The car suddenly started to miss fire and lose power. This puts the driver and others in dangerous situations. If it happens when pulling out into traffic and the car misfired it could cause a serious crash. Honda service diagnosed that a blown head gasket causes coolant to leak into the cylinders,
... leading to engine misfires and a sudden loss of motive power at highway speeds. This issue is well documented on 2016 to 2022 honda Accords. There are many documented cases of this exact premature failure. Honda is aware of this issue and has awarded some owners goodwill repair when outside of warranty. There is currently ongoing litigation because of the 10s of thousands of head gasket failures. My car has 75k miles and went out of warranty at 60k miles. My goodwill request was denied by local regional Honda Rep but I elevated the request to corporate Honda and currently waiting on their response.Read more

The infotainment screen has ghost/phantom touches. So the touchscreen registers taps I didn't make. This occurs while I’m driving, make it impossible to have my GPS navigation, or my Apple car play. It even freezes the screen. I’ve replaced my infotainment screen and the same error occurred a ye

BRAKE SYSTEM ERROR AND TURBO AND HEAD GASKET NEED TO BE CHANGED. SO MAY COMPLAINTS ABOUT THIS...
Incident date: Jun 1, 2026
While driving at 50 mph, the infotainment touchscreen began experiencing severe ghost touches, where the screen acts as if it is being repeatedly tapped by itself. The system rapidly cycled through menus on its own, making random phone calls via Bluetooth and changing the navigation map coordinates
... while I was in motion. This created a massive visual and mental distraction as I try to mute or stop the erratic behavior. Also When putting the vehicle in reverse, the phantom touches freeze the screen, blocking the backup camera feed entirely. The problem is worse on hot days. The dealer stated it is a known hardware delamination issue but I was told honda corporate told them not to replace it as the issue will re-occur do to their hardware malfunctioning. This is an extreme safety hazard and a distraction while driving. I am submitting this complaint to get honda to resolve this issue.Read more

2022 Honda Accord Infotainment “Ghost Touch” Defect I am reporting a recurring safety defect involving the infotainment touchscreen in my 2022 Honda Accord. The touchscreen intermittently registers false inputs (“ghost touches”) without any user interaction. During operation, the screen ra
... ndomly changes radio stations, switches applications, exits navigation, opens menus, changes settings, and becomes unresponsive. The issue occurs while driving and forces the driver to divert attention from the roadway in an effort to regain control of the system. This defect creates a significant distraction and increases the risk of a crash, particularly when navigation is being used in unfamiliar areas. In some instances, the screen becomes completely unusable for extended periods. My understanding is that many other 2021–2022 Honda Accord owners have reported identical symptoms, including screen replacements that failed to permanently resolve the problem. Numerous public complaints describe the same malfunction and indicate that the issue may be widespread. I believe this defect warrants investigation because it affects a driver interface that is relied upon during vehicle operation. Honda has not provided a permanent repair solution, and affected owners continue to experience recurring failures across multiple Honda models and trims. I respectfully request that NHTSA investigate this defect and determine whether a recall or other corrective action is warranted. Vehicle: 2022 Honda AccordRead more

Incident date: May 25, 2023
Component Affected: Electrical System / Vehicle Speed Control / Forward Visibility / Navigation / Infotainment Description of Safety Hazard: I am experiencing a severe, highly unpredictable, and dangerous hardware defect involving the factory infotainment system on my 2022 Honda Accord Sport SE. The
... screen suffers from a hardware failure known as "ghost touching," where the digital interface registers rapid, erratic touch inputs completely on its own without being touched. This defect creates an immediate and severe driver distraction while the vehicle is in motion by automatically placing uncommanded phone calls to random contacts, changing settings, turning the radio on, and forcing sudden, loud audio spikes that startle the driver. Furthermore, these chaotic system glitches directly impact critical driving, navigation, and safety systems: GPS Navigation Failure: The ghost touches actively disrupt the GPS navigation system. The screen will randomly force the GPS map on when not in use, or it will glitch, freeze, and misroute while I am actively trying to navigate heavy traffic. This renders the primary navigation entirely useless, leaving me stranded and forcing my attention away from the road to attempt a manual override. Cruise Control Dropout & Auditory Panic Hazard: The system intermittently triggers a critical error on the dashboard that completely disables the vehicle’s Cruise Control / Adaptive Cruise Control (ACC) systems at highway speeds. When this error flashes, the vehicle emits a loud, non-stop, continuous warning chime that refuses to turn off until the car's computer system randomly resets itself. This non-stop beeping creates an intense visual and auditory panic situation while driving at high speeds, completely shattering driver focus.Read more

Also I quick Google search of "2022 honda accord sport screen glitching" brings up tons of other people dealing with this same issue and there is apparently no fix for it. I had brought my Honda Accord into Honda of Muskogee to have the touchscreen looked at for ghost touching while the car was still under warranty. On the very first visit, the morning I brought it in the temperature was low and it was nice outside and they could not reproduce the issue. They also did
... not complete the oil change properly on my car, nor rotated the tires, they also mangled the nuts that hold the positive battery post down. This was on a brand new car when it happened. Fast forward to 2026, my manufacturer's warranty is expired. I have contacted Honda Corporate probably 3 times to see if they would honor the warranty on my factory unmodified radio that came with the car. I advised them I wanted any other dealership that is certified to do the work, besides Muskogee. That location has a vendetta against me now because I am reporting them for stealing people's money and not doing the work their customers paid for. Their customers are ASKING for work that they pretend they have no idea about. PROBLEM: The digitizer is failing due to bad parts/delamination of some sort, it causes frequent ghost touching which can disrupt important safety information on the screen such as navigation data on a fast highway. It also ghost touches and turns off the HVAC system so the microphone can hear you, in the summertime this is incredibly dangerous as you cannot bypass it to ensure you stay cool in a heat wave if the radio keeps doing this. I am afraid this distraction is either going to cause myself, or anyone else with a 2022 Honda Accord that - for whatever reason they have NOT issued a manufacturers' recall for defective digitizers. This radio has done this practically the entire time I have owned the car, I am so upset with it.Read more

