The safety recall (Hyundai Recall 227, NHTSA Recall 22V-312) repair on my 2013 Hyundai Sonata (previously completed by a Hyundai dealership prior to my purchase in April 2026) has suffered a critical mechanical failure. In May 2026, within 30 days of driving the vehicle, the recalled safety componen
... t completely failed while the vehicle was in operation on a public roadway. This sudden failure created an immediate safety hazard, resulting in complete fuel loss, forcing me to immediately pull over and have the vehicle towed to a repair facility (Meineke). An independent mechanical inspection confirmed that the exact component covered under the manufacturer's original safety recall had failed again. Because a critical, manufacturer-designated safety recall remedy failed to permanently correct the underlying defect within 30 days of use, this vehicle remains an active safety hazard.Read more

Knock sensor has detected a failure. Check engine light is flashing and the car is in limp mode.
Incident date: Sep 8, 2026
On September 8, 2026, while driving a 2013 Hyundai Sonata Limited at highway speed, the check-engine light began flashing and the vehicle suddenly lost power. The vehicle slowed to approximately 40 mph, then approximately 20 mph, before the engine completely shut off. I had to maneuver the vehicle t
... o the side of the highway while surrounding traffic continued traveling at highway speeds. This created a serious risk of a rear-end collision or other accident. The engine would not operate afterward and appears to have seized. The vehicle had approximately 100,000 miles at the time of the incident. There had been no clear warning of an imminent engine failure before the check-engine light began flashing and the vehicle lost power. The vehicle has not yet received a complete diagnostic inspection, so the precise cause of the failure is currently unknown. The vehicle is available for inspection upon request. No crash, fire, or injuries occurred, but the sudden and severe loss of power created an immediate safety hazard.Read more

I am reporting a serious engine failure involving my 2013 Hyundai Sonata. The vehicle's engine was previously inspected by a Hyundai dealership as part of a Hyundai engine recall. I followed Hyundai's instructions and brought the vehicle to the dealership for the required inspection. I was informed
... that the engine passed the inspection, and an orange marking was placed on the oil dipstick. The Hyundai dealership should have the service record documenting this recall inspection. Many years later, at approximately 120,000 miles, the same engine suddenly failed without warning and became locked/seized. A mechanic attempted to turn the engine and determined that it would not turn. I understand that many years had passed and the vehicle had accumulated additional mileage since the recall inspection. I am not claiming that the previous recall issue necessarily caused the current failure. However, I believe the circumstances warrant investigation because this is the same engine that had previously been inspected under a Hyundai engine recall and determined to have passed the inspection. The vehicle had been routinely maintained. Much of the routine maintenance was performed outside Hyundai dealerships by a mechanic in my family and by me, so the Hyundai dealership service history may not contain all maintenance records. I am concerned that the sudden engine seizure may represent a safety-related engine defect or may potentially be related to the previously recalled engine condition. An unexpected engine seizure can result in a sudden loss of propulsion and create a significant safety risk if it occurs while the vehicle is being driven. I am requesting that NHTSA review this complaint, including the vehicle's recall history and any related complaints or investigations involving the 2013 Hyundai Sonata, and determine whether this failure warrants further investigation. The Hyundai dealership that performed the recall inspection have documentation of that inspection.Read more

