Incident date: Jul 6, 2026
I am reporting a recall repair failure/delay involving my 2024 Chrysler Pacifica, VIN [XXX]. We received FCA’s recall letter for Important Safety Recall — Side Curtain Airbags, FCA Recall 06D, NHTSA Recall 26V-189. We scheduled a recall appointment with Pedder Chrysler Dodge Ram Jeep of Poway,
... 13811 Poway Rd, Poway, CA 92064, for XXX at 7:45 AM specifically because of this airbag safety recall. When we arrived, the dealer first said there was no recall, then later realized there had been a VIN-related mistake. They kept the vehicle. At 10:44 AM, the dealer texted that the vehicle was ready for pickup, but did not clearly disclose that the airbag recall remained unrepaired. Only after we called and asked for clarification did they say the airbag recall was not completed because the required airbag parts were unavailable. The repair invoice, RO #XXX, shows the airbag recall as “PARTS ON ORDER.” After pickup, NHTSA still showed 1 unrepaired recall for this VIN, status “Recall Incomplete.” The dealer refused to provide a written explanation documenting the VIN mistake, incomplete airbag recall, or ready-for-pickup communication. I am requesting NHTSA review this recall remedy failure/delay and the dealer’s handling of this safety recall appointment. This keeps the key evidence: the recall letter, invoice, and NHTSA active recall record. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

Vehicle went to turtle mode 3 separate times at highway speeds forcing a stop. Each time it was towed to the dealer. First two times little was done per the report and was returned. On the third time it has spent 7 weeks for a repair on the transmission not a replacement. The dealer also needed

The vehicle experienced a sudden, catastrophic brake-by-wire system failure while operating in one-pedal driving (Regeneration Boost mode). While decelerating at a low speed behind an 18-wheeler semi-truck, the vehicle failed to continue slowing down. I immediately depressed the physical brake pedal
... , but the vehicle completely ignored the foot pressure. The pedal and braking system became completely unresponsive, and the vehicle kept rolling forward uncommanded. Despite keeping the brake pedal fully depressed, the vehicle did not decelerate, resulting in a front-end low-speed collision with the metal rear bumper of the semi-truck. Because the collision occurred at a very low speed, the operator of the commercial truck was unaware of the impact and safely continued driving forward without stopping. The vehicle's brakes entirely refused to re-engage until I frantically pumped the physical brake pedal and cycled/pressed vehicle control power switches. This action seemingly forced the electronic braking modules to reboot or re-establish hydraulic pressure, allowing the vehicle to finally come to a stop. The vehicle was subsequently removed from the active roadway. This is a severe, life-threatening safety defect involving a total loss of electronic brake-by-wire and friction braking application without any mechanical override intervention, presenting an extreme hazard. The vehicle has been towed directly to an authorized Toyota dealership for a comprehensive electronic diagnostic pre-scan of the Skid Control ECU, Electronic Parking Brake system, and 12-volt auxiliary power systems to retrieve the stored DTC fault logs and freeze-frame data.Read more

I am writing to formally express my concern and frustration regarding the unresolved recall on my vehicle. It has now been over a year since the recall was issued, and my point of purchase dealer still has not received the necessary parts to complete the repair. This extended delay has moved beyond

I just bought a new Ram Rebel X 2025 in September. I have 7K miles and the front tires are shot due to factory misalignment! Local dealership South Hills Dodge is ZERO HELP. I opened a case with RAM. They said…“You have to drop off your truck at the dealership before we can escalate your case to
... a case manager”. My truck has been sitting outside the dealership since I dropped it off on Monday 4-27 and hasn’t moved!! The dealership initially said they would cover an alignment under warranty free of charge but not cover 2 new tires which were caused by the misalignment. They then told me I would have to open a case with RAM in order to get the tires paid for. In the meantime, I asked the service manager if he could get me the current alignment readings. He then lied to me and told me they weren’t able to take the current alignment measurements due to the fact that the front tires were so badly worn. I then took it to my local mechanic to get the current alignment readings without fixing anything so I could have this sheet for my records. It not only shows the toe being out of spec on both front tires but shows the Caster being out of spec. Therefore it was set that way when I purchased the vehicle brand new back in September. I talked to my case manager who was never able to get ahold of the dealership!! I was also never able to get a response to multiple questions through their text service. Finally they reach out after my vehicle sat for 4 days and say I have to pay 19.95 for a tire rotation in order to do the alignment. I then chose to take my vehicle to Jim Shorkey and they fix some recalls and RRTs but “needed to talk w case mgr about tire coverage” So then I have it out with this case manager about the amount of time and money I’ve wasted so far and in a nutshell he tells me that the first dealer said tires were not covered under warranty so they can’t cover them! Basically I was sent in a circle of wasted time by RAM corporation “customer service”!!!Read more