The dealer stated that the repair requires an IPMA software/module update and calibration. The quoted repair cost was approximately $1,100. This appears to be a software/programming-related condition affecting a driver assistance and brake-related safety feature, not normal wear and tear or a failed mechanical component. The warning is intermittent, but it has appeared multiple times. The vehicle is available for inspection upon request. No crash, injury, or fire has occurred. I am writing to express my concern with how Ford is handling Safety Recall 25S89. I have had numerous incidents where the camera has failed when backing up and have photos to document these instances. I brought my 2017 Edge to the dealership today and was told that it did not need replacement. So

... g up. The vehicle was inspected by a Ford dealership. The dealership confirmed DTC U3000:49 in the IPMA module and identified Ford TSB 23-2040 as applicable to the concern. Ford Customer Support was contacted and case number XXX was opened. Ford advised that no active warranty, recall, or customer satisfaction program currently provides repair cost coverage, and that because this involves Reverse Brake Assist, I may choose to file a safety concern report with NHTSA. Inadvertent Automatic Emergency Braking
On February 21, 2022, the Office of Defects Investigation (ODI) opened Preliminary Evaluation (PE22003) to assess reports of inadvertent activation of the Collision Mitigation Braking System (CMBS), an automatic emergency braking (AEB) system, in model year 2017-2019 Honda CR-V and 2018-2019 Honda A

Honda indicated that they were aware of a total of X,XXX reports that may relate to the alleged defect. Honda provided analysis of the alleged defect and alleges that some customers possibly had an inadequate understanding of the CMBS and its limitations. However, many consumer complaints allege that Honda dealerships were unable to reproduce the condition or state that they were informed that this is considered normal CMBS operation. To date, ODI has received a total of 1,294 consumer complaints of inadvertent activation of CMBS in 2017-2022 Honda CR-V and 2018-2022 Honda Accord vehicles. A total of 31 complaints alleged a crash and 50 alleged an injury. The Total column in the Failure Report Summary removes duplicate reports and shows the total number of reports with unique VINs from all Manufacturer, ODI, and EWR data sources. In some cases, there were multiple reports associated with a particular vehicle in which recurring failures were alleged. In total, there were X,XXX reports, 93 injury incidents and 47 crashes involving vehicles with unique VINs that may relate to the alleged defect. PE22-003 has been upgraded to an Engineering Analysis to further assess the scope, frequency, and potential safety related consequences of the inadvertent AEB activations. Front Seat Belt Assemblies may be Missing the Rivet that Secures the Quick Connector and Wire Plate
On April 4, 2024, the Office of Defects Investigations (ODI) opened RQ24006 to investigate American Honda Motor Co.’s (Honda’s) proposed remedy for recall 23V-782 as well as scope of the vehicles involved. Honda issued recall 23V-782 on November 21, 2023, which concerns certain model year (MY) 2
... 023-2024 Honda Accord, Honda Accord Hybrid, and Honda HR-V vehicles. Recall 23V-782 states that the driver and front passenger seat belt pretensioner assemblies may have been manufactured without a rivet that secures the pretensioner quick connector and wire plate. Recall documentation states that seat belt pretensioner assemblies that are missing the rivet will not properly restrain the occupant in the event of a collision, increasing the risk of injury.Subject defective seat belt assemblies are installed as original equipment for both the driver and front passenger positions for the HR-V, but only for the driver position for the Accord and Accord Hybrid. The recall remedy for 23V-782 instructs the technician to inspect both front seat belt pretensioner assemblies and, if necessary, replace them. The inspection procedure requires the technician to utilize a pull force gauge to determine whether the subject component is missing the subject rivet and requires replacement. Parts requiring replacement are determined by whether the pretensioner quick connector separates from the pretensioner assembly while the technician is pulling on the seat belt. If no separation occurs, then the part is determined to be not defective and not require replacement. There have been no reports alleging a failure of seat belt pretensioner assemblies after the inspect and replace remedy for recall 23V-782 was performed. Additionally, no comparable failures have been observed in field data for similar vehicles outside of the scope of the recall. As of the 4th quarter of 2024, the recall completion rate is at approximately 84% of the total recalled population. In view of the lack of failures after the remedy for recall 23V-782 was performed, and the lack of failures in similar vehicles outside of the scope of the recall, ODI is closing this Recall Query (RQ). The Agency reserves the right to take additional action if warranted by future circumstances.Read more

