors could indicate exposure to harmful substances. Tesla service center inspected the vehicle but reported no issues; I believe the inspection may not have been fully conducted as requested. The vehicle has not been independently inspected outside of Tesla. Police and insurance representatives have not been involved. Warning Signs Prior to Failure: * The touchscreen went black twice in the two days prior to July 27, 2025. * Unfamiliar key fob activity was detected on July 27, 2025. * The charger port opened by itself multiple times. * The vehicle locked while I was inside on one occasion. Attempts to Resolve: * Tesla’s mobile service rescheduling and service center inspection did not identify or resolve the issues. * Five privacy/data requests submitted to Tesla for access and diagnostic logs returned incomplete, empty, or irrelevant files. Desired Outcome: * A complete copy of all records and data related to my vehicle, including access logs, diagnostic logs, security/camera events, and system communications. * A thorough, transparent inspection of the vehicle by Tesla, conducted in my presence, with full documentation of findings provided.Read more

DTN Air Bag Inflator Rupture
The Office of Defects Investigation (ODI) has become aware of eight vehicle crashes in which a rupture of a Jilin Province Detiannuo Automobile Safety System Co., Ltd. (DTN) air bag inflator occurred during the deployment of the driver side air bag. All eight drivers in these crashes sustained serio
... us or fatal injuries that appear linked to the ruptures. Six of those drivers sustained fatal injuries and two sustained severe injuries. In all eight incidents, the subject inflators had been installed as replacement equipment after the vehicle was involved in a previous crash. Information gathered during NHTSA’s initial analysis of these incidents indicates that the inflators were potentially imported illegally into the United States. In conjunction with this safety investigation, NHTSA is also working with appropriate authorities to address any illegal activities associated with the importation of these inflators. As a result of the significant safety concerns relating to these inflators, as well as extensive information already obtained by ODI relating to their field ruptures, ODI is moving directly to the Engineering Analysis (EA) stage of its investigative process rather than first opening a Preliminary Evaluation. NHTSA has taken a variety of actions to address the risks posed by the importation of substandard or counterfeit air bag components. However, until recently, these risks appeared spread across a variety of components and manufacturers. As part of ODI’s continuous monitoring of this issue, the agency has obtained information to suggest that at least eight incidents involve ruptures of inflators manufactured by DTN. In parallel with this dedicated investigation into DTN inflators, ODI will continue to monitor reports of field incidents to evaluate risks from other substandard imported air bag components. In 2012, ODI first became aware of substandard or counterfeit air bag modules sold through online marketplaces, such as auction sites. NHTSA obtained examples of these air bag modules and conducted testing. None of the inflators equipped in these modules were manufactured by DTN. This testing revealed that the inflators consistently malfunctioned during deployments, such as by failing to deploy the air bag or expelling metal shrapnel due to a rupture. This led NHTSA to release a consumer advisory in October 2012 to alert the public and repair professionals to the potential dangers of substandard or counterfeit air bag modules. ODI also worked with major vehicle manufacturers, who released advisories and notifications concerning the use of such air bag modules. Since then, NHTSA has continuously monitored for reports of malfunctioning air bags that may be associated with substandard or counterfeit components. These monitoring efforts have been supported by the increased visibility the advisories have brought to the issue, and by several standing general orders in place as part of separate investigations into Takata and ARC inflators, which require certain manufacturers to report any alleged or suspected inflator field ruptures to NHTSA. Until 2017, ODI was not aware of any field incidents involving an abnormal deployment of an imported substandard or counterfeit air bag module. In September 2017, ODI became aware of a crash involving a model year (MY) 2009 Honda Civic where the air bag ruptured during deployment, killing the driver. After inspecting the vehicle and air bag parts, ODI determined that the air bag inflator was not original equipment and that the air bag module was likely a substandard import. Insufficient information existed to determine the manufacturer of the inflator in this incident, however, it does not appear to resemble those manufactured by DTN. On June 16, 2023, ODI received a Vehicle Owner Questionnaire (VOQ #11527380) alleging that the rupture of a driver side air bag caused fatal injuries to the driver of a MY 2020 Chevrolet Malibu. ODI’s assessment of the rupture indicated that the air bag