The low-pressure fuel pump module assembly, located directly beneath the rear passenger seat cushion, experienced a plastic housing failure. A stress fracture on the upper plastic fuel outlet nipple caused raw liquid gasoline to actively weep and pool on top of the module inside the passenger cabin.

RISK TO SAFETY: Raw liquid gasoline pooling inside the interior passenger compartment creates an severe, immediate fire and explosion hazard. Additionally, vehicle occupants were exposed to toxic fuel vapors while driving. INSPECTION & DEALER CONFIRMATION: An authorized Hyundai dealership inspected the vehicle and confirmed raw fuel leaking at the top of the fuel pump module assembly. This exact module had previously been removed and disassembled during a 2024 manufacturer recall service to replace the internal fuel filter and tank seal. Stress applied to the plastic fittings during that recall service appears to have caused the plastic top-hat to fracture over time under line pressure. Hyundai Corporate was notified of the safety hazard and prior recall history, but refused assistance. WARNING SYMPTOMS: The failure began with a strong, persistent fuel odor inside the vehicle cabin. No dashboard warning lights, check engine lights, or diagnostic trouble codes (DTCs) triggered prior to or during the failure. SUMMARY: A stress-fractured plastic fuel pump housing allowed raw fuel to pool in the passenger cabin following a prior manufacturer recall repair involving the disassembly of the same component.Read more

... orning, the engine would not start. The vehicle was towed to an independent mechanic on January 28, 2026, who determined that the engine was locked/seized. On February 11, 2026, it was towed to Wayne Hyundai for further diagnosis. The vehicle had only 51,533 miles and had received Hyundai’s Knock Sensor Detection System (KSDS) update. Hyundai denied warranty coverage, citing engine sludge, despite my providing a complete service history after resubmiting the missing receipts I had previously misplaced. My primary concern is safety, not the warranty dispute. A sudden engine seizure without warning could cause an unexpected loss of propulsion or create a dangerous situation if the engine fails while the vehicle is being driven. This concerns me because Hyundai Theta II engines have previously been associated with engine seizure and stalling, including issues addressed through The Hyundai Theta II Engine Settlement: and subsequent litigation involving certain 2019 Sonata vehicles. I respectfully request that NHTSA investigate whether a continuing or insufficiently addressed engine defect exists in affected 2019 Hyundai Sonata vehicles, particularly sudden engine seizure/no-start failures occurring without warning. I also ask NHTSA to examine whether existing recalls, campaigns, software updates, warranty extensions, and other remedies adequately protect owners when an engine fails suddenly before the KSDS system provides a warning.Read more

While driving a 2026 Tesla Model Y Premium on [XXX] at approximately [XXX] PM on [XXX] using Full Self-Driving (FSD) Supervised mode, the vehicle was traveling in the center lane (lane 2 of 3) when it ran over a large whole tire (commercial truck-sized) that was positioned in the center of the lane.
... The FSD system failed to detect or avoid the tire prior to impact. This incident demonstrates a critical safety limitation: Tesla FSD cannot detect large road obstacles such as a whole commercial truck tire positioned in the center of a highway lane at night. The FSD system showed no warning, took no evasive action, and did not alert the driver prior to the impact. Prior to the incident, there were no warning lamps, error messages, or any abnormal symptoms. All systems were functioning normally. Following the collision, a "Coolant Low - Schedule Service / Climate and driving performance may be limited" warning appeared immediately on the display screen. Upon inspection, all coolant had leaked out completely. Damaged components include: underbody, front bumper lower section, and cooling pump. The cooling pump damage is particularly concerning as it prevents proper battery temperature regulation, creating a significant risk of battery overheating and potential fire. The vehicle is currently at a Tesla-approved collision center. Inspection is still in progress. The vehicle has been reviewed by Tesla Service Center technicians and insurance representatives. Safety concern: Tesla FSD failed to detect and avoid a large, visible whole truck tire in the center of the highway lane at night, resulting in significant underbody damage, complete coolant loss, and cooling pump failure — posing serious battery fire risk. The vehicle was taken to a dealer, where it was determined that the tailgate would not open because the electrical system was disconnected, and the trim had fallen off. The contact stated that on several occasions while driving at various speeds, the dome light remained illuminated and turned off approximately 4-5 seconds later. Additionally, while the vehicle was in reverse(R) on several occasions, the dome light illuminated, and the contact was unable to see out of the rear window. Instead, they are advising me to operate this vehicle for an additional 2,000 miles to complete a corporate "consumption test" to prove the defect for warranty authorization. Operating a vehicle with a known, rapid, and erratic oil depletion defect for thousands of miles presents a severe, immediate safety hazard to my family and others on the road. If the engine oil drops below critical operating levels unexpectedly between manual checks, the engine faces an immediate risk of catastrophic metal-on-metal friction, thermal expansion, and sudden engine seizure at highway speeds. This creates an extreme driving liability, including an instantaneous loss of all motive power, a sudden loss of vital power steering and power braking assist (making the vehicle incredibly heavy and difficult to control or maneuver safely to a shoulder), and a high risk of an engine bay fire due to intense friction and fluid ignition. This is a known issue that I presented to them in March. At that time the PCV valve was replaced along with new spark plugs. The problem continues to persist. This matter is actively documented with Honda Corporate under Case #16466779. A manufacturer protocol should not supersede passenger safety by forcing the prolonged operation of a defective vehicle. The dealership has advised that this is a known issue with the 2019 Honda CRV with piston rings failing. This needs to be made public and a recall for Honda to fix.Read more

