An authorized dealer did a recall repair on my vehicle without my explicit consent. Not verbally nor in writing. Completion of said repair resulted in a botched outcome. Service manager was made aware but did not remedy the situation.

In May 2026 while driving home from her job in downtown St. Louis, my daughter was driving my equinox. While traveling on the highway at the speed limit of 55 miles an hour her brakes failed and she came within a few feet of rear ending a full gas tanker truck. She was able to maneuver to the should
... er of the highway and avoid an accident. After a few moments of the engine, idling the engine completely stopped working and shut down. After having the car towed to my local mechanic at a Midas shop, they began the diagnosis. I began similar research online to find cases of similar things happening. What was diagnosed is that the vacuum pump on the brake line was faulty and broke. Pieces of this pump made their way into my engine and ruined two of the cylinders in my engine. The car is now not drivable and needs a replacement engine. Chevy knew about this faulty, vacuum pump part and they did issue recalls on other vehicles using the same part. They have also issued technical bulletins to their technicians for a diagnosing this failed vacuum pump. I have tried to contact Chevy and they have offered me no assistance. I know a class action suit has been filed for this very same issue. Chevy is aware of this issue. Chevy‘s choice to use a part they knew was faulty. Almost led to the death of my daughter.Read more

The 2017 Hyundai Santa Fe was brought to dealership on 3/27/26 for an oil change and a computer update was completed for Emissions Improvement. Upon picking the vehicle up after service, it was noticeable that it was not shifting correctly. I spoke to the service department and asked if the memory
... was cleared with the update an if it was having to relearn how I drive and the said it should not be affected. This went on for a few days and I left the car with them to duplicate as there were3 no stored codes. After a few weeks, the Teck Line recommended a new transmission, which Hyundai offered goodwill assistance. As we were waiting for the transmission to arrive from being on backorder, a bulletin for Service Campaign 9C2 was release on 5/18/26 that described my experience with shifting issue almost exactly. The transmission was replaced and the drivability issues still persist. Hyundai says that I must let my car warm up for 1-3 minutes to allow a catalyst to reach temperature and not cause rough shifting. This rough shifting is the effect of the update and thus te reason that my transmission needed to be replaced. Further investigation showed that this update is a known issue and cannot be reversed once installed. Currently, I must start my car in 90 degree weather to warm it up as a result of this update which is not logical in any thought process. I have asked Hyundai to cover the balance of the new transmission as this is a result of thier faulty software update with no success.Read more

I was denied work on a network issue recall 3 separate times at the dealership I bought the car. The first time I was told it was because of a smell. If I clean the car, they will fix it. I noticed after they were in the car my backup camera didn’t work which I also thought might be a recall but f
... ound out it wasnt. I went ahead and cleaned it twice. I came back they alleged it smelled like cat litter and urine. I asked to speak to someone else because there was nothing in my car and I know it didn’t. There were also no stains, mold, anything like that. It smelled of cleaning products. But I took it home and scrubbed extensively even though I was confused by the comment. When I went there it was smelling like lemon. Zero stains, mold, nothing in the car but maybe a little sand and baking soda. The person went as far as to say wow the car is improved said but thry still sorta smell it and there’s stuff on the window (there wasn’t). At that point I knew I was being picked on because either they didn’t know how to fix it or didn’t want to work with me. It felt discriminatory and differential to others. I now have a super clean car but a network issue and no backup camera because of this. They claim now they fixed everything but a non-safety issue but I also still have two errors that say “telematics” and “networking issue have vehicle inspected.” I believe I have rights under NC lemon law as this is within 24 months of purchase with only 4600 milesRead more

On July 18, 2026, at approximately 10:00 AM EST, my son was manually driving a 2024 Tesla Model 3 at approximately 26 MPH when the steering suddenly became extremely stiff and difficult to turn. He attempted to steer away from a roadside barrier but was unable to turn the steering wheel sufficiently
... , resulting in a collision. There was no warning or indication observed on the dashboard before the steering suddenly became extremely stiff. Autopilot, cruise control, and Full Self-Driving were not activated; the vehicle was being driven manually. Fortunately, my son did not sustain any physical injuries. However, a sudden loss of normal steering control is a serious safety concern and could potentially cause severe injuries to the driver, passengers, pedestrians, or occupants of other vehicles. After the accident, the vehicle was towed to the insurance-designated facility and is currently being examined by the insurance adjuster. Within a few minutes of the accident, the vehicle also became completely locked, preventing us from retrieving personal belongings. I previously reported this incident to NHTSA under complaint number 11751698, but all required documentation was not available at that time. I am therefore submitting the complaint again with the additional information and documentation now available. I respectfully request that NHTSA investigate this incident and determine the root cause of the sudden steering stiffness and whether there is a safety-related defect or condition that could affect other vehicles.Read more