I am writing to formally report a serious and recurring safety defect with my 2025 Acura MDX A-Spec (VIN [XXX]) involving the Auto Idle Stop system. The engine repeatedly fails to restart after the Auto Idle Stop feature shuts it off. When my wife releases the brake pedal (for example, at a traffic
... light), the vehicle either stalls completely or experiences a major delay before restarting sometimes lasting more than 20 seconds, leaving the my wife unable to accelerate. This creates an immediate and unacceptable safety hazard, particularly in intersections or heavy traffic. I previously brought the vehicle to Acura of Langley regarding this exact issue. Despite that visit, the problem has continued. Most recently, on today, while my wife was driving with my kids, the vehicle again entered Auto Idle Stop at a traffic light. When the light turned green, the engine completely shut off and would not restart for more than 20 seconds. This left her stranded in traffic and unable to move the vehicle, creating a clear risk of collision. This is not an isolated complaint. The National Highway Traffic Safety Administration (NHTSA) has opened Engineering Analysis EA25004 into precisely this condition “No Restart After Auto Start/Stop Engages” affecting, among other models, 2016–2025 Acura MDX vehicles. American Honda has also issued dealer communications specifically seeking 2023–2025 MDX vehicles with client complaints of no auto re-start after Auto Idle Stop for inspection prior to any repair attempts. Class-action investigations and prior litigation involving related Idle Stop failures further underscore the known nature and seriousness of this defect. I am writing to formally request that Honda investigate and acknowledge a significant security and safety concern regarding the front radar module used on certain Honda vehicles equipped with Honda Sensing. The front radar module appears to be easily accessible from the front of the vehicle and can
... reportedly be removed by a thief in a matter of seconds without requiring specialized tools. Because of its location and lack of adequate theft protection, it has become an attractive target for theft. The theft of this component is more than a financial inconvenience. Once the radar module is removed, critical driver-assistance and safety features—including Adaptive Cruise Control (ACC), the Collision Mitigation Braking System (CMBS), Forward Collision Warning, and other Honda Sensing functions—may become unavailable until the sensor is replaced and professionally calibrated. This can leave owners without important safety technologies they relied upon when purchasing the vehicle. I respectfully ask Honda to investigate this issue, acknowledge the vulnerability, and consider implementing a remedy for affected vehicles. Possible solutions could include an improved anti-theft mounting system, a redesigned sensor location, or other security enhancements that make unauthorized removal significantly more difficult. As Honda owners, we should be able to feel confident leaving our vehicles parked without worrying that a critical safety component can be stolen in seconds. Beyond the financial burden of replacing the radar module, the loss of these safety features can affect the safe operation of the vehicle and diminish driver confidence. I urge Honda to take this concern seriously, communicate with affected owners, and evaluate whether a service campaign or safety recall is appropriate. At a minimum, I hope Honda will acknowledge the issue and work toward a solution that improves the security of the front radar module and restores owners' confidence in the safety of their vehiclRead more

NHTSA 22V-024, R21B90 Original case #11722884 Dealership, Stelly Nissan of Yorktown Heights, lied about the recall eligibility of my vehicle on March 5th, 2026. When I researched my own service records, 2014 Nissan Rogue VIN is actively included in Recall 22V-024 for the electrical connector fluid

They subsequently bypassed federal compliance by illegally marking the safety recall as closed or completed in the system without executing the physical repair. I have the physical invoice dated 11/25/2022, 0.00 parts/labor and visual inspection only, which contradicts what the recall procedure was defined as. Because the dealership skipped this mandatory safety remedy, the unaddressed fluid-wicking defect has now caused severe, cascading electrical system failures across multiple dashboard components. The vehicle remains an active electrical fire hazard as described in the original 22V-024 defect report. I am filing this new complaint specifically to report Dealer Recall Non-Compliance and Enforcement Violation. I request that NHTSA compel the manufacturer to execute the physical recall remedy immediately to mitigate the fire risk. As a matter of course, it is not currently a possibility to work with either the dealership, who has ceased communication with me, or Nissan Consumer Affairs who blatantly ignored that the recall was not performed and cited it as closed, a violation of Federal Law. My 2022 Volkswagen Jetta (purchased new) has a recurring Travel Assist/Adaptive Cruise Control safety malfunction that has existed since before approximately 24,000 miles. While driving with Adaptive Cruise Control or Travel Assist engaged, the vehicle repeatedly warns me to "Take Control of the Veh

