This model Civic Si was build by Honda with a defective clutch. Mine began to fail at less than 60K miles. It started in the higher RPM 5K to 6K in the taller overdrive gears. This clutch design was unable to support the torque of the motor, which led to its premature failure. It's well document
... ed in Honda forums that the clutch is weak and prone to early failure. It usually starts to fail in the upper gears under moderate acceleration. Once it starts to fail it does become a problem. Honda appears to be aware of the problem as they did have a TSB out that allowed for replacement for 5 years of 60K miles. However, reports on the internet seem to state Honda charges $2K just to tear down the car to look at the clutch. My car was low mileage and still hasn't hit 60K miles but outside the 5 year warranty. However, even if Honda had agreed to replace the clutch they reportedly leave the original dual mass flywheel, which would contribute to a shorter life span for the new clutch they just installed. The OEM manufacturer Exedy won't sell you the same clutch that Honda put into this car, and will only sell you the clutch from the type R. Thus it does appear clear that both Exedy and Honda were fully aware of the manufacturer defective clutch. A clutch like this should last over 100K miles. However, Honda rather then recalling them appears to have just thrown it to the wind and is willing to just let whatever happens pan out and see what's left . The failure of this clutch once it starts to slip does appear to be a relatively short process before total failure. Although it appears there has been no deaths so far there is a good chance someone will delay replacing that clutch because of cost, which could lead to a severe auto-accident or death. Even if Honda doesn't want to do a recall or reimburse their car owners they should at least issue an alert to owners about their defective clutch with free inspections. I own a 2021 Ram 1500 (“Build to Serve” edition). The airbag/SRS warning light came on around January 2025. The message states that the airbag system needs service. This indicates that the airbags may not deploy properly in the event of a crash, putting my safety and the safety of my passengers

They stated the part needed for repair is unavailable and have repeatedly told me there is a parts shortage and a manage allocation part. This has gone on for approximately 1 years, and the problem has not been repaired. The airbag warning remains on continuously. This issue has been documented by the dealer, but the manufacturer has not provided a remedy or replacement part. I am being forced to drive a vehicle with a known airbag safety defect because Ram/Stellantis has failed to supply the necessary repair parts in a reasonable time. I am requesting NHTSA assistance because this is a major safety concern and the manufacturer has not resolved it despite repeated contacts. The extent of corrosion compromises the subframe’s ability to safely support suspension loads and maintain alignment geometry. This presents a progressive structural safety risk, as continued operation may lead to sudden loss of suspension mounting integrity, steering instability, or loss of vehicle control without prior warning. Ambient temperature was approximately 12°F and we were stranded for about 2 hours awaiting a tow. The vehicle was towed and is currently at Knauz BMW (Lake Bluff, IL) and is available for inspection. The specific failed component/system is UNKNOWN at this time This is a repeat event. Approximately three days after purchase in New Jersey (6/25/2025 purchase date, 6/28 was the incident date), the same vehicle experienced a similar sudden loss of propulsion/power/shutdown while driving in heavy traffic while driving at ~60mph, again with my minor child passenger present. This same error occurred twice before the vehicle was taken to Morristown BMW and kept approximately 10 days for diagnosis/repair. Per the dealer, BMW technical assistance was consulted during that repair. The problem has recurred despite the prior repair. I believe this presents a safety defect due to sudden loss of propulsion while driving. I drive along tollway routes weekly, with speeds of 65mph+.Read more

Vehicle available for inspection upon request. Safety risk: loss of coolant can cause sudden engine overheating, power loss, or seizure while driving, creating risk of crash, stranding, or fire. Symptoms began November 2025 with “Check Coolant” warning; reservoir was below MIN. Added coolant once; warning has not returned, but pressure test proved ongoing internal leak. No visible external drips. This exact failure is a known manufacturing defect per Volkswagen Technical Service Bulletin 15-23-01 (Transaction 2071245/1, issued 08/17/2023), which states: “Original engine production head gasket design can lead to coolant leaks” and was redesigned as of engine DNKA 234 458 (July 27, 2023). Despite VW’s own bulletin documenting the defective gasket in all 2022–2023 Taos/Jetta 1.5T engines built before the fix, VW classifies the TSB as “information only” with no warranty coverage. VW Goodwill offered only 50% assistance on the $4,927 head gasket replacement and nothing toward a potential full engine replacement, leaving owner responsible for thousands of dollars in repairs on a documented factory defect in a low-mileage, young vehicle.Read more