I am writing to file a formal complaint regarding the service I received, and the dealership’s failure to address what I believe is a serious safety issue following recall repairs performed on my vehicle. On XXX, I brought my vehicle for a routine oil change and to complete three mandatory recall
... campaigns, including an ECM software update related to a misfire recall. Prior to this service, my vehicle had never exhibited any misfiring, transmission, or drivability issues. However, on Sunday, XXX, and again on Monday, XXX my vehicle began experiencing dangerous driving symptoms that started only after the recall work was completed. Specifically: The engine RPMs surge while accelerating, but the vehicle does not increase speed accordingly. The vehicle jerks when braking. The vehicle hesitates and feels unsafe while driving at highway speeds. These issues did not exist before Genesis performed the recall repairs. On XXX, I contacted my service advisor, to report these safety concerns. He assured me that a manager would contact me ASAP and determine the next steps. No one ever called me back. I called again that same day. During that conversation, Rick suggested that my worn brake pads and rotors were the reason my vehicle was “misfiring.” This explanation makes does not explain why my engine RPMs are flaring and the vehicle is failing to accelerate properly immediately after an ECM recall update. Once again, I was told someone would follow up, and again, no one did. On XXX, at approximately 9:00 AM, I called Genesis of Danvers again and spoke with Rick who told that if I dropped the vehicle off today, nothing could be done with it today, and that a manager would call me back. Got vehicle towed XXX. As of today XXX I still have no update on my vehicle. No phone card no email no loaner. I am becoming increasingly concerned by the lack of communication surrounding a vehicle that was towed because of a significant safety concern. While traveling with my fifth-wheel camper attached, my 2022 Ford F-350 Super Duty (6.7L Power Stroke) suddenly developed a complete no-crank condition. Prior to the failure, the vehicle displayed multiple electrical warnings, including U0100 (Lost Communication with PCM), High Engine Temperature, A
... dvanceTrac, Blind Spot System Fault, Hill Start Assist, and Pre-Collision Assist warnings. The vehicle ultimately became disabled and would not crank or start, leaving me stranded away from home while towing a trailer. Repair Description After troubleshooting the vehicle, I discovered severe internal corrosion inside the under-hood Battery Junction Box (BJB). The corrosion appeared consistent with Ford Technical Service Bulletin 23-2133 regarding moisture intrusion into the Battery Junction Box on certain 2020–2022 Super Duty trucks. I purchased a genuine Ford replacement Battery Junction Box from a Ford dealership and installed it. Immediately after replacement, the vehicle started normally and all warning messages disappeared. I retained the failed Battery Junction Box and have photographs documenting the internal corrosion. Safety Concern My concern is not simply the repair cost but the safety implications of this failure. The vehicle became completely disabled while towing a fifth-wheel trailer during travel. A sudden electrical failure affecting the Battery Junction Box can leave occupants stranded and may create hazardous situations if it occurs in remote locations or during travel. Because Ford has issued a Technical Service Bulletin addressing moisture intrusion into the Battery Junction Box, I believe this condition should be evaluated to determine whether additional action is warranted.Read more

My car was in shop for Smog inspection and failed. Mechanic told me that the most likely reason was that the engine has issues related to failing crankshaft position sensor which triggered the check engine light. He further explained that the engine with this condition will have abnormal oil consump
... tion. My experience is that is exactly what I wondered. No smoke in the exhaust, no oil leaks under the hood and no oil on the parking space at home nor work. Mechanic recommended I take the car to Hyundai dealer in Folsom. The service department there diagnosed my car and did not find fault with crankshaft position sensor but did find that there is a problem with timing mechanisms. They also said that the engine has difficulty starting, which I never experienced in over 50,000 I have driven it, save the time of battery replacement about 2 years ago. I decided to take my car back to my mechanic where he diagnosed it with timing issues. The abnormal engine noise was accounted for as a link to timing. In the interim I drive a rental at the rate of about $65.00 /day. I m in a quandary; if my mechanic repairs the timing issues, which may not be under warranty, does that invalidate the existing engine warranty? If I can get the warranty to cover the ongoing issues, we can save ourselves wasting time and money. If safety concerns are a priority, which is not, then we cannot sacrifice safety in the interest of celerity. In the meantime, my family are in the crossroads and I cannot accept that.Read more

Gas pedal hesitation at a rolling stop. There is a delay in acceleration during occasional rolling stops at locations such as yield signs and unmarked intersections on the roadway. When this happens the driver may depress the accelerator more than should be necessary causing tire spin and traction

