While attempting to enter a high-speed expressway, the transmission malfunctioned, preventing normal acceleration. The vehicle could only accelerate very slowly but managed to maintain a limited speed, creating a hazardous situation while trying to merge into fast-moving traffic. I was able to navig

The vehicle was towed to an authorized dealership service center, where technicians diagnosed a total internal failure of the CVT transmission requiring complete replacement. Mind you, the vehicle is under 6 years old and has fewer than 90,000 total miles, and this is the second time the transmission on this vehicle has suffered a total failure and required a complete replacement. Prior to this mechanical breakdown, there were no warning lights, abnormal symptoms, or performance issues indicating an impending transmission failure. ... rake failure occurred. In January 2024, I reported brake-related concerns to a Chevrolet dealership while the vehicle was under warranty, but the root cause was not diagnosed. In November 2025, after the brake pedal became hard, an independent repair facility inspected the vehicle and determined the brake problem was caused by a failing engine vacuum pump. They advised that the vehicle be repaired by a Chevrolet dealership. By January 2026, the brake condition had worsened to the point that the vehicle was no longer safe to drive and had to be towed. A Chevrolet dealership later confirmed that the engine vacuum pump had seized and damaged the camshaft, requiring replacement of both components. The vehicle remains inoperable. The failed vacuum pump caused loss of brake assist, resulting in a hard brake pedal, reduced braking ability, and an increased risk of a crash. I believe this represents a serious safety defect because failure of the vacuum pump significantly impairs the driver’s ability to stop the vehicle safely. I understand GM has issued service information regarding vacuum pump failures on this engine, and I am submitting this complaint because I believe other drivers may experience the same dangerous brake failure.Read more

Rusty Wallace Kia of alcoa confirmed the failure on 04/27/2026. They diagnosed the vehicle with DTCs P17EE and U1010. They provided a repair quote (RO XXX) for $9,431.48 to replace the corroded HCA and harness. YES. The vehicle was inspected by an authorized Kia dealer (Rusty Wallace Kia of alcoa ). They are refusing to perform the Recall SC276 mandated repair, claiming it was "already completed" with a fuse swap in Jan 2024, even though the safety defect has now manifested as a full component failure. Yes. The HEV warning light and Check Engine light appeared. Symptoms include "Check Hybrid System" messages and the vehicle failing to engage the engine clutch. These symptoms appeared recently at approximately 107,000 miles. Then when i checked the car it vanished, no error on dashboard ,so i took the car back and was driving error showed up at 109k miles again They asked to repair the vehicle and the repair is not in my budget and i told them that its the recall related issue and they are denying and saying recall was done and they dont open recall again and I found in the document they provided they just did the fuse replacement not HCA unit and The fuse swap did help with the hca corrosion and its not in drivable state and it becae a fire hazard. ... rage door and the corner of the garage/house. There was also a Tesla parked inside the garage that was knocked back about a foot and a half. The distance from the edge of driveway and street to the garage door is about 27 feet. This happened at 1:30 PM. The weather conditions were great, sunny and dry. Thankfully, no one was hurt. If my wife was another foot to the right, she would have hit the support post for the upper balcony, and there would have been a good chance of the balcony falling on top of her. Or, if someone was standing in front of my wife when she entered the driveway, they could have seriously been hurt! There were no warning lights on prior to the accident. This happened last Sunday, April 19, 2026. We had the car towed to a local body shop on the same day as the accident. The vehicle was towed away the very next day to our insurance company's salvage lot in Bakersfield. I do have all the information, address and phone number, of the lot where our car was towed to. Ten months later, the same dealer presented this identical fault as a $1,121.07 customer-pay repair on a vehicle under active 10-year/100,000-mile powertrain warranty. The repair was only approved after I contacted Kia Consumer Affairs directly — the dealer had not contacted them despite claiming otherwise. The February 2026 repair failed within 15 minutes of pickup. I made five documented phone calls to the dealer over 33 days reporting the failure. No action was taken. The April 2026 repair — sensor and hose replacement — failed within one hour of pickup on April 1, 