On numerous occasions, the vehicle has also applied the brakes unexpectedly at highway speeds when there was no apparent hazard in front of the vehicle. These unexpected braking events create a serious safety concern, especially in traffic. I drive between [XXX] and [XXX] approximately twice each month. During a typical one-way trip, the steering wheel recognition warnings may occur a dozen or more times while using Travel Assist or Adaptive Cruise Control. Unexpected braking has occurred repeatedly over the course of ownership. The vehicle has been taken to an authorized Volkswagen dealer multiple times for this same issue. Volkswagen has replaced the steering wheel multiple times, updated software, opened a Technical Assistance Center (TAC) case, and at least one technician duplicated the concern during a test drive. Despite these repeated repair attempts, the problem continues. I reported this issue to insurance. The facts are that when I was watching a family member’s toddler, I went to my vehicle to grab a stack of papers. While holding her, I reached in and grabbed the papers sitting directly on the back seat of the coupe vehicle (I.e., easy to reach). Upon grabbing
... the papers in one hand and holding her on my left hip, I shut the drivers side door and began walking toward the home. Immediately, I heard an odd sound like a rustle or possibly an animal scurrying. I looked around confused because I hadn’t seen anything and it wasn’t too dark. Then I looked closer and the entirety of my back windshield was shattered into a billion pieces leaking into my back seats and onto the car seat placed there for when I frequently pick up the toddler.

Repeated Failure and Rebooting of Mazda Connect Infotainment System I am the owner of a 2016 Mazda CX-5. The factory Mazda Connect infotainment system has developed a recurring defect in which the system repeatedly reboots to the Mazda logo while driving. Prior to complete failure, the navigation s
... ystem became intermittent and Bluetooth connectivity frequently disconnected, requiring repeated attempts to reconnect devices. The system now continuously cycles through the startup screen, making navigation, hands-free phone functions, and system settings inaccessible. These repeated reboots occur while the vehicle is in motion and are distracting to the driver. The flashing and rebooting screen repeatedly draws the driver's attention away from the road and interferes with the use of navigation and hands-free communication features that are intended to promote safe driving. After researching this issue, I found numerous reports from other Mazda owners experiencing the same symptoms involving the Mazda Connect system, navigation SD card, and Connectivity Master Unit (CMU). The frequency and similarity of these reports suggest that this is a widespread and well-known defect rather than an isolated incident. Because this defect affects driver attention and the safe operation of the vehicle's communication and navigation functions, I respectfully request that Mazda and the National Highway Traffic Safety Administration investigate this matter and require Mazda to issue a recall or establish a repair program for affected vehicles. My BMW's Head-Up Display (HUD) and Speed Limit Information system displays incorrect speed limits that do not match posted roadway signs. The HUD frequently shows "LTM" or an incorrect speed such as "25 mph" instead of the actual posted speed limit. This is a serious safety concern. Drivers rely on
... HUD speed limit information while driving. Inaccurate readings can lead to unsafe driving decisions, unintentional speeding, traffic violations, and increased accident risk. The vehicle was taken to an authorized BMW dealership for repair and kept for approximately one week. I was informed on June 4, 2026, and again on June 8, 2026, that the issue had been resolved. After each attempt, the defect remained completely unchanged. The dealership repeatedly misrepresented the repair as complete when it was not. I previously owned a BMW X5 for three years with no HUD or speed limit display issues, confirming this is a defect and not normal system behavior. The vehicle was purchased new in November 2025 and remains under warranty. Despite multiple failed repair attempts, the defect persists and continues to compromise safe vehicle operation. I respectfully request NHTSA investigate this as a potential safety defect and determine whether a Technical Service Bulletin (TSB), recall, or other corrective action is warranted.Read more

Rear tailgate opening and closing repeatedly on its own. Background: Arrived home from work yesterday evening, Jun 4, 2026. Parked my truck in the driveway. It was a hot sunny day. Turned the vehicle off, exited the vehicle, and locked the doors. Upon walking around behind my vehicle, the tailg

I attempted to manually close it to see if it would stop, I pushed the lock button on my key fob, and I pressed the tailgate button on the dash which would normally cause it to either open or close depending on its current position. None of those steps resolved the issue. I quickly googled the problem, identified the correct fuse in the dash, and pulled the endgate fuse to stop the tailgate from opening and closing on its own. With the fuse pulled, the tailgate does not function. I called my dealer who informed me it is a known issue with some Silverado trucks and that some had been recalled however he noted my 2022 was not included in the recall for this issue. The vehicle showed an error on the dash which stated "Action Required Steering Assist is Reduced Drive with Care" I tool the vehicle to the dealer I purchased it from McClusky Chevy at Kings Auto Mall in Mason Ohio. They informed me that the error code is C0460:58 and GM has a service bulletin 20-