Car had recall and new engine was put in April 2024. I’ve had the new engine for 25 months. It started doing the exact same thing as before when engine went out. No power, limp mode, engine violently shaking, rods knocking. Engine light came on. I parked it immediately and called dealership. They
... said it’s a refurbished engine not new so it does not have the full warranty. This new engine wasn’t new I just found that out this morning. Upon further research in Kia forums, these replacements blew up too. Like we all got the same engines that blew up. Can anything be done about this? This would be the 3rd engine in this Kia. It only had 30k miles on the new motor. Engines last longer than that. I’m now stuck as a travel nurse with finding another vehicle. This is just wrong. I missed work trying to find a car. Has there been any complaints about the replacement motors seizing up and blowing? Online searches show it happening to a lot of people with these Kia souls. Idk what your company does but everyone said to let you know. Idk how that helps but here it is. Can anything be done? My mechanic came by to look at it and said the motor is blown. It has an appointment at dealership for official diagnosis of blown motor.Read more

Subject: Safety Complaint – 2024 Honda Prologue – Clicking Noise When Turning (Possible CV Axle Defect) I am submitting a safety complaint regarding my 2024 Honda Prologue Touring due to a persistent clicking noise when turning, which appears to be related to the front axle or CV joint. Vehicl
... e Information: Year: 2024 Make: Honda Model: Prologue Touring Mileage:10300 VIN: [XXX] Description of Issue: The vehicle produces a noticeable clicking/popping noise when turning left or right, especially at low speeds such as in parking lots or during tight turns. The noise is consistent and reproducible. The issue appears to originate from the front axle/drivetrain area and raises concern about potential failure of a critical drivetrain component. Safety Concern: This defect may indicate premature wear or failure of the CV joint or axle assembly. A failure of this component while driving could result in loss of power to the wheels and create a dangerous driving condition, increasing the risk of an accident. Dealer Response: I brought the vehicle to an authorized Honda dealership. The dealer acknowledged the issue but stated that no repair or fix is currently available and advised me to contact Honda directly. The issue remains unresolved. Additional Notes: Based on my research, multiple other owners are experiencing similar issues with the same vehicle, suggesting this may be a widespread defect rather than an isolated incident. I am requesting that NHTSA investigate this issue as a potential safety defect affecting the 2024 Honda Prologue. Thank you for your attention to this matter. While driving my 2023 Cadillac Escalade (approx. 42,308 miles), the vehicle experienced a sudden and complete loss of power while accelerating on a busy highway. Engine power shut down without warning. I was able to pull to the shoulder, but the vehicle then became non-responsive and would not resta
... rt. The failure appears related to the electrical/charging system (12V battery, battery management system, alternator, or related modules). The vehicle is available for inspection upon request. This failure placed my family and others at serious risk. The loss of propulsion occurred in traffic, creating a collision hazard. After stopping, the vehicle had no heat and limited power while unable to start leaving us stranded on the shoulder of a heavily traveled highway in 10-degree weather. My two toddlers were in the vehicle, creating additional risk from traffic exposure and cold. This was a repeat incident. Approximately two weeks earlier, the vehicle experienced a similar power loss. A Cadillac dealership replaced the 12V battery and stated the issue was resolved. However, on January 22, 2026, I contacted the dealership and provided a video showing abnormal starting behavior, indicating the problem may not have been fixed. I followed up by phone the next day but received no response. Despite this prior repair and warning, the same failure occurred again. There were no clear warning lights or messages immediately prior to the power loss. After the failure, the vehicle was unable to restart. I also attempted to use OnStar, a paid emergency service, but was placed on hold for approximately 30 minutes. Due to unsafe conditions, my wife had to come pick us up to remove our children from danger. This incident involves a repeat loss of power while driving, prior warning signs, and failure of emergency support systems, raising serious safety concerns.Read more

Tesla’s Obstruction of Repairs Tesla deleted my original service complaint and annotations regarding these delivery defects. The associated service appointment was cancelled without notice or rescheduling. No repairs were performed despite the issues being raised during the warranty period. This p
... revented reasonable repair attempts and allowed the defects to persist. Current Nonconformities and Repair History The same or substantially similar defects continue and have worsened: • Recurring misalignment/alignment issues from delivery, impairing value and potentially safety. • Safety-related camera system failures: Persistent error codes on the camera(s) [specify if known, e.g., front/rear/ADAS-related], reducing visibility and affecting safe operation (e.g., backup, navigation, Autopilot/FSD if equipped). These are known Tesla issues potentially subject to recall (e.g., rearview camera display loss). • Power windows failure: Unresolved for at least 2 weeks despite service request; windows do not operate properly, impairing use and safety (e.g., emergency egress). Cumulative out-of-service time and failed repair attempts (including the obstructed initial one) meet or approach presumptions under § 1793.22(b): multiple attempts for the same nonconformities, with safety impacts. The vehicle remains nonconforming despite reasonable opportunity for Tesla to repair. nonconformities in the above vehicle that qualify it as a “lemon” under California Civil Code §§ 1793.2(d) and 1793.22 (Song-Beverly Consumer Warranty Act). These defects originated at delivery, were reported within the express warranty period, but Tesla failed to repair them—by deleting my original service complaint, annotating delivery issues without resolution, and cancelling the associated appointment—preventing conformity to warranty. The same issues persist and recur post-warranty expiration, substantially impairing the vehicle’s use, value, and safetyRead more