This is a report of a recurring, life-safety defect involving unintended vehicle movement in a 2024 Ford Expedition. The vehicle has experienced two separate incidents where it was placed in 'Park,' remained stationary for 13-30 seconds, and then began driving itself backward without driver interven
... tion or throttle input. Incident 1: Occurred on March 29, 2026 and I have this one on video. Incident 2: Occurred on April 8, 2026. The video clearly shows the vehicle remaining stationary and in 'Park' for 13 seconds before the unintended movement begins. This delay proves the movement is not a 'start-up rocking' sensation or 'normal clutch drag,' but a delayed mechanical or electronic failure of the park actuator or transmission system. I presented the vehicle for service at a Ford dealership on April 17, 2026. Despite my report of a delayed-onset rollaway, the dealership documented the issue as a 'normal and desired characteristic' of the 10-speed transmission. They characterized the event as a minor 'rocking' sensation that occurs immediately, which is factually contradicted by my video evidence showing a 13-second delay. This leaves me with an unsafe vehicle prone to unintended movement. I am requesting a formal investigation into a potential park-engagement failure.Read more

... be available and then they tried to turn around and ask me did I want to trade it in. So now I am stuck still paying for the jeep and I can not drive it because of the abs module. Subject: Safety Complaint – 2024 Honda Prologue – Clicking Noise When Turning (Possible CV Axle Defect) I am submitting a safety complaint regarding my 2024 Honda Prologue Touring due to a persistent clicking noise when turning, which appears to be related to the front axle or CV joint. Vehicl
... e Information: Year: 2024 Make: Honda Model: Prologue Touring Mileage:10300 VIN: [XXX] Description of Issue: The vehicle produces a noticeable clicking/popping noise when turning left or right, especially at low speeds such as in parking lots or during tight turns. The noise is consistent and reproducible. The issue appears to originate from the front axle/drivetrain area and raises concern about potential failure of a critical drivetrain component. Safety Concern: This defect may indicate premature wear or failure of the CV joint or axle assembly. A failure of this component while driving could result in loss of power to the wheels and create a dangerous driving condition, increasing the risk of an accident. Dealer Response: I brought the vehicle to an authorized Honda dealership. The dealer acknowledged the issue but stated that no repair or fix is currently available and advised me to contact Honda directly. The issue remains unresolved. Additional Notes: Based on my research, multiple other owners are experiencing similar issues with the same vehicle, suggesting this may be a widespread defect rather than an isolated incident. I am requesting that NHTSA investigate this issue as a potential safety defect affecting the 2024 Honda Prologue. Thank you for your attention to this matter. Dear National Highway Traffic Safety Administration (NHTSA),I am submitting this report to express a serious safety concern with my 2015 Buick Verano, which I believe is unsafe for shorter drivers due to poor visibility and design limitations.I received this car as a gift from my father, but the mom
... ent I drove it off the lot, I realized I could not safely operate it. I am 4 feet 11 inches tall, and this vehicle seems designed for much taller drivers. The windshield is too close, and there are several significant blind spots that make it extremely difficult to see vehicles, pedestrians, and obstacles around me.To try to fix this issue, we installed four extra mirrors and I used a booster seat, but these adjustments did not resolve the safety problems. I have experienced numerous close calls, including one incident where I nearly struck a pedestrian I could not see until he moved out of the way. Driving this car has become extremely stressful and frightening due to these visibility issues.I have spoken with several friends who are also under 5 feet 3 inches tall, and they reported the same difficulties when driving a Buick Verano. It appears that this model’s visibility design creates unsafe driving conditions for shorter individuals.I request that the NHTSA investigate this design concern to determine whether the 2015 Buick Verano meets safety standards for drivers of different heights, and whether design changes or warnings are necessary to protect shorter drivers.Thank you for your attention to this important safety issue.thank you.Read more