2026. Of additional concern: Recall SC288/SC288A (Fuel Tank Inspection and ECM Re-Update) was performed on this vehicle twice — July 2024 and August 2025. This caused electrical instability, warning lights, and vehicle failure. The battery was not adequately supporting the vehicle’s electrical system. The airbag and restraint system showed warnings caused by third-party wiring, indicating potential airbag or seatbelt malfunction. The fuel system was defective. The gas cap was broken and required tools to refuel the vehicle. Structural and body components were unsafe. A bumper bracket was loose and flapped while driving, and the passenger seat was unstable. Accessibility systems were also defective, including lift and door malfunctions affecting safe operation. United Access was notified immediately but did not correct these safety defects. Also, while in reverse the different camera views are unavailable because the screen won’t respond to your touches. This is a huge distraction while driving and makes for an unsafe driving experience. The dealer said years ago that they could replace the screen but the parts are on back order and it’s been months of no response from them every time we ask about a replacement unit. ... was improperly welded and mounted too low. This caused exhaust leaks, loud exhaust noise, poor acceleration, and safety concerns including potential exhaust exposure. Multiple professional repair facilities confirmed the exhaust system defect. The electrical system was defective. The vehicle was sold with the wrong size battery that was not properly secured. This caused electrical instability, warning lights, and vehicle failure. The battery was not adequately supporting the vehicle’s electrical system. The airbag and restraint system showed warnings caused by third-party wiring, indicating potential airbag or seatbelt malfunction. The fuel system was defective. The gas cap was broken and required tools to refuel the vehicle. Structural and body components were unsafe. A bumper bracket was loose and flapped while driving, and the passenger seat was unstable. Accessibility systems were also defective, including lift and door malfunctions affecting safe operation. United Access was notified immediately but did not correct these safety defects. Instead, they attempted to have the vehicle repaired using an extended warranty for pre-existing conditions. The vehicle was not safe to operate at the time of sale.Read more

The most severe symptom occurred when the car completely shut off on the highway which is very dangerous. Toyota Dealer verified the issue and informed me that the has DTCs for Torque Converter failure. Replaced Transmission at a cost of $10,150. . I respectfully request that Toyota consider offering a goodwill reimbursement for the repair of the transmission for my vehicle. Provide guidance to owners of similar vehicles to prevent accidents or injuries. ... mpted to turn the steering wheel as a truck was coming at me. The steering wheel would not turn or move at all. I was stuck in the middle of an intersection after it rolled to a stop. I turned on my flashers and looked at the dashboard to make sure my truck was in drive, which it was. A message was also displayed on the dashboard. It said accessory mode. I could hear my truck running. I cleared the message by pressing the ok button on the steering wheel. I put it in park and then into drive and the same message displayed again on the dashboard. I decided to turn off my truck and pressed the push button to turn it off (my foot was off the brake as I don’t need to press the brake to turn my truck off) and the starter engaged and I put the truck in drive and was able to press the brake and turn the steering wheel. I slowly drove the short distance home and called my husband which he instructed me to call Evansville Ford in Evansville, WI and had my truck towed there. Obviously, not having steering or brakes is a HUGE safety issue. Less than 24 hours earlier I had my entire family in that truck driving on the interstate. I can’t image that we would be alive if I had lost brakes and steering on the interstate. As of Dec 19th 2025, Evansville Ford has stated that they cannot find any issues, nor replicate the problem and returned my truck. I did not expect this issue to occur again because at the time of the previous problem, the dealership confirmed that issue was the timing chain and the VVT system, and replaced those components with new parts. If I were in a high-traffic area, I'm not confident that I would have been able to accelerate, stop, or safely pull over to the side of the road without putting myself and/or someone else in harms way. DTN Air Bag Inflator Rupture
The Office of Defects Investigation (ODI) has become aware of eight vehicle crashes in which a rupture of a Jilin Province Detiannuo Automobile Safety System Co., Ltd. (DTN) air bag inflator occurred during the deployment of the driver side air bag. All eight drivers in these crashes sustained serio
