I am filing this complaint regarding Honda's reimbursement program associated with the piston ring warranty extension related to engine misfire issues on certain vehicles. Prior to Honda issuing the warranty extension, I paid out of pocket to have the repair completed at an authorized Honda dealers
... hip. After Honda announced the warranty extension and reimbursement program, I submitted a reimbursement claim along with all required documentation and proof of payment in August 2025. My reimbursement claim has now been pending for approximately 11 months. Since submitting the claim, I have contacted Honda numerous times seeking an update. On multiple occasions, Honda representatives advised me that the reimbursement had been approved and was simply awaiting issuance of payment. However, no reimbursement has been received. Due to the lack of progress and conflicting information, I filed a complaint with my state's Attorney General's Office. In response to that complaint, Honda stated that the reimbursement claim was still under review, which directly contradicts what I had previously been told by Honda representatives. I have complied with all documentation requirements and have made repeated attempts to obtain a clear answer regarding the status of my claim. Despite this, I have not received reimbursement, a definitive status update, or a reasonable explanation for why a reimbursement claim related to a known warranty extension issue has remained unresolved for nearly a year. I am requesting review of this matter and whether Honda is properly administering and processing reimbursement claims associated with this warranty extension program within a reasonable timeframe.Read more

I own a 2024 Ford F-150 Lightning Platinum built in August 2024. The vehicle has been at the dealership for over 45 days for severe, unresolved electrical safety defects. I have attached photographic proof of the main defects. The first three photos show the driver's instrument display screen behin
... d the steering wheel completely blacking out on startup, blinding critical safety gauges like the speedometer. The center screen stays on, but the main cluster is totally dead. Fourth and fifth photos show the driver's seat memory failure. I manually set my lumbar support to a comfortable 3-4-3 setting and save it, but every time the truck sits, the seat completely deflates to a flat zero default on startup. This causes an immediate driving distraction and ergonomic hazard. The factory navigation also randomly shouts driving directions out of nowhere without any driver input. The dealership is currently trying to install a temporary "bypass wire" hack on my factory harness just to see if it fixes the seat memory. However, Ford's internal hotline (SSSC) refuses to authorize repairs for the black screen or phantom navigation until a tech physically catches it glitching on command. Ford’s policy is to return the vehicle unrepaired if it does not fail during active observation. I am requesting NHTSA to investigate Ford's active evasion of safety recall 25V540 (Ford 25S88). The official recall paperwork states the defective supplier software entered production on May 14, 2024. My truck was manufactured in August 2024—squarely within the defective window—yet Ford is using a model-year loophole to exclude 2024 vehicles from the recall population. They are denying owners a permanent factory software remedy and forcing dealers to use unapproved, temporary physical workarounds. The vehicle is at the dealership and available for federal inspection if need be.Read more

My 2014 Jeep Cherokee Trailhawk (Active Drive II / Sales Code DK4) has a confirmed DTC C14A7-97, indicating the PTU Motor is obstructed or blocked. This failure can lead to a sudden loss of drive power while in motion and a loss of the "Park" function, creating a significant rollaway risk. These are the same safety hazards identified in NHTSA Recall 20V-343 (W47). I am requesting that NHTSA investigate why 2014 model year vehicles with 2013 build dates are excluded from the XJ1 warranty extension for this known safety defect.Read more

I have had numerous safety warning messages display upon startup and also while driving. The messages are usually combined with a cold ambient temperature and do not display during warm temperatures. The FCA dealership has attempted to fix this issue on at least six different occasions so far withou

To date, FCA has replaced the original battery, they have updated, flashed the ECU module, performed NHTSA Recall B4C for flashing the cluster, and most recently they replaced the ORC module. FCA indicated that NHTSA-25V882 was not applicable to my VIN despite my VIN production date in April '25; within your suspect period. Original issue remains, FCA STAR case generated however cannot replicate error condition for FCA engineers. The next scheduled visit to FCA is to replace a telematics module for possible link to issue with the Uconnect5 system. This safety issue was first noted before 800 miles and continues through 2768 miles thus far. As this issue pertains directly to a vital Air Bag sensor issue and the inability to call for a SOS assistance, I expect the NHTSA to raise this concern with FCA USA and to determine the cause of this safety defect and issue a new Recall Notification for repair and/or replacement of affected parts.Read more