While driving at highway speed and the adaptive cruise control engaged, the vehicle applied the brakes without driver input. The vehicle slowed gradually over several seconds even though there was no vehicle, pedestrian, or obstacle present in front of the vehicle. Road and environmental conditions
... were clear and normal, including clear weather, dry roadway, and unobstructed lane of travel. Traffic was flowing normally, and there was no lead vehicle. The driver was attentive and did not press the brake pedal. The braking occurred without any warning, alert, or collision indication and resulted in an unexpected reduction in speed, creating a risk of a rear-end collision from following traffic and on one occasion road rage (which has occurred due to this condition) as the driver thought I "brake checked" him. The vehicle has been inspected by the local Kia dealership (Spitzer Kia of Mansfield) and they could not replicate the condition. Kia has also been notified and a case number assigned (25773402) but with no resolution. This issue has occurred multiple times under similar highway driving conditions, indicating repeatable uncommanded braking behavior. Additionally, KIA issued a “Voluntary Service Campaign” for 2024 Kia Telluride’s on July 31, 2025 for the same vehicle behavior. Unexpected braking at highway speeds creates sudden speed differentials and increases the risk of rear-end collisions, even when deceleration is gradual. The driver has limited ability to anticipate or prevent the event. No warning lights, audible alerts, or collision warnings were displayed. CONCLUSION The vehicle exhibits repeatable false or uncommanded activation of the automatic braking system during highway driving, consistent with a potential defect in the AEB/FCA system. This condition poses a safety risk to vehicle occupants and surrounding traffic and should be reviewed by NHTSARead more

Dear National Highway Traffic Safety Administration (NHTSA),I am submitting this report to express a serious safety concern with my 2015 Buick Verano, which I believe is unsafe for shorter drivers due to poor visibility and design limitations.I received this car as a gift from my father, but the mom

The windshield is too close, and there are several significant blind spots that make it extremely difficult to see vehicles, pedestrians, and obstacles around me.To try to fix this issue, we installed four extra mirrors and I used a booster seat, but these adjustments did not resolve the safety problems. I have experienced numerous close calls, including one incident where I nearly struck a pedestrian I could not see until he moved out of the way. Driving this car has become extremely stressful and frightening due to these visibility issues.I have spoken with several friends who are also under 5 feet 3 inches tall, and they reported the same difficulties when driving a Buick Verano. It appears that this model’s visibility design creates unsafe driving conditions for shorter individuals.I request that the NHTSA investigate this design concern to determine whether the 2015 Buick Verano meets safety standards for drivers of different heights, and whether design changes or warnings are necessary to protect shorter drivers.Thank you for your attention to this important safety issue.thank you.Read more

I purchased 4 new tires from Braman Honda on 2/2023. I drive the car daily but I also have a take home vehicle for work. On 12/4/25 and 12/5 my son, who is on military leave took the car from Boynton beach to Miami for an appointment. when he returned, he showed me a picture of the dash showing the
... low tire pressure light on. Up until now, there was no warning of any issues with the tires. I thought that there may be a nail in the tire but to my shock, the front driver side tire was missing chunks. The following week I had the car towed from my house to Braman Honda. My service advisor called to inform me that all 4 tires had tears and cracks in the sidewalls and must be replaced. Me and/or my son could have been hurt or killed while driving the car with the tires in this condition. My car is only serviced at Honda and I keep up with all tire rotations. On 2/21/25 a multi point vehicle inspection was performed and I declined the multi point inspection and tire rotation on 6/14/25 due to a time crunch. The warranty expired after 2 years so I had to finance 4 new tires. BF Goodrich will only file a claim if you replace with their tire brand. I do not trust their brand and even although they supplied the batch of dry rotted tires which was installed on my car, Honda nor BF Goodrich will help with the cost. I've read many similar cases like mine about these tires. BFGoodrich claims that the cracks are due to road hazards.Read more

