The injectors passed an air-fuel test. The technician then noted pressure releasing from the cooling system despite low coolant temperature. A head-gasket leak test was performed and FAILED. Honda diagnosed a blown cylinder head gasket and recommended approximately $4,906 in repairs, including spark plugs that had passed inspection only days earlier. I am concerned about the sudden failure of a major engine component in a well-maintained vehicle, particularly so soon after a Honda dealership inspection. All lights on the dashboard are on, car overheats and goes into limp mode, sometimes not accelerating over 20 MPH. My car went into limp mode on the highway, this scared me, so I haven't drove it since. Ive seen countless comments of users with the same make and model, experiencing the same issue. There is a hole in the engine block and it happed while I was driving, people could’ve been hurt on the road because of this mechanical failure. Vehicle has only been inspected by a private mechanic who found the problem. There was no warning of this besides a add air to tire warning.

The vehicle attempted to correct the "road departure" by steering the vehicle away from the perceived problem and violently engaged the brakes nearly causing a collision as it has done numerous times before. In this particular event, it was an extraordinarily dangerous situation given the location that it occurred (190ft in the air on a walled in 2 lane interstate highway). I have reported this to NHTSA and American Honda on multiple previous occasions and await a repair or even an acknowledgement of this extremely dangerous issue. These issues have caused serious concerns about my safety and the safety of others because I cannot depend on the vehicle’s safety systems or know when another component may fail. The vehicle and affected components are available for inspection upon request. I have attempted to get these problems addressed, but I have not been able to get a satisfactory resolution. The issues have been identified/diagnosed, and I am seeking further inspection to determine the full extent of the problems. I have not had the vehicle inspected by the police or an insurance representative, but I am trying to get the manufacturer/dealer involved so these issues can be properly documented and addressed. I am especially concerned because I have only had this vehicle since January 16, 2026, and I have already put a significant amount of money into it while still owing a substantial amount on the loan. I was also informed that the vehicle could potentially be considered a total loss within the next couple of months. I do not believe it is reasonable for me to be left responsible for a vehicle that I recently purchased from CarMax and that has developed or revealed so many serious problems. ... placed. In September 2025, at 207,539 miles, my vehicle was serviced at Joe Morgan Honda, an authorized Honda dealership. During that visit, the Honda technician documented that the cooling system fluid level was low and that no external leaks were found. The technician documented that they suspected the head gasket was causing internal coolant loss over time. I was instructed to monitor the vehicle and was provided an estimate of approximately $4,400 plus tax for head gasket replacement. I specifically asked the Honda service advisor whether the head gasket could be repaired at that time or whether anything could be done to prevent the suspected problem from progressing. I was told there was nothing that could be done at that point other than monitor the vehicle. Within weeks, multiple warning lights illuminated on my dashboard and the vehicle began experiencing significant drivability problems, including hesitation, misfiring, and loss of acceleration. The loss of acceleration was particularly concerning because the vehicle did not respond or accelerate normally while being driven. I took the vehicle to Integrity Automotive on September 25, 2025, at 208,583 miles. The engine was checked, and the ignition coils and spark plugs were replaced. Despite those repairs, the vehicle ultimately required major engine repairs on November 8, 2025, at 210,230 miles. The cylinder head gasket was replaced and the cylinder head was serviced/repaired. The fuel injectors, spark plugs, and serpentine belt were also replaced, and the cooling system and drivability/performance were checked. I have relied on Honda to perform the recommended maintenance and address any problems with the vehicle. When all of these warning lights and messages appeared, I took the vehicle to the Honda dealership. The dealership adjusted the valves and told me that everything was fine. Approximately one week later, I had to take the vehicle back because the problem was not resolved. At that time, I was told that the vehicle needed a head gasket. The vehicle had also previously required a turbo replacement. Because of the seriousness of the head-gasket diagnosis, I obtained a second opinion from another repair facility, and they also determined that the head gasket was the problem. I do not understand how a vehicle that I purchased new and have consistently maintained through Honda could experience these significant engine-related problems. I am especially concerned that numerous unrelated warning lights and safety-system messages appeared simultaneously before the head-gasket problem was diagnosed. Inadvertent Automatic Emergency Braking
On February 21, 2022, the Office of Defects Investigation (ODI) opened Preliminary Evaluation (PE22003) to assess reports of inadvertent activation of the Collision Mitigation Braking System (CMBS), an automatic emergency braking (AEB) system, in model year 2017-2019 Honda CR-V and 2018-2019 Honda A
... ccord vehicles. The reports allege that activation of the AEB system occurs while driving with no apparent obstruction in the vehicle's forward path, resulting in sudden vehicle deceleration. Honda indicated that they were aware of a total of X,XXX reports that may relate to the alleged defect. Honda provided analysis of the alleged defect and alleges that some customers possibly had an inadequate understanding of the CMBS and its limitations. However, many consumer complaints allege that Honda dealerships were unable to reproduce the condition or state that they were informed that this is considered normal CMBS operation. To date, ODI has received a total of 1,294 consumer complaints of inadvertent activation of CMBS in 2017-2022 Honda CR-V and 2018-2022 Honda Accord vehicles. A total of 31 complaints alleged a crash and 50 alleged an injury. The Total column in the Failure Report Summary removes duplicate reports and shows the total number of reports with unique VINs from all Manufacturer, ODI, and EWR data sources. In some cases, there were multiple reports associated with a particular vehicle in which recurring failures were alleged. In total, there were X,XXX reports, 93 injury incidents and 47 crashes involving vehicles with unique VINs that may relate to the alleged defect. PE22-003 has been upgraded to an Engineering Analysis to further assess the scope, frequency, and potential safety related consequences of the inadvertent AEB activations. The scope has been expanded to include assessment of model year 2020-2022 Honda CR-V and Accord vehicles. Front Seat Belt Assemblies may be Missing the Rivet that Secures the Quick Connector and Wire Plate