While driving at normal city speeds, the vehicle suddenly displayed a warning message stating “Stop vehicle and check power supply,” accompanied by a red battery warning indicator. Shortly after the warning appeared, the vehicle entered a reduced-power or “limp” mode, with severely limited a
... cceleration and speed. This created a safety concern as the vehicle could not maintain normal traffic flow. The high-voltage traction battery state of charge was normal at the time, indicating the issue was not related to battery depletion. Prior to the failure, there were no unusual driving conditions, accidents, or modifications to the vehicle. Ambient temperature was cool but within normal operating conditions. Due to the loss of power and safety risk, the vehicle was not driven further and was towed to an authorized Hyundai dealership for inspection. The problem was confirmed by the dealer, who diagnosed a failure of the Integrated Charging Control Unit (ICCU), which manages charging and power conversion between the high-voltage system and the 12-volt electrical system. The dealer determined that the ICCU requires full replacement. The vehicle remains at the dealership awaiting parts and is available for inspection. The failure resulted in sudden loss of drivability without prior meaningful warning, posing a potential hazard to the driver and surrounding traffic.Read more

I paid for a tow to the repair shop and they checked everything out and could find absolutely nothing wrong, no codes showing up, nothing. I had bought a new battery, was about to replace the ignition switch and/or starter but the shop had experience with Dodge 2014 before and the same issues with no apparent issues?? I did pay for another key and fob which wasted $200 because even afterwards the fob wouldn’t release due to the car not going into the park position 100% and I’m still having all these problems. It is definitely a safety issue and we all are left here driving this piece of junk that the manufacturer doesn’t seem to have any answer or as they say “remedy” for the issue. And that’s another thing- we are having ISSUES not an issue. This problem has more involved than the recall and for the amount of people that have submitted a complaint should be an eye opener for the manufacturer - because most people don’t even report it because they don’t know to and the service department is going to get the money! 12/31/2025 NHTSA, I am writing to formally report a serious safety issue involving my Ford F-150. On [XXX] in rural Alaska, my Ford F-150 experienced a flat tire in extreme winter conditions (approximately –15°F). I was traveling with my [XXX] child. When we attempted to change the tire, the
... lug nuts could not be removed using the factory tools provided with the vehicle. A tow truck was dispatched from the only available service on a holiday evening, which was 45 minutes away. The tow operator was then unable to remove the lug nuts with any of his professional equipment. We were subsequently informed that this is a known issue with Ford’s chrome-capped, two-piece lug nuts swelling or deforming, making them incompatible with standard sockets. As a result, what should have been a routine roadside tire change became a prolonged and dangerous situation for my child and me in life-threatening cold. We were stranded for 3 hours and nearly ran out of gas keeping the vehicle warm. It took a blowtorch provided by a passing Good Samaritan to complete the tire changeover. This failure created a significant safety hazard. A vehicle that cannot have a flat tire serviced during an emergency—especially in extreme weather—poses an unacceptable risk to occupants. This was not due to misuse, improper maintenance, or lack of tools, but rather an apparent design or materials defect that rendered the lug nuts unusable. Vehicle details: - Year/Model: 2017 Ford F-150 - VIN: [XXX] - Location of incident: Miles outside of Willow, Alaska This situation placed a minor child at risk in extreme winter conditions and should not occur in any Ford vehicle. The instrument cluster seems to fail more so in the hotter weather. The speedometer, RPMs and gas gauge just stop working altogether. This seems extremely similar to the issues of other Hyundai vehicles that were recalled. The blind spot monitoring turns on and off as it pleases and there is a me

The radio signal is very poor in the car, resulting in static and high pitched noises on the stations and cannot use regular stations. There also is a huge oil consumption and oil burn smell and leak. This also seems very common for other Hyundai vehicles that have been recalled. Having to put 4-5 quarts in per month and only drive it maybe 1,500 miles in that time frame. There is a pause bumpiness when having to accelerate sometimes as well, followed by failure of oxygen sensor smells and baffles within the muffler seem to often have weird sounds. Accident Statement – Instrument Panel Cluster Failure & Collision While driving my 2025 Ford Outerbanks, the Instrument Panel Cluster (IPC) screen suddenly went completely black without warning. At the time of the malfunction, I was attempting to merge into traffic on my left with my left turn si

The sudden loss of my IPC screen meant I could not see my speedometer, warning indicators, safety messages, or any visibility cues needed to safely judge traffic speed and spacing. This created an immediate hazardous situation and made it extremely difficult to manage both my own speed and the surrounding traffic patterns. As I recognized the danger, I attempted to brake and avoid a collision. During this moment, my steering wheel began to shake slightly—possibly due to the lane-keeping or lane-safety system responding. Despite my efforts to correct and avoid impact, the 18-wheeler collided with my vehicle. At the time of the crash, I had no visibility of any dashboard warnings, alerts, vehicle speed, or safety notifications due to the IPC failure. I am a current lessee of a 2023 Volkswagen ID.4 (VIN: [XXX] ), leased on [XXX], under a 36‑month agreement (Lease Account #: [XXX]). The vehicle is still under warranty at 23,000 miles. The navigation/GPS system has a persistent defect (location drift far from actual position), verified by dealer

