When the adaptive cruise control and automatic emergency braking systems become unavailable or fail to function properly, I cannot rely on features intended to assist in maintaining safety. The loss of these systems creates a potential safety risk for me, my passengers, and other motorists. The vehicle has been presented to an authorized Volkswagen dealer on multiple occasions for these concerns. The dealer has attempted repairs, but the problems continue to occur. Warning messages indicating that adaptive cruise control, front assist, or automatic emergency braking systems were unavailable appeared before and during the malfunction. I thought before they were just faulty sensors. The dealer never explained these features were not functioning. The symptoms first appeared while the vehicle was under warranty and have continued despite multiple repair attempts. The car was purchased brand new from the dealer.Read more

I am filing a safety complaint regarding a serious transmission defect in my vehicle. The drive and reverse gears are not functioning properly. After driving for a few minutes and parking, when I attempt to reverse on an inclined surface, the vehicle unexpectedly moves forward instead of backward.
... Similarly, when the vehicle is parked facing downhill and I shift into drive to move forward, it rolls backward. There are no warning lights or dashboard indicators alerting me to any malfunction. Due to this defect, I have nearly collided with objects multiple times within just 15 days of ownership. Most concerning, I almost hit another vehicle behind me while attempting to drive forward, as my vehicle suddenly rolled backward unexpectedly. This creates a serious safety hazard for me, my passengers, and other drivers. I purchased this used vehicle with less than 10,000 miles from EchoPark dealership. Unfortunately, I am unable to return the vehicle because their return policy is limited to 7 days. However, this issue appears to be a significant mechanical or transmission defect that poses an immediate safety risk.Read more

The vehicle was returned to me on February 1, 2025. On February 9, 2025, while parked in my driveway and not in operation, the vehicle caught fire. My husband and I were driving his Chevy Silverado with our two dogs in the back. Without any warning or any alert, our vehicle came to a complete stop in the middle lane on a three lane highway. Not a slow stop where there was any time. I’m confirming immediate. At that time, the vehicle would not

It’s truly a miracle that no one was killed. He put truck in neutral and pressed start button and was not able to go any faster then 5 mph and once again a miracle we were able to make it to shoulder of highway without being hit. The vehicle was shaking excessively and flashing things like emissions, traction, as our videos and pictures will show when we made it to side of highway. This is a disgrace that any vehicle would ever be manufactured with this outcome. I am deeply grateful that my husband and I and our two dogs are alive and not injured but this should NEVER happen again.Read more

We discovered severe mold contamination and water intrusion in our 2023 Toyota RAV4 Hybrid. My wife, our [XXX] child, and I have been experiencing dizziness and nausea for months whenever we were inside the vehicle, but we did not know the cause until the dealer (DLL) showed us extensive mold throu

The mold has spread into the floors, interior panels, and even the steering components. This is a serious health hazard. After doing research, we found many other RAV4 Hybrid owners reporting similar water leaks and mold growth issues. This raises the concern of a potential design or sealing defect in this model. This vehicle is unsafe to drive, especially with a young child. We are requesting a full safety investigation into the water intrusion, sealing integrity, and air quality risks associated with this model. Symptoms: • Dizziness • Nausea • Headache • Strong mold smell • Corrosion of interior components This is a serious safety and health issue, and we are asking NHTSA to investigate. The fire department arrived and extinguished the flames, and both fire and police reports were created documenting the incident. The vehicle was nearly new, with approximately 7,000 miles on it, no mechanical issues, and no prior maintenance warnings. Nothing unusual occurred during the drive, and the vehicle had been used under normal, paved-road conditions. On November 5, 2025, Volkswagen conducted an inspection at the IAA facility in Puyallup, WA. Volkswagen later denied liability, stating that burned and unburned vegetation was found in the engine compartment and suggesting that external organic material may have caused the fire. This explanation appears unreasonable, as vegetation in the engine bay does not normally ignite spontaneously in a modern vehicle operating under standard conditions. The vehicle is currently with State Farm at the IAA facility in Puyallup, WA, and is available for further inspection by NHTSA if needed. This incident posed a serious safety risk to the occupants and to other motorists, and may reflect a potential safety defect requiring further investigationRead more

I was driving my 2023 wrangler 4xe to work one Saturday morning. No weather conditions, no change of habit in my drive, and normal driving conditions in general. Half way there I felt a weird change and slight jerk while on the highway. After I noticed a light come on. I was unable to stop so I
... continued and monitored my gauges. All seemed normal except it kicked into gas only mode I noticed. Once I made it to my first red light it braked funny/differently. Once the light was green I accelerated but it was delayed and felt clunky. It almost jolted a little in order to go. I was five minutes from work so I continued on slowly. Once I made it to work I had to turn left through a double lane. Once clear I accelerated to turn into my lot. The jeep stayed lit but completely lost power and actually stopped in the middle of oncoming traffic. Thankfully one car slowed but the other planned to keep going. I panicked hitting the gas a few times until it finally jolted me up into the parking lot. Once I parked it I turned it off to breath and calm down. My child could have been in there. Had I not moved someone could have hit me and blamed me for the accident. After I parked it and turned it off it would not turn back on and shift again. But once I got a tow truck there to take to the dealer he was able to to turn it on. So I’m unsure how that’s safe either.Read more

Incident date: Oct 11, 2025
During an early season wet snow storm within minutes of driving the lights had become covered with snow and deflected the light in such a way that created a very unsafe driving environment. There was zero snow on the road because of it not being cold enough to stick but the deflection of the light
... and limited light on the road it was nearly impossible to see the road markings and lines to stay in the lane and created a very unsafe environment around me requiring me to drive significantly lower to try to stay on the road which could have caused an accident with other drivers who were not driving as slowly. There are a significant amount of reports of incidents like this across multiple forum platforms (ie: reddit) where drivers of the Rivian have indicated a drive that was very unsafe and something they had never experienced before. Rivian needs to create a retrofit to the existing headlamp that has a heating element of something to continue to melt the snow off the light as driving since the truck face is flat and the LEDs do not conduct enough heat to melt snow as driving.Read more
