When it shut down, a quick message flashed across the dash board--stating ability to shift had been lost (or something to that effect)--the vehicle then flipped into neutral and immediately lost power. My safety was put at great risk due to the fact that the entire vehicle shut down moments after I crossed over a very busy railway track. The vehicle had to be towed to a nearby Chevrolet dealership where it has been confirmed there was total engine failure as well as blown main electrical fuse.Read more

This is an extremely common issue plaguing Hyundai & Kia vehicles with the Theta III 2.5L Turbo engine, Such as Kia K5 GT, Hyundai Sante Fe, Hyundai Santa Cruz, & Hyundai Sonata N-Line. Its a failing ITM (integrated thermal management) unit or otherwise known as an ITMS unit. There is a rod within
... the ITM that prevents proper control of the flow of cooling the oil and engine coolant of the engine. Results in Cooling issues, such as a cold lower radiator hose while the engine is hot, or 'limp mode' during acceleration, that causes a sudden loss of power while trying to merge on the expressway. There has been other Kia models and years that have had TSB's related to fixing this issue, but Hyundai has released nothing. I have uploaded an example of one of the TSB's that do not apply to my car. Technical symptoms often include: Fans running 100% full duty (high) while the lower radiator hose stays cool. Or Fuel dilution or a 'rich' running engine. Because the engine is overcooling that keeps the car's fueling rich and in a open loop mode affecting emissions and contaminating oil. Personal Details of exactly what happened when i noticed mine was failing. I ran home on my lunch break, and while accelerating on a ramp to get up on a highway, my car started beeping and flashing "Engine Overheated" and i immediately let off the gas with preparation to pull over. The second i left off the gas, the coolant temp within 1-2 seconds went from a little over 3/4 the way up the gauge back to normal a little below half temp. I resumed driving a 3 mile drive back to work, with it showing normal temps the rest of the way. I popped the hood when i got to work to see if coolant was low, and it was completely normal, a little over halfway between the min/max lines. The car continues to run hot, often with the cooling fans running at 100%.Read more

The independent mechanic attempted to start the vehicle, however the vehicle failed to start. The independent mechanic inspected the vehicle and noticed that the gateway control modular was moist. The contact was informed that the gateway control module had become moist because of the broken seal, the vehicle shut down. The contact stated that the contact's wife had spilled a liquid substance into the vehicle. In addition, the contact was provided pictures of the corrosion. The contact referenced an unknown recall which had a similar failure description, however the contact's VIN was not associated. The dealer was made aware of the failure. The contact was informed that the repair was not covered under the recall. In addition, the contact was informed that the unknown recall only covered the seal. The contact was informed that the recall did not cover the replacement of the gateway control modular. The manufacturer was made aware of the failure and a case was filed. The contact was informed that the repair of the corroded seal was covered under the recall. The contact was advised to take the vehicle to the dealer for diagnostic testing. The failure mileage was approximately 103,000. VIN was unavailable.Read more

On March 21st, while driving my Hyundai vehicle, I noticed the vehicle began making a ticking sound. After stopping the vehicle, it slowly began catching on fire. The fire ultimately spread and resulted in the vehicle becoming a total loss. This created a serious safety concern and placed me at risk
... . I later learned there was a recall associated with the vehicle. However, I never received any recall notices because Hyundai reportedly had the wrong address on file. Had I been properly notified of the recall and potential safety risks, I would have done everything in my power to ensure the vehicle was inspected, repaired, and safe to drive. Because I was unaware of the recall issue, the vehicle continued to be operated without my knowledge of the potential danger. The incident caused significant financial and transportation hardship, including loss of reliable transportation and additional financial strain. Hyundai later reached a determination regarding the matter, but I remain concerned about the serious safety risk and the fact that I did not receive proper recall notification before the incident occurred. I am submitting this complaint because I believe the recall notification process and handling of this matter should be reviewed further due to the serious safety risk involved.Read more

Unknown. This was a certified preowned vehicle that was purchased on December 18, 2024. We have had NUMEROUS issues with the car since purchase but the most concerning has been the door actuators. The front door actuators on both sides failed in less than a year from date of purchase. I was told
... by the dealership where it was repaired that this was a common problem and to expect the rear ones to not be far behind. 4 months later, the rear ones are now not working. This creates a safety hazard, especially since this is the primary vehicle that my teenager drives with his younger brother. A malfunctioning door lock actuator can create dangerous situations, including the possibility that a door may fail to unlock or open, potentially trapping passengers in the vehicle during an emergency or out of the vehicle when my underage children are by themselves. Conversely, the door may appear to be locked when the mechanism has not actually engaged, leaving the vehicle vulnerable to unauthorized access. Faulty actuators may also cause erratic locking or unlocking behavior while the vehicle is in motion, which can create a safety distraction for the driver as well as a security risk. There were no warnings that this was an issue. My son thought the doors were locking (and unlocking). I am aware GM is already "tacking" this issue as I could find stats on how many people have reported, so it is enough that they are taking notice.Read more