My 2020 GMC Sierra 1500 Denali with the 6.2L L87 V8 just began experiencing a new rough idle with intermittent RPM dips. There are no engine codes or warning lights. Idle drops are noticeable, suddenly began this week. I am concerned due to ongoing NHTSA Engineering Analysis EA25-007 involving this
... engine. Some owners reported engine failure with little warning. The truck has right over 68,000 miles on it and has had every oil changed performed at a GM dealer. I have had to top off the oil between cycles and always used the recommended oil, being the Dexos Gen 3 due to the truck burning a small amount of oil. More detail on a possible cause. I believe it would be worth investigating the DFM technology. I will be performing a fuel cleaner and possibly a lifter cleaner and/or additive. I would like to tune the truck and turn off the DFM since this is what I have read causes the truck to burn oil and damage the lifters. However, I will wait until this Analysis is completed before deciding to change the truck's stock tunes.Read more

My 2017 Hyundai Tucson is experiencing severe oil consumption, which appears to be related to a known manufacturing defect affecting Hyundai engines from this period. My vehicle is burning through oil rapidly between normal oil change intervals, and it has begun showing signs of low oil levels despi
... te no external leaks. This creates a serious safety concern because the engine can fail without warning. The dealership has acknowledged that excessive oil consumption is a known issue for this engine but refuses to provide assistance because I do not have “enough oil change documentation” over the years. However, this issue is not caused by lack of maintenance — it is a widespread internal defect that Hyundai has issued multiple technical service bulletins for. My car has been maintained, and oil changes were performed at regular intervals, even if I do not have complete paperwork. The rapid oil burning is ongoing and dangerous. I am concerned about the engine seizing, stalling, or failing while driving, which could lead to a crash. This is a safety hazard that should be investigated and included with other Hyundai engine cases. Requested Action: I am asking NHTSA to investigate this oil consumption defect affecting the 2017 Hyundai Tucson and require Hyundai to provide a remedy, including repair or engine replacement, regardless of perfect maintenance documentation, since this problem stems from a manufacturing flaw.Read more

My 2024 Jeep Rubicon 4xe has experienced three separate incidents of complete power loss following the October 10, 2025 Uconnect software update (version FCA-MY24.U33.43-PROD). In two cases, the failure occurred while crossing active intersections, where the vehicle abruptly lost acceleration, flash
... ed gear indicators, and displayed a “Shift to Park and Press Brake + Start Button” message. The vehicle became unresponsive and required a lengthy restart. These incidents posed serious safety risks to myself, my young driver, and surrounding drivers. I’ve attempted resets and reviewed Jeeps online community, Jeep connect app, but no fix is available. Multiple owners are reporting identical failures online, suggesting a widespread and massive software fault. I urge NHTSA to investigate this issue and compel Jeep to issue a rollback or patch to restore safe operation immediately. I am also requesting NHTSA to legally require these vehicles to be disabled immediately until a robust fix is available to avert life threatening consequences. I cannot believe Jeep has not done this on their own!!!Read more

On April 30, 2024, while sitting at a traffic light waiting to turn, my Honda Pilot experienced a catastrophic engine failure. The driver heard a loud knocking sound and the engine failed. The driver was left to navigate to a safe area through traffic. The vehicle was towed to a nearby auto mechanic
... for evaluation. It was determined that a connecting rod had penetrated the engine block and that engine would not be salvageable. The vehicle was towed again to another auto mechanic, where the engine was removed and replaced with a used engine of the same make and model with other required support equipment (timing belt, water pump, miscellaneous belts and hardware) at a cost of $8656.00 Given the passage of approximately 15 months since the engine was replaced, the catastrophically failed engine is no longer available to be inspected. Thankfully the vehicle was stationary when the failure occurred or the driver may have been at greater risk. He did need to walk through a busy intersection to get to safety. The engine failure and replacement was performed and documented by an independent service center. The failed engine was not inspected by the manufacturer, police, or insurance carrier’s representatives. Only observed by the two independent service centers. There were no warning lamps, or no messages or other symptoms of the problem prior to the failure. This issue appears to be related to NHTSA investigation: PE25008Read more

