Defect Description

General Motors LLC (GM) is recalling certain model year 2009-2010 Chevrolet Impala vehicles manufactured April 25, 2008 through February 16, 2010. The front passenger seat frame may contact and damage the wires of the passenger presence sensor module.

Consequence

If the wires are damaged, the passenger presence sensory system may fail to recognize that the passenger seat is occupied, disabling the air bag. Damage may also cause the air bag fuse to short resulting in a loss of all air bags and seat belt pretensions. Both conditions increase the risk of inju... Read more

Remedy

GM will notify owners, and dealers will double wrap the wires with anti-abrasion tape and replace damaged wires as necessary, free of charge. The recall began on August 1, 2016. Owners may contact Chevrolet customer service at 1-800-521-7300. GM's recall campaign number is 36110.

Consequence

If the key is not in the run position, the air bags may not deploy if the vehicle is involved in a crash, increasing the risk of injury. Additionally, a key knocked out of the run position will cause loss of engine power, power steering, and power braking, increasing the risk of a vehicle crash.

Remedy

GM will notify owners, and dealers will install two key rings and an insert in the key slot or a cover over the key head on all ignition keys, free of charge. The recall began on September 23, 2014. General Motors (GM) is recalling certain model year 2008-2012 Chevrolet Impala police vehicles because the front lower control arms may fracture. This safety recall does not include non-police Chevrolet Impala vehicles.

Consequence

A broken control arm can result in the loss of control of the vehicle, increasing the risk of a crash.

Remedy

GM will notify owners, and dealers will replace both front lower control arms, free of charge. The safety recall began on August 21, 2012. Owners may contact the Chevrolet owner center at 1-866-694-6546.

Consequence

DURING A PASSENGER-SIDE AIR BAG DEPLOYMENT, PIECES OF THE INFLATOR TUBE COULD STRIKE AND INJURE VEHICLE OCCUPANTS AND THE AIR BAG CUSHION WOULD NOT INFLATE FULLY, REDUCING THE CAPABILITY OF THE BAG TO PROTECT THE PASSENGER.