Since the incident, I have installed a Bluetooth battery monitor. Data collected June 22, 2026 shows consistent parasitic drain throughout the day, with voltage declining from ~12.2V at rest and recovering only briefly when the DC-DC converter cycles (~14V spikes). The battery does not hold charge between cycles. No warning lamps appeared before or during the failure. This issue is documented across the shared Toyota/Subaru EV platform. The 2023-2025 Solterra and bZ4X are currently subject to class action litigation (Wade v. Toyota Motor North America, E.D. Cal., filed June 2025) alleging defective 12V charging systems. Warranty battery replacements do not resolve the problem because the parasitic drain continues. In October 2025, Subaru issued a service bulletin on parasitic drain testing, confirming the issue is systemic. The dealership service tech informed me that in order to service the vehicle and find the code that is making this issue, that I would have to continue driving the truck and bring it in to the dealership with the power steering and StabiliTrack warning. According to the technician, they can't do much due to that they don't know what is causing the power steering to malfunction. I haven't been able to take it to the dealership because the warnings have occurred while I'm far away from a dealership or diagnosis area. Also, I will not risk my safety maneuvering a vehicle that has lost electric power steering around a city where lots of left or right turns are necessary to drive to a locality. I experienced repeated stalling, loss of drive power while on the highway, limp mode, and all dashboard warning lights over several years. I brought the vehicle to an authorized Honda dealership multiple times and specifically asked whether these symptoms were related to the known fuel pump recall.
... Each time I was told the recall was not the cause. I was also told recall parts were unavailable or delayed. Based on dealership misdiagnoses, I paid out of pocket for multiple repairs including spark plugs and ignition coils which they claimed were the reason for my car acting up, that did not resolve the issue. Among other repairs I did on my own like replace the battery twice and what not. I have since received Honda’s official recall notice confirming that a faulty fuel pump causes the exact symptoms I reported. My vehicle is now completely inoperable it does not start and cannot be driven or transported. I have filed two claims with Honda Corporate, both denied based on mileage. I believe the vehicle’s failure is a direct result of Honda’s failure to address the recall in a timely manner and the dealership’s repeated misdiagnosis.Read more

My dealer ignored my request to even look at the issue, saying glass was not covered by their warranty and when I called Honda USA, they took the information and casually told me to 'have a blessed day'. My first big chip was repaired through my insurance in December 2025 and I will have 5 more chips looked at today by their preferred shop, Safelite. I have taken photos, as best I could, of the chips I discovered while washing the car in the bright sunlight. Having a clear field of vision is so critical to safe driving and the manufacturer should provide a durable windshield for passenger safety.Read more

Despite this, the manufacturer has stated that no further repairs will be approved, and the vehicle was returned with the defect still present. The vehicle has been out of service for more than 100 days for attempts to diagnose and repair this issue. The unresolved noise at highway speeds creates a safety concern because it may indicate an underlying drivetrain problem that could worsen or fail while driving. The vehicle was returned to a Kia dealership for inspection. I was informed that a cylinder issue was suspected; however, only a temporary repair involving a coil swap was performed. No comprehensive diagnostic testing was completed to confirm the root cause of the problem. After the repair, the vehicle continued to exhibit abnormal behavior, including shaking and lack of power. Safety concerns include battery failure that can cause me to be stranded and the fact that the previous owner has the ability to start my vehicle at any time, even while parked in my garage posing a risk to my family’s health/life with trapped carbon monoxide building up. Privacy concerns include the previous owners ability to track my vehicle, lock and unlock doors, access my contacts, call log, location, and more. This is a significant risk. The dealership should not have sold me a vehicle with a defective module without repairing it and now refuse to fix it. This should have been included in GMs factory recall for 2023 Tahoe TCU’s or a new recall should be issued. To me this issue should have been made a recall because of its criticality... the BSD system is a core safety feature of cars and reduces accidents substantially. There was no way for me to know my car has this problem unless it is made a recall. I have NOT had any accident related to this issue but i had to pick an incident date to complete this complaint. I have called KIA customer service at 1-800-333-4542 twice to find out whether the VIN number should be included in the service bulletin and the answer was no and whether i should report this to NHTSA with the possibility that certain VIN numbers were left out of the service bulletin in error. No answer as to what to do to investigate this was given by KIA. That's why i'm reporting this on here. The service bulletin has been attached to this complaint. My question to NHTSA is whether certain vehicles were left out of the service bulletin in error and whether the manufacturer is liable for the repair at no cost to the consumer. They are not sure if I will be able to get a rental car covered by Chrysler.