... other lights prior to that moment had illuminated on the instrument panel. The car is a 2017 Accord Sport with approximately 101k miles. Purchased in November 2021, this is the first time there’s ever been a problem and my car is maintained regularly. The car was towed to a local mechanic and I was told there was a misfire on cylinder 3, the valve spring is broken, the valve is bent, and the timing chain is loose. I was told I need a new engine. I don’t understand what happened because there was no warning or indication that anything was wrong. The safety concern is that the car died while driving on the highway and the vibration and jerking was scary trying to control the car while traveling at high speeds. I called Honda directly and I was told that my vin isn’t included in any recalls so there’s nothing they can do to help. I know there was a service bulletin in 2019 for the cam chain tensioner but I don’t know if this car was serviced since I bought it in 2021. I can’t afford to replace the engine and I’m still making payments. My VIN isn’t included with the fuel pump or oil pump recalls despite now including the L4 2.0 engines. This is my third Honda and I’ve never had anything like this happen before. The customer service rep said there was nothing they could do to help after confirming that my vin wasn’t included in any of the recalls. I had the car towed to a Honda dealership earlier this week for a deeper look into how and why this happened and the tech said the engine suffered a catastrophic failure. He also stated that he’s never seen anything like this before - referring to the broken valve spring. Valve springs are not wear and tear items.Read more

I got my new 2024 Chevy Silverado 2.7l turbomax custom 6 months ago. It shifts weird the first few times from a cold start. I have asked the salesman and he said yeah you have to let the truck warm up for 5 min or so. But even in doing so it still happens, worse the colder out it is. Yesterday I wen
... t to grab some food and went to take off onto the main road and when it tried to shift from first to second it felt like the transmission just stopped working for a few seconds then hit 2nd gear hard. I was told this was because the truck needs to get used to the way I drive it. Which I now have close to 4,000 miles on it and it still happens almost every time I shut the truck off and it cools down to ambient temperature. I know this is not normal, and a big safety concern because when I do go to pull out and this happens it feels like a total loss of power and does not take off like it should. I have yet to bring it into the dealership. But I have called about the issue and was told this is normal and would go away. Which it has not. So I am going to make a appointment to get it in the shop. I have also read that many others are having this same exact issue. In which case some people are saying they told them the same thing “that it’s normal”, I have heard others say they will change transmission fluid because the fluid they used is to thick, and have heard of others who had to have them change out the control module for the transmission and or reset it. I am just worried that I’m going to pull out on day and have this happen and get hit from the rear because I don’t have the power I should have to take off properly. Not to mention I should not have to let my car idle for 5-10 min before I can drive it. That’s a huge waste of gas, and time, plus having your direct injection idle makes a ton of carbon build up in the engine. I am not a certified technician but I have been working on my own vehicles for over 20 years now. So a brand new truck should not be doing this.Read more

Intermittent Loss of Electric Power Steering
The Office of Defects Investigation (ODI) is opening this Preliminary Evaluation to assess the scope, frequency, and potential safety consequences of alleged defects related to the electric power steering (EPS) system in model year (MY) 2017–2018 Chrysler Pacifica vehicles. ODI has received 94 Veh
... icle Owner Questionnaires (VOQs) reporting a condition in which the steering system exhibits a sticking sensation during gradual turns, particularly when the steering wheel is positioned near the 11 o'clock or 1 o'clock position. Complainants allege that during these events, increased steering effort is required to return the wheel to center. In many cases, this is followed by a sudden reactivation of EPS assistance, which may result in an oversteer condition. This behavior could increase the risk of a crash, especially at higher speeds. ODI has identified multiple VOQs describing similar symptoms. This investigation will assess the frequency, root cause, and potential safety-related consequences of the alleged EPS malfunction. To review the ODI reports cited in the Opening Resume ODI Report Identification Number document, go to NHTSA.gov.Read more

