Vehicle: 2017 Chrysler Pacifica Limited Gas (VIN: [XXX]) Component: Steering / Electrical Power Steering (EPS) Gear Rack Incident Description: I am filing this report in relation to active NHTSA Preliminary Evaluation PE25009 regarding the intermittent loss of electric power steering assist. Whil
... e driving on curved roads, the steering system exhibits a distinct "sticky" or binding sensation during gradual, sustained turns. This mechanical resistance is highly localized and most noticeable when holding the steering wheel slightly off-center, specifically near the 11 o'clock and 1 o'clock positions. When attempting to exit a curve, the steering wheel fails to smoothly return to center on its own. It requires a deliberate, manual "nudge" or extra physical driver effort to break past the sticky threshold. Once this resistance is overcome, the Electric Power Steering (EPS) assist suddenly reactivates, creating a brief oversteer condition that requires immediate correction to keep the vehicle tracking safely in its lane. The behavior feels heavily tied to an internal electronic fault within the steering gear module rather than standard lane-keep assistance intervention, as the binding sensation persists even with safety features deactivated. This creates an unpredictable driving dynamic, particularly at highway speeds where subtle, precise inputs are required. Additionally, this vehicle has an extensive history of rapid battery failure, requiring multiple complete replacements of both the main and auxiliary batteries within consecutive 12-month periods. I suspect a systemic voltage drop or a compromised electrical ground circuit (potentially related to Chrysler Recall V53 / NHTSA 19V-348) may be concurrently affecting or accelerating the failure of the highly voltage-sensitive EPS rack assembly. The temporary binding of the steering infrastructure represents a clear, progressive safety defect. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

The vehicle has an unresolved electrical/propulsion failure involving sudden shutdowns while driving and CAN C bus/lost-communication faults. Since the high-voltage battery was replaced through a recall-related repair, the vehicle has had repeated shutdown/loss-of-communication events, occurring do
... zens of times. In the most serious incidents, while driving at speed, the dash flickered, windshield wipers activated unexpectedly, headlights turned off, turn signals would not work, and steering locked or became difficult to control. The vehicle then completely shut off while still moving. After shutdown, the hazard lights would not work. One documented incident occurred on the interstate at about 70 mph. This puts occupants and others at risk because the vehicle can suddenly lose motive power and critical electrical/control functions at the same time, including lighting, signals, hazards, instrument display, and steering/control assistance. The vehicle has been inspected multiple times by a Chrysler dealership. Dealer records documented multiple modules losing communication on the CAN C bus. The dealer replaced the shifter module as a possible contributor, but the problem continued. The dealer later corrected loose pin tension at a communication/star connector, but the vehicle continued to shut down and later scan reports still showed widespread active/stored CAN and lost-communication faults across powertrain, hybrid control, body control, braking/ABS, steering, and shifter-related systems. The vehicle is available for inspection upon request. The problem has been confirmed by dealer scan reports and repair records, but the root cause remains unresolved. The manufacturer has been contacted through customer care and repair/scan documentation was submitted. The exact root cause is UNKNOWN, but dealer findings indicate an unresolved major electrical/CAN communication problem possibly involving the harness, connector network, or communication system.Read more

I pulled into the Dollar General and before I could get to my parking spot the vehicle made a beep sound and stalled and wouldn't move . There were no warning lights that came on. It wouldn't accelerate and I was nervous for the traffic that would be pulling in behind me. I put the vehicle in pa

To Whom It May Concern: I am submitting this complaint regarding a serious and ongoing safety defect involving the second-row driver-side seat belt in my Hyundai Palisade. I purchased this vehicle new on December 11, 2025, after being informed that the applicable seat belt recall repair had already
... been completed by the dealership on December 4, 2025. Despite the recall repair being performed prior to sale, the second-row driver-side seat belt continues to malfunction. On multiple occasions while the vehicle has been in motion, the seat belt has unexpectedly unlatched on its own while occupied by a passenger. This creates a severe safety hazard and leaves the passenger completely unprotected during travel. The issue has occurred repeatedly and has made the second-row driver-side seat unsafe to use. This defect is especially concerning because the vehicle was sold as repaired and safe following completion of the recall campaign. The continued failure of the seat belt system raises concerns that the recall remedy may be ineffective or incomplete. I am requesting that Hyundai Motor America and the National Highway Traffic Safety Administration investigate this matter immediately. I also request that Hyundai provide a permanent repair solution and determine whether additional Hyundai Palisade vehicles may still be experiencing similar seat belt latch failures even after recall service has been completed. Vehicle Information: •Year/Model: Hyundai Palisade •VIN: [XXX] •Purchase Date: [XXX] •Recall Repair Completion Date: December 4, 2025 •Selling Dealer: Holler Hyundai 1150 N Orlando Ave Winter Park FL 32789 •Current Mileage: 8,169 I am extremely concerned about the safety of my passengers and believe this defect presents a significant risk of injury in the event of sudden braking or a collision. Please contact me regarding next steps and investigation of this matter. Sincerely, [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