The contact stated that approximately 5 days later, on October 2, 2025, while reversing, there was a burning odor with black smoke coming from the exhaust pipe. The vehicle was towed to the mechanic and was diagnosed with coolant intrusion into the engine. The contact was informed that the engine needed to be replaced; however, the warranty only provided the replacement of 2 head gaskets. After 7 days of having the vehicle, while driving approximately 68 MPH, the vehicle started shaking with the check engine warning light illuminated. The vehicle was also idling roughly. The vehicle was towed to the mechanic and was diagnosed with fuel pump failure. The fuel pump was replaced; however, the vehicle was idling roughly, and there was an intermittent misfire in cylinders #1 and #6. The dealer was unable to identify which cylinders needed to be replaced. The contact discovered information of NHTSA Campaign Number: 25V274000 (Engine and Engine Cooling); however, the VIN was not available. The vehicle was not repaired. My 2017 Hyundai Tucson is experiencing severe oil consumption, which appears to be related to a known manufacturing defect affecting Hyundai engines from this period. My vehicle is burning through oil rapidly between normal oil change intervals, and it has begun showing signs of low oil levels despi
... te no external leaks. This creates a serious safety concern because the engine can fail without warning. The dealership has acknowledged that excessive oil consumption is a known issue for this engine but refuses to provide assistance because I do not have “enough oil change documentation” over the years. However, this issue is not caused by lack of maintenance — it is a widespread internal defect that Hyundai has issued multiple technical service bulletins for. My car has been maintained, and oil changes were performed at regular intervals, even if I do not have complete paperwork. The rapid oil burning is ongoing and dangerous. I am concerned about the engine seizing, stalling, or failing while driving, which could lead to a crash. This is a safety hazard that should be investigated and included with other Hyundai engine cases. Requested Action: I am asking NHTSA to investigate this oil consumption defect affecting the 2017 Hyundai Tucson and require Hyundai to provide a remedy, including repair or engine replacement, regardless of perfect maintenance documentation, since this problem stems from a manufacturing flaw.Read more

Intermittent Loss of Electric Power Steering
The Office of Defects Investigation (ODI) is opening this Preliminary Evaluation to assess the scope, frequency, and potential safety consequences of alleged defects related to the electric power steering (EPS) system in model year (MY) 2017–2018 Chrysler Pacifica vehicles. ODI has received 94 Veh
... icle Owner Questionnaires (VOQs) reporting a condition in which the steering system exhibits a sticking sensation during gradual turns, particularly when the steering wheel is positioned near the 11 o'clock or 1 o'clock position. Complainants allege that during these events, increased steering effort is required to return the wheel to center. In many cases, this is followed by a sudden reactivation of EPS assistance, which may result in an oversteer condition. This behavior could increase the risk of a crash, especially at higher speeds. ODI has identified multiple VOQs describing similar symptoms. This investigation will assess the frequency, root cause, and potential safety-related consequences of the alleged EPS malfunction. Loss of motive power due to an internal wiring connector short.
On July 22, 2022, the Office of Defects Investigation (ODI) opened investigation PE22-008 regarding complaints alleging a loss of motive power condition potentially due to a transmission malfunction in Model Years 2019-2021 Chrysler Pacifica Plug-In Hybrid Electric Vehicles (PHEV). On January 17,
... 2023, Fiat Chrysler Automobiles (FCA) issued safety recall (NHTSA Recall 23V-010) involving 2017-2023 Model Years (MY) Pacifica Plug-In Hybrid Electric Vehicles (PHEV) produced between August 12, 2016, through January 9, 2023. The recall states that the loss of motive power condition in these vehicles is caused by an internal transmission wiring connector that may short circuit, resulting in an unexpected engine shutdown. An unexpected engine shutdown can cause a loss of drive power, increasing the risk of a crash. In discussions with FCA on Recall 23V-010, ODI confirmed the recall was related to investigation PE22-008. The remedy for 23V-010 is to update the Power Inverter module (PIM) and/or Instrument Panel Cluster (IPC) software to provide drivers a message stating “Stop Safely - Vehicle Will Shut Off Soon” followed by a chime and MIL, PRNDL blinking (on cluster and shifter knob), low SOC (State of Charge) warning and reduced power mode. The recall remedy does not replace the defective hardware that was described in Recall 23V-010, and the wiring connector short still exists on these vehicles. To date, ODI has received 132 reports for the population of vehicles subject to recall 23V-010. These reports allege a loss of motive power condition that reduces the vehicles speed, followed by dashboard warning lamps displaying messages associated with recall 23V-010. The presence of these warning lamps and messages indicates the vehicle likely had the recall remedy applied. Follow up discussions with FCA confirm their remedy does not prevent a loss of motive power, but instead limits torque to the engine and after an unspecified number of miles or period of time, the engine will shut down. When this occurs, FCA relies on the owner to transport the vehicle to a dealership after they experience a loss of motive power condition, at which time the defective hardware is replaced. Furthermore, the dashboard messages introduced in the recall remedy may cause the driver to pull over in an unsafe location. Pulling over in an unsafe location may expose the occupants to undue harm. The Office of Defects Investigation is opening this Recall Query (RQ) to assess the adequacy, frequency, and safety consequence of the remedy for recall 23V-010. To review the ODI reports cited in the opening resume ODI report identification number document, go to NHTSA.gov.Read more

