This is a well-documented manufacturing defect that directly compromises passenger safety, drastically increases stopping distances, and threatens a complete loss of power assist while driving. Toyota explicitly acknowledged this inherent design flaw through its Customer Support Program (ZJB). However, they limited coverage to a strict 10-year expiration date from the first date of use. Because my vehicle has exceptionally low mileage and was driven gently, the component took longer to degrade, causing it to fail just outside Toyota's arbitrary time limit. Limiting safety coverage based on time rather than mileage penalizes owners of low-mileage vehicles for an engineered factory defect.Read more

On June 20, 2026 while driving the Toyota Prius I experienced a sudden brake failure. Initially I thought I might have pressed the accelerator instead of the brake. But then pressing the brake I found the car did not stop and the ABS brake, brake lights, and other lights lit up on the dash board.

The car was very slow to stop- I had to slam on the breaks again in the parking lot. At this point, I was very shaken up and so was my child asking if the car was safe. The shifting wheel was very hot. It wasn’t a sunny day and none of the other parts of the car was hot. At this point I was concerned about the electrical workings of the car and slowly drove back home which was just across Main street and across Channel. I then called Jeep and reported the problem. I also looked online to see if anyone had a similar experience and found a woman had a similar experience on 9/8/25. Road side assistance towed car to dealership. On 1/10/26 the same thing happened. Was backing out of driveway to do errands. Car slow to move when pushing accelerator pedal. Then accelerates very fast without much pressure on accelerator. Very slow to break had to slam on breaks to stop. In reverse, car started rolling forward first. Put back on charger and it seemed to reset everything and car worked normally. I was driving my 2001 Dodge Ram 20-30mph and experienced a total power loss, power steering, power brakes and clock reset. My husband was also in the pickup and can attest to issue. I am an older person so had a very hard time stopping the vehicle and attempting to steer it to the side of the road.

I got home, had the vehicle towed to the dealership. I explained issue, requested a total diagnosis of everything that was wrong but most importantly, I wanted the total power loss issue diagnosed, pinpointed and resolved. because I don't feel safe driving until issue is truly diagnosed and fixed. I gave them my lifetimecarmaxplus warranty . The techs found a list of parts that needed to be fixed besides the power loss problem, but told me they couldn't fix the total power loss because no codes had been thrown. I explained that since there was a total power loss it was reported by other Dodge techs that no codes could be retrieved. I asked the service manager to ask his tech what diagnostics were used and to what extent to determine the problem. He said he would call me after talking to his tech but never did. I went to the dealership 2 weeks later and asked what he found out from his tech. He was confused and apparently never did anything more about the situation. I called Chrysler warranty, they gave me a case number but again seemed oblivious to the total power loss and fixing this major safety issue. No one has attempted to address the problem or offered to have another mechanic inspect the vehicle. There were no codes that I saw on the dash and I did try to immediately retrieve any but none came up. The front passenger window locks also had been working intermittently 2 mos prior. This recall follows multiple previous recalls and investigations involving fire risk or electrical system failures on the Wrangler 4xe, including earlier campaigns related to the hybrid battery and software updates. The repeated safety issues raise concerns about the overall safety and reliability of this model. My vehicle is otherwise in excellent condition, but I can no longer use it as intended as a plug-in hybrid or safely park it in my garage. Jeep has not offered a timely fix, loaner vehicle, or buyback. Mazda released Technical Service Bulletin TSB #SA-031-21, which includes a redesigned cylinder head for the 2018–2019 CX-5 models. This bulletin demonstrates Mazda’s awareness of the design flaw. The TSB only covers turbo 2.5L engines and not normally aspirated engines. There is substantial evidence that this is a widespread defect in these model years, unrelated to owner misuse or vehicle mileage. Shortly after pickup, the car experienced loss of acceleration on the interstate, a strong odor of fuel, and a sudden loss of power, creating a dangerous and life-threatening situation. The vehicle had to be towed back to the shop. The manager, who is not a certified mechanic, admitted he personally “fixed” the issue by tightening the fuel pump himself. A Mercedes-Benz diagnostic inspection later confirmed multiple severe assembly errors, including missing fasteners, unsecured oil cooler, disconnected vent lines, and other safety-related defects in the fuel and cooling systems. These unsafe and negligent repairs created an extreme fire hazard, engine instability, and risk of sudden failure while driving. I am requesting a formal NHTSA investigation into this matter to ensure vehicle safety compliance and to prevent similar incidents affecting other consumers.Read more

Dear NHTSA, I am writing to formally report a serious safety incident involving my 2016 Kia Optima Hybrid, which caught fire and was completely destroyed on October 5, 2025. Incident Summary: While driving on a highway at approximately 65 mph, I noticed that the vehicle was not entering Hybrid Mo
... de, which is abnormal for its typical operation. I reduced my speed to 45 mph, then down to 35 mph as I began to observe smoke coming from the vehicle. At no point did the vehicle show signs of overheating, and there was no Check Engine Light until shortly before I was able to safely pull over. Within moments of stopping the vehicle and exiting with my passenger, the vehicle was engulfed in flames and ultimately burned to the ground. Fortunately, both occupants escaped unharmed. The vehicle has been inspected by my insurance provider; however, I have been waiting over two weeks for Kia to inspect the vehicle or provide any official response regarding a possible cause. This delay is concerning, especially given the severity of the incident and the potential safety risk to other drivers. My vehicle had to be towed to a repair shop, where I was informed that this is a known issue related to the MDPS (Motor Driven Power Steering) & ECU (Electronic Control Unit). The technician noted that Kia has issued a Product Improvement Campaign for some 2017–2018 Kia Forte models equipped with the 2.0L engine due to this very defect. However, my vehicle’s VIN was not included in the campaign, & the dealer has refused to provide assistance, stating that it is not their responsibility regardless of any service bulletins or improvement campaigns issued. This is deeply concerning, as it shows a clear disconnect between known safety issues and the manufacturer's accountability. There are numerous reports from other consumers experiencing the exact same failure outside the limited VIN range, indicating this issue may be much more widespread than Kia is acknowledging. It is unacceptable that a potentially fatal defect is being selectively addressed, especially when the manufacturer is aware of the problem. How many more incidents or near-accidents need to occur before a full recall is issued? This is not just a matter of inconvenience—it's a critical safety issue. The fact that Kia has already acknowledged the defect by issuing a product improvement campaign proves the problem exists. It is the responsibility of both the manufacturer and its dealerships to protect consumers by ensuring that all affected vehicles are repaired, not just those within an arbitrarily limited VIN range.