I am filing this complaint regarding a premature material failure on my 2024 Toyota Tundra. The factory-applied smoked overlay finish on the front grille is peeling and separating in large chunks, exposing the chrome underneath. This is occurring at approximately 46,000 miles and less than two years
... of ownership. This is not normal wear and tear, fading, or minor cosmetic deterioration. The finish is physically delaminating from the grille surface, which appears to indicate a defect in the manufacturing or application process of the factory-installed material. The vehicle has been properly maintained, has not been modified, and has not been subjected to misuse or environmental damage beyond normal driving conditions. Toyota and the dealership denied coverage, citing mileage limitations and categorizing the issue as cosmetic wear despite the obvious premature failure of the factory finish.Read more

Description of Safety Defect: The driver-side exterior rearview mirror vibrates excessively, escalating into a violent shake depending on vehicle speed, crosswinds, and road surfaces. The issue is highly pronounced at highway speeds (65+ mph) and deteriorates aggressively as speed increases, particu
... larly on open, wind-swept highways with like MA-3. The resulting violent vibration causes severe visual distortion, creating a blurred, "low-resolution" image that makes it incredibly difficult to accurately judge the distance, speed, and presence of overtaking vehicles. When checking the driver-side blind spot, this rapid blurring induces sudden, brief physical dizziness and vertigo, posing an immediate safety hazard during high-speed lane changes. The passenger-side mirror remains perfectly stable and clear under identical driving and road conditions. Attempts to Correct / Dealer History: The vehicle has been taken to an authorized dealership (Tulley Mazda, Nashua, NH) twice for this specific issue: On 03/18/2026 (Mileage: 5,862), Repair Order #XXX/1 noted the complaint. The technician verified the vibration on a road test but dismissed it as a "normal characteristic of the vehicle". On 04/21/2026 (Mileage: 6,846), Repair Order #XXX was opened because the condition worsened and began occurring at lower speeds. A ride-along was conducted, and the Shop Foreman confirmed the defect. Upon mirror disassembly, the technician found "loose internal components causing the issue". Current Status: Under warranty, the dealership replaced the body door mirror assembly (Part KJL1-69-181C) and the mirror glass/holder (Part TK71-69-1G7). However, the violent shaking remains completely unresolved and continues to worsen at high speeds. The dealership is now refusing further diagnostic or repair work, claiming the vehicle is "within spec," despite the persistent visual distortion and subsequent driver vertigo. No crash yet, but the defect is serious safety concern.Read more

Recently, my Jeep Wrangler Rubicon 4xe was affected by the 68C Fire Safety recall where the Jeep was stuck at the dealership for battery replacement for 6 weeks. The dealership didn't have a loaner vehicle to offer during the time of the Safety recall. I was asked to contact the Jeep customer care a
... bout securing a rental vehicle during the time. I got this email from Stellantis before I started the rental: " Thank you again for speaking with me today. As a follow-up to our call, your rental reimbursement has been approved. You may rent through a licensed and legitimate company—such as Enterprise, Hertz, Avis, or Budget—at a daily rate of $60 per day, based on the local rates in your area. Sales tax is included in this amount. Please note that optional services—including insurance, fuel, tolls, GPS, tips, and underage driver fees—are not reimbursable through Jeep Customer Care. Rental reimbursement applies only to warranty-related repairs and covers the time your vehicle is at the dealership, unless special arrangements have been approved. To process your reimbursement, please reply to this email with the following documents: Once we receive these documents, we’ll begin processing your reimbursement right away. If you have any questions, please feel free to reach out—I’m happy to help. " Jeep is now refusing the reimburse the non-optional charges saying that they only reimburse Time and Mileage and Taxes. Here are the required charges that Jeep is refusing to reimburse (Total of $337.71): Road Safety Fee (Colorado Mandatory): $229.62 (Total for both periods) Avis Mandatory Fees (FTP SR, License Recoup, Energy): $96.09 Extension Fee (Due to Repair Duration): $12.00 I spoke with the Customer Care multiple times that these are mandatory fees and as per their original email to me, they are supposed to only exclude optional fees from reimbursement. To Whom It May Concern: I am writing to formally notify you of a serious and dangerous safety defect involving the vehicle I purchased from Carvana on February 2, 2026. On April 9, 2026, while driving on the highway at 70 MPH, my truck stopped abruptly after experiencing a critical wheel failure.
... Upon exiting the vehicle, I notice that all the lug nuts were missing. I immediately located a tow driver that confirmed what I observed. The tow company towed the vehicles to a Dodge mechanic in Paducah, KY. Upon inspection by a certified mechanic, I was informed that the wheel failure was caused by either a improperly installed loose or too tightened lug nuts, resulting in damage to the wheel hub and wheel assembly, which condition represents a severe safety hazard that could have resulted in catastrophic injury or death. Prior to the malfunction, there were no warning lamps or messages until the actual event. When purchased, Carvana represented that its vehicles undergo thorough inspections and are safe for operation. However, this incident demonstrates a clear failure in inspection, preparation, or mechanical servicing, including but not limited to: •Failure to ensure lug nuts were properly installed and secured •Failure to identify an obvious and critical safety issue •Delivery of a vehicle that was not roadworthy at the time of sale A wheel detachment risk is not a minor and or normal wear and tear as stated by Carvana's warranty company Silver Rock Warranty and Carvana Warranty support team. The malfunction was a fundamental safety failure that any reasonable inspection would have identified and corrected prior to sale.Read more