Active Safety System Malfunction

On April 15, 2014, the Office of Defects Investigations (ODI) opened Preliminary Evaluation PE14-010 based on two complaints alleging incidents of sudden/severe uncommanded braking resulting in rear impact collisions in model year (MY) 2014 Chevrolet Impala vehicles.ODI analyzed all complaints relat... ed to allegations of unwanted brake activations while driving that were provided by GM or submitted to ODI from consumers and has not identified any additional incidents involving sudden, extended (greater than 1 second) autonomous braking.The two crash complaints that were the basis of PE14-010 involved rental vehicles equipped with an Electric Park Brake (EPB) system.Neither vehicle was equipped with Full Speed Range Adaptive Cruise Control (FSRACC) or Forward Collision Alert (FCA) systems. According to GM, brake lights will be illuminated if the service brakes are applied or the EPB system is activated while the vehicle is moving (dynamic EPB brake apply).In a dynamic EPB brake apply, the Electric Brake Control Module (EBCM) initiates a controlled deceleration while activating a chime and a telltale light.If the service brake is applied at any time during dynamic EPB brake apply, the system provides a substantially greater deceleration.Witness statements and analysis of crash data from the two incidents are consistent with inadvertent dynamic EPB brake applications.Both reports alleged beeping noise immediately before the accidents and one pre-crash data report shows a moderate initial vehicle deceleration followed by in increase in deceleration to approximately 0.6g just prior to the vehicle stopping and being struck from the rear.During this investigation, General Motors LLC (GM) inspected the two crash incident vehicles and examined each of the EPB switches to assess potential mechanical and electrical fault conditions.The testing and part examination did not find any evidence of mechanical or electrical failure of the EPB switches and GM did not identify any faults in the subject components or systems that could have caused or contributed to autonomous brake applications in the incident vehicles.According to GM, the evidence indicates the events were caused by inadvertent driver actuation of the EPB switch. Additionally, during this investigation, GM identified a defect condition in the EPB software in approximately 132,921 model year (MY) 2014 through 2015 Chevrolet Impala and MY 2013 through 2015 Cadillac XTS vehicles (Recall No. 14V-541) that could result in failure of the EPB to release after vehicle start-up.In this condition, the electronic parking brake piston actuation arm may not fully retract when the driver disengages the EPB, which may cause the brake pads to stay partially engaged with the rotor.According to GM, the parking brake indicator may not illuminate when the EPB is engaged and the driver may experience poor vehicle acceleration, undesired deceleration during idle coast-down, excessive brake heat, and premature wear to some brake components. If the vehicle is operated for an extended period of time in this condition, there is a potential for the rear brakes to generate significant heat, smoke, and sparks.GM and its supplier, Mando Pyeongtaek, upgraded the EPB software to address brake drag conditions and issued a recall bulletin on September 4, 2014 instructing dealers to reprogram the electronic parking brake control module on all affected vehicles. GENERAL MOTORS (GM) INITIATED A CUSTOMER SATISFACTION CAMPAIGN IN OCTOBER OF 2006 TO ADDRESS A FAILURE OF THE POWER STEERING HOSE AND/OR CLAMPS THAT COULD OCCUR WHEN THE ENGINE IS STARTED IN VEHICLES PRIMARILY OPERATED IN THE FOLLOWING STATES THAT EXPERIENCE EXTREMELY COLD TEMPERATURES:ALASKA, MAINE... , MINNESOTA, MONTANA, NEW HAMPSHIRE, NORTH DAKOTA, SOUTH DAKOTA, VERMONT, WISCONSIN AND WYOMING.AT THE TIME THIS PRELIMINARY EVALUATION (PE) WAS OPENED, ONLY 1 OF THE 33 COMPLAINTS RECEIVED BY THE OFFICE OF DEFECTS INVESTIGATION (ODI) WAS FROM A STATE COVERED BY THE CUSTOMER SATISFACTION CAMPAIGN.IN RESPONSE TO AN INFORMATION REQUEST SENT BY THE ODI, GM PROVIDED INFORMATION THAT INCLUDED 6 ALLEGED FIRES AND 1 ALLEGED MINOR CRASH POTENTIALLY RELATED TO THE ALLEGED DEFECT. ONE OF THE ALLEGED FIRES IS A DUPLICATE OF A REPORT RECEIVED BY ODI.OF THE FIRE INCIDENTS, 2 OF THE 6 ALLEGED FIRE INCIDENTS OCCURRED AFTER THE REMEDY DETAILED IN THE CUSTOMER SATISFACTION PROGRAM WAS PERFORMED.ONE OF THE VEHICLES WAS TOTALED AS A RESULT OF THE INCIDENT.IT IS UNDETERMINED WHETHER THE INCIDENTS WERE RELATED TO THE ALLEGED DEFECT OR WERE A BY PRODUCT OF THE REPAIR.OF THE OTHER FOUR ALLEGED FIRE INCIDENTS, 1 INCIDENT INVOLVED SMOKE ONLY AND NO VISIBLE FLAMES.BASED ON THE INFORMATION PROVIDED, THE REMAINING 3 FIRE INCIDENTS WERE LOCALIZED EVENTS THAT PRIMARILY INVOLVED DAMAGE TO THE POWER STEERING COMPONENTS AND SURROUNDING WIRING.ALL 3 OF THESE VEHICLES WERE SUBSEQUENTLY REPAIRED.NONE OF THE ALLEGED FIRE INCIDENTS OCCURRED WHILE THE VEHICLE WAS UNATTENDED.ODI IS AWARE OF 1 ADDITIONAL ALLEGED FIRE INCIDENT THAT WAS NOT INCLUDED IN GM'S INFORMATION REQUEST RESPONSE.BASED ON THE CONSUMER'S DESCRIPTION OF THE INCIDENT, THE FIRE WAS QUICKLY EXTINGUISHED BY A FRIEND AND THE VEHICLE WAS SUBSEQUENTLY REPAIRED AT A DEALERSHIP.IN TOTAL ODI IS AWARE OF 6 ALLEGED FIRE INCIDENTS WHERE THE CONSUMER ALLEGED FLAMES WERE VISIBLE.ALL OF THE ALLEGED FIRE INCIDENTS OCCURRED WHILE THE VEHICLE WAS STILL ATTENDED. ODI IS AWARE 1 MINOR CRASH ALLEGED TO BE RELATED TO A POWER STEERING FLUID LEAK.THE CONSUMER ALLEGED A MINOR CRASH OCCURRED AS THE CONSUMER WAS ATTEMPTING TO PULL INTO A DRIVEWAY AND HAD DIFFICULTY NEGOTIATING THE TURN DUE TO EXPERIENCING HIGH POWER STEERING EFFORTS AND STRUCK A SNOW BANK.IN A LETTER DATED SEPTEMBER 21, 2009, GM INFORMED ODI THAT A CUSTOMER SATISFACTION PROGRAM WILL COMMENCE IN DECEMBER OF 2009 FOR THE ENTIRE US. AS A REMEDY THE POWER STEERING HOSE ASSEMBLY FOR VEHICLES WITH V8 ENGINES AND THE POWER STEERING RETURN HOSE AND CLAMPS FOR VEHICLES WITH V6 ENGINES WILL BE REPLACED WITH UPGRADED COMPONENTS.BASED ON THE LOW FREQUENCIES AND MINOR SEVERITIES OF THE ALLEGED FIRE INCIDENTS AND CRASH,NO SAFETY RELATED DEFECT TREND HAS BEEN IDENTIFIED IN THE SUBJECT VEHICLES.GM'S EXPANDED CUSTOMER SATISFACTION PROGRAM WILL PROVIDE FREE REPAIRS TO VEHICLES EXCLUDED FROM THE ORIGINAL CAMPAIGN.ACCORDINGLY, THIS INVESTIGATION HAS BEEN CLOSED.THE CLOSING OF THIS INVESTIGATION DOES NOT CONSTITUTE A FINDING BY NHTSA THAT A SAFETY-RELATED DEFECT DOES NOT EXIST.THE AGENCY WILL CONTINUE TO MONITOR COMPLAINTS AND OTHER INFORMATION RELATING TO THE ALLEGED DEFECT IN THE SUBJECT VEHICLES AND TAKE FURTHER ACTION IN THE FUTURE IF WARRANTED.Read more
