It is a well-known issue across Mustang forms that the brake pedal sensor switch goes bad as well as the small rubber pad that presses against the brake pedal switch. This has happened to me multiple friends in my Mustang group as well as tons online. This is a serious issue because either it will m
... ake your vehicle believe the brakes are constantly being pressed letting you shift into gears while the brakes technically aren’t even depressed. That could result in someone getting ran over. Also, it makes it appear that you’re constantly pressing the brake pedal so drivers do not know when you are actually pressing the brakes. I had that issue, I resolved it, but then my switch failed later, which let me not even start the vehicle.Read more

My 2023 Kia Sportage EX Hybrid has experienced repeated engine-related failures since October 2022, requiring multiple dealer repairs. The safety issue escalated in 2026 when the vehicle experienced sudden loss of acceleration while driving on highways. In April/May 2026, the vehicle lost accelerati
... on twice within approximately 10–15 minutes. Warning messages appeared, including Check Engine Light and "Check Hybrid System – Stop Safely." The accelerator became unresponsive, requiring the vehicle to be moved to the shoulder. The dealer diagnosed P0030 and P1120 and replaced the Bank 1 Sensor 1 oxygen sensor and valve cover assembly with manufacturer technical support involved. Several months later, another highway loss-of-acceleration event occurred. The vehicle would not respond to the accelerator, RPM remained around 1,000, and it had to be safely moved off the road. The dealer diagnosed P2138 and replaced the accelerator pedal assembly. The repeated unexpected loss of acceleration creates a serious safety risk. The root cause remains unknown despite multiple repairs.Read more

This failure can lead to a sudden loss of drive power while in motion and a loss of the "Park" function, creating a significant rollaway risk. These are the same safety hazards identified in NHTSA Recall 20V-343 (W47). I am requesting that NHTSA investigate why 2014 model year vehicles with 2013 build dates are excluded from the XJ1 warranty extension for this known safety defect.Read more

I have had numerous safety warning messages display upon startup and also while driving. The messages are usually combined with a cold ambient temperature and do not display during warm temperatures. The FCA dealership has attempted to fix this issue on at least six different occasions so far withou
... t any change. To date, FCA has replaced the original battery, they have updated, flashed the ECU module, performed NHTSA Recall B4C for flashing the cluster, and most recently they replaced the ORC module. FCA indicated that NHTSA-25V882 was not applicable to my VIN despite my VIN production date in April '25; within your suspect period. Original issue remains, FCA STAR case generated however cannot replicate error condition for FCA engineers. The next scheduled visit to FCA is to replace a telematics module for possible link to issue with the Uconnect5 system. This safety issue was first noted before 800 miles and continues through 2768 miles thus far. As this issue pertains directly to a vital Air Bag sensor issue and the inability to call for a SOS assistance, I expect the NHTSA to raise this concern with FCA USA and to determine the cause of this safety defect and issue a new Recall Notification for repair and/or replacement of affected parts.Read more

On March 19, 2026, at approximately 1003 hours, I was traveling north on the [XXX] located in [XXX] . I was traveling in the number two lane and my speed was approximately 78 MPH when all of a sudden I had a loss acceleration power and my Kia EV9 began to decelerate rapidly and my dashboard began t
... o flash a loss of power warning. I had no control over my accelerator and could not speed up. Due to the rapid deceleration I had to take evasive action as northbound highway traffic was flowing smoothly. My clearest path was to move to the lefthand shoulder, which required me to cross the number one lane and the carpool lane. During this time, my Kia EV9 continued to decelerate. Once I got my vehicle onto the shoulder I placed my vehicle in park at which time my dashboard highlighted the front storage compartment of my vehicle in red. I assumed that meant my frunk was open. I exited my vehicle and checked my frunk and indeed found that my frunk was open and the first stage of the two-stage latch system was disengaged. Had the second stage of the latch system been disengaged it could have resulted in catastrophic consequences due to me traveling at highway speeds on a multilane highway. To be clear, I did not touch the interior compartment frunk release button and my keys were sitting in my left cargo short pocket located on the side of my left leg. I had also been traveling for approximately five to seven minutes and had entered the highway via the northbound [XXX] . I came to a stop at approximately the [XXX] area, which is approximately two miles north of [XXX] . I am aware that electric model Kia vehicles have a safety measure in which if the frunk or trunk is open the vehicle computer system does not allow the vehicle to engage in drive or reverse mode. Since I do have an electric vehicle, a diagnostic of my vehicle computer could reveal the cause. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

The radio signal is very poor in the car, resulting in static and high pitched noises on the stations and cannot use regular stations. There also is a huge oil consumption and oil burn smell and leak. This also seems very common for other Hyundai vehicles that have been recalled. Having to put 4-5 quarts in per month and only drive it maybe 1,500 miles in that time frame. There is a pause bumpiness when having to accelerate sometimes as well, followed by failure of oxygen sensor smells and baffles within the muffler seem to often have weird sounds. Have had the vehicle for 1 year and it was bought as a Hyundai certified pre-owned vehicle. It only has about 85K miles on it.Read more

