On 7/16/26 vehicle would not start; high-pitched noise heard near engine during no-start diagnosis. Towed to Goldy Chrysler Dodge Jeep Ram, Huntington WV. Per dealer invoice #27136: blown 500-amp main battery fuse found; after jumping past fuse, white smoke from exhaust and no coolant in system, dia
... gnosed as blown head gasket. Repair: both-side head gasket replacement plus fuse assembly. Vehicle in shop 7+ weeks, returned 9/4/26 with new unresolved issue: strong smell of burned electronics and antifreeze in cabin. All failed components available for inspection upon request. Safety risk: head gasket failure/coolant intrusion caused engine hydrolock, a sudden uncommanded loss of engine power that can occur without warning, including while driving in traffic. Burned-electronics smell raises electrical/fire risk; antifreeze odor raises cabin fume exposure and window-fogging visibility concerns. Confirmed by dealer: Yes. Goldy diagnosed and documented the fuse failure, coolant loss, and head gasket failure in writing, and performed the repair. Manufacturer/police/insurance inspection: Not by police or insurance. Spoke by phone with a Chrysler HQ rep 5 times (8/13-9/1/26) who was informed; no confirmed independent physical inspection by manufacturer beyond dealer's own diagnosis. Warning signs: P0304 misfire on cylinder 3 appeared first, progressing to complete no-start with high-pitched noise, prompting the tow-in and hydrolock diagnosis.Read more

Incident date: Sep 3, 2026
The van has a consistent steering catch when traveling most pronounced at 11 and 1. The steering hangs and then releases which causes the van to travel unexpectedly into the other lane of traffic. There are no faults or other indicators visible with the vehicle to warn the driver of an issue. The is

Despite this known pattern, no recall currently exists covering this specific defect for 2019 model year vehicles. This is a safety concern because coolant intrusion into a cylinder can cause sudden misfires, rough running, loss of power, and potential stalling while driving — creating a hazard in traffic, particularly at highway speeds or during turns/merges. I contacted Chrysler Case Management regarding this issue. The manufacturer has attempted to attribute responsibility to the dealership rather than acknowledging this as a manufacturer defect in the engine design/manufacturing, despite the well-documented pattern across multiple model years of the same engine. I am experiencing an intermittent steering problem with my 2019 Chrysler Pacifica Limited. While driving, particularly when making slight turns or small steering corrections, the steering wheel sometimes feels as though it sticks or binds in the turned position instead of smoothly returning toward c
... enter. When this occurs, I have to apply additional force to the steering wheel to correct the vehicle’s direction. Once enough force is applied, the steering releases and moves, which can cause the vehicle to correct more abruptly than intended. This occurs at both lower and higher driving speeds and is especially noticeable during gradual turns and small steering corrections near the center position. The problem creates a safety concern because the vehicle can continue traveling in the direction of the slight turn until I deliberately apply additional steering force. This makes maintaining the intended direction of travel less predictable. There are currently no power-steering or other steering-related warning lights displayed on the dashboard. When the vehicle is stationary with the engine running, the steering wheel turns smoothly and I have not been able to reproduce the sticking sensation. The problem occurs while the vehicle is being driven. I took the vehicle to an authorized Chrysler dealership and reported the steering concern. The dealership test-drove the vehicle but stated that they could not reproduce the problem and considered the steering behavior normal. However, the problem continues to occur during normal driving. I have also been able to record video demonstrating the steering behavior. The vehicle remains available for further inspection. The exact component causing the problem is UNKNOWN. I am reporting the issue because the intermittent resistance in the steering and the additional force required to correct the vehicle’s direction could create a loss-of-control risk if the condition worsens or occurs unexpectedly during a turn.Read more

NHTSA Complaint Narrative — Draft On July 9, 2026, in the evening, my leased 2026 Subaru Outback Touring caught fire while we were driving home. We were stopped in a left-turn lane waiting on a left-turn signal. When the light turned, the vehicle in front of us moved forward — which, in hindsig
... ht, was fortunate, because molten metal was sprayed across that exact spot moments later. The engine effectively exploded, throwing molten metal from the melted hood and engine components into the air around the car. We exited the vehicle immediately, leaving it burning in the left-turn lane. The fire department, based only about half a mile away, arrived within approximately 4 minutes, but it still took them roughly 35 minutes to fully extinguish the fire given its intensity. The vehicle had just been returned that day (or very recently) from a 3.5-week internal engine repair at the dealership. This was the third time I had reported the same underlying engine problem to Subaru — the recent lengthy repair was the first real attempt by the dealer to address it, and it failed within days of the vehicle being returned to me. My spouse and I were in the car moments before the explosion. Had we been delayed even briefly — for example, had the car ahead of us not moved forward when it did — we believe we could have been seriously injured or killed by the explosion and flying molten debris. The vehicle was a total loss. I am filing this complaint because: This was the third report of the same engine issue prior to the fire, indicating a recurring, unresolved defect rather than an isolated incident. The failure occurred immediately after a dealership repair specifically intended to fix this problem, suggesting the underlying defect was not — or could not be — properly corrected. The nature of the failure (explosive engine fire while driving, throwing molten metal across an active traffic lane) presents a serious risk of injury or death to occupants, other drivers, and bystandersRead more

