... re involving the brake booster, brake actuator, and ABS system at only 58,769 miles. The vehicle was diagnosed with Diagnostic Trouble Code C1391 (Abnormal Leak of Accumulator Pressure). According to my mechanic, the condition is caused by an internal brake fluid leak within the brake booster assembly with master cylinder. The recommended repair includes replacement of both the brake booster assembly with master cylinder and the brake booster pump assembly. The estimated cost of repair is approximately $4,100. This concerns me because the brake system is a critical safety component, and I believe such a failure at fewer than 60,000 miles is highly unusual and may pose a safety risk to drivers and passengers. Toyota previously issued a Warranty Enhancement Program for this issue, but my vehicle was denied coverage because the program expired on August 31, 2021, and the secondary coverage expired on February 20, 2022, ten years after the vehicle's first use date. Toyota declined any out-of-warranty assistance despite the vehicle's exceptionally low mileage. The fault cascade included a HIGH VOLTAGE ISOLATION FAULT (BMS_f123), drive inverter warning (DI_w126), and multiple battery management warnings. The instrument cluster displayed a red emergency warning: "Vehicle shutting down — PULL OVER SAFELY." The vehicle required towing. The failure was confirmed by the service center's own follow-up invoice, which documented "trapped air pockets in the coolant system following the most recent service" as the cause. The corrective repair was performed at $0 as a Goodwill service — the service center's internal acknowledgment of liability. The service advisor made a written text admission of causation the following day, then retracted it in a recorded phone call. Both are preserved. No post-repair Isolation Resistance test results were provided. The HV isolation fault may indicate latent thermal damage to HV insulation that could cause a safety failure at higher mileage outside any remaining warranty. ... ely after unplugging the charger, the vehicle activated Walk-Away Door Lock and locked all doors despite the phone key remaining inside the vehicle. I was locked out without access to my phone and had to ask a stranger to call 911 and contact my husband. Cabin temperatures rapidly became dangerous for an infant. My husband was eventually able to remotely unlock the vehicle using the Tesla app, narrowly avoiding forced entry by emergency responders. The vehicle failed to detect the phone key inside the cabin and improperly initiated locking while a vulnerable child occupant was present. This created a serious risk of heat injury or death. The vehicle also failed to prevent the lockout despite an infant occupant being seated in the second row, raising concerns about whether rear-seat occupancy detection and child safety protections functioned properly during the event. I am requesting investigation into Tesla’s proximity sensing, phone key detection, walk-away lock behavior, and child occupant safety protections during charging/unplugging events. ... , the dash cluster started warning to shut off the engine immediately due to a lack of oil. I know there was plenty of oil in the engine because I just had the oil changed at a shop two weeks prior to the incident. I was able to verify that it was a total of 22 seconds from the time I noticed the smoke to the time I was able to pull off safely and shut the engine off. At that time, the engine had not seized, and I was able to manually shut off the engine on my own. I never tried to restart the engine and I had it towed to a shop that was nearby. 48 hours later the mechanic inspected it and discovered that the oil cooling line had somehow disconnected from the engine block, which had resulted in a catastrophic loss of oil. When that repair was made, the mechanic discovered that the engine would not start either by hand or automatically and deemed the engine to be seized. The engine in question is the Chevrolet 6.2L L87 engine that was involved in a manufacturer recall for models made in 2020 through 2024. My vehicle is the 2019 model and although there have been numerous occasions that I’ve been able to find in only a short period of research that revealed similar issues with a 2019 L87 6.2L engine, there still is no official recall on my specific model year. It was 1:30 in the morning, and I was rushing my sick dog to the emergency vet when this happened. Later that afternoon I was informed that the engine had failed and a replacement would be ordered. There was no prior indication of a problem or warning/CEL’s. The engine failed after the dealership had completed the engine recall ( pico test and 0-40W oil change ) related to the vehicle. I am reporting this in hopes that it helps with the investigation. This has been very disappointing for me as I have supported the brand of vehicle for many years. Driving a car with this issue that does not alert with a check engine light is a major threat to driver and passenger safety as it can result in engine seizure and vehicle fires. The engine issues began at the least before 58,000 miles, with total failure occuring at roughly 96,000 miles. Hyundai is known for this issue, particularly in turbo model engines. Only once during ownership of the car did the check engine light come on prior to engine failure. When checked, it was described as a catalytic converter issue. The service center also did an oil change on the car, and the light went off. The check engine light did not come on again for more than a year. The check engine light did not come on until after the issue was diagnosed by the independent service center it was taken to in January 2026.Read more

I noticed in the fall of 2024 that my transmission stared shifting hard between 30-40 mph during acceleration. I had the local Ford dealership check out the issue and they stated I needed a new transmission and included an estimate. I decided to seek information from 2 independent transmission shops
... and they could not confirm that I needed the transmission replaced at that time. The vehicle has been driving fine and I recently started to notice the hard shift again at 30 mph. Every once in a while, when the hard shift occurs, it seems like I lose power to the drive train and my vehicle slows down to about 15 mph and vehicles need to go around me. It only lasts for a few seconds and then the acceleration will continue to drive normal. I do notice an increase in RPM's from time to time as well while shifting around 30 mph but does not feel like it is slipping. Once through this area of shifting, the vehicle operates normally shifting through other gears. There were no warning lights or other symptoms leading to this issue. I did notice that there was an article posted with a possible transmission issue from this year vehicle that is being investigated concerning sudden down shifting while driving. My car suffered a failure of the Integrated Charge Control Unit ("ICCU") which makes the car underivable. This is the third time that I have had an issue with the ICCU. The first two times, it failed to charge the 12V battery, leading to a loss of all 12V systems and being unable to drive the car.

While I was driving on the highway I heard a rattling noise in the engine bay and every single light and system showed a malfunction on my dash. Then I was not able to accelerate and had to roll to a stop. This was extremely dangerous both for me and for the other vehicles on the highway because the
... speed limit was 55 MPH and between not knowing why all of my system warning lights turned on I didn't even notice that I was not accelerating anymore for a few moments. The worst part was that the area in which I was driving had no shoulder and it was by a body of water. When I was relatively safe out of the way I called a tow truck to take me to a repair shop a block away from my residence in the event it was something minor. The first repair shop looked at it and stated it was an engine issue and I should take it to the dealer/repair shop we normally take it to. I then needed to get it towed again to the other shop and the problem was confirmed to be a catastrophic engine failure by the Honda dealer/repair shop which we always take our vehicle to for maintenance (literally got an oil change and filters replaced less than a month prior) and had to pay to get our engine replaced. The original engine is not available for inspection as we had to get it replaced. The vehicle was not inspected by the manufacturer because they stated there was no open recall on our vehicle even if there were other vehicles with the same year make and model with an open recall on an issue that could cause something like this.Read more