... rs, quick reaction time. Incident mid day, no pedestrians, distractions, no moving traffic. Vehicle doors shut, seat belt on. Not on incline. Pulled into parking spot I use weekly, spot has parking curb. Was at a full stop. Realized I had not pulled into the spot quite far enough, and attempted to inch vehicle forward. (Do not recall any warning lights, but this car usually alerts at literally anything - even shadows.) Very lightly applied gas to inch forward, slowly. Felt slight "scrape" of undercarriage on the parking curb. Put car in reverse, lightly applied gas to inch back. Car did not rev, no screeching tires, it was at a standstill previously. Was in Eco mode. All seemed normal, until vehicle suddenly launched UP the parking curb, full force, and into a building pillar directly ahead. The car never moves with that much force, I'm not a pedal-to-the metal driver. Eco mode on. No airbag deployment, injuries, etc. Sheriff took report for the building, simple incident. I couldn't make sense of it. I know how overly cautious I am while parking, what I do, and don't do. Took responsibility, but was still analyzing a week later, and explained what I remembered to the mechanic. He guessed right away. Said Sonatas were notorious for it. He said they're known for randomly applying the ebrake unexpectedly - and also randomly releasing the ebrake - and aggressively (and silently) switching from electric to gas when met with resistance, aka the parking curb. Insurance totalled the car. 2.5 years old, 17k miles. This was my 3rd Hyundai in 20 years, others were great. Discovered 40,000 Elantra hybrids, same year as my Sonata, were recalled due to "unintended acceleration."Read more

... ic powered seat movement. The second-row seat began reclining and moving without warning, and my wife’s head became trapped between the seatback and the side pillar as the seat continued to move. The powered seat did not immediately stop when resistance was encountered, creating a dangerous entrapment situation involving the head and neck area. The seat had to be manually stopped to free her. Fortunately, serious injury was avoided, but the situation posed a significant risk of head, neck, or crushing injury, especially in a family vehicle frequently used to access the third row. Safety Concern: The second-row automatic seat movement can be triggered accidentally and does not appear to have adequate obstruction detection or pinch protection. When someone is leaning into the third row (which is common when assisting children), the seat movement can create a severe head or body entrapment hazard. This appears to be a serious safety defect involving powered seat movement and inadequate obstruction detection.Read more

I begged them in the final days of me having the car to fix it and saying I didn’t feel safe and they wouldn’t without an extensive amount of money I could not provide . I was excited to finally hear there was a recall, only to find it was only for 2014 GMC Acadia's. The transmission have failed again and yes it is available for inspection at All Automation in Bakersfield, CA on Quantico. During the course of breaking down the my safety and the safety of my children was at risk. My vehicle was jumping, jerking the RPM's were all over the place and at one point in the course of driving on the freeway it felt like it lost power and nearly stopped. This has been an ongoing problem since the purchase of the vehicle. It has been inspected and diagnosed by All Automatic. There were no warnings, messages or symptoms that were constant. It was sporadically slipping up until the time it gave out. ... valve before I took it to the Toyota dealership for confirmation. SAFETY CONCERN: This is a known defect affecting multiple Toyota models from 2019-2023. Coolant bypass valve failures can lead to engine overheating, potential engine damage, and loss of vehicle control if the engine fails while driving. This creates a serious safety hazard. MANUFACTURER RESPONSE: Toyota issued Customer Support Program 24TE04 covering this exact defect for 2020-2021 Corollas and certain RAV4 models. However, they arbitrarily excluded 2022-2023 Corolla Cross vehicles despite using THE IDENTICAL DEFECTIVE PART (F0010 coolant bypass valve) in all gas models through early 2024. Toyota dealership charged me $99 for a diagnostic to confirm what was already known. The repair quote was nearly $1,000. Toyota Corporate denied goodwill assistance, stating they only provide it to "loyal customers" who have spent significant money on servicing or own multiple Toyotas. CONSUMER IMPACT: This is the same defect that led to the class action lawsuit Barrientos v. Toyota. Toyota settled privately and created CSP 24TE04, but deliberately excluded newer model years with the same defective part. Consumers with 2022-2023 vehicles are forced to pay $900-1,300 out-of-pocket for a manufacturing defect that Toyota has already acknowledged in other model years. REQUESTED ACTION: I am requesting NHTSA investigate why Toyota's Customer Support Program excludes 2022-2023 model years when they contain the identical defective coolant bypass valve. Toyota should be required to extend CSP 24TE04 to cover ALL vehicles with part #16671-F2010 or F0010, regardless of model year.Read more