Loss of Direction Control
On May 6, 2021, the Office of Defects Investigation (ODI) opened Engineering Analysis (EA21-001) to assess a sudden loss of steering control, in Model Year (MY) 2013-2015 Honda Accord vehicles. Consumers alleged while driving the steering wheel would pull to the left or the right independently. Stee
... ring pull may pose a risk of loss of directional control. Honda stated that they believed the inadvertent steering input was caused by a loose magnet located within the electronic power steering (EPS) system. Due to poor formulation of the adhesive, this magnet can become loose and move up to 2 degrees around the steering column. When the magnet moves, the torque sensor generates a signal that results in unwanted steering in either direction. Honda stated the steering force required to maintain directional control with a loose magnet is a maximum of 3-pound feet. The maximum force of the unwanted steering input can be overcome by the driver and is similar to driving on grooved pavement. The Vehicle Research Test Center (VRTC) sent over 4,000 questionnaires to 2013-2015 Honda Accord vehicle owners. Of the 637 responses, 58 consumers experienced unwanted steering. Consumers experiencing unwanted steering did not report difficulties maintaining directional control. There were no crashes or injuries reported to VRTC. Testing done by VRTC was not able to reproduce the allegations. Analysis of all relevant data indicates that the subject condition has a relatively low failure rate. The poor adhesive formulation was addressed by the supplier with three (3) counter measures in the manufacturing process. There has been a declining trend in consumer complaints to NHTSA which indicates possible exhaustion of parts prone to failure. In view of the low rate, declining complaint trend, the nature of failure, and resolution of the adhesive formulation, this Engineering Analysis is closed. The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist. The agency reserves the right to take additional action if warranted by new circumstances. The ODI complaints cited above can be viewed at www.NHTSA.gov under the following ODI identification numbers: 11538714, 11528538, 11528008, 11522953, 11496557, 11495837, 11494328, 11478680, 11477167, 11476758, 11476263, 11471198, 11470001, 11467026, 11466561, 11466016, 11465626, 11464143, 11463558, 11462759, 11461500, 11460777, 11460191, 11455540, 11454334, 11452198, 11448114, 11447666, 11444888, 11444823, 11444172, 11443324, 11443243, 11443105, 11441374, 11439520, 11438345, 11437261, 11436824, 11436350, 11435994, 11435079, 11434969, 11434545, 11434442, 11434381, 11433824, 11433745, 11433555, 11433368, 11433176, 11431206, 11430465, 11430253, 11430009, 11429253, 11428301, 11427449, 11426841, 11426340, 11425928, 11425261, 11425230, 11423611, 11423245, 11422445, 11422395, 11422351, 11422220, 11422138, 11422069, 11421683, 11421589, 11421540, 11420573, 11419338, 11419270, 11418147, 11418143, 11417768, 11417742, 11417570, 11417437, 11416804, 11416699, 11416401, 11416378, 11416375, 11416155, 11416117, 11416084, 11416051, 11416032, 11416028, 11416024, 11416000, 11415933, 11415915, 11414105, 11406220, 11398736 ,11387075, 11387049, 11375580, 11364698, 11361125, 11351660, 11349961, 11327909, 11327170, 11301957, 11289067, 11282525, 11277659, 11258017, 11256914, 11218752, 11208077, 11196050, 11194358, 11189901, 11165075, 11163481, 11154891Read more

SRS Control Module Failure
As discussed further below, and by letter dated January 22, 2016, Honda is recalling certain model year (MY) 2008 through 2010 Honda Accords to replace a defective air bag/SRS control module that can result in the Supplemental Restraint System (SRS) becoming fully inoperative (disabled).See NHTSA Sa
... fety Recall 16V056 at SaferCar.gov for further details.Honda's response to an agency Information Request letter indicated that the SRS module was manufactured by tier-one supplier Continental and contained a power supply ASIC manufactured by tier-two supplier Atmel. The original ASIC was susceptible to a moisture ingress issue due to inadequate ASIC passivation layer.Water ingress could result in moisture reaching the Titanium Nitride (TiN) layer in the ASIC causing corrosion and breakage of electrical circuits. The damaged ASIC then causes failures of the SRS module resulting in 1) incomplete boot-up when power is supplied to the module or 2) the inability of the module to establish either internal or external bus communications. In either case the air bag readiness indicator light on the instrument cluster is illuminated and all air bags and seat belt pretensioners are disabled.During the investigation, ODI obtained the SRS module from a complainant crash vehicle where the air bags did not deploy despite a significant frontal impact. An inspection of the module was performed at a Continental facility. The inspection confirmed that the module had the ASIC fault thus preventing the SRS module from completing the boot-up process. An attempt was made to recover stored EDR data from the module however none was present; this outcome is consistent with an Atmel ASIC failure.A second SRS module alleged to have the ASIC problem was inspected during the same visit. This second module was unable to communicate with the test equipment. The ASIC was replaced on this module and full function was restored to the module confirming the ASIC as the cause of the SRS module fault.According to Honda, tier-two Atmel implemented three different corrective actions in early 2008.Implementation of the corrective actions into module production by tier-one Continental, and into vehicle production by Honda resulted in three distinct vehicle populations. The first population consists of certain non-consecutively built MY 2008 to 2010 Accords with modules that did not contain any of the corrective actions. This population of 341,444 vehicles exhibited a significant level of module failure in service and is thus being recalled. The second population of 382,693 non-consecutively built MY 2008 to 2010 Accords contained two of the three corrective actions. This population exhibited a significantly lower level of SRS module failure, compared to the first population, but a marginally higher level compared to the third population.Honda is conducting a 15 year unlimited mileage extended warranty program for this second population.Under the program Honda will replace the SRS module free of charge for vehicles that experience an SRS module malfunction due to ASIC failure.Owners eligible for the extended warranty will be notified by Honda in writing. The third population consists of vehicles containing all three corrective actions, this population exhibits a normal level of SRS module failure.Based on the safety recall and extended warranty this investigation is closed. The VOQ reports cited above can be reviewed at SaferCar.gov under the following identification numbers: 10871612, 10869953, 10810554, 10788238, 10781994, 10778712, 10765025, 10761583, 10759019, 10758622, 10747850, 10735806, 10734761, 10733519, 10715339, 10703521, 10676517, 1065Read more