Ignition-off high voltage battery fire (Remedy effectiveness of Recall 22V-077)
On February 11, 2022, Fiat Chrysler Automobiles (FCA) issued a safety recall (NHTSA Recall 22V-077) on all 2017-2018 Pacifica Plug-In Hybrid Electric Vehicles (PHEV) produced between August 12, 2016 through August 7, 2018. The recall addresses ignition “OFF’ fires that originate at the high volt
... age (HV) battery pack assembly. An ignition ‘OFF’ vehicle fire can result in an increased risk of occupant injury, injury to persons outside the vehicle, and property damage, with little to no warning. At the time of the recall filing the root cause of the vehicle fires was unknown and under investigation by FCA and suppliers. A software remedy designed to improve thermal event detectability by monitoring battery diagnostics is currently being installed on subject vehicles as an interim remedy. As of November 23, 2023, the root cause of the battery failure is unknown. From December 2022 through December 2023, NHTSA has received four consumer complaints (VOQs) alleging vehicle thermal events involving recalled 2017 and 2018 Chrysler Pacifica PHEVs. Additional information collected from complainants confirmed a thermal event originating near or at the HV battery pack after the Recall 22V077 remedy had been applied to the vehicle. Follow up meetings with FCA and suppliers in November and December 2023 to discuss the ongoing root cause investigation and post-recall remedy incidents indicated a recent increase in HV battery thermal events. Furthermore, a review of NHTSA complaint data indicated the post-recall HV battery thermal event complaint rate now exceeds pre-recall levels. The Office of Defects Investigation (ODI) is opening this Recall Query (RQ) to review the effectiveness of the original recall remedy, understand the root cause of the battery fires, investigate additional reports of Pacifica PHEV HV battery fires, and to increase monitoring of the manufacturer root cause investigation. Transmission fault leading to loss of motive power
On July 22, 2022, the Office of Defects Investigation (ODI) opened PE22008 to investigate allegation of loss of motive power, potentially due to transmission malfunction, in certain Chrysler Pacifica vehicles. In a January 17, 2023 letter to NHTSA, Fiat Chrysler Automobiles (FCA) submitted recall no
... tice 23V-010 involving a transmission motor wiring defect in 67,118 model year (MY) 2017-2023 Chrysler Pacifica plug-in hybrid electric vehicles (PHEVs) manufactured from August 12, 2016 through January 9, 2023. FCA determined that an internal transmission connector short could result in an unexpected engine shutdown under certain conditions. Follow up correspondence with FCA confirmed this defect results in an electric motor position trouble code that was the basis for opening PE22008, therefore NHTSA is closing this investigation. The 23V-010 recall remedy is a software update to address the hardware failure associated with PE22008. When a hardware failure is detected, the vehicle limits motive power while instructing the driver to pull over to a safe location. NHTSA is concerned that drivers will pull over to an unsafe location when the warning message associated with the recall remedy displays on the dashboard. Due to the nature of the recall remedy and related reports, NHTSA has opened Recall Query RQ24004 to assess the recall remedy. The reports and failure report summary values included with this closing action are related to allegations of Recall 23V-010 remedy inadequacy. The Agency reserves the right to take additional action if warranted by future circumstances. Stall/Loss of Motive Power
By letter dated November 30, 2017, the Center for Auto Safety (CAS) petitioned the National Highway Traffic Safety Administration requesting it open an investigation into allegations of engine stalling in the model year (MY) 2017 Chrysler Pacifica, a new vehicle platform. In support of its petition,
... CAS cited 57 NHTSA complaints alleging an engine stall in the subject Pacifica vehicles, in addition to other information it provided. On December 1, 2017, NHTSA's Office of Defects Investigation opened Defect Petition 17-003 as requested by CAS.ODI sent an information request letter to Fiat Chrysler Automobiles (FCA), the manufacturer of the subject Pacifica vehicles, on December 12, 2017 requesting information related to engine stalling in Pacificas. The letter requested FCA provide the information by January 23, 2018.On January 11, 2018, and after technical discussions with ODI, FCA filed a defect information report advising the agency that it would conduct a safety recall of certain MY2017 Chrysler Pacificas to address a condition resulting in engine stalling without warning. The recall would address nearly 154,000 MY2017 Pacificas manufactured with engine control software that did not contain a routine intended to address loss of crankshaft position information required to keep the engine from stalling.FCA noted the condition could lead to a stall without illuminating a Malfunction Indicator Lamp, and without setting any Diagnostic Trouble Code.Given these conditions, dealer technicians would be unable to properly diagnose, much less correct the cause of the stalling, which resulted in additional stalling events and significant consumer frustration.ODI reviewed consumer complaints it received through the date of FCA's recall filing and identified 88 alleging engine stalling. The ODI complaints generally describe stalling that occurs at low engine (RPM) and/or vehicle speeds, and are consistent in that regard with most of the 445 reports FCA provided. Also both the ODI and FCA reports note that the engine could be readily restarted after stalling. Both circumstances are consistent with the root cause FCA identified and is conducting a recall to address.Additionally, FCA was able to identify an older vehicle population that used the same engine as the MY2017 Pacifica, however the vehicles were manufactured with the remedy level software; according to FCA reports of stalling in this population were significantly lower. No crash or injury reports were identified in either the ODI or FCA reports.Certain MY2017 and all MY2018 Pacificas were manufactured with the remedy level software and are thus not included in the recall action. In view of the recall action FCA has announced, ODI considers the petition moot and is closing the action without a grant or deny decision. The closing of the petition does not constitute a finding by NHTSA that a safety-related defect does not exist, and the agency reserves the right to take additional action if warranted by future circumstances.Reference (ODI) numbers for the above NHTSA complaints:11060960, 11060808, 11060711, 11057809, 11057520, 11057493, 11057171, 11057169, 11057025, 11055566, 11055031, 11054808, 11054746, 11054717, 11054274, 11054085, 11053669, 11053588, 11053553, 11053322, 11052905, 11052892, 11052654, 11052336, 11051390, 11051158, 11048832, 11048728, 11048720, 11048661, 11048626, 11048175, 11047936, 11047831, 11047793, 11047687, 11047684, 11047399, 11045061, 11042619, 11040610, 11040502, 11040294, 11039494, 11035468, 11034698, 11033417, 11032016, 11030729, 11029903, 11025266, 11025119, 11023691, 11023539, 11022835, 11022154, 11021223, 11019457, 11016083, 1101Read more

