*The black water tank of our 2024 Forest River NoBo 20.4 was found to be leaking. Upon inspection, it was discovered that the factory installed tank heating pad applied to our black water tank suffered a severe electrical and structural malfunction. The carbon fiber heating element inside the pad wa
... s found to be bent, concentrating heat to an extreme degree. The element experienced a high-resistance thermal event, melting completely through the protective heating pad and burning a hole directly through the structural plastic of the black water tank it was applied to. * This created a severe fire hazard for two individuals who had been using the travel trailer at the time of the finding. *This issue is consistent with current NHTSA recalls on this component. RV manufacturers like Grand Design and WInnegago have already issued urgent Federal safety recalls and "Do Not Drive" advisories on vehicles equipped with ThermaHeat pads due to high resistance and severe terminal overheating. *There were no warning lamps or advisories from control panels to indicate the issue and potential danger.Read more

My display screen (infotainment) that has almost all the setting to my car on it has been glitching every single time I drive my car. It has what is being described as "ghost touch". It will just flick back and forth between all different settings to my car, turn on and off the radio and go between

I believe it is a major hazard, it as startled me so many time by just randomly blasting a radio station while im driving, messing with my navigation on maps while driving, I have been distracted so many times when I should have been focused on the road but instead I am trying to stop it from changing settings, calling people, trying to cancel routes its setting on my maps and ect. The problem is a "known issue" according to the Honda dealership I bought it from and took in to look at for repair. Also I quick Google search of "2022 honda accord sport screen glitching" brings up tons of other people dealing with this same issue and there is apparently no fix for it. Honda said you can buy a new screen but it may still do it since there is no official fix for this. 2018 Ford Escape SE 1.5L 4 cylinder EcoBoost engine. I was driving on a major 2 lane state road going 65 miles an hour when the warning light came on that the engine was overheating and pull over immediately. There was no prior warning. As soon as I stopped I looked at the temperature indicator and
... it returned to a normal level. I tried to drive again and got the same urgent message to stop. There was very little grass on the side of the road where I had to pull off and cars and trucks were passing us going 60-70 miles per hour. I had to call a tow truck to get the vehicle towed to the local Ford dealership. 3 hours sitting on the side of the road and did not get picked up for 3 hours in the dark. Ford ran a diagnostic test the next day and informed me in writing that the engine needed to be replaced due to coolant leakage into the engine block. I would also need the cabin coolant heater pump and the sparkplugs replaced. The total would be over $8,200. I had the vehicle in for service three months ago due to a check engine light and did the requested repair. The informed me now that it was for a different issue. Ford is not taking any responsibility for this known problem. I was the original owner and was never informed of the issue.Read more

... consistent and requires significantly more effort to stop the vehicle. This is a known internal fluid leak within the brake booster assembly that causes the accumulator pump to run continuously and ultimately reduces braking performance — a direct safety hazard. Why This Requires Federal Attention: This is not an isolated incident. Hundreds of 2013 Prius owners have reported identical symptoms, with associated diagnostic trouble codes C1391, C1252, C1256, and C1253. Toyota acknowledged this defect by issuing Technical Service Bulletin TSB-0089-20 and a Customer Support Program (ZJB) providing limited extended warranty coverage. However, that program was not well publicized . I was never informed as a vehicle owner . And it has now expired, leaving thousands of owners with a dangerous defect and repair bills costing thousands . A safety defect of this magnitude — one that can result in sudden, significant loss of braking ability — should not have an expiration date on its remedy. Toyota has not issued a mandatory recall, and owners continue to unknowingly drive vehicles with compromised brake systems. Requested Action: I respectfully urge NHTSA to: 1.Open a formal investigation into this defect across all affected 2013–2015 Toyota Prius model years 2.Require Toyota to issue a mandatory safety recall 3.Compel Toyota to cover repair costs for all affected owners regardless of mileage or age of vehicleRead more

While flat towing my 2022 Ford F‑350 behind a motorhome using Ford’s Neutral Tow Mode procedure, the vehicle suffered a catastrophic transmission failure. The Ford dealer documented this clearly on the service invoice: “HAPPENED WHILE THE VEHICLE WAS FLAT TOWED.” After the tow, the truck had
... severe slipping, delayed engagement, high RPM with no movement, and a wrench light. The dealer confirmed fault codes P0741, P1744, and P27B4. The dealer found catastrophic internal damage: “fluid is burnt with metal debris… filter is packed with metal debris… torque converter has came apart and filled transmission with metal debris… it is catastrophic failure.” These components are available for inspection. The dealer also documented a low battery condition: “batteries. low state of charge.” Ford’s Neutral Tow Mode relies on electronic control modules that require stable voltage, but Ford provides no warning that low voltage can cause the transfer case to exit Neutral Tow Mode. This creates a safety risk because the transmission can be rotated internally without lubrication while being towed, which can lead to sudden drivetrain lockup or loss of control. The problem was confirmed by the Ford dealer, and the vehicle was fully inspected by them. The only warning symptom occurred after towing: wrench light, slipping, and no movement. There were no warnings during the tow itself. This failure mode is not disclosed by Ford and cannot be detected or prevented by the owner.Read more