ODI reviewed complaint data, available design, test, and field data on the model year (MY) 2
... 011 Hyundai Sonata, MY 2012-2013 Kia Forte and Forte Koup vehicles. ODI also evaluated data from the automotive air bag system supplier, ZF-TRW (ZF), to review vehicles that may contain the same air bag control unit (ACU) used in the subject vehicles. Hyundai Motor America (HMA) issued recall No. 18V-137 for MY 2011-2013 Sonata and MY 2012 Sonata Hybrid vehicles. Kia Motors America (KMA) issued recall No. 18V-363 for MY 2010-2013 Kia Forte and Forte Koup, MY 2011-2013 Optima, MY 2011-2012 Optima Hybrid, and MY2011-2012 Sedona vehicles. The two recalls address vehicles containing ACUs manufactured without adequate circuit protection by installing a wire harness filter kit to prevent internal damage to the ACU. In the event of a crash, ACU damage may result in the disablement of the vehicle's airbags. Each respective manufacturer has mailed letters to affected vehicle owners informing them of the remedy and its availability. ODI plans to evaluate the susceptibility of the subject ACU to electrical signals, as well as other vehicle factors that can either lead to, or reduce the likelihood of, an electrical overstress event in other vehicles manufactured with the subject ZF ACU.Accordingly, this Preliminary Evaluation has been upgraded to an Engineering Analysis (EA19-001), to expand the scope of the investigation to include the Tier-one supplier and any manufacturers who installed the subject ACU in production vehicles. ... dai) filed a defect report with the agency stating that it would conduct a safety recall on 977,778 model year 2010-2014 Sonata vehicles produced from December 11, 2009 through May 29, 2014 and model year 2011-2015 Sonata Hybrid vehicles produced from December 2, 2010 through January 9, 2015.The remedy for the recall was to inspect the front seat belt anchor connector for proper attachment to the seat belt pre-tensioner.ODI continued the investigation in order to pursue a remedy that would be more lasting, rather than a one-time inspection.The connector is designed so that it can be removed from the pre-tensioner, when required, during vehicle maintenance.If it is not properly connected when originally installed or when re-installed after maintenance, the connector could potentially release in a crash. On September 4, 2017 Hyundai filed an amended defect report and advised the agency that the remedy would be modified to provide a permanent label, to be attached to the connector, at the time of inspection.The label provides graphic instructions and illustration as to the proper orientation of the connector lock when it is properly installed.On October 4, 2017 Hyundai filed an additional defect report (see 17V-671) to have the vehicles that have had only theinspection procedure performed return to dealers to have the permanent label applied to the connector.ODI is closing this investigation based on Hyundai's recall action to add a permanent label to the connector. The EPB is released by the control switch or by selecting a drive gear with the transmission shifter when the ignition is on.If the EPB has not been used within 1000 kilometers (621 miles) the system is designed to perform an Automatic Adjustment Function (AAF) of the pads to check for proper clearance. This function is a pad adjustment only and is preformed with the vehicle in park with the ignition off. It does not set the parking brake or turn the brake indicator light on. Hyundai has identified an error within the EPB control logic that can cause the pads to remain in contact with the rotors at various levels of force after the AAF is completed. This condition could cause the rear brakes to drag at the next drive cycle and may generate some levels of heat and smoke. The EPB control logic error affects model year 2016 vehicles produced from the start of production through March 16, 2016.Vehicles produced from March 17, 2016 incorporate an updated EPB control logic to prevent brake pad drag. On May 11, 2016 Hyundai notified its dealer network of Service Campaign TFF to update the EPB control logic in the affected vehicle population. Dealers will reflash the EPB software to prevent brake pad drag.There are no reports of accidents or injuries as a result of this issue. The incident that alleged fire could not be confirmed as there were no repairs to the vehicle other than replacement of the rear brake rotors and pads.ODI is closing this investigation based on the manufacturer's Service Campaign of the affected vehicles and in consideration that the the effect of brake drag in this instance does not appear to present an unreasonable risk to motor vehicle safety. Seat Belt Buckle Electrical Malfunction
On July 22, 2014 the Office of Defects Investigation (ODI) opened a preliminary evaluation to further analyze allegations of seat belt pretensioner malfunctions on model year (MY) 2006-2008 Hyundai Sonata vehicles.ODI received 131 unique reports alleging a failure within the seat belt pretensioner s