SYSTEMS AFFECTED: Electrical System (12V Power Supply / Low-Voltage), Climate Control (HVAC System / Thermal Management), Towing / Recovery DESCRIPTION OF SAFETY HAZARD & DEFECT: My 2022 Rivian R1T experienced a sudden, complete electrical failure due to a defective HVAC system, resulting in a dange
... rous hard-bricked state that rendered the vehicle totally inoperable. Approximately 30 days prior to the incident, the vehicle underwent a comprehensive inspection at a Rivian Service Center where both the HVAC system and 12-volt battery system were tested and cleared. Shortly after, a primary internal failure/glitch within the HVAC equipment created a high parasitic power draw on the vehicle's low-voltage circuit while parked. The HVAC module failed to enter sleep mode, continuously draining the dual 12-volt AGM battery array below critical operating thresholds. This rapid, deep-discharge condition completely disabled the vehicle's primary high-voltage contactors and low-voltage control modules, locking out all vehicle functions, electronic doors, and drive controls. The vehicle could not be restarted, jump-started normally, or safely shifted into neutral, necessitating an emergency flatbed tow to the service center. Technicians confirmed that the primary HVAC equipment failure directly caused the full depletion and destruction of the 12-volt electrical system. This defect presents an immediate safety risk. An uncommanded HVAC-induced power drain that completely depletes the 12-volt system can cause sudden electrical failure, vehicle bricking, loss of hazard lights, and lock-out conditions, potentially stranded occupants in dangerous environments or busy roadways. Rivian is treating this catastrophic multi-system failure as non-warranty maintenance, refusing to address a clear pre-existing latent defect in their low-voltage power management and thermal architecture. REQUESTED ACTION: Investigation into 2022 Rivian R1 HVAC power management defects causing severe 12V drainRead more

I am reporting a safety defect involving the infotainment touchscreen in my 2018 Subaru Outback). The touchscreen has developed visible "bubbles" under the screen lamination, with lines and bubbling appearing beneath the surface of the display. As the weather got warmer, the issue worsened, and the
... screen constantly registers false touch inputs (“ghost touches”) without any physical contact. While driving, the infotainment system repeatedly activates buttons and menus on its own. The vehicle will unexpectedly: - Change radio stations - Rapidly cycle through menus and settings - Trigger Bluetooth phone functions - Attempt to call contacts from my phone when connected via Bluetooth This creates a significant driver distraction and safety hazard. The screen behavior is unpredictable and often continuous while driving, forcing me to repeatedly look away from the road to determine what the system is doing. The issue is especially concerning because the system can initiate phone calls or rapidly switch screens while the vehicle is in motion, making it difficult to safely operate the vehicle without distraction. Based on online research, this appears to be a widespread issue affecting multiple Subaru vehicles with similar infotainment systems, suggesting a possible design or manufacturing defect rather than isolated wear and tear. I believe this defect presents an unreasonable safety risk because it creates repeated and unavoidable distractions for the driver while the vehicle is in operation. This is my fist Subaru, I cannot believe how many times I have heard that this is a "common issue" and yet Subaru will not admit fault. This is a worse distraction than texting and driving and should be approached with the same respect when it comes to safety.Read more

Since shortly after buying my F-150 truck, I have had an ongoing issue with the Brake problem notification light coming on. I have spoken to the local Ford Dealership about this issue several times and had no resolution. They told me they could do an inspection, but it would cost me over $500, and
... only if they found a warranty issue that they were responsible for would they reimburse me the cost of the inspection. They also said that if the notification light was not on when they did the inspection, they would be unlikely to find an issue even if one existed. The warning light can go days or weeks without appearing and can stay on anywhere from a few minutes to an hour or so. I have tried to get the truck to the dealership several times, but it always goes off before I can get there (the dealership is about 40 minutes drive from my house). I am concerned because the warning light comes on more often these days but I still can not get the truck to the dealership with the light on. As the issue appears to be increasing in frequency, I am getting more concerned. The first time this appeared was approximately August 2024.Read more