Electric Power Steering Failure
The Office of Defects Investigation (ODI) opened PE14-033 to investigate 24 complaints alleging loss of power steering while driving in model year (MY) 2013 Honda Accord vehicles equipped with electric power steering (EPS).Analysis of service data for the original 24 complaints and 33 additional com
... plaints received after PE14-033 was opened found that 85 percent of the vehicles with diagnostic trouble codes available to identify the faulty component had a code indicating torque sensor failure (DTC 53).In its response to ODI's information request letter for PE14-033, Honda identified two manufacturing process conditions that may have affected the quality of some early production torque sensors.Analysis of warranty returns and manufacturing processes determined that a relatively small number of torque sensors were potentially affected by the conditions, which were corrected by the supplier (Bourns) relatively early in production.Analysis of warranty data indicates that both conditions were early-life failure concerns and most of the affected sensors have already failed, resulting in a cumulative failure rate of less than 0.2 percent.Of the 20 VOQ's that appear to be related to torque sensor failure, 6 reported failure dates in 2013, 12 in 2014, and just 2 to date in 2015.ODI's analysis identified 2 minor crashes with no injuries that may have been related to torque sensor failure in the subject vehicles. This investigation is closed because the low failure rate and a declining trend indicate that failures are rare. The closing of this preliminary evaluation does not constitute a determination that no defect exists or that power steering failures do not present an unreasonable risk to safety. Inadvertent Side Air Bag Deployment
ODI has identified 293 incidents of alleged inadvertent deployment of the side air bags in model year (MY) 2008 Honda Accord 4-door vehicles. This total represents all incidents reported to both ODI and Honda including those from Honda's warranty system (note: the total shown above eliminates all dup
... licative reports to ODI and Honda). This investigation has revealed that when the driver or front passenger door is shut, the driver-side or passenger-side side curtain air bag can deploy, and in some cases, the side seat-mounted torso air bag can deploy as well.Fourteen people allegedly suffered injuries caused by the deploying air bags.In June 2008, near the end of MY 2008 Accord 4-door production, Honda changed the crash parameter for door closing force, which is embedded in the electronic control unit's software code, in order to reduce the incidents of the inadvertent side air bag deployments.More specifically, the software design change was made on June 5, 2008 for the Accords built in the U.S. and on June 30, 2008 for those built in Japan.ODI found the number of the inadvertent deployment incidents declined significantly for the vehicles with the new crash parameter setting, i.e., the late-built MY 2008 vehicles and all MY 2009 vehicles.Honda reported the Accord 2-door model utilizes a different software/crash parameter than the subject 4-door model. This Preliminary Evaluation has been upgraded to an Engineering Analysis (EA14-004) to determine, among other things, the risk of air bag deployment injuries to vehicle users in vulnerable positions. The ODI reports cited above can be reviewed online at http://www-odi.nhtsa.dot.gov/owners/SearchNHTSAID under the following identification numbers: 10232206, 10235294, 10255323, 10268158, 10268191, 10269514, 10276755, 10279446, 10280675, 10308292, 10319979, 10342520, 10353536, 10408925, 10436506, 10447151, 10458732, 10468196, 10468200, 10493407, 10494971, 10522291, 10524389, 10543092, 10544473, 10545258, 10547591, 10554073, 10563629, 10565311, 10565423, 10576518, 10579241, 10584501, 10585442, 10606937, 10608973.Read more

Braking Performance
The Office of Defects Investigation (ODI) received a defect petition requesting that it open an investigation and recall MY 2005 Honda Accord Hybrid vehicles for a possible safety defect related to braking performance. The petitioner in a defect petition letter and in a complaint filed with ODI under
... Vehicle Owner Questionnaire (VOQ) 10329383 alleging ineffective braking performance of the Honda Accord Hybrid vehicle while driving over rumble strips along side of the highway and simultaneously attempting to the brake. The petitioner alleges this began a chain of events, where the vehicle accelerated uncontrollably, the passenger attempted to stop the vehicle by pulling the parking brake and eventually the vehicle crossing over into traffic flowing in the opposite direction. The result was a crash fatally injuring the passenger and severely injuring the petitioner and two occupants of another vehicle. During the defect petition review, ODI analyzed complaints involving similarly equipped Honda Accord Hybrid vehicles and first and second generation Honda Civic Hybrid vehicles, reviewed data provided to ODI by Honda in response to an information request letter and performed testing of braking performance of a 2005 Honda Accord Hybrid vehicle at NHTSA's Vehicle Research and Test Center in East Liberty, Ohio.Based on this review, ODI does not believe additional investigation is likely to result in a finding that a defect related to motor vehicle safety exists. Therefore, in view of the need to allocate and prioritize NHTSA?s limited resources to best accomplish the agency's safety mission, the petition is denied. This action does not constitute a finding by NHTSA that a safety-related defect does not exist. The agency will take further action if warranted by future circumstances. For more information, see the Federal Register notice in the DP10-004 file (www-odi.nhtsa.dot.gov/defects).Read more