... ystem, which was typically preceded by SRS light illumination.ODI requested more information from Hyundai in order to understand the nature of the failure and how the airbag deployment algorithm, the seat belt pretensioners, and/or the airbag function may be affected. In response to ODI?s information request (IR), Hyundai submitted 764 unique consumer and field reports and 19,847 warranty claims related to a failure in the front seat belt buckle assembly potentially causing a malfunction of the seat belt pretensioners.In it's response, Hyundai advised that if the resistance specifications for the seat belt buckle pretensioner circuit, located in the seat belt stalk, exceed the nominal range, a diagnostic trouble code will be set and the SRS warning indicator lamp will illuminate.The presence of this stored code will not affect the ability of the SRS Control Module (SRSCM) to detect a collision and command deployment of the pretensioners and airbags, if warranted.In fact, the wiring harness connector may be unplugged from the seat belt buckle pretensioner, and the signal to deploy the pretensioner will still be commanded and recorded.MY 2006-2008 Sonata?s utilize a dual pretensioner system by incorporating pretensioners into both the driver and front passenger seat belt retractor and the seat belt buckle assembly.The retractor pretensioner is located inside the B-pillar and winds (pretensions) the seat belt webbing back into the retractor housing during a collision, helping to remove seat belt slack and restrain the upper torso and hips.In contrast, the buckle pretensioner is located within the stalk of the seat belt assembly and acts to supplement lower limb protection.Both pretensioners are controlled by the SRSCM but utilize separate wiring and connectors.The seat belt retractor pretensioner circuitry and operation is independent of the seat belt buckle pretensioner connector and circuitry.Hyundai produced analysis indicating that increased resistance in the seat belt pretensioner which occurs in vehicle service does not result in a loss of buckle pretensioner.Hyundai further advised that in the unlikely event that a seat belt buckle pretensioner may not deploy due to increased resistance, the retractor pretensioner would still activate.Hyundai advised that the subject vehicles maintain their 5 STAR NHTSA safety rating using the retractor based pretensioner only.ODI notes that MY 2009-10 Sonatas reverted back to a single pretensioner system, and do not utilize seat belt buckle pretensioners. Hyundai advised its intention to initiate a warranty extension program to address concerns with the seat belt buckle prentensioner faults in MY 2006-2008 Hyundai Sonata vehicles.This action will extend the warranty coverage for this issue for 15 years or 250,000 miles, whichever occurs first.Given Hyundai's action and the minimal safety consequence this issue presents, further use of agency resources does not appear to be warranted.Accordingly, this investigation is closed.The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist.The agency will monitor the issue and reserves the right to take further action if warranted by the circumstances. The Office of Defects Investigation (ODI) is aware of 7 complaints and field reports and 10 warranty claims on the subject vehicles that allege the intermediate steering shaft became loose or in some cases separated from the upper steering column and/or the steering gearbox, causing (or potentially
... causing) the hand wheel to become mechanically de-coupled from the road wheels resulting in a complete loss of steering capability.The reported causes include excessive free play or separation of the intermediate steering shaft upper and/or lower universal joint cross shaft/yoke; missing, loose or damaged universal joint pinch bolts; improperly assembled intermediate steering shafts with misaligned splines or insufficient spline engagement preventing the universal joint pinch bolt from being secured in the retaining groove when fastened.On September 20, 2010, the manufacturer notified the agency that it would conduct a safety recall (NHTSA Recall No. 10V-426) to address the alleged defect in approximately 139,500 subject vehicles produced from December 11, 2009 through September 10, 2010.In addition to inspecting the steering shaft, the manufacturer will also update the electric power-assist steering control software to address complaints that the steering wheel vibrates or shakes as a result of a a motor driven power steering malfunction.The manufacturer began notifying affected owners by written letter on October 14, 2010.To date, 119,383 subject vehicles have been serviced under the recall campaign and 58 subject vehicles have received replacement parts as a result.Accordingly, this investigation is closed.Read more