No one explained to me that this could cause catastrophic failure and lead to a crash before I left the dealership. They could not replace it as they said they do not have the part and it is on national backorder. Additionally they told me the repair would cost me $3,400. The vehicle has 42k miles on it and this repair is not under warranty as the warranty that would have covered it expired in March of 2026. I have taken the vehicle to a different GM dealer - Tom Gill GMC in Fairfield Ohio - they are waiting on the part and I have a case opened with GM regarding the costs. You would think that if there is a risk of a catastrophic failure which could cause harm to my self or someone else - the dealer would have an obligation to tell me so before i drive off. Subsequent to the 68C recall software update, my vehicle is suffering from intermittent loss of acceleration. Even with a full charge of battery, when driving the 2025 jeep wrangler 4xe in EV mode, occasionally the car will shut of EV mode and transition to gas/hybrid mode. The problem is the veh

It would have been a head on collision if the other driver hadn't slammed on their breaks. This was my last time with the vehicle and it has been sitting at the dealership since January. The dealership service department tells me Stellantis is not working on a fix for this issue, even though it is a serious safety defect. Since then I have been pursuing lemon law however this process is taking forever. They refused to provide a loaner car. I was renting a vehicle but it was too costly. I ultimately had to buy another vehicle, even thought I spent $70,000 on a brand new jeep that is sitting at a dealership. They claim no fault found. The dealership drove it a total of 10 miles in EV mode. No fault found. The dual clutch PowerShift DCT transmission has failed. When placed in reverse, the engine RPM increases when accelerating but the vehicle does not move at all. While driving forward, the vehicle struggles to reach 10mph and the transmission slips severely. The vehicle is essentially undriveable. T
... he vehicle has gotten stuck on neighborhood roads on 3 separate occasions due to complete loss of reverse function. I was stuck for 30 minutes to an hour which put me in way too much preassure and possibly, I could have ended up in danger. The vehicle had to be physically pushed by hand to move it. This created a dangerous situation for myself and others on the road. An independent mechanic diagnosed and replaced the clutch actuator assembly and fuel pump. After repairs the problem persists unchanged. The mechanic has noted the vehicle requires a full transmission replacement. This is a known defect with the Ford PowerShift DCT transmission affecting multiple model years. Repair costs have exceeded $1,500 with no resolution. The failed transmission puts safety at risk as the vehicle cannot accelerate properly in traffic and has no reverse function. Problem has been confirmed by an independent service center. The vehicle experienced a failure of the steering system, specifically the steering gearbox. The steering intermittently sticks, binds, and resists input, followed by sudden release, causing unpredictable response and difficulty maintaining control. The vehicle also pulls unexpectedly, creating a se
... rious safety hazard, especially at highway speeds. The vehicle is unsafe to operate and is currently at a dealership for inspection. Prior to failure, symptoms included increasing steering resistance, inconsistent feel, and intermittent sticking, which worsened over time until the vehicle became undrivable. The issue has been diagnosed and confirmed by an authorized dealership, which determined the steering gearbox requires replacement. The dealership indicated the failure was not due to misuse, damage, or lack of maintenance. There are similar steering-related failures reported in comparable vehicles; however, this vehicle is excluded based on VIN eligibility. Despite similar failure, no recall coverage has been provided. This failure creates a high risk of loss of control and potential crash, with no warning lights or alerts prior to failure. The estimated repair cost exceeds $4,300 including parts and labor. The manufacturer has been contacted, but no resolution has been provided. This failure occurred during normal operation and was not caused by driver input or external damage. The sudden loss and return of steering control creates a dangerous condition that could result in a crash without warning. Given similar reported issues, this warrants investigation into a potential safety defect.Read more
