This creates a significant safety hazard, especially on interstate highways or roads without a safe shoulder. If the vehicle is turned off and restarted, the warning message disappears, but the check engine light remains illuminated. After the next ignition cycle, the check engine light also disappears, leaving no visible indication of the problem. The issue began occurring approximately once per year but has become progressively more frequent. It now occurs almost daily. During a recent 300-mile trip, the warning appeared four separate times. It also commonly occurs twice during a 60-mile drive. The increasing frequency has made the vehicle unreliable and unsafe to operate on highways. The vehicle has been taken to an authorized Chrysler dealership. However, the dealer stated they cannot diagnose or repair the problem unless the warning is actively displayed when the vehicle arrives. Because the warning clears after restarting the vehicle, the dealer has been unable or unwilling to identify the cause or perform a repair. I also contacted Chrysler customer service. I was informed that because there is no recall related to this issue, they could only refer me back to the dealership for diagnosis under any applicable warranty coverage. This defect poses a serious safety risk because the vehicle may lose propulsion while traveling at highway speeds, requiring an emergency maneuver to reach the shoulder while surrounded by fast-moving traffic. 2019 Chrysler Pacifica Hybrid experienced a serious charging system failure during routine overnight charging using a standard Level 2 home charger that had previously worked without issue. In the morning, the charging connector was physically trapped in the vehicle charge port and could not be remo
... ved despite repeated normal troubleshooting attempts, including repeated unlock cycles, charger power cycling, breaker reset, and reseating attempts. Because the Pacifica Hybrid disables operation while a charging connector is detected, the vehicle became completely undrivable and required towing. Multiple AAA tow operators were unable to move the vehicle normally. We ultimately had to personally research and guide the tow operators through the manual neutral override procedure in order to get the vehicle into neutral and onto the tow truck. The dealership has suggested that the connector may have fused or arced together during charging and is attempting to characterize the situation as “not the vehicle’s fault.” Regardless of the precise electrical mechanism, this event occurred during ordinary intended use of the vehicle’s charging system. The end result was that the vehicle trapped the connector, immobilized itself, and stranded the owner without any meaningful owner-accessible recovery method. This is not ordinary wear and tear or a cosmetic inconvenience. This was a charging-system immobilization event involving a vehicle platform that already has a documented history of charging-system recalls and software-related charging concerns. The vehicle was being used exactly as intended. No misuse, modification, or improper operation occurred. A vehicle shouldn't become completely inoperable during routine charging. I am requesting review of this matter as a significant charging-system failure and requesting goodwill assistance regarding the substantial diagnostic and repair costs now being imposed on the customer for an event that occurred during normal operation.Read more

... the dash. The issue has become more frequent over time, so that it went from happening a couple times a week to multiple times in one trip, not always when going around a turn. Just a few days ago, the passenger got in and buckled, yet the system showed that it was not buckled. The seat belt was securely latched. The alarm continued to sound and the dash showed a red X on that seat plus a red picture of a person wearing a seat belt. It was a short (about 1 mile drive) and the alarm went off the whole time. We had to reconfigure the vehicle so that seat is no longer being used. We contacted the dealer and they stated that our VIN has no recall or Service Bulletins related to the issue. We are taking it in to be inspected later this week. The dealer would not order any parts or schedule a replacement until they tested it themself.Read more

