... to myself and others on the road. Following the incident, I took the vehicle to a Hyundai dealership for inspection and diagnosis. The dealership advised that the vehicle’s VIN is not currently covered by a Hyundai recall for this issue. What is particularly concerning is that Hyundai has issued recalls involving power steering/electric power steering failures and related loss-of-steering-assistance concerns on various Hyundai models and model years. Given the nature of the failure and the similarities among reported power steering defects, I am concerned that this vehicle may be experiencing the same or a substantially similar safety-related defect, despite Hyundai currently stating that this particular VIN is not included in a recall. I respectfully ask NHTSA to: Review this incident and the underlying power steering failure associated with this 2015 Hyundai Elantra and determine whether the issue represents a broader safety defect. I understand that not every mechanical failure qualifies as a safety defect or recall. However, a sudden and unexpected loss of power steering assistance is fundamentally different from an ordinary wear-and-tear repair because it can affect the driver’s ability to safely control the vehicle. I am asking NHTSA to please review this matter as a potential safety defect. This creates a severe safety hazard by instantly dropping vehicle velocity on active interstates and high-speed roadways, leaving the vehicle unable to accelerate out of harms way or merge safely, placing myself, my passengers, and surrounding motorists at immediate risk of a high-speed rear-end collision.The malfunction has been repeatedly confirmed and documented by authorized GM service agent Rountree Moore Chevrolet (Lake City, FL) across three separate service visits. My 2022 MDX purchased new in 2021 experienced an unexpected loss of AWD functionality at 5 years old with 77000 miles when the splines of the transfer case sheared with the transmission. The failure reverted the car to FWD and became very easy to lose traction in anything but ideal road conditions.
... There were no signs of this failure. No warning lights, no indication on the AWD power distribution monitor, and no sounds or other indications. I rigorously followed all maintenance schedules and performed all regularly scheduled maintenance at the Acura dealership. There have been numerous times when I am attempting to accelerate from a stop while turning onto a busy road where the unexpected loss in traction resulted in me nearly causing an accident. I took in my vehicle for service on 5/22/26 and they confirmed the issue is the transfer case spline failure as noted by many other Acura SH-AWD vehicles with the active NHTSA investigation. I would like to add my complaint to that filing and highlight the insidious danger that comes with the failure when the vehicle suddenly does not perform as expected. Given that my vehicle was barely behind the powertrain warranty period when the failure occured, I am now expected to cover an enormous bill that covers the replacement of the transmission and transfer case.Read more

The manufacturer was notified of the failure and sent a field inspector, who inspected the vehicle and informed the contact that the inspection report would be completed on the same day; however, the manufacturer would follow up with the contact within 45-60 days. It has been reliable, well cared for, and in excellent condition. Recently, when the brake line indicator came on, I immediately took the vehicle to a mechanic to have it inspected. I was informed that the brake fluid reservoir was empty and that there was a leak present. Upon further inspection, the mechanic identified that the leak was coming from the rear of the vehicle. It was then determined that the issue involved the rear brake line located near the gas tank. This is extremely concerning, not only because it directly impacts the safety of the vehicle, but also because, based on the research I have conducted, this appears to be a common issue with this particular model. Given that this is a critical safety component, it is troubling that such a failure would occur despite the vehicle being properly maintained. Based on what I have researched, I feel that I have a valid complaint. This does not seem to be the result of neglect or lack of care, but rather a potential defect or known issue associated with this vehicle. As an owner who has taken every step to properly maintain and care for this car, it is both frustrating and disappointing to now be dealing with such a serious problem. I am requesting that this matter be reviewed thoroughly and that an appropriate resolution be provided. I attempted to steer back to the right and applied braking; however, the vehicle did not respond as expected. Electronic stability control did not appear to engage or correct the loss of control. The vehicle then deviated from its intended path, overturned, and struck a fence. The vehicle was declared a total loss. No other vehicles were involved, and no injuries occurred. Safety risk This failure created an immediate and serious safety risk. The loss of steering and braking effectiveness could have resulted in severe injury or fatality. The rollover demonstrates a critical safety concern. Warning signs No warning lights, messages, or symptoms were present prior to the failure. The event occurred suddenly without notice. At the dealership, the problem was confirmed and reproduced by their technicians. They diagnosed: •Fuel leakage from the high-pressure fuel pressure regulator •Fuel contamination in the engine oil These components and diagnostic findings are available for inspection upon request. This issue poses a significant safety risk. Fuel leakage and fuel-in-oil contamination increase the potential for an engine fire, and the hesitation, sputtering, and stalling at intersections present a risk of unexpected loss of power in traffic, which could result in a collision. The vehicle previously had its engine replaced under a Kia recall in 2020, and the current symptoms are consistent with known Kia engine and fuel system defects addressed in past recalls and technical service bulletins. The vehicle has been inspected only by the dealership so far; it has not yet been inspected by the manufacturer, police, or insurance representatives. Kia Consumer Affairs has been contacted and is reviewing the case. I am requesting that NHTSA review this issue as a potential defect trend involving the high-pressure fuel system on this model, especially in vehicles with prior recall-related engine replacements. Upon inspection, I discovered that the tire had been shredded by the road—the sidewall had catastrophically failed, and the inner structure had separated. The tire had good tread and showed no signs of damage before the incident. This was a sudden and dangerous failure that placed me at serious risk of losing control of the vehicle. It occurred under normal driving conditions at moderate freeway speed. There were no prior warning signs, dashboard messages, or visible damage. The tire was manufactured in October 2021 at Pirelli’s Rome, Georgia plant. The failure appears consistent with belt separation or internal delamination. The damaged tire has been preserved and is available for inspection. I believe this tire may be part of a defective batch and am reporting this for safety investigation. I feel because this incident happened at a later time the Steering Lock Unit became defective. I've noticed the warning light on my dashboard for awhile but it was off and on and have been taking my car for maintenance over the years at Premier Nissan (now called Capitol Nissan in San Jose) and thought maybe the tech would find the problem. When we brought the vehile home, the fan was super loud and would come on when ever it wanted too, whether the vehicle was running or shut off. It would also runn continuously even though it was not hot out nor had the car been running. We took the car back to the dealership. They looked into it. Ordered apart that never came. They had our car for 3 weeks and finally after arguing amongst themselves determined the relay switch should be replaced. Once we brought the car home, again the fan is super loud and runs randomly. The dealership has washed their hands of this situation. Telling us we are welcome to take it to another dealership as they believe the fan is running properly. We never had any indication that the fan was running improperly to begin with. My car was quiet and ran wonderfully. Now it is loud and actually embarrassing. Our friend was walking by and heard a loud noise like an airplane. He turned the corner and said wow that's your fan in the explorer, that isn't right. This sucks. I love my explorer but because of this I really don't. I believe it is not fixed and maybe never needed to be fixed. We followed all of the channels thru Ford to have this remedied.