The car directly ahead of me, by three car lengths began to decelerate quickly, and therefore I applied greater force to the brake pedal. It was at the very moment I applied greater brake force that the car actually began to accelerate, instead of slow down. This caused me to apply even more brake pedal downforce, and after what felt like all four wheels "shuddering", the can started to slow down and come to a stop. Other than the initial error message about the shifter in gauge cluster, there were no other error lights, sounds or messages when the unintended acceleration occurred. The conditions at the time this happened was it was 50 degrees F, and raining. The car was in Wet/Snow mode, with brake regen locked at Level 1.Read more

However, fully mechanical braking is not affected and, if the IB system is not functioning properly, the parking (emergency) brake will be automatically activated to aid in braking performance and help maintain vehicle stability and control. REMEDY: The Integrated Brake (IB) system will be replaced for free by an authorized BMW center which should take about two to three hours. RECALL STATUS: Recall INCOMPLETE. Loss of Motive Power
The National Highway Traffic Safety Administration (NHTSA) received a letter dated July 13, 2022, petitioning the Agency to initiate a safety defect investigation into loss of motive power allegations attributed to oil drain pan assembly failures on certain Hyundai Motor America, Inc. and Kia Americ
... a, Inc. vehicles spanning Model Years (MY) 2005-2021. The Petitioners cited examples of complaints indicating that the subject vehicles were experiencing rapid loss of engine oil due to oil drain plug back outs, resulting in loss of motive power and/or catastrophic engine damage. Additional complaints were provided which alleged that the oil drain pans lack sufficient structural integrity and are susceptible to cracking. The petition itself can be reviewed at NHTSA.gov under ODI number 11481745. On September 12, 2022, the Office of Defects Investigation (ODI) opened Defect Petition (DP22-003) to evaluate the Petitioners’ request. The Petitioners have provided additional complaints within four supplemental letters dated January 31, 2023, March 21, 2023, May 23, 2023, and August 4, 2023. ODI’s evaluation included the review and analysis of the complaints and supporting information submitted by the Petitioners, as well as additional complaints submitted directly to NHTSA which referenced either the oil drain plug or a crack in the oil pan. After reviewing the available data, ODI has not identified evidence that would support opening a defect investigation into the oil drain pan assemblies of the subject vehicles. Based on a review of the available information, a loss of motive power resulting from an oil drain plug back out has not occurred between the initial sale of a vehicle and the completion of its first oil change. If during an oil change service, a technician fails to remove the existing drain plug gasket before installing a new gasket, it is likely that a proper seal between the drain plug and the oil pan will not be made. An improper seal between the drain plug and oil pan could lead to the backing out on the drain plug, resulting in the sudden loss of oil while driving. Additionally, the provided examples of oil pan cracks were localized to the immediate vicinity of the oil drain plug and/or appeared to be the result of direct-contact with a tool during servicing. There is no information to support that a vehicle-based defect exists relating to the oil pan assembly, and instead the documented incidents are related to the failure to follow the proper maintenance procedures during oil change service. In October 2023, Hyundai published a Technical Service Bulletin (TSB) that provided detailed instructions for oil change service on its gasoline powered vehicles, specifically highlighting the need to remove and replace the existing drain plug gasket prior to reinstalling the drain plug. After a thorough review of the material submitted by the petitioner, the information already in NHTSA's possession, and the potential risks to motor vehicle safety implicated by the petitioners' allegations, NHTSA does not believe that a formal investigation is warranted. Therefore, the petition is denied. A Federal Register Notice (FRN) further detailing NHTSA’s reasons for denial of the petition will be published. Engine Fires
On March 29, 2019, the Office of Defects Investigation (ODI) opened two Preliminary Evaluations (PE19-003 on Hyundai and PE19-004 on Kia) to broadly investigate incidents of non-crash fires (regardless of origin or operational status of the vehicle) on various model year Hyundai and Kia vehicles.
