I kept pressing on the gas to get going and the van wouldn't move. Meanwhile traffic behind me is gaining on me and I was rear-ended. After being hit I tried to move it out of the travel lane and it still wouldn't move. After all that I looked at the dash and it said to move forward, release the electronic parking brake (or something to that effect). I pushed the button to turn it off, and was able to get onto the shoulder. I believe the root cause of the crash was that the van automatically turned on the parking brake on it's own, and I did not want it on. If it hadn't, I could've accelerated away and not gotten rear-ended.Read more

The dealer cleared the codes, and the vehicle was then taken to another dealer, where it was diagnosed and determined that the ground strap needed to be replaced. The vehicle was not repaired. The manufacturer was notified of the failure and informed the contact that the part was on back order and might take six months for the part to do the repair to be available. The dealership advised me that the vehicle had coolant intrusion involving cylinder 4 and that the engine would need to be replaced. Because this was a serious diagnosis, I then took the vehicle to another reputable automotive repair shop for a second opinion. That shop also advised that the engine needed to be replaced due to the coolant intrusion issue. This creates a safety concern because coolant intrusion can lead to misfires, engine damage, overheating, loss of power, stalling, or engine failure while the vehicle is being driven. I was more than 250 miles from home and had to avoid driving the vehicle because of the risk of further engine damage or possible failure on the road. The vehicle is available for inspection upon request. The issue was inspected by the dealership and by an independent automotive repair shop. No crash, fire, injury, or death occurred. The check engine light came on while driving during a vacation. No other warning message is known at this time.Read more

After examining the vehicle, the technician advised that there was no evidence of an external impact, such as a rock strike or other object, that would have caused the glass to break. Because the window appeared to shatter without any apparent outside force, I am concerned that this may indicate a defect in the rear glass or its installation. Before the snow fell, the crevice was mostly covered with a shield to keep snow from falling into it. The wipers were put into the "snow position" but when used again today, the wipers have again failed to operate in sync. The drivers side wiper doesn't move fully and the passenger side wiper does. Car wipers should work when it snows, but especially when precautions have been taken. This is extremely unexpected and atypical.Read more

The system did not return to normal functionality until the vehicle was parked and shut off overnight. This indicates a deep software logic error that resists manual resets and only clears after a prolonged power-down cycle. This failure resulted in a significant safety hazard. The dimness rendered the navigation system unusable, causing extreme driver distraction as I had to take my eyes off the road to attempt to read the screen. The unpredictable nature of this glitch means it could recur at any time, potentially disabling access to critical vehicle safety information or navigation during transit. I performed a manual software reboot and cycled the ignition multiple times, yet the screen remained dim. The hardware clearly had the capacity for full brightness (as evidenced by the backup camera), but the software refused to restore daytime levels for any other function until the car performed a "deep sleep" cycle overnight.Read more

The van has been to the dealership 4 times for these messages. They have replaced components, tried to find loose wires and connectors, replaced the 12V battery, and other fixes. The van will drive normally for a few weeks, and then they come back. On the latest visit (4th since 73B recall) it's been at the dealership for 31 days and counting. The dealership is waiting on Chrysler to figure out a fix for the van. The vehicle has over 50 stored messages. This recall was supposed to monitor the hybrid battery for early indications for a fire, but according to the original recall all batteries are assumed to have the manufacturing defect that leads to spontaneous fires. This van is unsafe, and we never know when it will work correctly. The van operated normally before the recall software was installed.Read more

The dealer diagnosed the vehicle and determined that the lifters and engine sensors had failed. The vehicle was not repaired. The contact received notification of NHTSA Campaign Number: 25V274000 (Engine and Engine Cooling). The dealer was notified of the recall, and the contact was informed that the dealer was waiting for a response from the manufacturer regarding the recall repair. The dealer later advised that an attempt to rebuild the engine was planned. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The vehicle was not repaired. The manufacturer was not informed of the failure. The failure mileage was approximately 56,000.Read more

On [XXX], I was driving the vehicle and accelerating onto the [XXX] freeway from an on-ramp in northern Phoenix, Arizona. During acceleration, the truck suddenly and violently downshifted, producing an abrupt jerking motion that forcibly displaced the vehicle out of the accelerating gear and into a
... lower gear without warning or driver input. The incident required us to exit the freeway and pull over. The event caused a momentary but pronounced loss of smooth propulsion at a critical merge point on to the freeway, creating an obvious risk of loss of control and collision with other motorists. The sudden lock-up and downshift, spike in RPMs, and deceleration were entirely inconsistent with normal operation and were objectively dangerous under highway-speed conditions. My passenger and I were jerked forward into our seatbelts. After this incident, I had the vehicle's transmission checked at a small shop, which stated they did not find any codes or issues, but recommended a full transmission rebuild at a cost of several thousand dollars. Finding no diagnostic codes or issues, I continued using my truck until it unexpectedly failed the following month on a freeway. In December 2025, while I was exiting a freeway in Mesa, Arizona, the F-150’s 6R80 transmission downshifted again without being prompted, causing similar critical safety concerns. But this time the transmission was stuck in gear and was unsafe to operate on the highway or city streets. It had to be towed. A repair shop determined that the output speed sensor was found faulty at all times with diagnostic trouble codes of P720, P0722, and P2535, requiring complete replacement of the Leadframe Assembly. To the best of my knowledge and belief, this is consistent with other recalls that Ford has issued related to the failure of its 6R80 transmissions in F-150s and similar vehicles for years 2011-2014, which are known to Ford to have a risk of crashing due to the sudden downshifting concerns. ... rvice Trailer Light,” “Service Antilock Brake System”, Service Electronic Braking”, “Service Trailer Brake System” The dealership diagnosed the problem as a faulty ABS Module. I was informed that the part would cost approximately $1,400, plus the cost of ordering the part, and that the part may take up to four (4) years or more to arrive—if it ever becomes available at all. I contacted multiple Dodge dealerships and received the same response regarding the extreme delay and unavailability of the ABS Module. I was also told that the vehicle is unsafe to drive in its current condition. This is especially concerning, as this is my only vehicle, and I cannot use it due to the safety risks. Despite this, Dodge customer service informed me that no assistance can be provided unless I first pay for and order the part, even though the part may not arrive for years. Additionally, the dealership is unwilling to buy back the vehicle at a reasonable price due to the unresolved ABS issue, leaving me without a safe, drivable vehicle and without support or alternatives. I have confirmed that there is no recall listed for this issue, yet the ABS Module is a critical safety component, and the lack of part availability for years is unacceptable for a loyal customer who has maintained the vehicle properly and driven it minimally. I am respectfully requesting that Dodge Corporate provide A clear explanation for the extreme parts delay, A prompt solution—such as expedited part sourcing, coverage or assistance with repairs, or a reasonable buyback/exchange option, Guidance on how Dodge intends to support customers facing major safety issues caused by unavailable parts.Read more

