Turbo cooler and turbocharger failure, diagnosis and replacement on February 4 and February 17, 2026 by a certified GM dealership. Replacement of these parts was not included in service bulletin N222372590 (issued 12/22/22), which covered 100% replacement for the exact parts for the exact engine for
... a number of other GM vehicles manufactured from 2016-2022. The total repair cost for these two part replacements was $4,951.11 for a car that was purchase just two months prior for $17,000. A great disappointment! Code for issue in both cases was P0299. The failed parts resulted in significant engine power loss, which made it unsafe to drive as acceleration away from intersections and on entering the highway was poor!Read more

The vehicle is experiencing an apparent fuel-system malfunction. The vehicle has a very strong raw gasoline odor, significant difficulty starting, and appears to be consuming fuel at an abnormally high rate. After filling the fuel tank completely, I drove approximately 20 miles at most and the fuel
... gauge decreased by approximately one-quarter of a tank. The engine oil also has a strong gasoline odor, indicating that fuel may be entering the engine oil. The exact failed component has not yet been diagnosed. The vehicle is available for inspection upon request. The strong raw gasoline odor and apparent rapid loss of fuel create a concern for a fuel leak and potential fire hazard. The vehicle also has difficulty starting and shuts the engine off when the automatic stop/start system activates while stopped at a traffic light. This creates a safety concern because the vehicle may not restart normally when traffic begins moving. I am concerned that the vehicle may have a fuel-system defect similar to the high-pressure fuel pump/fuel-line issue previously recalled on certain 2016-2018 Chevrolet Malibus. No. The problem has not yet been diagnosed or confirmed by a dealer or independent service center. The symptoms have been personally observed while operating the vehicle. This was my first step as this happened at 10:30pm. No. The vehicle has not yet been inspected by the manufacturer, police, insurance representatives, or another official representative. I am reporting the problem because of the severity of the symptoms and the potential safety risk. There were no warning lamps or messages that I noticed before the problem occurred. The first symptoms were a very strong gasoline odor, unusually rapid fuel loss, and significant difficulty starting the vehicle. The vehicle also shuts off completely when the automatic stop/start system activates while stopped at a traffic light. These symptoms began recently.Read more

I have a 2017 Chevy Malibu that in 2022 had a shift malfunction causing it to become stuck in reverse and roll backwards. It ended up having to be towed and was repaired in May 2022. On August 25 and 26, 2026 the car is now experiencing another SHIFT TO PARK malfunction whereby the vehicle DOES NO
... T detect that the shifter is in the park position. It shows a "shift to park" error on the car's dashboard and will not let me turn the car off until it detects it is in park which takes multiple times of shifting and cycling the car through the gears or repeatedly jiggling the gear selector. This is a temporary workaround and NOT a safe or satisfactory solution as this can cause a serious or even fatal accident! This Malibu seems to have a continuing problem with the shift malfunctioning and GM needs to recognize this is a continual problem and resolve it by recalling this issue AGAIN before someone is hurt or killed. I am requesting GM repair this at their expense or replace my vehicle as it seems to be a LEMON!Read more

While driving my 2017 Chevrolet Malibu LT equipped with the 1.5L turbocharged engine, the brake vacuum pump failed due to the pump’s mechanical drive/tang shearing. The failure resulted in loss of vacuum assistance to the power brakes, creating a significantly harder brake pedal and a serious safe
... ty concern because braking assistance can be reduced and stopping may require substantially more effort. After removing the failed pump, I found that part of the pump’s drive mechanism had sheared off and had to be retrieved from the camshaft area. I am retaining the failed pump and broken component. I am requesting that this failure be investigated as a potential safety defect because a failure of a component responsible for brake-assist vacuum can affect the vehicle’s ability to stop safely.Read more

(Transmission and motor) I had taken my car to the dealership on XXX for recalls and all the signal were fine before going there. They took my car back to do the recalls and my son and I waited . Once they were done we left closer to closing time is noticed there was a burn mark on my dash board tha
