However, my vehicle is outside the program limits despite experiencing what appears to be the same type of failure reported by many other Prius owners. Since November, I have faced difficulty obtaining the correct parts, reports of nationwide backorders or limited availability, conflicting information regarding updated Toyota part numbers, and concerns about repair providers advertising new OEM parts that may actually be aftermarket or incorrect components. This creates a safety concern because owners experiencing brake failures may be unable to obtain timely repairs using verified, appropriate parts. Repair estimates have ranged from approximately $3,000 to over $4,000, including a Toyota dealership estimate of approximately $4,100. This creates a significant barrier to restoring a vehicle to safe operation, especially involving a component Toyota has previously addressed through a customer support program. I request that NHTSA review this complaint and similar reports to determine whether further investigation is warranted regarding Toyota Prius brake actuator failures, parts availability issues, and whether additional manufacturer action or support is needed. ... platforms sharing the same EPS rack/gearbox. COMPONENT: Electric power steering (EPS) rack/steering column is suspected. Vehicle is in my possession and available for inspection upon request. RISK: Steering failing to self-center, plus intermittent stickiness during turns/corrections, risks needing sudden extra force to move the wheel, which could cause overcorrection and loss of control, especially at highway speed or during lane changes. DEALER CONFIRMATION: Honda Lisle, ILtest drove the vehicle on 7/22/26 and verbally confirmed notchiness. He later left a voicemail stating the only fix is full power steering rack replacement, verbally quoted at $5,500. Nothing was documented in writing on the repair order; no diagnostic findings or formal quote provided despite my request. OTHER INSPECTIONS: Vehicle was at Honda Lisle, IL on 7/22/26 for an unrelated grille-shutter code that set off all of the dash lights. No inspection by police, insurance, or other parties. On 07/10/2026 at approximately 1:10 a.m., the parked and unoccupied vehicle caught fire while turned off. The vehicle had not been driven for several hours. A neighbor was awakened by the horn/alarm and popping noises, observed fire near the passenger-side cab/engine area, and called 911. The fire r
... apidly spread throughout the cab, engine compartment, rocker-panel areas, wheel wells, and truck bed, creating an explosion hazard and threatening a nearby occupied residence and garage. The fire department extinguished the blaze, but the vehicle was completely destroyed. Before the fire, the vehicle had experienced recurring coolant warnings, a DEF-system warning, and delayed starting associated with the main battery. No smoke, fuel odor, intentional ignition source, charging device, or known external fire source was present before the incident. Weather was calm. The exact failed component and cause remain UNKNOWN. A firefighter stated that the apparent fire origin was near the passenger side of the vehicle. Not to mention the risk it put my family in as I was driving with my wife and daughter when the module failed and caused total power failure. I was driving down the road with my family in the vehicle when without any warning the truck completely shut down and didn’t have any power. I brought it to the dealership I bought it from and they stated they fixed the leak and replaced the RFH. Fast forward to this past week, there was another leak in the rear window. The dealership then stated it was a faulty back window unit that needs to be completely replaced. Stellantis (formerly FCA) previously acknowledged this defect through extended warranty coverage. However, my vehicle’s model year was later excluded despite being affected by the same known issue. This exclusion is particularly concerning given that the defect ultimately caused collateral electrical damage beyond the window itself. The vehicle suddenly lost power and stalled in moving traffic, creating a significant safety hazard. I was forced to attempt to move the vehicle out of active traffic lanes while other vehicles were traveling at highway speeds. The unexpected loss of propulsion increased the risk of a crash, serious injury, or death to myself, my passengers, and other motorists. The vehicle was taken to a Hyundai dealership after each incident. However, the dealership did not provide an explanation for the HEV warning light, loss of power, or stalling condition. The manufacturer determined the damage was impact-related and not covered under warranty. Recommended repairs include replacement of the right rear knuckle, upper longitudinal link, toe link, hub bearing, and halfshaft. The failure resulted in a significant change in rear suspension geometry and raised concerns about vehicle safety. I complained to the rental staff that they rented me a car that had been recalled. The attendant said they apparently didn't know about the recall until I returned the car. I find that hard to believe. I checked NHTSA's website a day after I returned the car [XXX] ), and it said that the Kia Telluride I rented had been recalled on February 23, 2026. The reason for the recall was because "THE DRIVER AND PASSENGER SEAT BACK FRAME ASSEMBLIES MAY FAIL TO PROPERLY RESTRAIN OCCUPANTS IN A CRASH". That's a terrifying thought. According to Avis's recall policy [XXX] they're not supposed to rent the car and they are supposed to notify me about the recall. Avis did neither of these things. Luckily the trip was accident-free and my daughter and I are physically fine. But I think Avis should be held accountable for allowing me to drive a rental car that may have had a higher likelihood of killing us in a crash. At the very least they didn't notify me in any way. They weren't even going to mention anything about the recall, and I had to ask about it after I saw what they wrote on the windows. I also submitted a review afterward (Avis sent me a standard email asking about my experience), and I explained my frustration about them renting me a recalled car. Avis replied, but it was just a standard apology and they didn't say anything about the recall. All around a terrible experience. The dealership inspected the vehicle and acknowledged brake/rotor-related vibration but classified it as normal wear and tear and declined warranty repair. Inspections by Others: The vehicle has been inspected by an authorized Volkswagen dealership. No police or insurance inspections were involved. Warning Lamps or Prior Symptoms: No warning lights or dashboard messages were displayed prior to or during the issue. The service manager called me and said they tested the truck, but it is still showing active codes. A GM advisor called me saying the truck should be ready for pickup this Friday. However, this afternoon, I got a call from the service manager saying there is another issue—the engine wiring harness—and they will order the part; now it will not be ready until next week. GM owes me my life. I do not want to drive the vehicle. The service department people at Curry Chevrolet are very professional, but did I pay and spend a lot of money for a garbage truck? Told me as long as my car is drivable I’ll be fine. This vehicle has an open recall related to engine/performance defects. History includes: Complete engine failure in 2024 (vehicle retained for two months). Recurring symptoms reported to dealership. Check engine light illuminated; dealership advised vehicle was safe to drive. Vehicle retained again without repair completion. No technician availability; recall repair not completed before crash. I relied on dealership assurances that the vehicle was safe. This defect creates a serious risk of unintended acceleration and loss of braking control. Then the car started shifting really hard, getting stuck in gears and shutting off. I had to get it towed to my house. I’m thinking there’s a bad PCM, TCM or BCM in this model and it just hasn’t been mentioned yet. Historically, Ford says it’s the spark plugs often and other issues and take several times to fix the issue. Last time it has a shut off problem, they tried two different things before replacing a solenoid somewhere and it lasted for a while up until now. The contact stated that the vehicle failed to start after multiple attempts. The contact left the vehicle and returned to the vehicle the following day. The contact managed to drive the vehicle to the residence. The vehicle was then taken back to the dealer, where an unknown repair was performed; however, the failure recurred two days later. The contact later received a recall notice for NHTSA Campaign Number: 25V586000 (Fuel System, Gasoline); however, the part to do the recall repair was not yet available. The dealer was notified of the failure but refused to acknowledge the recall. The manufacturer had yet to be notified of the failure. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The vehicle was not repaired. The failure mileage was unknown.Read more