Sudden shattering of the sunroof while driving can: Distract the driver and lead to loss of control Cause injury from falling or flying glass Create hazardous driving conditions, especially at highway speeds Dealer Response: I reported the issue to a Toyota dealership, but they denied warranty coverage, stating it was not covered without providing evidence of impact damage. Additional Notes: There was no visible point of impact, which suggests a possible stress fracture or manufacturing defect rather than external damage. Request: I request that NHTSA investigate this issue as a potential defect, as spontaneous sunroof glass failure while driving presents a significant safety hazard.Read more

I've noticed periodically when the car is braking at start stop traffic in autopilot mode, the brake is very jerky and I can actually hear the pedal jerking and engaging. I wrote it off at the time in my head as normal operation of the car. I have brake pressure offset codes and brake booster mia co
... des. Sometimes I've noticed when I brake, randomly the pedal is super stiff and it feels like the car is driving on ice, as if the brakes aren't working or slowing me down at all, but I thought nothing of it at the time. Today on the 14th, I got into an accident at a relatively slow speed that in any other scenario I should have been able to stop. Yet I hit a parked car because as I was braking, I felt the pedal go very very stiff and unresponsive, and I carried the same speed as when I started braking right until impact, it seems as if I didn't slow down one bit. The brakes did not respond as they did mere seconds before and I hit a parked car. I understand teslas may throw codes left and right in service mode that may mean nothing, but I am worried about the brake booster code and wonder if maybe it caused the accident I was involved in. My friends told me they heard about tesla brake issues and that I should look into it and when I went to go check the codes hours later there they were.Read more

Axle clicking initially only while making turns left and right - now when it drives straight the axle sometimes clicks as well.
Incident date: Dec 1, 2025
The vehicle is experiencing multiple serious mechanical failures affecting the brake system, powertrain, and steering/wheel components. The brake system produces a constant hissing sound and appears to have lost braking assist. When the brake pedal is depressed the vehicle continues to roll forward
... and requires excessive force to stop. The vehicle has rolled forward while braking at intersections, creating a significant safety hazard for both the driver and surrounding traffic. These symptoms were first reported on February 4, 2026. Shortly afterward the vehicle was inspected by a Chrysler dealership, which documented a brake vacuum leak in the braking system. Despite that inspection and documentation, the braking condition has continued to worsen and the vehicle no longer feels safe to operate. The braking performance is inconsistent and stopping distance appears to be increasing. In addition to the braking failure, the vehicle experiences powertrain issues including transmission hesitation, delayed shifting, and loss of power during acceleration and when driving uphill. The vehicle also has severe vibration and instability originating from the front driver-side wheel or steering area while driving, which affects vehicle stability and control. These combined mechanical failures involving braking, drivetrain operation, and steering/wheel stability create a serious safety risk during normal operation, particularly while braking in traffic or attempting to maintain control of the vehicle. The vehicle currently feels unsafe to operate due to these worsening conditions and unresolved mechanical defects.Read more

... uggle to downshift, staying at a higher RPM than it should, such as being stuck at 4000 RPM while driving 30mph, and it would stay like this for about 1-2 minutes. Additionally, from time to time, the reverse gear will not engage at all, this is a major safety concern, God forbid I am somewhere I need to back out from and there is obstruction in the front, only God knows what trouble I would be in. There have been recalls for such cars. It is unfortunate that Ford has not opened an active recall for others with this issue. Please help, this car costs too much, and I fear for my wife and newborn's safety with transmission issues.Read more

The horn on my 2025 Hyundai Tucson Limited failed without warning in mid January 2026 and no longer functions. This creates a serious safety risk because I was unable to warn another driver while they were merging into my lane on the highway. Without a functioning horn, I cannot alert other drivers
... to avoid potential collisions. The vehicle was inspected by Balise Hyundai in Springfield, MA, on January 23, 2026, and the dealership confirmed the horn requires replacement. However, they cannot cannot order the replacement part because it is unavailable, possibly due to a redesign or manufacturer change. Hyundai Consumer Affairs has acknowledged the defect and confirmed there is no timeline for repair. They are not allowing dealerships to order the old part and do not have a timeline on when the redesigned part will be available. Because the horn does not function, my Massachusetts State Inspection sticker is expired and cannot be renewed until the horn is repaired, making the vehicle illegal to operate. There were no warning lamps or prior symptoms before the failure. The vehicle is available for inspection upon request.Read more