Ignition Shift Interlock
On August 4, 2010, American Honda Motor Co. (Honda) submitted a Defect Information Report (DIR) to NHTSA regarding a ignition interlock defect in approximately 384,220 model year (MY) 2003 Honda Accord and Civic and MY 2003 and 2004 Honda Element vehicles with automatic transmissions (Recall 10V-364
... ).In a September 22, 2010 supplemental DIR, Honda expanded the scope of the recall population based on manufacturing and complaint data. The total population for the complete recall is approximately 551,498 vehicles. The information listed in this closing report refers only to the subject vehicles, MY 2003 Accords with automatic transmissions.According to Honda's DIR, Under certain conditions the interlock lever of the ignition switch may unexpectedly deform, which can allow the interlock function of a vehicle with an automatic transmission to be defeated if the driver does not follow the typical shut-down procedure of shifting the gear selector to the Park position before rotating the ignition key to the off position. Removal of the ignition key when the gear selector of a vehicle with an automatic transmission has not been shifted to the Park position can allow the vehicle to roll away, increasing the risk of a crash.Honda's dealers will be instructed to inspect the ignition interlock switch and, if necessary, replace the original interlock pin and lever within the ignition switch and replace them with new, redesigned parts. This investigation is closed.Read more

ODI HAS RECEIVED 16 COMPLAINTS ALLEGING FAILURES OF THE IGNITION PARK-SHIFT INTERLOCK THAT ALLOWED THE IGNITION KEY TO BE REMOVED FROM THE IGNITION SWITCH WITHOUT THE TRANSMISSION BEING SHIFTED TO PARK IN MODEL YEAR (MY) 2002 AND 2003 HONDA ACCORD VEHICLES WITH AUTOMATIC TRANSMISSIONS.ELEVEN OF THE
... COMPLAINTS ALLEGE THAT THE FAILURE OF THE INTERLOCK RESULTED IN A ROLLAWAY CRASH.ONE OWNER ALLEGED THAT SHE WAS ALMOST INTO HER HOUSE WHEN SHE NOTICED THE CAR ROLLING OUT OF THE GARAGE AND DOWN THE DRIVEWAY AND THAT WHEN SHE TRIED TO STOP THE CAR IT ALMOST RAN OVER HER, BEFORE CRASHING INTO A CABLE BOX AT THE END OF THE DRIVEWAY.ANOTHER OWNER ALLEGED THAT THE VEHICLE ROLLED OUT OF THE DRIVEWAY AND STRUCK A TREE ACROSS THE STREET.THERE HAVE BEEN 28 COMPLAINTS MADE TO HONDA REGARDING IGNITION PARK-SHIFT INTERLOCK FAILURES IN THE SUBJECT VEHICLES, 10 OF WHICH ALLEGE ROLLAWAY CRASHES. IN OCTOBER 2003, AMERICAN HONDA MOTOR COMPANY INITIATED RECALL 03V-423 TO CORRECT AN IGNITION PARK-SHIFT INTERLOCK DEFECT IN CERTAIN MY 1999 HONDA ODYSSEY, MY 1997-99 ACURA CL, MY 1999 ACURA TL, AND MY 1998-99 HONDA ACCORD VEHICLES.IN JANUARY 2005, HONDA INITIATED ANOTHER RECALL TO ADDRESS DEFECTIVE IGNITION PARK-SHIFT INTERLOCKS (RECALL 05V-025), THIS TIME COVERING MY 1997-2001 HONDA PRELUDE, MY 1999-2000 ACURA TL, AND CERTAIN MY 1999-2002 HONDA ACCORD VEHICLES.NOT ALL MY 2002 HONDA ACCORDS WERE COVERED UNDER RECALL 05V-025.THE REMAINING MY 2002 AND ALL MY 2003 ACCORD VEHICLES ARE COVERED IN THE SCOPE OF THIS RECALL QUERY. THIS INVESTIGATION HAS BEEN UPGRADED TO AN ENGINEERING ANALYSIS(EA09-001) TO CONTINUE TO ASSESS THE SCOPE AND FREQUENCY OF THE ALLEGED DEFECT.Read more

SRS READINESS INDICATOR ILLUMINATION
THE SUBJECT VEHICLES ARE DESIGNED WITH AN OCCUPANT POSITION DETECTION SYSTEM (OPDS) FOR THE PASSENGER SEAT TO SUPPRESS THE SIDE AIRBAG IN THE EVENT OF AN OUT-OF-POSITION OCCUPANT.ON SOME VEHICLES, THE OPDS UNIT AND SENSOR GRID MAY BE SUSCEPTIBLE TO HIGH MOISTURE CONTENT AND/OR TO SPORADIC ELECTRICAL
... INTERFERENCE THAT WOULD RESULT IN THE ILLUMINATION OF THE SRS READINESS INDICATOR.IN THIS CONDITION, OPDS SUPPRESSION WILL NOT FUNCTION, BUT THE SIDE AIRBAG WILL STILL DEPLOY IN THE EVENT OF A SIDE CRASH. HONDA ISSUED A TECHNICAL SERVICE BULLETIN IN APRIL 2004 TO ADDRESS THIS PROBLEM. HOWEVER, THE PROBLEM PERSISTS EVEN AFTER THE WARRANTY PERIOD EXPIRES.AS A RESULT, SOME OWNERS MAY NOT BE OBTAINING THE NECESSARY REMEDIAL SERVICE WORK.CONTINUED ILLUMINATION OF THE SRS INDICATOR CAN MASK THE DETECTION OF OTHER AIRBAG COMPONENT FAULTS IF THEY OCCUR. IN MARCH 2006, HONDA ISSUED A SERVICE BULLETIN (06-009) TO EXTEND THE WARRANTY ON SRS ILLUMINATION AND OPDS ISSUES TO 10 YEARS OR 150,000 MILES (WHICHEVER OCCURS FIRST).HONDA WILL SEND NOTIFICATION LETTERS TO OWNERS OF THE SUBJECT VEHICLES AND WILL PROVIDE A NO COST REPLACEMENT OF THE OPDS UNIT AS NEEDED.IN ADDITION, OTHER MY 1999-04 HONDA AND ACURA VEHICLES ARE ALSO COVERED BY THE BULLETIN.THIS ACTION TAKEN BY THE MANUFACTURER IS SUFFICIENT TO RESOLVE THE ISSUE RAISED BY THIS INVESTIGATION.THE CLOSING OF THIS INVESTIGATION DOES NOT CONSTITUTE A FINDING BY NHTSA THAT NO SAFETY-RELATED DEFECT EXISTS.THE AGENCY RESERVES THE RIGHT TO TAKE FURTHER ACTION IF WARRANTED BY THE CIRCUMSTANCES.Read more