... 2023, Fiat Chrysler Automobiles (FCA) issued safety recall (NHTSA Recall 23V-010) involving 2017-2023 Model Years (MY) Pacifica Plug-In Hybrid Electric Vehicles (PHEV) produced between August 12, 2016, through January 9, 2023. The recall states that the loss of motive power condition in these vehicles is caused by an internal transmission wiring connector that may short circuit, resulting in an unexpected engine shutdown. An unexpected engine shutdown can cause a loss of drive power, increasing the risk of a crash. In discussions with FCA on Recall 23V-010, ODI confirmed the recall was related to investigation PE22-008. The remedy for 23V-010 is to update the Power Inverter module (PIM) and/or Instrument Panel Cluster (IPC) software to provide drivers a message stating “Stop Safely - Vehicle Will Shut Off Soon” followed by a chime and MIL, PRNDL blinking (on cluster and shifter knob), low SOC (State of Charge) warning and reduced power mode. The recall remedy does not replace the defective hardware that was described in Recall 23V-010, and the wiring connector short still exists on these vehicles. To date, ODI has received 132 reports for the population of vehicles subject to recall 23V-010. These reports allege a loss of motive power condition that reduces the vehicles speed, followed by dashboard warning lamps displaying messages associated with recall 23V-010. The presence of these warning lamps and messages indicates the vehicle likely had the recall remedy applied. Follow up discussions with FCA confirm their remedy does not prevent a loss of motive power, but instead limits torque to the engine and after an unspecified number of miles or period of time, the engine will shut down. When this occurs, FCA relies on the owner to transport the vehicle to a dealership after they experience a loss of motive power condition, at which time the defective hardware is replaced. Furthermore, the dashboard messages introduced in the recall remedy may cause the driver to pull over in an unsafe location. Pulling over in an unsafe location may expose the occupants to undue harm. The Office of Defects Investigation is opening this Recall Query (RQ) to assess the adequacy, frequency, and safety consequence of the remedy for recall 23V-010. Ignition-off high voltage battery fire (Remedy effectiveness of Recall 22V-077)
On February 11, 2022, Fiat Chrysler Automobiles (FCA) issued a safety recall (NHTSA Recall 22V-077) on all 2017-2018 Pacifica Plug-In Hybrid Electric Vehicles (PHEV) produced between August 12, 2016 through August 7, 2018. The recall addresses ignition “OFF’ fires that originate at the high volt
... age (HV) battery pack assembly. An ignition ‘OFF’ vehicle fire can result in an increased risk of occupant injury, injury to persons outside the vehicle, and property damage, with little to no warning. At the time of the recall filing the root cause of the vehicle fires was unknown and under investigation by FCA and suppliers. A software remedy designed to improve thermal event detectability by monitoring battery diagnostics is currently being installed on subject vehicles as an interim remedy. As of November 23, 2023, the root cause of the battery failure is unknown. From December 2022 through December 2023, NHTSA has received four consumer complaints (VOQs) alleging vehicle thermal events involving recalled 2017 and 2018 Chrysler Pacifica PHEVs. Additional information collected from complainants confirmed a thermal event originating near or at the HV battery pack after the Recall 22V077 remedy had been applied to the vehicle. Follow up meetings with FCA and suppliers in November and December 2023 to discuss the ongoing root cause investigation and post-recall remedy incidents indicated a recent increase in HV battery thermal events. Furthermore, a review of NHTSA complaint data indicated the post-recall HV battery thermal event complaint rate now exceeds pre-recall levels. The Office of Defects Investigation (ODI) is opening this Recall Query (RQ) to review the effectiveness of the original recall remedy, understand the root cause of the battery fires, investigate additional reports of Pacifica PHEV HV battery fires, and to increase monitoring of the manufacturer root cause investigation. Transmission fault leading to loss of motive power
On July 22, 2022, the Office of Defects Investigation (ODI) opened PE22008 to investigate allegation of loss of motive power, potentially due to transmission malfunction, in certain Chrysler Pacifica vehicles. In a January 17, 2023 letter to NHTSA, Fiat Chrysler Automobiles (FCA) submitted recall no
... tice 23V-010 involving a transmission motor wiring defect in 67,118 model year (MY) 2017-2023 Chrysler Pacifica plug-in hybrid electric vehicles (PHEVs) manufactured from August 12, 2016 through January 9, 2023. FCA determined that an internal transmission connector short could result in an unexpected engine shutdown under certain conditions. Follow up correspondence with FCA confirmed this defect results in an electric motor position trouble code that was the basis for opening PE22008, therefore NHTSA is closing this investigation. The 23V-010 recall remedy is a software update to address the hardware failure associated with PE22008. When a hardware failure is detected, the vehicle limits motive power while instructing the driver to pull over to a safe location. NHTSA is concerned that drivers will pull over to an unsafe location when the warning message associated with the recall remedy displays on the dashboard. Due to the nature of the recall remedy and related reports, NHTSA has opened Recall Query RQ24004 to assess the recall remedy. The reports and failure report summary values included with this closing action are related to allegations of Recall 23V-010 remedy inadequacy. The Agency reserves the right to take additional action if warranted by future circumstances. Stall/Loss of Motive Power
By letter dated November 30, 2017, the Center for Auto Safety (CAS) petitioned the National Highway Traffic Safety Administration requesting it open an investigation into allegations of engine stalling in the model year (MY) 2017 Chrysler Pacifica, a new vehicle platform. In support of its petition,
... CAS cited 57 NHTSA complaints alleging an engine stall in the subject Pacifica vehicles, in addition to other information it provided. On December 1, 2017, NHTSA's Office of Defects Investigation opened Defect Petition 17-003 as requested by CAS.ODI sent an information request letter to Fiat Chrysler Automobiles (FCA), the manufacturer of the subject Pacifica vehicles, on December 12, 2017 requesting information related to engine stalling in Pacificas. The letter requested FCA provide the information by January 23, 2018.On January 11, 2018, and after technical discussions with ODI, FCA filed a defect information report advising the agency that it would conduct a safety recall of certain MY2017 Chrysler Pacificas to address a condition resulting in engine stalling without warning. The recall would address nearly 154,000 MY2017 Pacificas manufactured with engine control software that did not contain a routine intended to address loss of crankshaft position information required to keep the engine from stalling.FCA noted the condition could lead to a stall without illuminating a Malfunction Indicator Lamp, and without setting any Diagnostic Trouble Code.Given these conditions, dealer technicians would be unable to properly diagnose, much less correct the cause of the stalling, which resulted in additional stalling events and significant consumer frustration.ODI reviewed consumer complaints it received through the date of FCA's recall filing and identified 88 alleging engine stalling. The ODI complaints generally describe stalling that occurs at low engine (RPM) and/or vehicle speeds, and are consistent in that regard with most of the 445 reports FCA provided. Also both the ODI and FCA reports note that the engine could be readily restarted after stalling. Both circumstances are consistent with the root cause FCA identified and is conducting a recall to address.Additionally, FCA was able to identify an older vehicle population that used the same engine as the MY2017 Pacifica, however the vehicles were manufactured with the remedy level software; according to FCA reports of stalling in this population were significantly lower. No crash or injury reports were identified in either the ODI or FCA reports.Certain MY2017 and all MY2018 Pacificas were manufactured with the remedy level software and are thus not included in the recall action. In view of the recall action FCA has announced, ODI considers the petition moot and is closing the action without a grant or deny decision. The closing of the petition does not constitute a finding by NHTSA that a safety-related defect does not exist, and the agency reserves the right to take additional action if warranted by future circumstances.Reference (ODI) numbers for the above NHTSA complaints:11060960, 11060808, 11060711, 11057809, 11057520, 11057493, 11057171, 11057169, 11057025, 11055566, 11055031, 11054808, 11054746, 11054717, 11054274, 11054085, 11053669, 11053588, 11053553, 11053322, 11052905, 11052892, 11052654, 11052336, 11051390, 11051158, 11048832, 11048728, 11048720, 11048661, 11048626, 11048175, 11047936, 11047831, 11047793, 11047687, 11047684, 11047399, 11045061, 11042619, 11040610, 11040502, 11040294, 11039494, 11035468, 11034698, 11033417, 11032016, 11030729, 11029903, 11025266, 11025119, 11023691, 11023539, 11022835, 11022154, 11021223, 11019457, 11016083, 1101Read more

