Defect Description
Ford Motor Company (Ford) is recalling certain 2015-2017 Mustang vehicles. Water and road salt may corrode the front seat belt anchor pretensioner cables.
Consequence
A corroded cable can break, preventing the seat belt from properly restraining an occupant, and increasing the risk of injury during a crash.
Remedy
Dealers will inspect and replace both front seat belt anchor pretensioner assemblies, as necessary. In addition, dealers will remove sections of the carpet and underlayment that contact the cables. Symptoms: the vehicle will not start without a jump, repeated low-battery and SOS/telematics warnings, blank infotainment display, and full battery depletion after the vehicle sits unused for as little as 3 days, even with a battery maintainer in use. Dealer cloud diagnosis confirmed two contributing faults. First, elevated parasitic/quiescent current draw from control modules not entering proper sleep state, measured at 1.6A versus a normal value near 0.01A. Second, inadequate 12V charging. The dealer replaced the 12V and telematics backup batteries and applied software updates to multiple modules (including TCU and BCM), yet the failures recurred. This VIN has an open safety recall on MHEVs (NHTSA 26V-248, manufacturer D126) describing a DCDC converter internal fault that causes loss of 12V system charging and a "Stop Safely, Electrical Fault Detected" warning, with potential loss of motive power, coasting to a stop, and loss of exterior lighting. The remedy is not yet available. Safety risk: loss of 12V charging and battery depletion can cause stalling, loss of motive power in traffic, no-start and stranding, and loss of lighting, all of which increase crash risk. On one occasion the vehicle barely started after sitting at an airport lot, nearly leaving my family stranded. The problem has been reproduced and confirmed by both a franchised dealer and an independent service center. The vehicle and the failed batteries are available for inspection on request. Warning messages first appeared before the current failure and have recurred across multiple battery replacements.Read more

I got the car back on February 19. On February 20, I again had a rough idle on start up. I brought the car back to Vann York Honda and advised them that I was experiencing the same issue as before. The mechanic brought it to the back and advised me that he could not find anything wrong with the vehicle and that it is running fine. I expressed my concerns with the vehicle and he stated that without a error code or check engine light being on, it would be difficult to diagnose. The technician and the advisor said to keep driving it until a light comes on saying there is a problem since they could not find an issue. in the following 2 weeks, I had regular rough idles on start up and the fuel efficiency dropped approximately 6 miles per gallon. When I filled the car with gas, the fuel range dropped from 540 to 370. When I called about that issue, they said they reset the computer when performing the previous service on the vehicle and it is taking time for the computer to "learn" the car again. My truck was recently held for two weeks for service. After I lodged a complaint, the General Manager suggested I seek service elsewhere. This recommendation was made after I responded to an email stating that, following the completion of repairs to the headliner, third brake light, and brakes/rotors, I would no longer be utilizing Crain's services. The headliner sustained water damage due to improper installation by your technician, and fingerprints were also evident. I firmly believe in transparent communication regarding service progress, which was notably absent in this instance. Both the General Manager and Service Manager deferred responsibility, instructing me to contact GM directly, only for GM to inform me that direct contact was inappropriate. This resulted in a continuous cycle of unaddressed concerns with the service manager. Currently, I am operating an $80,000 vehicle that has incurred damage from a technician and exhibits a ticking engine. Furthermore, the driver's side mirror, which was previously replaced due to shaking at highway speeds, is now exhibiting the same issue, despite a technician's assertion that this is normal. I towed it to the closest dealer and when they checked the codes (the next day), they said there were no codes and they could not replicate the problem. As a note, the rain was off and on, nothing that was worse than anything I have driven in previously. I drive main roads and did not go through any large puddles or anything similar. This is a major safety issue and I am terrified that it could happen while driving on a highway where I may not have time to get out of the way of other traffic (car literally stopped with no option of continuing to coast). When placed in drive, it would go backward, and when in reverse, it would go forward. It was officially diagnosed with transmission failure after I had it towed to my nearest dealership. I’ve maintained this vehicle responsibly and have never driven it roughly or taken it off-road. Still, I was quoted nearly $10,000 to replace the transmission, which is outrageous for a three-year-old SUV. Ford refuses to cover the repair, even though this seems like a serious manufacturing issue. I’m still making payments on the vehicle, and now it’s completely unusable. ... 2013-2018 Ford Flex models. My vehicle is a 2017 Ford Flex, and it recently experienced a failure of the right rear toe link, consistent with the failure mode described in the recall. However, after contacting my local Ford Dealership (Rowe Ford, Westbrook, ME) and the Ford Motor Company, I was informed that my VIN is not included in the recall coverage. I had the right rear toe link repaired and paid for it. I am concerned that the same issue could occur in my left rear toe link, so I am working to have that part preemptively replaced at my own expense. I believe that the exclusion of my vehicle by VIN may be inappropriate for the following reasons: 1. Model year and component match: My Flex falls well within the model years (2013-2018) covered by the recall, and the failed part is the exact component under scrutiny. 2. Failure occurred during normal driving conditions and almost caused an accident. 3. Ford's recall criteria appear overly narrow, possibly omitting vehicles that still contain defective parts, like my vehicle. I respectfully request that NHTSA review this matter and determine whether the VIN range for this recall should be expanded to include additional vehicles like mine that have experienced identical part failures with potential life threatening safety concerns. I have uploaded invoices from my local dealership repair facility. One invoice is for the work to replace the failed right rear toe link and the other is for preemptive replacement of the left rear toe link that I chose to commission on my own.Read more

