VCI Emergency Vehicle Specialists (VCI) is recalling certain 2017-2018 Ford F-550, 2014 E-350 and E-450, 2015 F-250 and F-450, 2016 E-450, 2017 E-350, F-450 and E-450, 2018 F-350, 2010 E-450, 2005-2006 E-450, 2009 E-450 and Chevrolet C4500-based Type I and Type III ambulances equipped with ASA Electronics Voyager monitors used to display the back-up camera image. The monitors may unexpectedly revert back to the factory default settings which may cause the camera image to be reversed.
Remedy: VCI Emergency Vehicle Specialists will work with ASA to notify the owners, and remedy the displays, free of charge. The recall began May 9, 2019. Owners may contact ASA Electronics at 1-800-384-4400.

The contact stated that while reversing the back over prevention camera malfunctioned and displayed a black or distorted screen, creating a rear visibility hazard for the driver. The contact indicated that the failure was present after the vehicle had been repaired under NHTSA Campaign Number: 19V401000 (EQUIPMENT). The cause of the failure was not yet determined. The manufacturer and local dealer were notified of the failure, but no assistance was provided. The failure mileage was 140,000.

The contact stated that the failure obstructed the road's visibility, which prevented the driver from safely operating the vehicle. The vehicle was taken to the dealer, and the contact was informed that a diagnostic test was needed to determine the cause of the failure. The vehicle was not diagnosed or repaired due to the cost. Noted strange "thumping" sound in truck. Took to certified local mechanic who has worked on our vehicles since 2016. This truck has just over 117,000 miles, I am the original owner (ordered while stationed in Afghanistan) and there has never been an accident, there has been no reason for removal of the body from the frame unless the Ford dealership who repaired the missing wire harness when the truck was less than 90 days old had to remove the truck bed. Inspection of vehicle noted the bolts holding the body to the frame were missing. Vehicle is currently being repaired. The risk is the body could have come off the frame and potentially caused bodily or property damage. I contacted Ford but have not received any feedback other than the call center laughing at me for the situation. There were no warning lights on the dash nor codes given to my scanning tool by the vehicle. Mechanic stated this is the third F-Series of this generation they have seen this year with the exact same problem. Truck cannot be moved due to safety concerns, locked out/tagged out of service. Repairs will cost approximately $2,500 including time and materials.

The contact owned a 2015 Ford F250. The contact stated that while driving at 35 MPH, the vehicle veered into the lane of oncoming traffic. As a result, the contact's vehicle hit an incoming vehicle head-on. As a result, the brakes failed to function, the contact's vehicle jumped over the embankment, and the vehicle came to a stop about 75 ft. down the embankment. The driver-side and passenger-side air bags did not deploy. Unknown if any warning lights were illuminated. A police report was filed. EMS was called. However, the contact refused to go to the hospital. The contact suffered injuries to the left arm and below the shoulder blade that did not require medical attention. The vehicle was not diagnosed or repaired. The insurance company was notified, and the vehicle was towed to the insurance storage yard. The manufacturer was not made aware of the failure. The failure mileage was approximately 58,000.

The contact noticed that the fire started before the engine completely started. The contact was the only occupant of the vehicle and was able to exit the vehicle safely. The local fire department was called and extinguished the fire. The vehicle was towed to a local tow yard. The vehicle was condemned as a total loss by the insurance company. The contact stated that no injuries were sustained. In addition, the contact stated that there was minor damage to the roof and the garage door of the contact's house. The manufacturer was made aware of the failure and a case was filed. The contact was advised to contact the NHTSA Hotline and report the failure. The failure mileage was approximately 50,000.

The contact stated that on several occasions while driving at speeds over 65 MPH, or while driving over a bump in the road, the vehicle experienced the "Death Wobble”, and was shaking violently. The vehicle was taken to an independent mechanic where it was diagnosed that the suspension, shock absorber, and dampener needed to be replaced. The vehicle was not yet repaired. ELECTRICAL SYSTEM Jun 26, 2025
The electical system seems to have a bad connection. This can sometimes be triggered when releasing the parking brake. Most often the issue occurs when driving where all gauges go offline and numerous errors are thrown on the display. Most times you can reconcile the issue by wiggling the wiring harness under the stearing wheel located near the parking brake. This vehicle has not been inspected. I have talked to local repair shops and they have verbally quoted prices too high to trace it down. It does not seem to be temperature related. Unplugging the wiring harnesses and plugging them back in does seem to help for a short period of time. This has occurred over 30 times in the past 2 years. The truck has less than 40,000 miles on it. The truck will not start when it is having this issue but thankfully will continue to run if already started.

The contact stated that a second motorist alerted the contact that fuel was leaking from underneath the vehicle. The contact stopped to inspect the vehicle and after raising the hood, it was discovered that diesel fuel was leaking from the top fuel filter. The contact later purchased and replaced the fuel filter, and the vehicle was repaired. The cause of the failure was not yet determined. The investigation into the F-150 sudden transmission Downshifts into a lower gear at highway speeds needs to be expanded and also include F-250 6.7 L diesel. It has happened in my F-250 twice while traveling on the interstate at highway speeds, down shifts from gear 6 to gear 3. VERY DANGEROUS

The upper fuel filter plastic housing cracks and leaks fuel on hot parts and has produced fires on other vehicles, not this one. I have had this replaced twice in the past, this will be the third time. There are no warning lamps or other warnings. Only the smell of fuel.

We have even purchased new high end tires as recommended with no resolution. We are unable to haul our RV out of fear of an accident. It has occurred at high speeds on the highway and at lower speeds on roads. This is very scary when it occurs. Coming to a full stop in a one lane HOV lane is not only dangerous for us but also dangerous for those traveling behind us. This has been occurring for years with the most recent occurrence this week. After researching we discovered this route to file a complaint.

Lethal death wobble , could have easily crashed on the interstate without having both hands on the wheel. This is insane, had the front track bar and front steering stabilizer replaced, still no fix.

While driving on interstate highway I hit a bump and the front end began to wobble until I could brake and stop I took it to my front end shop and spent ~ $2K Next trip after repair had same problem when I hit a bump. After two more returns to the same shop and almost all parts replaced the truck did it again last week This continued wobble could be deadly and needs to be fixed. I plan to call ford but cannot believe this is not a recall issue