I am majorly concerned because our daughter is supposed to use this vehicle in February 2026 and the way the recall repairs are trending, it may take months or years to remedy. I am writing this correspondence so you are aware of the recall and put it on your radar to track. I plan on writing again if a significant amount of time has passed with no remedy per Ford's direction if we are having difficulty getting our vehicle repaired in a "reasonable" amount of time. While stopped at a stop light at an intersection in our 2021 Hond Odyssey, the idle auto off feature kicked in, and then as we let off the brake and went to hit the gas to move forward the car shut off completely. The car had to be restarted by applying the brake and pressing the start/stop ignition
... button. This is not the first time this has happened with this vehicle. This also happened a series of times in 2023 and Honda performed a series of software updates, replaced the battery, and replaced a sensor on the battery. However, now two years later the idle auto off issue occured again.Read more

I drive a 2017 Jeep Cherokee and have experienced repeated instances of power loss while driving. The vehicle will suddenly jerk, hesitate, or feel like its struggling to maintain momentum, especially at lower speeds or when turning. On one occasion, it felt like the transmission slipped and I brief
... ly lost acceleration, which was very alarming and could have caused a crash if I been merging or crossing traffic. I've also had the "Service 4WD" warning light appear intermittently. I took the vehicle to both a jeep dealership and an independent shop for diagnostics, but neither was able to find an active trouble code. However, the symptoms have continued and are getting worse. After reading about the recent recall (NHTSA Campaign 25V011) involving the Power Transfer Unit (PTU) in 2017-2019 Jeep Cherokees, I believe this may be an issue I am experiencing. At this time, Jeep has not released a fix, and I'm concerned that the vehicle is unsafe to drive until the PTU can be inspected and repaired. This seems to be a known and widespread issue that puts drivers at risk, and I'm hoping this report helps move the investigation forward.Read more

Hood on 2025 Toyota Grand Highlander vibrates/flutters due to poorly designed aluminum construction. Very distracting and causes drivers to take eyes off the road. Toyota has issued T-SB-0111-24 for the 2024 Grand Highlander for this specific issue but my dealership told me that it doesn't apply to
... the 2025 but the hood part number is the same for both years. The constant vibration could eventually "work harden" the thin metal and ultimatlly allow the hood to break off the vehicle. I am writing to formally file a complaint regarding a prolonged and troubling service experience at Jaguar Land Rover Buckhead, related to my 2018 Range Rover Sport (approximately 49,000 miles), and to express dissatisfaction with the handling of a prior related incident. My vehicle has been the dea
... lership approx. 2 weeks with no loaner and me missing days from work and not able to transport my children to school. For past context, In 2023, I experienced a faulty ground stud issue that left me stranded on the highway during my morning commute. I later learned this exact issue had been subject to a recall on the Discovery model — but not on mine — despite identical symptoms. As a result, I was issued a $675 Owner Loyalty Certificate (OLC) by Molly B. of Land Rover USA, which I appreciated, but which I believe did not fully compensate for the risk and inconvenience caused. Recently, I returned to the another dealer, Jaguar Land Rover Buckhead, in May 2025 for a new issue. I was told there was a broken exhaust bolt that delayed repairs. I specifically asked the service advisor if I was responsible for the bolt breaking during service and was told I was not, though it was framed as a “common issue” across many vehicles. I respectfully disagreed, noting I’ve owned multiple vehicles with higher mileage and never encountered such a failure. I also suggested that if this is so common, it may reflect a design flaw that Land Rover should address. Additionally, I was misled by the service advisor, Cole Eidson, who sent me a message on Tuesday, May 28th, stating the bolt was already removed and the exhaust installation was underway. I later discovered this was not true, and that the bolt had not yet been extracted at the time of that message. This misrepresentation caused unnecessary confusion, delay, and loss of trust. This pattern — first with the unresolved ground stud failure, then with misinformation about repairs and unexplained delays — leaves me deeply disappointed withRead more