... t wasn’t there before the recalls. Going forward on the way home. We were waved down by a man stating that my signal and brake light weren’t working . So that next day we took my car back to the dealership to see what happen, why all the sudden different alerts were coming up. They had the same mechanic relook at the car and a when finish 31/2 hour latter was told I needed a new transmission, costing $7384.94 which I don’t have . They charge me $179.00 just to confirm what they said about needing a new transmission. I was devastated and confused why I wasn’t told that the day before. So a few days later I took it to another machanic to see if the could see why the lights weren’t working and he ended up putting new bulbs in and got them working but also he put it in a diagnosis machine 2 of them and couldn’t get a reading at all and said I should take it back to find out why he couldn’t get a reading. I was so frustrated at that point I just was upset! I then had a friend of the family mechanic look at it and he said it wasn’t the transmission it was a knock sensor that I needed so I ordered it. When I called him to install the knock sensor he was too busy to do it and it had already been a whole month of waiting so I reached out to a place in Cumberland RI to see if he could install the knock sensor had it towed there and he kept it for three days and said it was a transmission computer and he tried two of them and neither of them worked and charged me $150 for that I was humiliated and crying , I went there to have my car towed home seen him while waiting for the tow truck to come he said I needed a new transmission and now a motor, crying again. 2 incidents of Sudden Unintended Acceleration. No warning lights etc in either. Car damaged but available for inspection. Now encountering difficulties initating Toyota investigation. 1st incident (4/18/25; mileage 967) occurred after a hard left turn. After turn onto uphill street, I took my foo
... t from accelerator to slow and maintain distance from a car backing down hill ahead. Instead, car self-accelerated and shot uphill, over speed bump with increasing speed. Did not respond to braking until it was close to hitting a parked car. Stopped with a slam; probably brake assistance engaged. No damage or injury, but I was shaken. Took car immediately to Toyota dealer to inspect. Was told RoB confirmed an event but not its nature or cause; identified nothing wrong. They hypothesized cruise control might have been triggered accidentally (no evidence, not plausible). Claimed they submitted report to Toyota national center. After 2nd incident, Toyota national said they had no record of 1st event. Their standard safety inspection protocols were not followed. Returned car to me without repair or follow up. 2nd incident ensued. 2nd self-acceleration event (2026) also occurred after hard left turn to make U turn at low speed, braking while completing turn. When I took my foot off brake, before I could press it on accelerator, car shot diagonally across street. Jumped curb and traveled at considerable speed 100' up landscaped slope. Destroyed 6 large shrubs and 3 trees 9' tall. Did not stop despite constant driver braking. Forward collision prevention system did not engage to prevent uprooting trees. Car stopped only after I turned steering wheel sharply to left to avoid hitting a full-size tree trunk. Then car responded to braking, acceleration ended, car stopped next to tree without impact. Terrifying life-threatening event but only damaged landscape and car. Police came, assessed incident, determined not a police matter, did not file police report.Read more

A few weeks ago i noticed that a car passed me and the little blind spot alert light didn't light up. when i arrived at my destination I did some scanning through the menus and i discovered that the blind spot (side assist) was not on, but when i attempted to turn it on, the car said "function not
... available". After some investigation (using a odbeleven scanner) i discovered that the blind spot assist functionality has a fault with "no communication". This system, upon failure, give NO indication anywhere, plus this also means my rear back up alert ( a kid running behind my car, for example, which would normally cause a huge alert, nothing happens). I also learned that there is a technical bulletin : [XXX] related to this. This should be a recall, its dangerous to have no indication that a safety feature system like this is completely nonfunctional (without an ODBeleven read). a light on the dash should alert me, or a message on the screen. this is just plain dangerous! Since the release of V14, the Full Self-Driving (FSD) system automatically defaults to operating approximately 7 MPH above the posted speed limit. This occurs on all road types, including city streets and highways. When FSD is engaged, the vehicle sets the cruising speed above the detected speed li