This kind of unexpected shutdown while driving is a serious safety concern. I was lucky to avoid a collision, but if this had happened on a freeway or near traffic, the outcome could have been worse. ... n automatic button. There is a sensor that controls that door. It is never in danger of being hit or disturbed in use by an ambulatory person. However, when it is used with a ramp, the second row of seats is removed and replaced with a passenger-side ramp. The ramp is 30 inches wide. The normal power wheelchair is 26 to 29 inches wide. This leaves only 2 inches MAXIMUM on either side of the ramp for the wheelchair. It is virtually impossible to NOT hit the sensor when going in or out by wheelchair!! I have had this van for about two years. I use it about once every two weeks. Most times I have an assistant driving. I have only used it about 50 times TOTAL, in two years. On [XXX], I had a doctor's appointment. I activated the automatic door and ramp. I rolled up the ramp, and parked in the passenger area. That seat has been removed for the wheelchair. My assistant pushed the button to activate the ramp to go back under-floor. It did so. BUT the sliding door on the passenger side did NOT close at all! Eventually he managed to manually slide the door shut. If I had been driving alone, I would have been stranded right there---at the doctor's office, shopping center, etc., as I do NOT have the strength to manually close that door! This is clearly a danger to disabled people using this modified Honda Odyssey. Honda and BraunAbility should be required to protect and reinforce the sensor area so users will NOT easily damage the sensor with light use of this van! I was left stranded 90 miles away from home in freezing temperatures during a snowstorm on Christmas night. My vehicle was towed to a dealership 140 miles from home as GM would not cover the cost of towing to the dealer closest to my home (68 miles away). After numerous phone calls with GM as well as the GM dealer, I was informed 9 days later that my entire engine (6.2L V8) would need replaced. I was also informed that there are two vehicles ahead of mine that require replacement of their engines and that the engines are back-ordered. They have been unable to provide an estimated timeframe on when my vehicle will be returned to me in working condition, stating it "could be months." When I was informed of the need for an engine replacement, I placed a request for a manufacturer repurchase which was ultimately denied the following week. The agent responsible for managing my case informed me that "GMC requires two to three major failures" before they will consider a repurchase approval. The dealership was not able to provide me a loaner vehicle and I was informed that GM only reimburses $44 per day toward a rental vehicle. My occupation and location require reliable transportation in harsh winter conditions so a truck is the only reasonable option. The dealership charges $75 per day to rent a basic truck. Prior to this incident, the vehicle did not have any major issues and received routine maintenance services as recommended by the manufacturer. It was never involved in any accidents. It only had 17,553 miles and was purchased brand new in October 2023.Read more

Inability to shift gears on acceleration
The Office of Defects Investigations (ODI) analyzed complaint data provided by Ford as well as complaints submitted to ODI from consumers.In total, there were 364 unique reports indicating various shift quality issues while driving.Ford identified several factors that may contribute to shift quality
... concerns in the subject vehicles, including cold transmission, clutch stay-out at high engine speeds, gear clash or grinding, and gradual loosening of clutch plate bolts in some early production vehicles.The largest percentage of complaints indicated higher than expected shift efforts in cold ambient temperatures.These complaints were related to transmission fluid viscosity and the higher shift efforts resolved themselves with the drivetrain warming.Ford published technical service bulletin TSB 11-3-18 to inform technicians that a lower viscosity fluid was available for use to address the cold shift issues.The clutch stay-out condition typically occurs at engine speeds greater than 4000 rpm.It does not prevent the selection of any gear, but may delay clutch re-engagement depending on how long the driver maintains the higher engine speed.Ford identified rotating inertia of the clutch components as the root cause and replacement clutch pedal parts were made available under Ford TSB 10-19-4.A number of reports alleged increasing difficulty selecting gears along with gear clash or grinding.Grinding or notchy gear shifts may be caused by gradual loosening of clutch plate fasteners, which is promoted by certain aggressive shifting.This condition is progressive in nature and initially results in symptoms related to shift quality/feel.Clutch plate bolt loosening will not result in inability to select any gear unless the progressive symptoms are ignored until complete clutch failure occurs.ODI?s analysis identified only five complaints alleging an inability to shift into gear due to loose clutch plate bolts.None of these resulted in a stall and gear shifting could occur before engine start.A revised fastener was designed and implemented in production and is available for service through a special service message.There is no indication of loss of motive power or unreasonable safety risk associated with the alleged defect in the subject vehicles.This preliminary evaluation is closed.Read more