He actually demonstrated how to do it. The Honda Dealerships are aware of the problem and admit it’s a manufacturing design flaw. I have tolerated this for several years thinking Honda would offer compensation for the problem. However, yesterday when it was working, an alert appeared in the middle of the display about an issue with my fuel cap. I’m aware that the fuel cap issue is related to a recall, but if the display had not been working I wouldn’t have seen the alert because I wasn’t aware that alerts appear on that display and I realize now that I can miss an urgent alert. To me that’s a huge safety issue! I’ve seen hundreds of complaints online about this, and I know it’s caused by cracks in the solder. I would like for Honda to be accountable and let customers have this repaired at no charge.Read more

I am filing a formal complaint regarding a serious safety concern with my 2021 CHEVROLET SILVERADO, following the completion of recall N252494001 performed by Galleria Chevrolet in Dallas, TX. Prior to this recall work and related inspections, my vehicle operated without any abnormal engine noise.

After the recall inspection and a subsequent fuel injector repair performed by the dealer, my vehicle began producing a bottom-end engine noise that had never been present before, and ultimately we have catastrophic engine failure. What is highly concerning is that the dealership staff previously documented the situation in writing as follows: “We performed the recall inspection and the truck passed. After performing an injector repair now there is a bottom-end noise when it's running. Did the failed injector possibly affect the validity of the test? That may be the case. We know that the truck did not experience the noise before the inspection; is it possible that the steps we took affected the truck in a way that got the noise to show itself? That’s what we need to determine.” Despite this acknowledgment, the dealership is now denying any responsibility, classifying the damage as unrelated “wear and tear”. This reversal appears highly suspicious, as it contradicts their own observations and written notes. It suggests that the dealership and GM may be refusing to fulfill their obligations under the recall and repair warranty, leaving me with a vehicle that has a blown engine and a significant safety risk. Given the timing of the noise, occurring directly after recall work and injector repair, and the internal documentation from the dealership itself, I request NHTSA investigate whether: This issue is part of a broader trend affecting other vehicles under the same recall. GM and its dealers are properly honoring their obligations to fully repair vehicles impacted by recalls. Consequential damage resulting from recall work is being systematically denied coverage.Read more

The dealer said that it was the torque converter and that it was very bad. The car is too new to have these types of catastrophic issues. It was $9000 to resolve the issue according to the dealer. I had to go somewhere else. Also the screen has problems where it turns blue, and stops working, where the music playing will also stop and you cannot skip to another song. A hard reboot is the solution, but on a daily basis? I know that other models, the Aviator and Corsair, have the same issues. I have the same problem. When contacting Lincoln dealer for the same issue, they state there is no recall. I really like the Lincoln but there seem to be quite a few problems. Will these be placed in a recall list? Inability to shift gears on acceleration
The Office of Defects Investigations (ODI) analyzed complaint data provided by Ford as well as complaints submitted to ODI from consumers.In total, there were 364 unique reports indicating various shift quality issues while driving.Ford identified several factors that may contribute to shift quality
... concerns in the subject vehicles, including cold transmission, clutch stay-out at high engine speeds, gear clash or grinding, and gradual loosening of clutch plate bolts in some early production vehicles.The largest percentage of complaints indicated higher than expected shift efforts in cold ambient temperatures.These complaints were related to transmission fluid viscosity and the higher shift efforts resolved themselves with the drivetrain warming.Ford published technical service bulletin TSB 11-3-18 to inform technicians that a lower viscosity fluid was available for use to address the cold shift issues.The clutch stay-out condition typically occurs at engine speeds greater than 4000 rpm.It does not prevent the selection of any gear, but may delay clutch re-engagement depending on how long the driver maintains the higher engine speed.Ford identified rotating inertia of the clutch components as the root cause and replacement clutch pedal parts were made available under Ford TSB 10-19-4.A number of reports alleged increasing difficulty selecting gears along with gear clash or grinding.Grinding or notchy gear shifts may be caused by gradual loosening of clutch plate fasteners, which is promoted by certain aggressive shifting.This condition is progressive in nature and initially results in symptoms related to shift quality/feel.Clutch plate bolt loosening will not result in inability to select any gear unless the progressive symptoms are ignored until complete clutch failure occurs.ODI?s analysis identified only five complaints alleging an inability to shift into gear due to loose clutch plate bolts.None of these resulted in a stall and gear shifting could occur before engine start.A revised fastener was designed and implemented in production and is available for service through a special service message.There is no indication of loss of motive power or unreasonable safety risk associated with the alleged defect in the subject vehicles.This preliminary evaluation is closed.Read more