... mit without driver input. There is no setting that allows the driver to enforce strict compliance with the posted limit. Available speed settings only permit equal or greater deviation above the speed limit. The driver cannot manually lock or command the system to remain at or below the posted limit while FSD is active. The only way to prevent speeding is to fully disengage FSD. This is not a malfunction. The vehicle is operating as designed under V14. This creates a safety concern because: 1.The system is programmed to exceed legal speed limits by default. 2.The driver does not retain meaningful authority to ensure lawful operation while automation is engaged. 3.Automatic speeding increases crash risk in construction zones, school zones, and changing speed areas. 4.Drivers may receive citations despite not intentionally selecting an unlawful speed. Because this behavior was introduced via V14 software, it likely affects all Tesla vehicles running FSD V14, not just this vehicle.Read more

While driving on a highway, the vehicle intermittently experienced a sudden loss of engine power during acceleration, accompanied by a flashing check engine warning light. The most serious event occurred while merging into traffic, when the vehicle failed to accelerate normally and briefly lost powe
... r, creating a risk of being struck by faster-moving vehicles. After pulling over and restarting the vehicle, normal acceleration temporarily returned, but the problem has recurred multiple times. The malfunction appears to be related to an engine misfire or powertrain fault because the check engine light flashes during the events and an OBD scan has shown misfire-related diagnostic codes (random/multiple misfire and a cylinder-specific misfire). The condition is intermittent and typically occurs after the vehicle has been driven for approximately 10–45 minutes. The vehicle remains available for inspection. The vehicle has been presented to authorized service departments multiple times for diagnosis. The dealerships have reported that they were unable to duplicate the issue during road testing and no lasting repair has been completed. The manufacturer has been contacted and the vehicle has been inspected/diagnosed through authorized service channels, but the underlying cause has not been identified. The failure continues to occur intermittently and is a safety hazard because it affects acceleration in traffic. Incident list 10/25/2025 (~12:25 PM; ~3,108 miles) — While accelerating to merge onto a highway, the vehicle lost power and the check engine light flashed. The driver pulled over, shut the vehicle off, restarted, and the condition temporarily resolved, allowing the trip to continue. 10/27/2025–11/04/2025 — The same loss-of-power/misfire behavior recurred intermittently, typically lasting a few minutes before clearing. 11/05/2025 (~8:01 AM; ~3,641 miles) — The check engine light flashed again and symptoms consistent with misfire/loss of power were present. I'm writing this for my father because he might not have the technical capacity to do this. He is [XXX] old. He bought a car last year brand new from Toyota. It is a Hybrid Camry that has some options that allow the car to follow traffic, lane keep etc. On [XXX] he had a Hybrid System Malfunction t
... hat caused the car to accelerate while at a traffic light ready to turn left. The acceleration caused his car to collide with the car in front of him. He tried to press the brakes but was not able to do it fast enough. The car in front had no damage. His car has a lot of damage. The other driver drove off. His car was damaged so much that it was not drivable. There was radiator fluid on the road. This was on [XXX]. He took the car in immediately after back to the Toyota dealership. The car is still there. It has been almost 40 days now with no resolution. No car, no rental. The dealership is saying Toyota (the headquarters has to deal with it), and Toyota did a 3rd party inspection on Nov 11, 2025. And now they are saying they will do another Toyota inspection between 30 and 45 business days from the initial inspection. So that takes us into mid January 2026 with my dad stranded with no car or having to uber or pay out of pocket to rent a car. On top of that, the dealership now has told my dad he has to get the car off their lot or start accumulating fees. They performed an EDR report. Attachin below. I am also attaching several pictures of the error messages I got including proximity notification system malfunction. And other documents. This vehicle is affected by NHTSA Recall 25V644 (starter motor – fire risk). The recall remedy has been listed as “Remedy Not Available” since September 25, 2025, and remains unavailable more than two months later. BMW did not notify me about this safety recall. I only discovered it after a t
