I am reporting an ongoing, catastrophic safety defect and corporate suppression regarding Chrysler Recall 03A (NHTSA Campaign 23V010) on my 2022 Chrysler Pacifica Hybrid. The official recall remedy has repeatedly failed to prevent total electrical system depletion and unexpected motive power shutdow
... n in the field, leaving my family in extreme danger. On March 3, 2023, at 6,000 miles, the vehicle suffered a complete, un-warned motive power shutdown while I was driving my two young children on a rural highway. The vehicle stalled dead on an incline in the middle of a lane. The brakes failed to engage automatically, so I couldn’t evacuate my children because I had to keep my foot on the brake. Cars had to swerve around our vehicle at 55 MPH, as it was on a blind corner. My children were deeply traumatized; my autistic child was completely inconsolable and now suffers from an ongoing, severe psychological fear surrounding this vehicle stopping unexpectedly. We were only saved from a crash because a passerby stopped to help after about 15 minutes. The dealership subsequently applied the Recall 03A software update and replaced the 12V battery. This recall remedy has utterly failed. The replacement 12V battery died again in May 2025 and died a third time in June 2026, proving a chronic electrical defect exists. The dealership refused to perform parasitic draw testing, refused diagnostics, and simply charged the battery, stating 'it's fine.' Chrysler corporate support subjected me to a cycle of unresponsive case managers. One manager promised a buyback review within 3 days, but instead, a subsequent manager closed my file. Corporate explicitly refused to escalate the vehicle to their STAR technical team and flatly refused to provide copies of my warranty and service records. I explained to both corporate and the dealership that I don’t feel safe driving this car without further diagnostics but corporate refused to authorize, stating they stood by the dealer.Read more

My 2022 GMC Yukon XL with the 6.2L L87 engine was previously subject to NHTSA Recall 25V-274 / GM Recall N252494000 for L87 engine loss of propulsion. The recall was closed by the dealer as “PicoScope Inspection — Engine Passed / Update Engine Oil Type,” with the oil/cap/filter remedy. The ve
... hicle was later brought back to the dealer for engine noise/knocking, check-engine light, and engine oil light/oil warning concerns. The dealer has now confirmed that the dipstick was not picking up oil, the oil level was severely low, the oil condition was severely degraded, and abnormal engine noise was heard. My VIN also shows active GM Special Coverage N252494003 through 1/15/2032 or 150,027 miles. The dealer previously stated that the underlying PicoScope pass data from the recall inspection is no longer available to the dealership after submission. This is not the first oil-related concern. During a prior service visit in September 2025, around the same time the L87 recall was completed, the dealer documented an engine oil low message and an upper oil pan leak causing the low-oil message. I am concerned the L87 engine may be failing after the recall remedy was previously marked complete, creating a risk of engine failure or loss of propulsion while driving. I am also concerned the current engine/oil/noise condition is being treated as customer-pay/out-of-warranty before the root cause is fully diagnosed or reviewed by GM under Special Coverage N252494003.Read more

My 2022 Chrysler Pacifica PHEV has experienced three separate complete losses of motive power at highway speed, each preceded only by a brief dashboard warning to pull over safely. First occurrence: April 18, 2026. Second: May 7, 2026 — vehicle towed to Spitzer Motor City (Cleveland, OH), an autho
... rized Stellantis dealer. Dealer replaced the Power Inverter Module hardware under warranty (R.O. 42463CY). Third occurrence: May 17, 2026 — just five days after the repair. Notably, the battery was at 40% on the third occurrence, meaning the engine had not cycled on, distinguishing this failure from the prior two. The root cause is the internal transmission wiring harness connector short identified in NHTSA Recall 03A (23V-010). Per NHTSA's own ODI investigation (RQ24-004), the software recall remedy does not prevent loss of motive power. TSB 21-008-23 / Warranty Extension XE1 identifies full transmission replacement as the correct fix. The dealer did not perform a transmission replacement. The safety risk is severe — complete, sudden loss of drive power at interstate speeds with minimal warning creates immediate rear-end collision risk. The defect has been confirmed by the authorized dealer. The transmission assembly is available for inspection. No police or insurance involvement. This VIN currently shows zero open recalls, suggesting the incomplete repair was logged as satisfying Recall 03A.Read more