As I read the above comments/complaints on the 2014 Dodge Dart, I have realized that we all are having the same issues with the recall, the car not engaging in park, and the Keyfob not working properly where it takes from 5-30 minutes to turn the car off because it is still in the acc mode. Now the
... car has started yet ANOTHER problem that it doesn’t want to start. I paid for a tow to the repair shop and they checked everything out and could find absolutely nothing wrong, no codes showing up, nothing. I had bought a new battery, was about to replace the ignition switch and/or starter but the shop had experience with Dodge 2014 before and the same issues with no apparent issues?? I did pay for another key and fob which wasted $200 because even afterwards the fob wouldn’t release due to the car not going into the park position 100% and I’m still having all these problems. It is definitely a safety issue and we all are left here driving this piece of junk that the manufacturer doesn’t seem to have any answer or as they say “remedy” for the issue. And that’s another thing- we are having ISSUES not an issue. This problem has more involved than the recall and for the amount of people that have submitted a complaint should be an eye opener for the manufacturer - because most people don’t even report it because they don’t know to and the service department is going to get the money! There is and has been something going on with the entire braking, parking, transmission, cable, the last recall my car I was told wasn’t affected however the brakes went out totally without any warning or problem before hand. Said there was fluid or something that messed it up and it cost me $2000 to repair it when the exact issue was going on with the dodge dart 2013-2014 but of course MY CAR was not in that recall even though it had the same problem as the cats which were affected by the recall. I’ll never never purchase another Dodge and would advise anyone else not to purchase one.Read more

12/31/2025 NHTSA, I am writing to formally report a serious safety issue involving my Ford F-150. On [XXX] in rural Alaska, my Ford F-150 experienced a flat tire in extreme winter conditions (approximately –15°F). I was traveling with my [XXX] child. When we attempted to change the tire, the
... lug nuts could not be removed using the factory tools provided with the vehicle. A tow truck was dispatched from the only available service on a holiday evening, which was 45 minutes away. The tow operator was then unable to remove the lug nuts with any of his professional equipment. We were subsequently informed that this is a known issue with Ford’s chrome-capped, two-piece lug nuts swelling or deforming, making them incompatible with standard sockets. As a result, what should have been a routine roadside tire change became a prolonged and dangerous situation for my child and me in life-threatening cold. We were stranded for 3 hours and nearly ran out of gas keeping the vehicle warm. It took a blowtorch provided by a passing Good Samaritan to complete the tire changeover. This failure created a significant safety hazard. A vehicle that cannot have a flat tire serviced during an emergency—especially in extreme weather—poses an unacceptable risk to occupants. This was not due to misuse, improper maintenance, or lack of tools, but rather an apparent design or materials defect that rendered the lug nuts unusable. Vehicle details: - Year/Model: 2017 Ford F-150 - VIN: [XXX] - Location of incident: Miles outside of Willow, Alaska This situation placed a minor child at risk in extreme winter conditions and should not occur in any Ford vehicle. The sudden loss of my IPC screen meant I could not see my speedometer, warning indicators, safety messages, or any visibility cues needed to safely judge traffic speed and spacing. This created an immediate hazardous situation and made it extremely difficult to manage both my own speed and the surrounding traffic patterns. As I recognized the danger, I attempted to brake and avoid a collision. During this moment, my steering wheel began to shake slightly—possibly due to the lane-keeping or lane-safety system responding. Despite my efforts to correct and avoid impact, the 18-wheeler collided with my vehicle. We purchased the above referenced vehicle new back in 2022 and have received numerous recalls since 2023. Many of these recalls have no remedy or remedies not yet available. I am majorly concerned because our daughter is supposed to use this vehicle in February 2026 and the way the recall repairs are trending, it may take months or years to remedy. I am writing this correspondence so you are aware of the recall and put it on your radar to track. I plan on writing again if a significant amount of time has passed with no remedy per Ford's direction if we are having difficulty getting our vehicle repaired in a "reasonable" amount of time. There is no reason Ford should be producing faulty, no remedy vehicles. For the airbag the car was sold to me without a working airbag. It’s in the steering wheel (I have been told this by a Chevrolet dealership) and the part that is broken is the part that will make it inflate in case of crash. This is scary to me. I was just in my first (and hopefully only) accident
... a month before in a more steely car and while no airbags were deployed it still hurts. The second issue is with the fuel pump or sensor - the coding is very unsure but the dash (including the mechanical gauge) says the car will go from half tank to empty on an 8 mile drive. It has no leaks on the underside and I had it into the mechanic twice. The second time because it stumbled and stalled at a stop light. Before dying - half tank. August 6th they just scanned it with their computer I got no paperwork from them. On the 15th I didn’t get any paperwork either. I will include the shop’s number.Read more