THE OFFICE OF DEFECTS INVESTIGATION (ODI) PRODUCED AN INFORMATION REQUEST (IR) LETTER DATED OCTOBER 14, 2004 WHICH DEFINED THE SUBJECT VEHICLES AS MODEL YEAR 1999 - 2001 HONDA ACCORD VEHICLES WHICH WERE NOT WITHIN THE SCOPE OF PRIOR RECALL ACTION (03V423) HONDA CONDUCTED TO RESOLVE AN INTERLOCK DEFE
... CT.HONDA SUBMITTED RESPONSES ON NOVEMBER 29 AND DECEMBER 21, 2004.THE FAILURE REPORTS NOTED ABOVE HAVE BEEN DETERMINED FROM ODI'S ANALYSIS OF NHTSA'S COMPLAINT DATABASE AND AN INITIAL ANALYSIS OF THE MANUFACTURER'S IR RESPONSE DATA (FURTHER ANALYSIS REQUIRED).SOME OF THE HONDA REPORTS ARE DUPLICATIVE OF ODI.TAKING INTO ACCOUNT A SHORTER SERVICE LIFE, THE FAILURE INCIDENCE OF THE SUBJECT VEHICLES IS SIMILAR TO THAT DETERMINED WHEN ODI CONDUCTED PRELIMINARY EVALUATION (PE) 03-013, WHICH INVOLVED INTERLOCK FAILURES ON OLDER ACCORDS SUBSEQUENTLY RECALLED UNDER THE 03V423 ACTION.ACCORDINGLY, THE PETITON IS GRANTED AND ODI HAS OPENED PE05-002 TO FURTHER INVESTIGATE THE MATTER.Read more

FRONTAL DRIVER AIR BAG
DURING TWO OF 13 NHTSA RESEARCH & DEVELOPMENT COMPATIBILITY CRASH TESTS, THE DRIVER-SIDE FRONTAL AIRBAG FAILED TO PROPERLY DEPLOY DUE TO A LARGE TEAR IN THE AIRBAG FABRIC.BOTH R&D TEST VEHICLES THAT CONTAINED A FAILED AIR BAG WERE 4-DOOR, 4-CYLINDER (L4) VEHICLES.SUBSEQUENT STATIC AND DYNAMIC TESTS
... CONDUCTED BY HONDA REVEALED THAT DURING SEVERE CRASH CONDITIONS, THE AIRBAG FABRIC CAN SNAG THE INSIDE SURFACE OF THE AIRBAG MODULE COVER AT THE EMBLEM ATTACHMENT POINTS.HONDA REPORTS THAT THE AIR BAG MODULE AND COVER DESIGN ARE UNIQUE TO THE 4-DOOR, 4-CYLINDER SUBJECT VEHICLES.ON NOVEMBER 17, 2004, HONDA NOTIFIED THE AGENCY THAT IT WOULD RECALL 228,932 MODEL YEAR (MY) 2004 AND 28,428 EARLY-MY 2005 ACCORD L4 SEDANS (UP THROUGH EARLY-OCTOBER, 2004).AS PART OF THE RECALL REMEDY, HONDA WILL INSTALL AN ADDITIONAL PIECE OF PROTECTIVE FABRIC/FLAP BETWEEN THE AIRBAG AND THE INSIDE AIRBAG MODULE COVER.SEE NHTSA RECALL 04V551 FOR DETAILS.IN ADDITION, THERE ARE APPROXIMATELY 250 EXPORTED ACCORD VEHICLES INCLUDED IN THIS RECALL. THE V-6 MODELS AND THE 2-DOOR, 4-CYLINDER MODELS ARE NOT INCLUDED BASED ON DIFFERENT AIR BAG AND AIR BAG MODULE COVER DESIGNS.THIS INVESTIGATION IS CLOSED.Read more