COMPLAINANTS ALLEGE THAT A FIRE ERUPTED UNDER THE HOOD, NEAR THE FRONT OF THE VEHICLE.IN CHRYSLER'S RESPONSE TO ODI'S NOVEMBER 2, 2007 INFORMATION REQUEST, CHRYSLER ASSERTS THAT THE ALLEGED DEFECT OCCURS ONLY IN THE 4.0L ENGINES AND THE CONSUMER WILL EXPERIENCE A LOSS OF POWER STEERING FLUID OR A NO
... ISY POWER STEERING PUMP BEFORE SEEING ANY EVIDENCE OF SMOKE OR FIRE.CHRYSLER ALSO STATES THAT A TUBE NUT ON THE HIGH PRESSURE POWER STEERING LINE WAS DISCOVERED TO BE CROSS THREADED DURING THE ASSEMBLY PROCESS AND DID NOT ALLOW PROPER SEALING OF THE O-RING.THE POWER STEERING SYSTEM IN THE 4.0L ENGINE IS ASSEMBLED AT THE CHRYSLER PLANT.THE POWER STEERING SYSTEM FOR THE 3.8L ENGINES ARRIVES AT THE CHRYSLER PLANT PRE-ASSEMBLED.CHRYSLER FOUND THAT IN THE 4.0L ENGINE, IT WAS POSSIBLE TO CROSS-THREAD THE TUBE NUT ON THE HIGH PRESSURE LINE DURING ITS ATTACHMENT TO THE STEERING GEAR, THUS CREATING AN IMPROPER SEAL.CHRYSLER IMPLEMENTED ASSEMBLY PLANT PROCESS CHANGES TO ELIMINATE THE POSSIBILITY OF CROSS-THREADING, AND APPLIED ADDITIONAL QUALITY CONTROL MEASURES TO DETECT POWER STEERING FLUID LEAKS.CHRYSLER STATED THAT WARRANTY AND FIELD DATA SUGGESTS THAT THESE PROCESS CHANGES HAVE MINIMIZED THE POSSIBILITY OF CROSS-THREADING.WITH REGARD TO THE VEHICLES PRODUCED PRIOR TO THE PRODUCTION CHANGE, A REVIEW OF THE WARRANTY CLAIMS INDICATES THAT THE ALLEGED DEFECT HAS NOT DECLINED IN THE 4.0L POPULATION.ODI IS UPGRADING THIS PRELIMINARY EVALUATION TO AN ENGINEERING ANALYSIS, EA08-006, TO FURTHER EVALUATE THE ISSUE.Read more