I am reporting a serious safety concern involving my 2022 Nissan Rogue equipped with the 1.5L engine. While driving in normal traffic, my vehicle suddenly displayed an "Engine Malfunction" warning with no prior indication of an engine problem. Immediately after the warning appeared, the vehicle rap
... idly lost power and could no longer maintain the speed of surrounding traffic. I had very limited ability to accelerate or safely maneuver, creating a significant risk of being struck from behind or becoming stranded in an active travel lane. There was no advance warning that a catastrophic engine failure was imminent. The vehicle ultimately became inoperable and was towed to an authorized Nissan dealership. The dealership diagnosed the engine with coolant leaking into the cylinders and determined that replacement of the engine assembly, turbocharger, and catalytic converter was required at an estimated cost exceeding $10,000. My vehicle had previously experienced engine-related concerns and was included in Recall Campaign R25D1. That recall was completed, but less than 30 days later the engine failed catastrophically. Based on this sequence of events, I am concerned that the existing recall may not identify or address all engine conditions that can result in sudden loss of motive power and catastrophic engine failure. I have since learned of numerous publicly reported complaints from other owners describing similar failures involving coolant intrusion, sudden loss of power, and complete engine replacement in 2022 Nissan Rogue vehicles equipped with this engine. I respectfully request that NHTSA investigate whether these engine failures represent a broader safety defect and whether additional corrective action is necessary to protect the driving public.Read more

On June 7, 2026, while Full Self-Driving (FSD) was engaged, my 2025 Tesla Model Y drove itself forward out of a parking spot and struck a stationary landscape rock directly in its path. The vehicle did not slow, brake, or steer to avoid the obstacle before impact. The rock was a fixed object roughly
... 20 inches tall, clearly visible in the vehicle's forward (windshield) camera from a complete standstill, before FSD began moving the car. It stayed visible as the car advanced and dropped into the camera's near-field blind zone just before contact. The vehicle struck it with no apparent detection or avoidance. This is a serious safety concern beyond the resulting property damage. An obstacle of this height is in the same range as a small child or a crouching person. An automated driving system that fails to detect and respond to a fixed, clearly visible object in its direct path presents a significant risk of injury. The dashcam footage (windshield camera) is available and shows the obstacle visible from a standstill. The vehicle's front bumper camera feed is not saved to dashcam, so only the manufacturer's internal logs can confirm whether the obstacle was detected and how it was classified. I reported this to the manufacturer's service team and requested a log review; they declined to confirm detection and directed me elsewhere. No warning lamps or alerts appeared before the impact. Footage and repair documentation available on request.Read more

Surprisingly the next morning I went into the garage to check on what was happening with the vehicle and to my surprise the vehicle had stopped running. I then disconnected The 12 Volt battery and left it disconnected for an hour or two. After this time I reconnected the battery. I again attempted to start the vehicle and surprisingly the vehicle started up and all functions in the car seem to be performing normally. At this point I drove the car about a mile to the Autonation Jeep Chrysler dealership and had conversation with service technician. Surprisingly he told me that Jeep has had this problem with other vehicles and the solution is to disconnect the 12 Volt battery for 20 or 30 minutes and then reconnect the battery. At that point the car seems to reset itself and it will run normally. End This car of mine only had 78 miles on it This is totally unacceptable. Furthermore this seems to be a problem with other Jeep vehicles. It is my opinion that Jeep Needs to correct this problem so I will not be stuck with a car that's running in my garage and I can't shut it off. All in all it seems to me this is a software problem that needs to be reworked and corrected.Read more

My 2013 Hyundai Tucson with 89,417 miles experienced complete engine failure due to severe rod knock from connecting rod bearing wear. This vehicle was brought to our local Hyundai dealership with confirmed diagnosis. This is a well-documented manufacturing defect in the 2.4L Theta II engine. This
... vehicle was bought 10/28/205 with 29,494 miles. The vehicle was properly maintained with regular oil changes from time of purchase- records attached. Hyundai denied warranty coverage under the TXXM extended program citing incomplete Campaign 966 (KSDS software update) and improper maintenance. However, I never received any recall notices or campaign notifications. When inquiring about notices, Hyundai cited an address that I have not lived at for over 10 years. This sudden engine failure has left me without a vehicle with Hyundai refusing to address this known problem with their engines. I request that NHTSA investigate this issue as well as Hyundai's inadequate notification practices.Read more

Lincoln MKC 2017 Engine failure due to Coolant leak. Bought new Lincoln MKC 2017. The current mileage is 52, 500. Upon Starting, engine would shake for about 10 seconds. The Engine light would come ON. Engine light would turn OFF and ON for few weeks. The Dealership diagnostic indicated code P0301
... . I was advised Full Engine replacement. Currently the Engine is not starting at all. I have heard it's a design issue with the Engine. I have contacted multiple Lincoln dealerships, and each one has its own engine replacement costs. Lincoln Concierge declined my request to help in anyway although it seems to be a design issue with the Engine.Read more

While driving the steering will lock up and force a jerk when trying to get the steering wheel to move. At times there is a shudder while turning at slow speeds. Originally it was only present while turning as you are on an on ramp. It now appears also while driving straight.

The back up camera on these cars are terrible and blurry. I talked to the dealership and they don’t seem to be concerned with the issue. Very low quality.