While traveling at normal highway speeds (55 MPH), the vehicle's battery capacity rapidly dropped from 80-90% down to 10% in less than 30 seconds, resulting in activation of the vehicle's "turtle mode" and power throttling, which made it impossible to maintain posted highway speed of 55 MPH. A warni
... ng appeared on the dashboard, which stated, "Warning Power Reduced". The vehicle traveled at approximately 40 MPH creating a serious hazard to others on the roadway who were traveling at the posted speed limit of 55 MPH. Upon pulling over to the side of the freeway, the battery charge rebounded back up to 75%, but maintained power throttle, making it unsafe to reenter the roadway, as the vehicle was incapable of accelerating to the posted speed limit of 55 MPH. It was extremely slow to accelerate at all, making it from 0 - 40 in about a minute, which again, was extremely hazardous to others on the roadway. Nissan has since acknowledged that this was due to a factory defect, and covered under their factory warranty. I have attached photos of the warning message displayed, and have videos of the rapid battery loss, but those files are too large to attach.Read more

The rear passenger side tire hit a pothole and it caused all of the airbags to deploy on April 6, 2026. The impact should not have set the airbags off. This has caused a severe rash/burn on my son's face and neck. The vehicle is now being totaled by my insurance company and forcing me to have to
... purchase a new vehicle and my insurance rates to go up. Three days later, April 9, 2026, a safety recall on the over-sensitive airbags was issued for this Odyssey. There were no other warnings leading up to this. Insurance has inspected the vehicle and deemed it a total loss.Read more

Glendale Dodge (CA) is actively using Federal Fire Risk Recall 68C to fraudulently conceal a separate, critical High-Voltage hardware failure on my 2021 Jeep Wrangler 4xe. Upon intake, my vehicle’s internal VIP diagnostic scan showed an active P0E15 code (Electric Coolant Heater Failure). Per Stel
... lantis Technical Service Bulletins, this code indicates a severe failure requiring a physical hardware replacement to ensure the high-voltage system operates safely. To avoid performing this costly warranty repair, the dealership deliberately falsified the diagnosis on Final Invoice #31349. Under Line C (Customer States Check Engine Light is On), the dealership officially recorded the cause and correction as: 'DUE TO RECALL MODULE UPDATES AND BAD BATTERY.' This is an affirmatively false written diagnosis. A dead 12V battery and a recall software update do not cause, nor do they resolve, a P0E15 High-Voltage Electric Coolant Heater failure. The dealership lied on the legal invoice to scrub the hardware defect from the record. They then applied the 68C software patch as a smokescreen, falsified the 'Date Out' to 12/26/2025 to prematurely close the federal recall ticket, and attempted to return a vehicle that remains mechanically compromised and unsafe. This is intentional warranty fraud masking a high-voltage system failure. Stellantis and its authorized repair facility are refusing to properly remedy the vehicle. The vehicle is currently held at the dealership pending an active fraud investigation by the California Bureau of Automotive Repair (Case #6596-WS4PV6).Read more

Dear Sir or Madam, I am submitting a formal safety complaint to the National Highway Traffic Safety Administration regarding my Kia Sorento. Over the past six months, this vehicle has required replacement of the engine, transmission control module (TCM), and the entire transmission. Although the en
... gine and transmission were replaced at no cost to me, the extent of these major repairs within such a short period raises serious concerns about the vehicle’s overall safety and reliability. In mid-January, shortly after picking up the vehicle from the dealership following the transmission replacement, the vehicle suddenly stalled while I was driving through an intersection. The engine lost power without warning, leaving me stopped in active traffic. To get the vehicle moving again, I had to shift into park, turn the engine completely off, and then restart it before it would drive again. This unexpected loss of power in the middle of an intersection created a dangerous situation with oncoming traffic approaching. Thankfully, my children were not in the vehicle at the time. After repeated major component failures and this stalling incident, I no longer feel safe driving this vehicle. A vehicle that loses power without warning poses a serious risk to the driver, passengers, and other motorists. I respectfully request a thorough investigation into this matter, including whether there are known defects, recalls, or related complaints involving this model. I am prepared to provide repair records and documentation upon request. Sincerely, [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

Vehicle began displaying check engine light. Diagnostic code P219B (Bank 2 air-fuel ratio imbalance) was identified. Vehicle was serviced and a valve-related repair was performed. Shortly after, engine performance worsened and further inspection revealed metal shavings in engine oil and confirmed in
... ternal lifter/valvetrain failure. Oil levels were normal and maintenance records are available. No warning of low oil pressure or overheating occurred prior to failure. Independent service center and inspector confirmed internal mechanical failure. Engine failure created risk of sudden loss of power while driving. This appears consistent with documented Pentastar 3.6L valvetrain/lifter issues reported by other owners. Concern is potential for sudden engine malfunction and safety risk due to power loss.Read more