inflator was not original equipment but was instead manufactured by DTN, a Chinese company (also known as Yoshibayashi Toku theanol Automotive Safety Systems Co. Ltd.). From June 2023 to July 2024, ODI became aware of four additional ruptures involving substandard air bag modules equipped on Chevrolet Malibu vehicles. At the time, insufficient information existed for these crashes to determine who manufactured the ruptured inflators. As a result, NHTSA released a consumer advisory on July 10, 2024, alerting car buyers and owners to be aware of cheap, substandard replacement air bag inflators that can cause death or serious injury in a crash. The advisory urged consumers who own or are considering the purchase of a used vehicle to learn the vehicle’s history and ensure the vehicle has genuine air bag inflators. In March 2025, NHTSA learned about another rupture involving a suspected substandard, aftermarket inflator that was equipped in a MY 2017 Hyundai Sonata. The driver of this vehicle sustained fatal injuries that appeared related to the rupture. In August 2025, NHTSA received a similar report of a fatal air bag rupture in a MY 2019 Hyundai Sonata. Most recently, in October 2025, ODI received a report of a fatal air bag rupture in a MY 2020 Chevrolet Malibu. Photographs of the air bag components in these three crashes indicated that the ruptures all involved inflators manufactured by DTN. Further investigation of the inflator fragments in three of the prior incidents confirmed that they also involved DTN inflators. Photographs of the components involved in one of the other incidents also strongly suggest that the ruptured inflator was manufactured by DTN. This opening resume exceeded space limitations and is continued in an attachment uploaded to the investigation.Read more

Turn Signal Lamps
GM designed the MY 2004 and 2005 Chevrolet Malibu and Malibu Maxx vehicles with a daytime running lamp (DRL) system that uses the same filament (the high power filament of a dual filament bulb) as the front turn signals. The high power filament operates at full battery voltage when functioning as a D
... RL.The DRL is illuminated whenever the headlamps are off, increasing the duty cycle of the front turn signal bulb. The MY 2003 and earlier Malibu vehicles (Malibu Maxx was not manufactured for sale in the U.S. prior to MY 2004) used a headlamp based design, operated at a reduced voltage, for the DRL function; GM reverted back to this design on the MY 2006 and later Malibu and Malibu Maxx vehicles. Filament based bulbs have a finite life (hours of illumination) for any given operating voltage. As a result, increasing the duty cycle of a bulb typically decreases the life span of the bulb.GM identified and addressed two front turn signal/DRL and rear turn signal bulb quality issues early in MY 2004 production, however vehicle warranty claims for failed bulbs continued to occur. To address the high warranty levels of bulb failures (90% of the warranty claims submitted to ODI by GM were bulb failures and 10% were socket failures), GM issued a technical service bulletin in March 2006 advising dealers to use a new specification of bulb when addressing service failures, and to also inspect the electrical sockets for thermal damage and replace if necessary. In June 2006 (after subject vehicle production ended) GM changed the turn signal bulb specification to that of the TSB bulb in vehicle production also.To date, ODI has not identified any allegations of crash, injury, or fire related to the alleged defect even though the subject vehicles have experienced 5 - 6 years of field exposure.ODI's analysis of consumer complaints, both VOQ and manufacturer, shows a declining trend. The warranty rate for turn signal bulb replacement is high however statistical analysis of the warranty data suggests that the majority of the original equipment bulbs have already been replaced.GM conducted a safety recall (06V-263) for MY 2004-2005 Cadillac XLR to replace the front turn signal/DRL bulbs and the electrical sockets. However GM advised that the turn signal/DRL bulbs/sockets on the subject vehicles differ from those on the recalled XLRs in several respects, including using bulb sockets with four electrical terminals as opposed to three, which improves the electrical connection, allows the bulb to operate at a lower temperature, and adds stability to the bulb in the socket; operating at a lower voltage, which increases vehicle bulb life; using vented headlamps, which allows bulbs to run cooler than those installed in a non-vented XLR headlamp; and using a clear bulb, which runs cooler than the amber bulbs installed on the XLR.Additionally, the warranty claim level related to subject vehicle sockets is unremarkable compared to other defect investigations into defective socket assemblies.A safety-related defect trend has not been identified at this time.Further use of agency resources does not appear to be warranted.Accordingly, this investigation is closed. The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist. The agency will take further action if warranted by the circumstances.See attached report for further information.Read more