I am filing this safety complaint on behalf of myself and my wife, [XXX] , the primary driver of our 2021 Ford Escape PHEV, with active under Recall 25SC4 (no remedy available). ... brid mode transition, always without prior warning. The only warning is a brief sound like air escaping immediately before shutdown. At shutdown: engine cuts completely, steering wheel locks rigid and cannot be turned, brake pedal locks hard and cannot be depressed, gear selector locks in Drive and cannot be changed. The driver has zero ability to steer, brake, or move the vehicle. Must activate hazards and wait 1-5 minutes for restart while immobilized in an active traffic lane. Incidents occurred: (1) February 2026: multiple times, dealer retained vehicle 5 days, found nothing. (2) Early April 2026: multiple times, including one witnessed by a police officer who directed traffic around immobilized vehicle. Dealer kept 3 days, blamed the original Ford-supplied charger. Replacement did not fix defect. (3) May 1, 2026: 4 times in one day with my wife only in the car, including on the Dolphin [XXX] near Miami Int [XXX] 3:30PM Friday rush hour, with our [XXX] child in the vehicle on thr [XXX] r occasions. Ford's own recall documentation states steering and braking remain functional during shutdown. This is directly contradicted by our firsthand experience. Both are completely inoperable. Dealer on second attempt blamed original Ford-supplied charger — replacement did not resolve defect. This vehicle is subject to active class action [XXX] et al. v. Ford Motor Company (Case XXX). Yesterday when I was driving in the carpool HOV lane in the Los Angeles area where some sections of the freeway have lane markers that are very close to the center divider and the car decided to randomly turn off ADAS without warning which resulted in a car swipe against the center divider and damaged the front bumper as well as a broken side rearview mirror. I did a random search online and found lots of complaints from other Genesis/Hyundai/Kia owners who have similar issues am surprised why nothing has been done about it so I decided to report this. The repair would cost me almost 4 thousand dollars all because of the manufacturer defect that probably could be easily fixed with over the air update. Jan 14, 2026 took the truck back AGAIN for engine ticking, noise, loss of propulsion/speed, running sluggish. Dealer kept it until Jan 21, 2026. Dealer said they drove it and the noise is normal injector noise. They did nothing other than drive it. They will not investigate any further as it wasn't pulling codes or warning lights. It is still not running right and I'm waiting for it to have a major failure. It is not safe to drive. The first motor did not show any warnings either. GM really needs to get this figured out soon. No vehicle with 52,000 miles should have 3 motors put in it. This is not quality work and concerns for safety are real.Read more

... phic internal pump failure, resulting in an immediate "Low Oil Pressure" warning and forced engine shutdown. Safety Hazard: Stalling at Speed: This defect causes an immediate loss of propulsion. If this occurs during highway merging or high-speed maneuvers, the driver faces an abrupt loss of power steering and braking assist, significantly increasing the risk of a high-speed rear-end collision. Imminent Seizure: Total oil pressure loss can lead to crankshaft seizure, potentially locking the drive wheels and causing a loss of vehicle control. Technical Evidence: BMW has released an updated part (P/N 11419895359) that replaces the plastic internals with a metal housing, acknowledging the original design is insufficient for the thermal loads of the engine. Post-repair inspection of my vehicle showed significant carbonized "soot" in the filter, indicating that the pump's underperformance caused localized overheating of the lubricant prior to the final failure. Request: NHTSA should open an investigation into the B58 oil pump (2019–2021 models) to determine the failure rate of the plastic-ring units. The ACC will just turn off without warning. A yellow warning light turns on to signal driver it is not operational. This has happened many times. The warning light also occurs when the system is NOT turned on. The FCC warning comes on intermittently and states like the ACC service immediately.

Despite the widespread CVT issues in Nissan vehicles and identical symptoms reported in other Rogue models, Nissan refused to repair my CVT because they claim the Rogue Sport is not included in the extended CVT warranty. I also requested Goodwill Assistance due to the clear safety risk, but this was denied as well. The dealer quoted me around $1,500 just to diagnose the CVT problem, with repair costs likely much higher. It is unreasonable for a critical safety component to fail at this mileage and for Nissan to reject all support. This issue continues to occur unpredictably, and the vehicle can suddenly lose power during normal driving, putting me and others at serious risk. Nissan has refused responsibility despite this being a well-documented, safety-related CVT defect. I am filing this complaint because the malfunction poses a risk of crashes and injuries, and Nissan has declined to address a known safety problem.Read more