... hird-party platform automatically deactivated the vehicle due to the open recall. In addition, this vehicle has recently developed starting issues — it now struggles to start and requires multiple attempts. This may indicate that the defective starter component described in the recall is already failing. Combined with the stated fire risk, this creates a serious daily safety concern. BMW advises owners to park outside and away from structures, but provides no remedy, no timeline, and no assistance. Multiple inquiries to BMW Recall Support were answered with the same message: there is no known timeframe for parts or repair. I am filing this complaint because the manufacturer has not provided a remedy, has not communicated adequately, and the defect appears to be progressing in a way that increases the risk described by NHTSA. I respectfully request that NHTSA review the extended delay, lack of remedy, lack of notification, and the presence of active symptoms related to this safety defectRead more

DTN Air Bag Inflator Rupture
The Office of Defects Investigation (ODI) has become aware of eight vehicle crashes in which a rupture of a Jilin Province Detiannuo Automobile Safety System Co., Ltd. (DTN) air bag inflator occurred during the deployment of the driver side air bag. All eight drivers in these crashes sustained serio
... us or fatal injuries that appear linked to the ruptures. Six of those drivers sustained fatal injuries and two sustained severe injuries. In all eight incidents, the subject inflators had been installed as replacement equipment after the vehicle was involved in a previous crash. Information gathered during NHTSA’s initial analysis of these incidents indicates that the inflators were potentially imported illegally into the United States. In conjunction with this safety investigation, NHTSA is also working with appropriate authorities to address any illegal activities associated with the importation of these inflators. As a result of the significant safety concerns relating to these inflators, as well as extensive information already obtained by ODI relating to their field ruptures, ODI is moving directly to the Engineering Analysis (EA) stage of its investigative process rather than first opening a Preliminary Evaluation. NHTSA has taken a variety of actions to address the risks posed by the importation of substandard or counterfeit air bag components. However, until recently, these risks appeared spread across a variety of components and manufacturers. As part of ODI’s continuous monitoring of this issue, the agency has obtained information to suggest that at least eight incidents involve ruptures of inflators manufactured by DTN. In parallel with this dedicated investigation into DTN inflators, ODI will continue to monitor reports of field incidents to evaluate risks from other substandard imported air bag components. In 2012, ODI first became aware of substandard or counterfeit air bag modules sold through online marketplaces, such as auction sites. NHTSA obtained examples of these air bag modules and conducted testing. None of the inflators equipped in these modules were manufactured by DTN. This testing revealed that the inflators consistently malfunctioned during deployments, such as by failing to deploy the air bag or expelling metal shrapnel due to a rupture. This led NHTSA to release a consumer advisory in October 2012 to alert the public and repair professionals to the potential dangers of substandard or counterfeit air bag modules. ODI also worked with major vehicle manufacturers, who released advisories and notifications concerning the use of such air bag modules. Since then, NHTSA has continuously monitored for reports of malfunctioning air bags that may be associated with substandard or counterfeit components. These monitoring efforts have been supported by the increased visibility the advisories have brought to the issue, and by several standing general orders in place as part of separate investigations into Takata and ARC inflators, which require certain manufacturers to report any alleged or suspected inflator field ruptures to NHTSA. Until 2017, ODI was not aware of any field incidents involving an abnormal deployment of an imported substandard or counterfeit air bag module. In September 2017, ODI became aware of a crash involving a model year (MY) 2009 Honda Civic where the air bag ruptured during deployment, killing the driver. After inspecting the vehicle and air bag parts, ODI determined that the air bag inflator was not original equipment and that the air bag module was likely a substandard import. Insufficient information existed to determine the manufacturer of the inflator in this incident, however, it does not appear to resemble those manufactured by DTN. On June 16, 2023, ODI received a Vehicle Owner Questionnaire (VOQ #11527380) alleging that the rupture of a driver side air bag caused fatal injuries to the driver of a MY 2020 Chevrolet Malibu. ODI’s assessment of the rupture indicated that the air bag inflator was not original equipment but was instead manufactured by DTN, a Chinese company (also known as Yoshibayashi Toku theanol Automotive Safety Systems Co. Ltd.). From June 2023 to July 2024, ODI became aware of four