Inability to shift gears on acceleration
The Office of Defects Investigations (ODI) analyzed complaint data provided by Ford as well as complaints submitted to ODI from consumers. In total, there were 364 unique reports indicating various shift quality issues while driving.Ford identified several factors that may contribute to shift quality
... concerns in the subject vehicles, including cold transmission, clutch stay-out at high engine speeds, gear clash or grinding, and gradual loosening of clutch plate bolts in some early production vehicles. The largest percentage of complaints indicated higher than expected shift efforts in cold ambient temperatures. These complaints were related to transmission fluid viscosity and the higher shift efforts resolved themselves with the drivetrain warming.Ford published technical service bulletin TSB 11-3-18 to inform technicians that a lower viscosity fluid was available for use to address the cold shift issues. The clutch stay-out condition typically occurs at engine speeds greater than 4000 rpm.It does not prevent the selection of any gear, but may delay clutch re-engagement depending on how long the driver maintains the higher engine speed.Ford identified rotating inertia of the clutch components as the root cause and replacement clutch pedal parts were made available under Ford TSB 10-19-4.A number of reports alleged increasing difficulty selecting gears along with gear clash or grinding.Grinding or notchy gear shifts may be caused by gradual loosening of clutch plate fasteners, which is promoted by certain aggressive shifting. This condition is progressive in nature and initially results in symptoms related to shift quality/feel.Clutch plate bolt loosening will not result in inability to select any gear unless the progressive symptoms are ignored until complete clutch failure occurs.ODI?s analysis identified only five complaints alleging an inability to shift into gear due to loose clutch plate bolts.None of these resulted in a stall and gear shifting could occur before engine start.A revised fastener was designed and implemented in production and is available for service through a special service message. There is no indication of loss of motive power or unreasonable safety risk associated with the alleged defect in the subject vehicles. This preliminary evaluation is closed.Read more

ACCELERATOR PEDAL INTERFERENCE
ON APRIL 3, 2006, FORD MOTOR COMPANY SUBMITTED A DEFECT INFORMATION REPORT TO NHTSA CONCERNING A DEFECT CONDITION THAT COULD RESULT IN INTERFERENCE BETWEEN THE ACCELERATOR PEDAL AND FLOOR CARPETING IN APPROXIMATELY 19,140 MODEL YEAR 2003 AND 2004 MUSTANG COBRA VEHICLES (FORD RECALL NO. 06S43/NHTSA R
... ECALL 06V-108, COPY ATTACHED).FORD WILL RECALL THE VEHICLES TO INSTALL A SHIELD BENEATH THE ACCELERATOR PEDAL TO PREVENT THE PIVOT PIN ON THE REAR SURFACE OF THE PEDAL FROM COMING INTO CONTACT WITH THE FLOOR CARPETING DURING HEAVY THROTTLE APPLICATION.IN THOSE VEHICLES WHERE THE FLOOR CARPETING DOES NOT FIT FLUSH WITH THE SHEET METAL, THE UNIQUE REAR SURFACE PROFILE OF THE MUSTANG COBRA PEDAL MAY ALLOW THE PIVOT PIN OF THE PEDAL TO CATCH IN THE CUTOUT ON THE FLOOR CARPETING BEHIND THE PEDAL, INTERFERING WITH THE PEDAL'S ABILITY TO RETURN; THE RESULT OF WHICH COULD BE UNEXPECTED, CONTINUED THROTTLE APPLICATION.DURING THE ENGINEERING ANALYSIS STAGE OF THE INVESTIGATION, ODI ALSO EXAMINED THE POSSIBLITY OF THE SAME PHENOMENA OCCURING IN NON-COBRA MUSTANGS.HOWEVER, THE DIFFERENT DESIGN OF THE ACCELERATOR PEDALS ON THE NON-COBRA MUSTANGS RESULTS IN SIGNIFICANT DIFFERENCES IN THE POTENTIAL CONTACT BETWEEN THE ACCELERATOR PEDAL AND THE CARPETING, WITH THE BASE OF THE PEDAL PAD ACTING TO FORCE THE CARPET AWAY FROM THE PIVOT PIN.THIS ENGINEERING ANALYSIS HAS BEEN CLOSED.Read more

ACCELERATOR PEDAL INTERFERENCE
PE05-038 WAS OPENED ON JULY 7, 2005, WITH 11 CONSUMER COMPLAINTS TO ODI ALLEGING INCIDENTS OF THROTTLE STICKING DUE TO ACCELERATOR PEDAL INTERFERENCE WITH THE CARPET.ODI HAS RECEIVED AN ADDITIONAL 24 COMPLAINTS SINCE THE PE WAS OPENED.DURING PE05-038, FORD SUPPLIED COMPLAINT AND WARRANTY INFORMATION
... REGARDING 86 ADDITIONAL INCIDENTS OF ACCELERATOR PEDAL INTERFERENCE IN MODEL YEAR 2003 TO 2004 MUSTANG COBRA VEHICLES.EIGHTY-FIVE OF THESE HAD CONCERNS OF ACCELERATOR PEDAL STICKING, 70 OF WHICH WERE ANALYZED AS BEING RELATED TO CARPET INTERFERENCE.ODI IS CONTINUING ITS ANALYSIS OF THE CAUSAL FACTORS IN THE REMAINING 15 INCIDENTS.THIS PRELIMINARY EVALUATION HAS BEEN UPGRADED TO AN ENGINEERING ANALYSIS.Read more