IGNITION-PARK INTERLOCK
SIX OF THE ODI COMPLAINTS ARE DUPLICATIVE OF HONDA COMPLAINTS, AS WELL AS FOUR OF THE CRASHES.THE ONE INJURY OCCURRED WHEN A ROLLAWAY VEHICLE STRUCK THE EXITING OPERATOR WHO HAD INADVERTENTLY PARKED THE VEHICLE IN AN UNSECURED STATE (NOT IN PARK, BRAKE NOT ENGAGED).HONDA IDENTIFIED THE WARRANTY CLAI
... MS FROM COMPLAINT DESCRIPTIONS THAT CONTENDED THE INTERLOCK SYSTEM FAILED TO OPERATE PROPERLY.HONDA PRESENTED THE RESULTS OF THEIR INVESTIGATION INTO INTERLOCK SYSTEM FAILURES AT AN OCTOBER 16, 2003 MEETING WITH ODI IN WHICH HONDA IDENTIFIED A SUPPLIER AND AN ENGINEERING SPECIFICATION CHANGE FOR THE IGNITION CYLINDER BODY (CB), A SUBCOMPONENT OF THE IGNITION LOCK ASSEMBLY.RESULTS FROM HONDA?S ANALYSIS DEMONSTRATED A STRONG CORRELATION BETWEEN THE PRODUCTION DATE RANGE OF VEHICLES THAT EXPERIENCED HIGHER INTERLOCK WARRANTY CLAIM RATES AND THE IMPLEMENTATION DATES OF THE CHANGES.AN ENGINEERING SPECIFICATION FOR A SURFACE TREATMENT PROCESS FOR THE CB WAS ELIMINATED AT THE BEGINNING OF MY 1998.THE SUPPLIER OF THE CB WAS CHANGED IN FEBRUARY 1999.VEHICLES BUILT WITHIN THIS TIME PERIOD CONTAIN A CB WITH POOR SURFACE HARDNESS THAT CAN WEAR PREMATURELY.AS THE WEAR PROGRESSES, THE INTERLOCK SYSTEM BECOMES INCAPABLE OF PREVENTING KEY REMOVAL WHEN THE SHIFTER IS IN A NON-PARK POSITION.INTERLOCK FAILURE INCREASES THE RISK OF A VEHICLE ROLLAWAY AND SUBSEQUENT CRASH OR INJURY WHEN VEHICLES ARE PARKED UNSECURED.HONDA ACCORD VEHICLES SUBJECT TO THIS CHANGE INCLUDE MY 1998 AND MY 1999 THROUGH FEBRUARY 1999 PRODUCTION.VEHICLES PRODUCED AFTER THIS DATE ARE NOT AFFECTED BY THIS CONDITION AND THUS ARE NOT CONSIDERED AT RISK FOR FAILURE.ODI NOTES THAT 12 OF THE 13 VOQ REPORTS FALL WITHIN THIS PRODUCTION RANGE (THE 13TH DOES NOT HAVE A VIN AND CANNOT BE VERIFIED).THE ABOVE POPULATION REPRESENTS THE NUMBER OF THE ACCORD VEHICLES TO BE RECALLED.HONDA ALSO REPORTED THAT CERTAIN ODYSSEY AND ACURA CL AND TL MODELS WERE AFFECTED AND WOULD ALSO BE RECALLED.A TOTAL OF 570,120 VEHICLES WILL BE RECALLED.UNDER RECALL 03V-423, HONDA WILL INSPECT VEHICLES AND REPLACE COMPONENTS (INCLUDING THE CB) ON VEHICLES WITH EXCESSIVE WEAR.VEHICLES NOT SHOWING WEAR WILL HAVE A REDESIGNED PART INSTALLED WHICH PREVENTS WEAR.Read more

IGNITION-PARK INTERLOCK SYSTEM
SIX OF THE ODI COMPLAINTS ARE DUPLICATIVE OF THE MANUFACTURER COMPLAINTS, AS WELL AS FOUR OF THE CRASHES.THE NOTED INJURY OCCURRED WHEN A ROLLAWAY VEHICLE THAT HAD BEEN INADVERTENTLY PARKED IN NEUTRAL STRUCK THE EXITING OPERATOR.THE WARRANTY CLAIM COUNT SHOWN FOR OTHER REPRESENTS INCIDENTS WHERE HON
... DA'S ANALYSIS OF CLAIM DETAIL INDICATES THE INTERLOCK SYSTEM MALFUNCTIONED.THE INTERLOCK SYSTEM UTILIZES A LEVER TO BLOCK IGNITION KEY REMOVAL WHEN THE TRANSMISSION IS NOT IN PARK.HONDA HAS IDENTIFIED A WEAR CONDITION OF THE INTERLOCK LEVER THAT CAN CAUSE THE SYSTEM TO MALFUNCTION; WHEN WEAR OCCURS THE KEY CAN BE REMOVED FROM THE IGNITION WHILE THE SHIFTER IS IN A NON-PARK POSITION.HONDA DOCUMENTS INDICATE THAT A MANUFACTURING PROCESS MAY BE THE CAUSE OF INTERLOCK LEVER WEAR, AND THAT THIS PROCESS WAS REVISED IN JANUARY OF 2001.AN ENGINEERING ANALYSIS WILL BE OPENED TO FURTHER INVESTIGATE THIS CONCERN AND ITS SCOPE.Read more