... During the PE investigations, Hyundai and Kia issued several recalls to address vehicle fires, including those due to engine failures involving various engine types. The remedies for the engine failure recalls typically involve engine inspection (to detect existing damage), and if necessary, engine replacement. Additionally, both manufacturers have been installing an engine control software modification known as Knock Sensor Detection Software (KSDS) which is intended to detect impending engine failure, alert the driver, and limit engine power (to lessen likelihood of engine failure).KSDS is being installed under recalls and non-safety field actions, and both companies have extended limited engine warranties (see recall files). On December 22, 2021, ODI upgraded PE19-003 and PE19-004 to this Engineering Analysis (EA21-003, covering both Hyundai and Kia) to evaluate the scope of the prior recalls related to engine failures/fires and the efficacy of the recall remedies for the recalls, as well as the long-term viability of related programs and non-safety field actions being conducted by Hyundai and Kia. The former issue involved approximately 550,000 MY 2013 Hyundai Elantra, MY 2011 Kia Sorento, and MY 2012-2015 Kia Rio. The latter issue involved approximately 2.5 million recalled vehicles, which have been identified as displaying potential remedy concerns: Hyundai recalls 15V-568, 17V-226, 20V-746, and 21V-727; and Kia recalls 17V-224, 19V-120, 20V-750, and 21V-844. Regarding the recall scope issue, among all of the scope vehicles identified at EA opening, ODI found only the MY 2011 Kia Sorento vehicles equipped with both the Theta II 2.4L 4-cylinder Multi-Point Injection (MPI) and the Lambda II 3.5L 6-cylinder MPI engines experienced relatively high rates of reported fires. In December 2023, Kia issued recall 23V-877 that will cover about 80,000 MY 2011 Kia Sorento vehicles equipped with the Theta II 2.4L MPI engines. The remedy will involve an inspection, and if necessary, a replacement of the engine, as well as the installation of KSDS. It should be noted that in 2021 these vehicles were included in a KSDS product improvement campaign with warranty extension. For the MY 2011 Sorento vehicles equipped with the Lambda II 3.5L engines, ODI believes that future engine failures/fires involving these vehicles will likely be due to a lax in adhering to the proper or scheduled engine maintenance (regular engine oil changes), rather than a design or manufacturing engine issue. Regarding the issue of the efficacy of the recall remedies, ODI found the majority of the recalled Hyundai and Kia vehicles, by model, experienced lower reported fire rates after the remedies were applied. Furthermore, ODI found that for most vehicles that did not receive the recall remedies, by model, experienced relatively high fire rates. To address this problem, Hyundai informed ODI in December 2023, that it will renotify all of its owners covered by the Hyundai recalls of engine failures/fires every 8 months for the next 3 years. In December 2023, Kia stated that it will renotify the majority of its owners every 8 months for the next 3 years.Several previously recalled Kia models are excluded from the renotification due to their lower reported rates of engine fires. Overall, the renotification actions will help to increase the recall completion rates, thus reduce the frequency of engine fires in the future. In view of the recall action being taken by Kia and renotification actions being taken by both Kia and Hyundai, ODI is closing this Engineering Analysis. Also, both companies have been released from the 2020-iniated Consent Orders (Kia since November 2022 and Hyundai since November 2023). The agency reserves the right to take additional action if warranted by future circumstances. Accordingly, Model Year (MY) 2005-2016 Kia Optima/K5, MY 2006-2015 Kia Sorento, MY 2007-2016 Hyundai Elantra, MY 2007-2016 Hyundai Santa Fe, MY 2006-2015 Hyundai Sonatas are all part of the subject population. The petition was originally filed as a Vehicle Owner Questionnaire that can be reviewed at NHTSA.gov under reference (ODI) number 11439675.On November 12, 2021, ODI opened Defect Petition DP21-003 to evaluate the petitioner?s request. ODI performed trend analyses of the VOQs related to vehicle speed control issues for all the subject vehicles and found that none of the 52 Model/Model Year Hyundai/Kia vehicles demonstrated any actionable trend that warranted an investigation.Petitioners submitted 1119 VOQs and non-VOQ cases in which sudden uncommanded acceleration (SUA) or loss of motive power (LOMP) incidents