... us or fatal injuries that appear linked to the ruptures. Six of those drivers sustained fatal injuries and two sustained severe injuries. In all eight incidents, the subject inflators had been installed as replacement equipment after the vehicle was involved in a previous crash. Information gathered during NHTSA’s initial analysis of these incidents indicates that the inflators were potentially imported illegally into the United States. In conjunction with this safety investigation, NHTSA is also working with appropriate authorities to address any illegal activities associated with the importation of these inflators. As a result of the significant safety concerns relating to these inflators, as well as extensive information already obtained by ODI relating to their field ruptures, ODI is moving directly to the Engineering Analysis (EA) stage of its investigative process rather than first opening a Preliminary Evaluation. NHTSA has taken a variety of actions to address the risks posed by the importation of substandard or counterfeit air bag components. However, until recently, these risks appeared spread across a variety of components and manufacturers. As part of ODI’s continuous monitoring of this issue, the agency has obtained information to suggest that at least eight incidents involve ruptures of inflators manufactured by DTN. In parallel with this dedicated investigation into DTN inflators, ODI will continue to monitor reports of field incidents to evaluate risks from other substandard imported air bag components. In 2012, ODI first became aware of substandard or counterfeit air bag modules sold through online marketplaces, such as auction sites. NHTSA obtained examples of these air bag modules and conducted testing. None of the inflators equipped in these modules were manufactured by DTN. This testing revealed that the inflators consistently malfunctioned during deployments, such as by failing to deploy the air bag or expelling metal shrapnel due to a rupture. This led NHTSA to release a consumer advisory in October 2012 to alert the public and repair professionals to the potential dangers of substandard or counterfeit air bag modules. ODI also worked with major vehicle manufacturers, who released advisories and notifications concerning the use of such air bag modules. Since then, NHTSA has continuously monitored for reports of malfunctioning air bags that may be associated with substandard or counterfeit components. These monitoring efforts have been supported by the increased visibility the advisories have brought to the issue, and by several standing general orders in place as part of separate investigations into Takata and ARC inflators, which require certain manufacturers to report any alleged or suspected inflator field ruptures to NHTSA. Until 2017, ODI was not aware of any field incidents involving an abnormal deployment of an imported substandard or counterfeit air bag module. In September 2017, ODI became aware of a crash involving a model year (MY) 2009 Honda Civic where the air bag ruptured during deployment, killing the driver. After inspecting the vehicle and air bag parts, ODI determined that the air bag inflator was not original equipment and that the air bag module was likely a substandard import. Insufficient information existed to determine the manufacturer of the inflator in this incident, however, it does not appear to resemble those manufactured by DTN. On June 16, 2023, ODI received a Vehicle Owner Questionnaire (VOQ #11527380) alleging that the rupture of a driver side air bag caused fatal injuries to the driver of a MY 2020 Chevrolet Malibu. ODI’s assessment of the rupture indicated that the air bag inflator was not original equipment but was instead manufactured by DTN, a Chinese company (also known as Yoshibayashi Toku theanol Automotive Safety Systems Co. Ltd.). From June 2023 to July 2024, ODI became aware of four additional ruptures involving substandard air bag modules equipped on Chevrolet Malibu vehicles. At the time, insufficient information existed for these crashes to determine who manufactured the ruptured inflators. As a result, NHTSA released a consumer advisory on July 10, 2024, alerting car buyers and owners to be aware of cheap, substandard replacement air bag inflators that can cause death or serious injury in a crash. The advisory urged consumers who own or are considering the purchase of a used vehicle to learn the vehicle’s history and ensure the vehicle has genuine air bag inflators. In March 2025, NHTSA learned about another rupture involving a suspected substandard, aftermarket inflator that was equipped in a MY 2017 Hyundai Sonata. The driver of this vehicle sustained fatal injuries that appeared related to the rupture. In August 2025, NHTSA received a similar report of a fatal air bag rupture in a MY 2019 Hyundai Sonata. Most recently, in October 2025, ODI received a report of a fatal air bag rupture in a MY 2020 Chevrolet Malibu. Photographs of the air bag components in these three crashes indicated that the ruptures all involved inflators manufactured by DTN. Further investigation of the inflator fragments in three of the prior incidents confirmed that they also involved DTN inflators. Photographs of the components involved in one of the other incidents also strongly suggest that the ruptured inflator was manufactured by DTN. Turn Signal Lamps