Inability to shift gears on acceleration
The Office of Defects Investigations (ODI) analyzed complaint data provided by Ford as well as complaints submitted to ODI from consumers. In total, there were 364 unique reports indicating various shift quality issues while driving.Ford identified several factors that may contribute to shift quality
... concerns in the subject vehicles, including cold transmission, clutch stay-out at high engine speeds, gear clash or grinding, and gradual loosening of clutch plate bolts in some early production vehicles. The largest percentage of complaints indicated higher than expected shift efforts in cold ambient temperatures. These complaints were related to transmission fluid viscosity and the higher shift efforts resolved themselves with the drivetrain warming.Ford published technical service bulletin TSB 11-3-18 to inform technicians that a lower viscosity fluid was available for use to address the cold shift issues. The clutch stay-out condition typically occurs at engine speeds greater than 4000 rpm.It does not prevent the selection of any gear, but may delay clutch re-engagement depending on how long the driver maintains the higher engine speed.Ford identified rotating inertia of the clutch components as the root cause and replacement clutch pedal parts were made available under Ford TSB 10-19-4.A number of reports alleged increasing difficulty selecting gears along with gear clash or grinding.Grinding or notchy gear shifts may be caused by gradual loosening of clutch plate fasteners, which is promoted by certain aggressive shifting. This condition is progressive in nature and initially results in symptoms related to shift quality/feel.Clutch plate bolt loosening will not result in inability to select any gear unless the progressive symptoms are ignored until complete clutch failure occurs.ODI?s analysis identified only five complaints alleging an inability to shift into gear due to loose clutch plate bolts.None of these resulted in a stall and gear shifting could occur before engine start.A revised fastener was designed and implemented in production and is available for service through a special service message. There is no indication of loss of motive power or unreasonable safety risk associated with the alleged defect in the subject vehicles. This preliminary evaluation is closed.Read more

ACCELERATOR PEDAL INTERFERENCE
ON APRIL 3, 2006, FORD MOTOR COMPANY SUBMITTED A DEFECT INFORMATION REPORT TO NHTSA CONCERNING A DEFECT CONDITION THAT COULD RESULT IN INTERFERENCE BETWEEN THE ACCELERATOR PEDAL AND FLOOR CARPETING IN APPROXIMATELY 19,140 MODEL YEAR 2003 AND 2004 MUSTANG COBRA VEHICLES (FORD RECALL NO. 06S43/NHTSA R
... ECALL 06V-108, COPY ATTACHED).FORD WILL RECALL THE VEHICLES TO INSTALL A SHIELD BENEATH THE ACCELERATOR PEDAL TO PREVENT THE PIVOT PIN ON THE REAR SURFACE OF THE PEDAL FROM COMING INTO CONTACT WITH THE FLOOR CARPETING DURING HEAVY THROTTLE APPLICATION.IN THOSE VEHICLES WHERE THE FLOOR CARPETING DOES NOT FIT FLUSH WITH THE SHEET METAL, THE UNIQUE REAR SURFACE PROFILE OF THE MUSTANG COBRA PEDAL MAY ALLOW THE PIVOT PIN OF THE PEDAL TO CATCH IN THE CUTOUT ON THE FLOOR CARPETING BEHIND THE PEDAL, INTERFERING WITH THE PEDAL'S ABILITY TO RETURN; THE RESULT OF WHICH COULD BE UNEXPECTED, CONTINUED THROTTLE APPLICATION.DURING THE ENGINEERING ANALYSIS STAGE OF THE INVESTIGATION, ODI ALSO EXAMINED THE POSSIBLITY OF THE SAME PHENOMENA OCCURING IN NON-COBRA MUSTANGS.HOWEVER, THE DIFFERENT DESIGN OF THE ACCELERATOR PEDALS ON THE NON-COBRA MUSTANGS RESULTS IN SIGNIFICANT DIFFERENCES IN THE POTENTIAL CONTACT BETWEEN THE ACCELERATOR PEDAL AND THE CARPETING, WITH THE BASE OF THE PEDAL PAD ACTING TO FORCE THE CARPET AWAY FROM THE PIVOT PIN.THIS ENGINEERING ANALYSIS HAS BEEN CLOSED.Read more

PE05-038 WAS OPENED ON JULY 7, 2005, WITH 11 CONSUMER COMPLAINTS TO ODI ALLEGING INCIDENTS OF THROTTLE STICKING DUE TO ACCELERATOR PEDAL INTERFERENCE WITH THE CARPET.ODI HAS RECEIVED AN ADDITIONAL 24 COMPLAINTS SINCE THE PE WAS OPENED.DURING PE05-038, FORD SUPPLIED COMPLAINT AND WARRANTY INFORMATION
... REGARDING 86 ADDITIONAL INCIDENTS OF ACCELERATOR PEDAL INTERFERENCE IN MODEL YEAR 2003 TO 2004 MUSTANG COBRA VEHICLES.EIGHTY-FIVE OF THESE HAD CONCERNS OF ACCELERATOR PEDAL STICKING, 70 OF WHICH WERE ANALYZED AS BEING RELATED TO CARPET INTERFERENCE.ODI IS CONTINUING ITS ANALYSIS OF THE CAUSAL FACTORS IN THE REMAINING 15 INCIDENTS.THIS PRELIMINARY EVALUATION HAS BEEN UPGRADED TO AN ENGINEERING ANALYSIS.Read more