were alleged.For the 1119 reports, 220 were incidents involving crashes, from which 69 subject Hyundai-Kia vehicles were inspected by Hyundai/Kia dealer technicians, Hyundai/Kia engineers, and/or third-party representatives.ODI found the inspections identified no vehicle defects related to the ETC system.Of the 220 crash-involved reports, 13 Event Data Recorder (EDR) records were downloaded successfully and 11 of those indicated driver error: (i.e.) pedal misapplication.A condition where the accelerator pedal is mistakenly applied instead of the brake pedal when the driver wants to stop the vehicle.In addition, ODI reviewed a scientific journal paper published in Forensic Science International in 2016 by Mr. Park et al ., as well as related test videos. ODI does not believe there is any evidence of uncontrollable SUA occurrences in any of the tests reviewed. NHTSA's Office of Vehicle Safety Compliance (OVSC) reviewed the allegations regarding the potential noncompliance with Federal Motor Vehicle Safety Standard No. 124.Based on the information provided, the Agency concludes there is insufficient information for us to determine if a possible non-compliance exists.Finally, both Hyundai and Kia informed ODI that all their vehicles manufactured after 2012 were equipped with an ETC management system, in which the engine speed will go to idle when the system sees signals from both the brake pedal and accelerator pedal at the same time; this is commonly known as the brake-throttle override system.After thoroughly assessing the material submitted by the petitioner, information already in NHTSA's possession, information submitted by Hyundai and Kia in response to an Information Request letter, and the potential risks to safety implicated by the petitioner's allegation, NHTSA does not believe that a formal investigation is warranted. ... assembly had detached and was forced downward by the deploying air bag.The metal bracket caused a laceration to the driver's ear and other possible injuries.Approximately 75,000 MY 2012 Hyundai Elantra vehicles, built in Korea and sold in the U.S., have stiffeners in the headliners while the MY 2012 Elantra vehicles built in the U.S. utilize a different headliner that does not use a stiffener. Thus, the latter vehicles are not included in this investigation.Hyundai has inspected the headliner assemblies from eight Elantra vehicles (which were sold in the Korean market) with production dates in the same range as the field incident vehicle.Hyundai reported to ODI that the metal brackets from these vehicles were found to be securely attached to the headliners.Hyundai also has conducted several static deployment tests in attempts to replicate the field incident. In one test, the metal bracket detached from the headliner but the test condition did not replicate or explain the condition observed in the incident vehicle.This Preliminary Evaluation has been upgraded to an Engineering Analysis (EA12-008) to further assess whether incidents like the single field incident could potentially occur with other subject vehicles.In addition to the subject vehicles, ODI also will collect and review information on MY 2011 and 2013 Elantra vehicles that have headliners with stiffeners similar to those in the subject vehicles. ... 29, HYUNDAI CAMPAIGN 086).THE RECALLED VEHICLES ARE EQUIPPED WITH FUEL PUMPS MANUFACTURED WITH UPPER BODY COMPONENTS FROM A DIFFERENT SUPPLIER THAN THOSE MANUFACTURED PRIOR TO NOVEMBER 5, 2007.ACCORDING TO HYUNDAI, THE NEW SUPPLIER'S UPPER BODY COMPONENTS HAD INCORRECT DIMENSIONAL CHARACTERISTICS RESULTING IN MISALIGNMENT OF THE ARMATURE SHAFT, WHICH CAUSED SOME OF THE FUEL PUMPS TO EXPERIENCE POOR BRUSH-TO-COMMENTATOR CONTACT CONDITIONS THAT COULD RESULT IN ENGINE STALL. THE USE OF ETHANOL BLEND FUEL, CONTAINING UP TO 10 PERCENT ETHANOL, CAUSES A FILM BUILD-UP THAT CREATES HIGH RESISTANCE BETWEEN THE BRUSH AND COMMUTATOR.THE MISALIGNED ARMATURE SHAFT PREVENTS THE BRUSH AND COMMUTATOR FROM MAKING SUFFICIENT CONTACT TO OVERCOME THE HIGH RESISTANCE FILM BUILD-UP, THEREFORE PRODUCING LESS PUMPING PRESSURE THAN REQUIRED TO SUPPLY THE FUEL INJECTION SYSTEM. SIXTY-FOUR OF THE COMPLAINTS (72%), INCLUDING THE CRASH AND INJURY ALLEGATION, INVOLVE THE RECALLED VEHICLES.THE COMPLAINT RATE IN THE SUBJECT VEHICLE POPULATION IS 98.5 PER 100,000 VEHICLES, COMPARED WITH 21.4 IN THE REMAINING POPULATION, WHICH HAS ON AVERAGE BEEN IN SERVICE OVER A YEAR LONGER.THE WARRANTY CLAIM RATE IS ALSO SIGNIFICANTLY HIGHER IN THE RECALLED VEHICLES, WHICH HAD 569 CLAIMS (0.9 PERCENT CLAIM RATE) COMPARED WITH 126 CLAIMS IN THE REMAINING POPULATION (0.1 PERCENT CLAIM RATE).