RL.The DRL is illuminated whenever the headlamps are off, increasing the duty cycle of the front turn signal bulb.The MY 2003 and earlier Malibu vehicles (Malibu Maxx was not manufactured for sale in the U.S. prior to MY 2004) used a headlamp based design, operated at a reduced voltage, for the DRL function; GM reverted back to this design on the MY 2006 and later Malibu and Malibu Maxx vehicles. Filament based bulbs have a finite life (hours of illumination) for any given operating voltage.As a result, increasing the duty cycle of a bulb typically decreases the life span of the bulb.GM identified and addressed two front turn signal/DRL and rear turn signal bulb quality issues early in MY 2004 production, however vehicle warranty claims for failed bulbs continued to occur.To address the high warranty levels of bulb failures (90% of the warranty claims submitted to ODI by GM were bulb failures and 10% were socket failures), GM issued a technical service bulletin in March 2006 advising dealers to use a new specification of bulb when addressing service failures, and to also inspect the electrical sockets for thermal damage and replace if necessary.In June 2006 (after subject vehicle production ended) GM changed the turn signal bulb specification to that of the TSB bulb in vehicle production also.To date, ODI has not identified any allegations of crash, injury, or fire related to the alleged defect even though the subject vehicles have experienced 5 - 6 years of field exposure.ODI's analysis of consumer complaints, both VOQ and manufacturer, shows a declining trend.The warranty rate for turn signal bulb replacement is high however statistical analysis of the warranty data suggests that the majority of the original equipment bulbs have already been replaced.GM conducted a safety recall (06V-263) for MY 2004-2005 Cadillac XLR to replace the front turn signal/DRL bulbs and the electrical sockets.However GM advised that the turn signal/DRL bulbs/sockets on the subject vehicles differ from those on the recalled XLRs in several respects, including using bulb sockets with four electrical terminals as opposed to three, which improves the electrical connection, allows the bulb to operate at a lower temperature, and adds stability to the bulb in the socket; operating at a lower voltage, which increases vehicle bulb life; using vented headlamps, which allows bulbs to run cooler than those installed in a non-vented XLR headlamp; and using a clear bulb, which runs cooler than the amber bulbs installed on the XLR.Additionally, the warranty claim level related to subject vehicle sockets is unremarkable compared to other defect investigations into defective socket assemblies.A safety-related defect trend has not been identified at this time.Further use of agency resources does not appear to be warranted.Accordingly, this investigation is closed.The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist.The agency will take further action if warranted by the circumstances.See attached report for further information.Read more

... APPARENTLY ORIGINATE IN CORROSION PITS FOUND BENEATH REGIONS OF DAMAGED EPOXY COATING.CONSUMER COMPLAINTS AND WARRANTY DATA SHOW THAT THE FAILURES OCCUR PREDOMINANTLY IN THE ?CORROSION STATES? THE INVESTIGATION, IT WAS FOUND THAT THE FAILURES WERE THE RESULT OF FRACTURE OF THE PASSENGER-SIDE WINDSHIELD WIPER PIVOT HOUSING (SUBJECT COMPONENT).SUCH A FRACTURE LED TO AN IMMEDIATE AND IRRECOVERABLE LOSS OF WINDSHIELD WIPER FUNCTION.GMREPORTED THAT THE MATERIAL COMPOSITION (50% GLASS-REINFORCED ACETAL) OF THE SUBJECT COMPONENT WAS INSUFFICIENT TO WITHSTAND STRESSES IMPOSED BY CERTAIN FROZEN PRECIPITIATION ACCUMULATIONS.THE SUBJECT COMPONENT MATERIAL WAS CHANGED TO THE STRONGER 50% GLASS-REINFORCEDNYLON-6, EFFECTIVE JANUARY 98.OBSOLETE SERVICE PART WERE DISCARDED.GM WILL REPLACE THE SUBJECT COMPONENTS IN 129,427 SUBJECT VEHICLES FOUND IN 28 STATES (AK, CO, CT, DE, ID, IL, IN, IA ME, MD, MA, MI, MN, MT, NE, NH, NJ, NY, ND, OH, PA, RI, SD, UT, VT, WV, WI, WY), THE DISTRICT OF COLUMBIA, AND ALL CANADIAN PROVINCES.THESE STATES ACCOUNTED FOR 91% OF THE CONSUMER COMPLAINTS IDENTIFIED ABOVE.ODI WILL MONITOR THIS COMPAIGN FOR ADEQUATE EFFCTIVENESS AND, IF NECESSARY, TAKE FURTHER ACTION IN THE FUTURE.FURTHER EXPENDITURE OF AGENCY RESOURCES ON THIS MATTER IS UNJUSTIFIED AT THIS TIME.THE ENGINEERING ANALYSIS IS CLOSED.Read more
