SYSTEMIC POWERTRAIN SAFETY DEFECT: Catastrophic Loss of Propulsion & Failure of Dual-Motor Redundancy. 1. Incident & Safety Risk: While traveling in the far-left HOV lane of a 6-lane interstate highway, the vehicle experienced an uncommanded, unwarranted Automatic Emergency Braking (AEB) interventi
... on during a controlled lane change. This sudden deceleration instantly triggered a complete powertrain collapse and total loss of propulsion. The vehicle came to a dead stop in a live highway lane. The driver (me), passenger, and our [XXX] infant were stranded in lane 3 of a 6 lane highway for ~35 minutes until Highway Patrol units arrived to manually push the vehicle across live traffic to the shoulder. This near-miss was an immediate, severe life-safety crisis not just for the occupants but also to others on the road. 2. Symptoms & Alert Codes: There were zero warning symptoms prior to the incident. Immediately upon the power collapse, the touchscreen displayed critical faults. Active diagnostic telemetry confirmed alert codes: DI_a137 (No Capable Drive Units) DI_a063 (System Graceful Power Off) 3. Manufacturer Confirmation & Inspection Status: The failure was confirmed in writing by the manufacturer’s service center. The diagnostic invoice explicitly states: "The rear drive inverter experienced an unintended reset due to an error. Sudden power reduction and shutdown while driving." The vehicle is currently held at the manufacturer's facility and is fully available for inspection. 4. Engineering Assessment: As a mechanical engineer, my assessment is the surge through the AEB event to the inverter’s DC-link was unable to damp this load, forcing a hardware logic reset. Crucially, the software logic failed to partition this fault. I own a 2019 GMC Sierra 1500 Denali equipped with the 6.2L L87 engine. On or around December 31, 2025, the vehicle developed a loud knocking noise from the engine that became worse with RPM increase. The dealership initially diagnosed valvetrain-related failure and replaced multiple top-end components including lifters, rocker arms, camshaft-related parts, belts, pulleys, hoses, and associated hardware. After reassembly, the engine reportedly started briefly and then failed again. The dealership then performed additional teardown and determined the lower end of the engine had also failed. I was informed by dealership staff that rod bearing failure is “very common” on the L87 6.2L engines. Due to the extent of the damage, the vehicle ultimately required a complete long block replacement. My truck remained out of service from approximately 12/31/2025 until 5/1/2026, a period of roughly four months. During that time, there were repeated delays involving diagnosis, inspections, warranty approvals, and additional teardown procedures. The vehicle had approximately 111,858 miles at the time of failure and had been regularly maintained. Prior to the engine failure, the truck exhibited abnormal engine ticking/knocking noises under load and higher RPM operation. The final repair performed was replacement of the engine long block assembly. I am submitting this report because the dealership indicated this type of lower-end/rod bearing failure is common on the GM 6.2L L87 platform, and I believe this issue may represent a broader reliability and safety concern affecting other owners.Read more

... rear end, accompanied by violent clanking and grinding noises from the driveline area. The gearbox then stalled, leaving the vehicle disabled in the middle of the highway. Upon inspection, it was determined that a rear differential mounting bolt had sheared, causing the differential to shift violently out of position. This sudden movement resulted in the driveshaft breaking, which further destabilized the vehicle and led to a severe and immediate loss of control. This failure occurred without warning and created an extremely dangerous situation. The sudden loss of drivetrain integrity at highway speed placed both myself and nearby motorists at serious risk of a crash. A component as critical as a differential mount or driveshaft should not fail under normal operating conditions. This incident strongly indicates a serious design or manufacturing defect affecting the vehicle’s powertrain and structural safety. Requested Action: I urge the NHTSA to investigate rear differential mounting and driveshaft failures on this vehicle model. A visual inspection was performed and they informed me that there is a leak and it appears to be coming from the top of the tank or an area above the tank. It is hard to identify the exact source without lowering the tank. They mechanic also informed me that Ford has a safety recall issue for something similar, 13S04. I checked with Ford and while there is a recall for the same model and year of my vehicle, my 2013 Explorer is not a part of the recall. Around 5/22 I called in requesting an update and was put on the phone with the service tech to describe my issues On 6/6/25 John from courtesy ford left a 2 minute long rambling voicemail, 6/3 he said he received a fax. He said he spoke to service department and I could drive the car, that I could be driving the car now. "he wasn't sure what was going on with it" They've driven it and can't find anything. Inability to shift gears on acceleration
The Office of Defects Investigations (ODI) analyzed complaint data provided by Ford as well as complaints submitted to ODI from consumers.In total, there were 364 unique reports indicating various shift quality issues while driving.Ford identified several factors that may contribute to shift quality
... concerns in the subject vehicles, including cold transmission, clutch stay-out at high engine speeds, gear clash or grinding, and gradual loosening of clutch plate bolts in some early production vehicles.The largest percentage of complaints indicated higher than expected shift efforts in cold ambient temperatures.These complaints were related to transmission fluid viscosity and the higher shift efforts resolved themselves with the drivetrain warming.Ford published technical service bulletin TSB 11-3-18 to inform technicians that a lower viscosity fluid was available for use to address the cold shift issues.The clutch stay-out condition typically occurs at engine speeds greater than 4000 rpm.It does not prevent the selection of any gear, but may delay clutch re-engagement depending on how long the driver maintains the higher engine speed.Ford identified rotating inertia of the clutch components as the root cause and replacement clutch pedal parts were made available under Ford TSB 10-19-4.A number of reports alleged increasing difficulty selecting gears along with gear clash or grinding.Grinding or notchy gear shifts may be caused by gradual loosening of clutch plate fasteners, which is promoted by certain aggressive shifting.This condition is progressive in nature and initially results in symptoms related to shift quality/feel.Clutch plate bolt loosening will not result in inability to select any gear unless the progressive symptoms are ignored until complete clutch failure occurs.ODI?s analysis identified only five complaints alleging an inability to shift into gear due to loose clutch plate bolts.None of these resulted in a stall and gear shifting could occur before engine start.A revised fastener was designed and implemented in production and is available for service through a special service message.There is no indication of loss of motive power or unreasonable safety risk associated with the alleged defect in the subject vehicles.This preliminary evaluation is closed.Read more