COMPLAINANTS ALLEGE THAT A FIRE ERUPTED UNDER THE HOOD, NEAR THE FRONT OF THE VEHICLE.IN CHRYSLER'S RESPONSE TO ODI'S NOVEMBER 2, 2007 INFORMATION REQUEST, CHRYSLER ASSERTS THAT THE ALLEGED DEFECT OCCURS ONLY IN THE 4.0L ENGINES AND THE CONSUMER WILL EXPERIENCE A LOSS OF POWER STEERING FLUID OR A NO
... ISY POWER STEERING PUMP BEFORE SEEING ANY EVIDENCE OF SMOKE OR FIRE.CHRYSLER ALSO STATES THAT A TUBE NUT ON THE HIGH PRESSURE POWER STEERING LINE WAS DISCOVERED TO BE CROSS THREADED DURING THE ASSEMBLY PROCESS AND DID NOT ALLOW PROPER SEALING OF THE O-RING.THE POWER STEERING SYSTEM IN THE 4.0L ENGINE IS ASSEMBLED AT THE CHRYSLER PLANT.THE POWER STEERING SYSTEM FOR THE 3.8L ENGINES ARRIVES AT THE CHRYSLER PLANT PRE-ASSEMBLED.CHRYSLER FOUND THAT IN THE 4.0L ENGINE, IT WAS POSSIBLE TO CROSS-THREAD THE TUBE NUT ON THE HIGH PRESSURE LINE DURING ITS ATTACHMENT TO THE STEERING GEAR, THUS CREATING AN IMPROPER SEAL.CHRYSLER IMPLEMENTED ASSEMBLY PLANT PROCESS CHANGES TO ELIMINATE THE POSSIBILITY OF CROSS-THREADING, AND APPLIED ADDITIONAL QUALITY CONTROL MEASURES TO DETECT POWER STEERING FLUID LEAKS.CHRYSLER STATED THAT WARRANTY AND FIELD DATA SUGGESTS THAT THESE PROCESS CHANGES HAVE MINIMIZED THE POSSIBILITY OF CROSS-THREADING.WITH REGARD TO THE VEHICLES PRODUCED PRIOR TO THE PRODUCTION CHANGE, A REVIEW OF THE WARRANTY CLAIMS INDICATES THAT THE ALLEGED DEFECT HAS NOT DECLINED IN THE 4.0L POPULATION.ODI IS UPGRADING THIS PRELIMINARY EVALUATION TO AN ENGINEERING ANALYSIS, EA08-006, TO FURTHER EVALUATE THE ISSUE.Read more