SEAT FRAME BRACKET FAILURE
THIS PE WAS OPENED BASED ON 10 CONSUMER COMPLAINTS IN THE ODI DATABASE ALLEGING THAT THE POWER ASSISTED SEATS WERE FAILING IN CERTAIN 1998-2002 HONDA ACCORD VEHICLES.FAILURES ARE CONCENTRATED IN THE 1998-99 MODEL YEAR VEHICLES.THE BRACKET THAT ATTACHES THE SEAT BOTTOM TO THE SEAT BASE IS FRACTURING
... DUE TO: 1-THE WELD CONDITION OF BRACKET, 2-THE DRIVERS WEIGHT, AND 3-THE IMPACT ON THE SEAT WHEN THE DRIVER ENTERS THE VEHICLE AND SITS DOWN.OVER TIME THE BRACKET THAT ATTACHES THE SEAT MAY FAIL AT THE LEFT FRONT CORNER OF THE SEAT BOTTOM.THE OTHER 3 POINTS OF ATTACHMENT OF THE SEAT BOTTOM TO THE SEAT BASE STAY ATTACHED AND THE SEAT MOVEMENT IS LIMITED.THE ATTACHMENT OF THE SEAT BOTTOM TO THE SEAT BACK DOES NOT FAIL.THE CONDITION IS MOST APPARENT DURING RAPID ACCELERATION CAUSING THE SEAT TO TILT REARWARD.ALTHOUGH A FEW COMPLAINTS REPORT CONCERN OVER LOSS OF VEHICLE CONTROL, NO LOSS OF CONTROL INCIDENTS, CRASHES OR INJURIES ARE REPORTED.IN RESPONSE TO CUSTOMER COMPLAINTS, HONDA WILL PROVIDE OWNERS OF 1998 AND SOME 1999 ACCORDS A NO-COST REINFORCEMENT FOR THE SEAT AT THE LEFT FRONT CORNER.A SAFETY-RELATED DEFECT HAS NOT BEEN IDENTIFIED AT THIS TIME AND FURTHER USE OF AGENCY RESOURCES DOES NOT APPEAR TO BE WARRANTED.ACCORDINGLY, THIS INVESTIGATION IS CLOSED.THE CLOSING OF THIS INVESTIGATION DOES NOT CONSTITUTE A FINDING BY NHTSA THAT A SAFETY-RELATED DEFECT DOES NOT EXIST.THE AGENCY WILL TAKE FURTHER ACTION IF WARRANTED BY THE CIRCUMSTANCES.Read more

SUDDEN ACCELERATION
SUMMARY OF INFORMATION FROM MANUFACTURER:INFORMATION FROM HONDA RELEVANTTO THIS PE WAS PROVIDED BY LETTER DATED 8 SEPTEMBER AND A PRESENTATION TONHTSA ON 2 OCTOBER.HONDA HAS ISSUED NO TECHNICAL SERVICE BULLETINS OROTHER NOTIFICATIONS TO DEALERS RELATING TO FUEL FLOW AND DELIVERY, THROTTLECONTROL, OR
... ENGINE MALFUNCTIONS THAT MAY RESULT IN UNINTENDED SUDDENACCELERATION.THE AFFECTED VEHICLES ARE EQUIPPED WITH A CARBURETOR DESIGNWHICH, ACCORDING TO HONDA'S EVALUATION, WILL NOT PERMIT ENGINE RPM TOEXCEED A FAST IDLE CONDITION OF 2000 RPM WITHOUT DRIVER ACTUATION OF THETHROTTLE. TEST DATA WERE PROVIDED TO SHOW THAT AN ENGINE SPEED OF 2000 RPMWITH THE TRANSMISSION IN THE PARK POSITION WOULD, WHEN SHIFTED INTODRIVE, YIELD A VEHICLE SPEED OF 8 MPH AFTER APPROXIMATELY 10 SECONDS.OTHER TEST DATA WERE PROVIDED TO SUPPORT HONDA'S POSITION THAT FAILEDENGINE MOUNTS WOULD NOT CONTRIBUTE TO SUDDEN VEHICLE ACCELERATION ASALLEGED IN THIS PE.ANALYSIS:THE FOLLOWING ARE CONSIDERED SIGNIFICANT OBSERVATIONS AND/ORANALYTICAL ASSESSMENTS OF THE FACTS IN THIS PE FILE: OTHE ALLEGED INCIDENTS ARE SIMILAR TO THOSE FROM OTHER PE/EA FILESIN THAT THEY TEND TO EMPHASIZE OCCURRENCE OF THE PHENOMENON WHENTHE TRANSMISSION IS SHIFTED FROM PARK INTO ANOTHER GEAR. OA HIGH PERCENTAGE OF THE REPORTED OCCURRENCES ALSO INVOLVEACCIDENTS,BUT THE RELATIVELY LOW PERCENTAGE OF INJURY ACCIDENTSSUGGESTS THAT MOST ARE LOW SPEED COLLISIONS. OHONDA HAS NOT IDENTIFIED, AND ODI'S ANALYSES HAVE NOT DISCOVERED,A TREND OF MECHANICAL OR ELECTRICAL COMPONENT FAILURE WITH ACAUSAL RELATIONSHIP TO THE ALLEGED OCCURRENCES.IN ADDITION TO THE ABOVE, ANALYSIS OF THE RATES OF ALLEGED INCIDENTS,ACCIDENTS, AND INJURY ACCIDENTS WITH RESPECT TO THE TOTAL VEHICLEPOPULATION OF 746,000, SHOWS THAT THE HONDA ACCORD COMPARES FAVORABLYTO OTHER VEHICLES REPORTED TO HAVE EXPERIENCED SIMILAR OCCURRENCES.THIS ANALYSIS, TOGETHER WITH OTHER INFORMATION ACQUIRED, SHOWS THAT NODEFECT TREND EXISTS TO JUSTIFY CONTINUATION OF INVESTIGATIVE ACTIVITY.PROPOSED ACTION:CLOSE THIS PRELIMINARY EVALUATION.===Read more