The contact stated that the vehicle failed to start after multiple attempts. The contact left the vehicle and returned to the vehicle the following day. The contact managed to drive the vehicle to the residence. The vehicle was then taken back to the dealer, where an unknown repair was performed; however, the failure recurred two days later. The contact later received a recall notice for NHTSA Campaign Number: 25V586000 (Fuel System, Gasoline); however, the part to do the recall repair was not yet available. The dealer was notified of the failure but refused to acknowledge the recall. The manufacturer had yet to be notified of the failure. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The vehicle was not repaired. The failure mileage was unknown.Read more

The contact stated that after crashing into the brick wall, the seat belt became unlatched. There were no warning lights illuminated. The driver sustained injuries to the wrist, which had struck the steering wheel, and the left hand which struck the door on impact. Medical attention was provided at the scene by the EMT and at the hospital the following day. The steering wheel air bag, driver's side side-curtain air bags, and passenger’s side air bags deployed during the crash. A Police report was filed. The vehicle was towed to a tow lot. The insurance company later towed the vehicle to a tow lot. The local dealer was contacted and referred the contact to the manufacturer. The vehicle was not diagnosed or repaired. The manufacturer was contacted and sent an investigator to inspect the vehicle. The inspector determined that the driver’s seat belt was not latched during the crash after performing six push-pull tests on the seat belt. The vehicle was deemed a total loss by the insurance company. The failure mileage was approximately 10,000.Read more

If you are in a urban/remote area which I am no dealerships for about 50 miles away can be a serious problem being stranded on the I 15 freeway.