FUEL TANK PUNCTURES
EA06-010 WAS OPENED BASED ON INFORMATION GATHERED IN PRELIMINARY EVALUATION PE06-009.PE06-009 WAS OPENED BASED ON INFORMATION OBTAINED IN A PEER INFORMATION REQUEST AS PART OF A PRELIMINARY EVALUATION (PE05-050) OF FUEL TANK PUNCTURE IN MODEL YEAR (MY) 2003 THROUGH 2006 NISSAN MURANO VEHICLES.IN JAN
... UARY 2006, NISSAN SUBMITTED A DEFECT INFORMATION REPORT TO ODI THAT IDENTIFIED A DEFECT IN THE FUEL TANK SHIELDING IN APPROXIMATELY 209,000 MY 2003 THROUGH 2006 NISSAN MURANO VEHICLES.TO REMEDY THE PROBLEM, NISSAN INSTALLED SHIELDS ON THE EXPOSED FRONT SURFACES OF THE RECALLED VEHICLES.PEER DATA COLLECTED FOR MY 2004 THROUGH 2005 PACIFICA VEHICLES DURING PE05-050 INCLUDED 120 FIELD REPORTS DOCUMENTING TANK REPLACEMENTS DUE TO PUNCTURE OR DAMAGE BY ROAD DEBRIS AS PART OF A PARTS RESTRICTION PROGRAM CONDUCTED AT THE REQUEST OF THE TANK SUPPLIER (INERGY). ANALYSIS OF THE INFORMATION PROVIDED BY CHRYSLER DURING PE05-050, PE06-009 AND EA06-010 SHOW THAT THE FRONT SURFACE OF THE FUEL TANKS IN THE SUBJECT PACIFICA VEHICLES IS NOT COMPLETELY PROTECTED FROMROAD DEBRIS BY THE FLOOR PAN OR OTHER BODY STRUCTURE.THE TANKS WERE COMPARED WITH PEER VEHICLES, INCLUDING SEVERAL IDENTIFIED BY CHRYSLER AS SHARING SIMILAR DESIGN CHARACTERISTICS (UNSHIELDED HIGH-DENSITY POLYETHYLENE PLASTIC FUEL TANKS IN A SADDLEBAG CONFIGURATION).SOME OF THE PEER DESIGNS REVIEWED BY ODI PACKAGED THE TANKS IN RECESSED CAVITIES IN THE FLOOR PAN OR USED SHIELDS TO PROTECT EXPOSED SURFACES ON THE FRONT OF THE TANK.IN GENERAL, THESE TANKS HAD THE LOWEST COMPLAINT RATES FOR TANK PUNCTURE BY ROAD DEBRIS. HOWEVER, OTHER PEER DESIGNS REVIEWED BY ODI HAD PACKAGING SIMILAR TO THE SUBJECT VEHICLES AND SOME OF THESE ALSO HAD SOME COMPLAINTS OF TANK PUNCTURE.NONE OF THE PEER VEHICLES ANALYZED BY ODI HAD TANKS PUNCTURE DATA COMPARABLE TO CHRYSLER'S FIELD REPORTS FROM ITS PART RESTRICTION PROGRAMS, SO THAT DATA COULD NOT BE USED IN PEER COMPARISONS. ANALYSIS OF THE COMPLAINT DATA SHOWS THAT THE RATE FOR THE SUBJECT VEHICLES IS ABOUT HALF THAT OBSERVED IN THE RECALLED MURANO VEHICLES AND NOT SIGNIFICANTLY HIGHER THAN SOME OF THE PEER VEHICLES ANALYZED WITH SIMILAR DESIGN CHARACTERISTICS.TESTING BY THE NHTSA VEHICLE RESEARCH AND TESTING CENTER (VRTC) SHOWED DIFFERENCES BETWEEN THE MURANO AND PACIFICA FUEL TANK PACKAGING AND EXPOSURE TO ROAD DEBRIS THROWN BY THE FRONT TIRES.ACCORDINGLY, THIS INVESTIGATION IS CLOSED.THE CLOSING OF THIS INVESTIGATION DOES NOT CONSTITUTE A FINDING BY NHTSA THAT A SAFETY-RELATED DEFECT DOES NOT EXIST.THE AGENCY WILL CONTINUE TO MONITOR COMPLAINTS AND OTHER INFORMATION RELATING TO THE ALLEGED DEFECT IN THE SUBJECT VEHICLES AND TAKE FURTHER ACTION IN THE FUTURE IF WARRANTED.SEE DOCUMENT FILE FOR ADDITIONAL INFORMATION REGARDING THIS RESUME.Read more

CHRYSLER HAS NOTED THAT THEY HAD IN PLACE FROM JANUARY 2003 TO JUNE 2005 A PARTS RESTRICTION PROGRAM ON PACIFICA FUEL TANK REPLACEMENTS WHICH MAY HAVE MADE THE NUMBER OF REPORTS ARTIFICIALLY HIGH IN COMPARISON TO MANUFACTURERS WHICH DID NOT HAVE A SIMILAR PROGRAM IN EFFECT.CHRYSLER ALSO QUESTIONED WHETHER MANY OF THE FIELD REPORTS OF FUEL TANKS REPLACED DUE TO DAMAGE FROM UNKNOWN CAUSE WERE ACTUALLY PUNCTURED. THIS INVESTIGATION HAS BEEN UPGRADED TO AN ENGINEERING ANALYSIS (EA06-010) TO GATHER ADDITIONAL INFORMATION CONCERNING THE ALLEGED DEFECT IN THE SUBJECT VEHICLES.Read more