FUEL TANK PUNCTURES
EA06-010 WAS OPENED BASED ON INFORMATION GATHERED IN PRELIMINARY EVALUATION PE06-009.PE06-009 WAS OPENED BASED ON INFORMATION OBTAINED IN A PEER INFORMATION REQUEST AS PART OF A PRELIMINARY EVALUATION (PE05-050) OF FUEL TANK PUNCTURE IN MODEL YEAR (MY) 2003 THROUGH 2006 NISSAN MURANO VEHICLES.IN JAN
... UARY 2006, NISSAN SUBMITTED A DEFECT INFORMATION REPORT TO ODI THAT IDENTIFIED A DEFECT IN THE FUEL TANK SHIELDING IN APPROXIMATELY 209,000 MY 2003 THROUGH 2006 NISSAN MURANO VEHICLES.TO REMEDY THE PROBLEM, NISSAN INSTALLED SHIELDS ON THE EXPOSED FRONT SURFACES OF THE RECALLED VEHICLES.PEER DATA COLLECTED FOR MY 2004 THROUGH 2005 PACIFICA VEHICLES DURING PE05-050 INCLUDED 120 FIELD REPORTS DOCUMENTING TANK REPLACEMENTS DUE TO PUNCTURE OR DAMAGE BY ROAD DEBRIS AS PART OF A PARTS RESTRICTION PROGRAM CONDUCTED AT THE REQUEST OF THE TANK SUPPLIER (INERGY). ANALYSIS OF THE INFORMATION PROVIDED BY CHRYSLER DURING PE05-050, PE06-009 AND EA06-010 SHOW THAT THE FRONT SURFACE OF THE FUEL TANKS IN THE SUBJECT PACIFICA VEHICLES IS NOT COMPLETELY PROTECTED FROMROAD DEBRIS BY THE FLOOR PAN OR OTHER BODY STRUCTURE.THE TANKS WERE COMPARED WITH PEER VEHICLES, INCLUDING SEVERAL IDENTIFIED BY CHRYSLER AS SHARING SIMILAR DESIGN CHARACTERISTICS (UNSHIELDED HIGH-DENSITY POLYETHYLENE PLASTIC FUEL TANKS IN A SADDLEBAG CONFIGURATION).SOME OF THE PEER DESIGNS REVIEWED BY ODI PACKAGED THE TANKS IN RECESSED CAVITIES IN THE FLOOR PAN OR USED SHIELDS TO PROTECT EXPOSED SURFACES ON THE FRONT OF THE TANK.IN GENERAL, THESE TANKS HAD THE LOWEST COMPLAINT RATES FOR TANK PUNCTURE BY ROAD DEBRIS. HOWEVER, OTHER PEER DESIGNS REVIEWED BY ODI HAD PACKAGING SIMILAR TO THE SUBJECT VEHICLES AND SOME OF THESE ALSO HAD SOME COMPLAINTS OF TANK PUNCTURE.NONE OF THE PEER VEHICLES ANALYZED BY ODI HAD TANKS PUNCTURE DATA COMPARABLE TO CHRYSLER'S FIELD REPORTS FROM ITS PART RESTRICTION PROGRAMS, SO THAT DATA COULD NOT BE USED IN PEER COMPARISONS. ANALYSIS OF THE COMPLAINT DATA SHOWS THAT THE RATE FOR THE SUBJECT VEHICLES IS ABOUT HALF THAT OBSERVED IN THE RECALLED MURANO VEHICLES AND NOT SIGNIFICANTLY HIGHER THAN SOME OF THE PEER VEHICLES ANALYZED WITH SIMILAR DESIGN CHARACTERISTICS.TESTING BY THE NHTSA VEHICLE RESEARCH AND TESTING CENTER (VRTC) SHOWED DIFFERENCES BETWEEN THE MURANO AND PACIFICA FUEL TANK PACKAGING AND EXPOSURE TO ROAD DEBRIS THROWN BY THE FRONT TIRES.ACCORDINGLY, THIS INVESTIGATION IS CLOSED.THE CLOSING OF THIS INVESTIGATION DOES NOT CONSTITUTE A FINDING BY NHTSA THAT A SAFETY-RELATED DEFECT DOES NOT EXIST.THE AGENCY WILL CONTINUE TO MONITOR COMPLAINTS AND OTHER INFORMATION RELATING TO THE ALLEGED DEFECT IN THE SUBJECT VEHICLES AND TAKE FURTHER ACTION IN THE FUTURE IF WARRANTED.SEE DOCUMENT FILE FOR ADDITIONAL INFORMATION REGARDING THIS RESUME.Read more