My vehicle originally underwent an engine replacement performed by the dealership under a manufacturer recall. The engine was replaced at no cost to me. However, only two days after receiving the vehicle back from the dealership, the transmission failed. The dealership has since stated that they ca
... nnot determine whether the transmission was already failing or whether damage may have occurred during the engine replacement process. Specifically, they indicated there is uncertainty surrounding whether work performed during the engine installation may have contributed to a wiring harness melting, which may have affected transmission operation. Despite acknowledging that fault cannot be determined, Nissan Corporate has offered to cover only 50% of the transmission replacement cost, leaving me responsible for $3,112. This partial payment offer was communicated through Walker Jones Nissan. I do not believe it is reasonable for me to bear half of this expense under the circumstances for the following reasons: • The transmission failure occurred only two days after a major recall repair performed by the dealership. • Prior to the engine replacement, the vehicle was operating without transmission failure. • The dealership has admitted uncertainty as to whether their repair work contributed to the issue. • I have been without my vehicle for approximately two months while this situation remains unresolved. • During this time, I have continued making loan payments and maintaining insurance coverage despite having no use of the vehicle. Given the timing of the failure immediately following recall service and the dealership’s inability to rule out repair-related causes, I respectfully believe full coverage of the transmission repair is warranted as a matter of fairness and customer responsibility.Read more

4/6/2026 vehicle stalled in traffic multiple times, entered "Limp Mode", unable to accelerate after stopping or after deceleration. I had to press the gas to the floor to get the vehicle to move. While driving, if I had to brake or lay off the gas at all, the vehicle would not accelerate back to t
... he original speed quickly. This put myself and my four children at risk of being rear-ended. I drove with the hazard lights on to Kia of Wesley Chapel Service center. The Battery Management System Warning displayed at the time of the stalling. The Kia Access App displayed the BMS Warning and code P0AA600 (High Voltage Isolation Fault). Initially, Kia of Wesley Chapel Service Advisor stated No warnings displayed and No historical or active codes found. I provided a screenshot of the Kia Access App showing the active BMS Warning and active P0AA600 code at 2:48PM on 4/8/2026. The Service Advisor maintained that they could not find any active or history codes. At 3:00PM the same day, the Kia Access App suddenly showed "No Issues Found". The code was obviously manually cleared. I filed a complaint with Kia Customer Care the same day (4/8/2026) and requested a Kia Technical Specialist be assigned to the case. On 4/12/2026, the original Kia Service Advisor stated "the history code has finally appeared which was the one presented in the screenshot but we cannot replace parts based off of a code that is no longer active unfortunately". He stated they would "keep driving and testing" to try and duplicate the issue. The Kia Access App shows the last trip was 4/7/2026 (the day after I dropped off the vehicle). Interestingly, the vehicle was not moved from the drive up lane at the Service Center until 10:40AM that day, after being dropped off at approximately 6PM the evening prior. I believe they could not get the car to move the evening of 4/6/2026. The vehicle remains at Kia of Wesley Chapel. I am sending the Motor Vehicle Defect Notification to Kia Corporate today.Read more

I am reporting a serious safety issue involving unintended acceleration in my vehicle. The vehicle accelerates unexpectedly without driver input (specifically without pressing the “resume” function), requiring immediate braking to regain control. This issue has occurred numerous times over the p
... ast 4–5 months. Incidents have primarily happened at low speeds (approximately 20–25 mph or less), including while making turns, exiting parking lots, and once in our yard, where the vehicle nearly struck our home. The problem has occurred both when adaptive cruise control was active and when it was turned off. In the most recent incident, adaptive cruise control was off, and the vehicle suddenly accelerated while traveling in about 20–25 mph on the main street. The driver had to apply the brakes forcefully to avoid a collision with the vehicle in front. There have been no warning lights, alerts, or messages prior to these events. During at least one incident, a “braking required” message appeared only after the unintended acceleration had already begun. The vehicle has been taken to a GMC dealership multiple times, but the issue has not been duplicated by them. The dealer suggested the possibility of accidental activation of the “resume” button; however, this did not occur. The vehicle is currently at the dealership (since April 6) and is available for inspection. This defect presents a significant safety hazard to the occupants of the vehicle and to others on the road.Read more