additional ruptures involving substandard air bag modules equipped on Chevrolet Malibu vehicles. At the time, insufficient information existed for these crashes to determine who manufactured the ruptured inflators. As a result, NHTSA released a consumer advisory on July 10, 2024, alerting car buyers and owners to be aware of cheap, substandard replacement air bag inflators that can cause death or serious injury in a crash. The advisory urged consumers who own or are considering the purchase of a used vehicle to learn the vehicle’s history and ensure the vehicle has genuine air bag inflators. In March 2025, NHTSA learned about another rupture involving a suspected substandard, aftermarket inflator that was equipped in a MY 2017 Hyundai Sonata. The driver of this vehicle sustained fatal injuries that appeared related to the rupture. In August 2025, NHTSA received a similar report of a fatal air bag rupture in a MY 2019 Hyundai Sonata. Most recently, in October 2025, ODI received a report of a fatal air bag rupture in a MY 2020 Chevrolet Malibu. Photographs of the air bag components in these three crashes indicated that the ruptures all involved inflators manufactured by DTN. Further investigation of the inflator fragments in three of the prior incidents confirmed that they also involved DTN inflators. Photographs of the components involved in one of the other incidents also strongly suggest that the ruptured inflator was manufactured by DTN. This opening resume exceeded space limitations and is continued in an attachment uploaded to the investigation.Read more

Turn Signal Lamps
GM designed the MY 2004 and 2005 Chevrolet Malibu and Malibu Maxx vehicles with a daytime running lamp (DRL) system that uses the same filament (the high power filament of a dual filament bulb) as the front turn signals. The high power filament operates at full battery voltage when functioning as a D
... RL.The DRL is illuminated whenever the headlamps are off, increasing the duty cycle of the front turn signal bulb. The MY 2003 and earlier Malibu vehicles (Malibu Maxx was not manufactured for sale in the U.S. prior to MY 2004) used a headlamp based design, operated at a reduced voltage, for the DRL function; GM reverted back to this design on the MY 2006 and later Malibu and Malibu Maxx vehicles. Filament based bulbs have a finite life (hours of illumination) for any given operating voltage. As a result, increasing the duty cycle of a bulb typically decreases the life span of the bulb.GM identified and addressed two front turn signal/DRL and rear turn signal bulb quality issues early in MY 2004 production, however vehicle warranty claims for failed bulbs continued to occur. To address the high warranty levels of bulb failures (90% of the warranty claims submitted to ODI by GM were bulb failures and 10% were socket failures), GM issued a technical service bulletin in March 2006 advising dealers to use a new specification of bulb when addressing service failures, and to also inspect the electrical sockets for thermal damage and replace if necessary. In June 2006 (after subject vehicle production ended) GM changed the turn signal bulb specification to that of the TSB bulb in vehicle production also.To date, ODI has not identified any allegations of crash, injury, or fire related to the alleged defect even though the subject vehicles have experienced 5 - 6 years of field exposure.ODI's analysis of consumer complaints, both VOQ and manufacturer, shows a declining trend. The warranty rate for turn signal bulb replacement is high however statistical analysis of the warranty data suggests that the majority of the original equipment bulbs have already been replaced.GM conducted a safety recall (06V-263) for MY 2004-2005 Cadillac XLR to replace the front turn signal/DRL bulbs and the electrical sockets. However GM advised that the turn signal/DRL bulbs/sockets on the subject vehicles differ from those on the recalled XLRs in several respects, including using bulb sockets with four electrical terminals as opposed to three, which improves the electrical connection, allows the bulb to operate at a lower temperature, and adds stability to the bulb in the socket; operating at a lower voltage, which increases vehicle bulb life; using vented headlamps, which allows bulbs to run cooler than those installed in a non-vented XLR headlamp; and using a clear bulb, which runs cooler than the amber bulbs installed on the XLR.Additionally, the warranty claim level related to subject vehicle sockets is unremarkable compared to other defect investigations into defective socket assemblies.A safety-related defect trend has not been identified at this time.Further use of agency resources does not appear to be warranted.Accordingly, this investigation is closed. The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist. The agency will take further action if warranted by the circumstances.See attached report for further information.Read more