FUEL TANK PUNCTURES
ON FEBRUARY 22, 2006, ODI OPENED PE06-009 TO INVESTIGATE REPORTS THAT THE FUEL TANKS ON MODEL YEAR (MY) 2004 TO 2005 CHRYSLER PACIFICA VEHICLES MAY BE PUNCTURED BY CONTACT WITH ROAD DEBRIS.THE NUMBER OF REPORTS SUBMITTED BY CHRYSLER WAS SIGNIFICANTLY HIGHER THAN THOSE SUBMITTED BY OTHER MANUFACTURER
... S WHO WERE REQUESTED TO SUBMIT DATA AS PART OF INVESTIGATION PE05-050 INTO MY 2003 TO 2006 NISSAN MURANO VEHICLES THAT MAY HAVE HAD INSUFFICIENT SHIELDING OF THE FUEL TANK TO PREVENT PUNCTURES AFTER CONTACT WITH ROAD DEBRIS.NISSAN ISSUED RECALL 06V-003 INVOLVING INCREASING THE SHIELDING OF THE FUEL TANKS TO REDUCE THE LIKELIHOOD OF TANK PUNCTURES. CHRYSLER HAS NOTED THAT THEY HAD IN PLACE FROM JANUARY 2003 TO JUNE 2005 A PARTS RESTRICTION PROGRAM ON PACIFICA FUEL TANK REPLACEMENTS WHICH MAY HAVE MADE THE NUMBER OF REPORTS ARTIFICIALLY HIGH IN COMPARISON TO MANUFACTURERS WHICH DID NOT HAVE A SIMILAR PROGRAM IN EFFECT.CHRYSLER ALSO QUESTIONED WHETHER MANY OF THE FIELD REPORTS OF FUEL TANKS REPLACED DUE TO DAMAGE FROM UNKNOWN CAUSE WERE ACTUALLY PUNCTURED. THIS INVESTIGATION HAS BEEN UPGRADED TO AN ENGINEERING ANALYSIS (EA06-010) TO GATHER ADDITIONAL INFORMATION CONCERNING THE ALLEGED DEFECT IN THE SUBJECT VEHICLES.Read more