... 023-2024 Honda Accord, Honda Accord Hybrid, and Honda HR-V vehicles. Recall 23V-782 states that the driver and front passenger seat belt pretensioner assemblies may have been manufactured without a rivet that secures the pretensioner quick connector and wire plate. Recall documentation states that seat belt pretensioner assemblies that are missing the rivet will not properly restrain the occupant in the event of a collision, increasing the risk of injury.Subject defective seat belt assemblies are installed as original equipment for both the driver and front passenger positions for the HR-V, but only for the driver position for the Accord and Accord Hybrid. The recall remedy for 23V-782 instructs the technician to inspect both front seat belt pretensioner assemblies and, if necessary, replace them. The inspection procedure requires the technician to utilize a pull force gauge to determine whether the subject component is missing the subject rivet and requires replacement. Parts requiring replacement are determined by whether the pretensioner quick connector separates from the pretensioner assembly while the technician is pulling on the seat belt. If no separation occurs, then the part is determined to be not defective and not require replacement. There have been no reports alleging a failure of seat belt pretensioner assemblies after the inspect and replace remedy for recall 23V-782 was performed. Additionally, no comparable failures have been observed in field data for similar vehicles outside of the scope of the recall. As of the 4th quarter of 2024, the recall completion rate is at approximately 84% of the total recalled population. In view of the lack of failures after the remedy for recall 23V-782 was performed, and the lack of failures in similar vehicles outside of the scope of the recall, ODI is closing this Recall Query (RQ). The Agency reserves the right to take additional action if warranted by future circumstances.Read more

Honda stated that they believed the inadvertent steering input was caused by a loose magnet located within the electronic power steering (EPS) system. Due to poor formulation of the adhesive, this magnet can become loose and move up to 2 degrees around the steering column. When the magnet moves, the torque sensor generates a signal that results in unwanted steering in either direction. Honda stated the steering force required to maintain directional control with a loose magnet is a maximum of 3-pound feet. The maximum force of the unwanted steering input can be overcome by the driver and is similar to driving on grooved pavement.The Vehicle Research Test Center (VRTC) sent over 4,000 questionnaires to 2013-2015 Honda Accord vehicle owners. Of the 637 responses, 58 consumers experienced unwanted steering. Consumers experiencing unwanted steering did not report difficulties maintaining directional control. There were no crashes or injuries reported to VRTC. Testing done by VRTC was not able to reproduce the allegations. Analysis of all relevant data indicates that the subject condition has a relatively low failure rate. The poor adhesive formulation was addressed by the supplier with three (3) counter measures in the manufacturing process.There has been a declining trend in consumer complaints to NHTSA which indicates possible exhaustion of parts prone to failure. In view of the low rate, declining complaint trend, the nature of failure, and resolution of the adhesive formulation, this Engineering Analysis is closed. The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist. The original ASIC was susceptible to a moisture ingress issue due to inadequate ASIC passivation layer.Water ingress could result in moisture reaching the Titanium Nitride (TiN) layer in the ASIC causing corrosion and breakage of electrical circuits.The damaged ASIC then causes failures of the SRS module resulting in 1) incomplete boot-up when power is supplied to the module or 2) the inability of the module to establish either internal or external bus communications.In either case the air bag readiness indicator light on the instrument cluster is illuminated and all air bags and seat belt pretensioners are disabled.During the investigation, ODI obtained the SRS module from a complainant crash vehicle where the air bags did not deploy despite a significant frontal impact.An inspection of the module was performed at a Continental facility.The inspection confirmed that the module had the ASIC fault thus preventing the SRS module from completing the boot-up process.An attempt was made to recover stored EDR data from the module however none was present; this outcome is consistent with an Atmel ASIC failure.A second SRS module alleged to have the ASIC problem was inspected during the same visit.This second module was unable to communicate with the test equipment.The ASIC was replaced on this module and full function was restored to the module confirming the ASIC as the cause of the SRS module fault.According to Honda, tier-two Atmel implemented three different corrective actions in early 2008.Implementation of the corrective actions into module production by tier-one Continental, and into vehicle production by Honda resulted in three distinct vehicle populations.The first population consists of certain non-consecutively built MY 2008 to 2010 Accords with modules that did not contain any of the corrective actions.This population of 341,444 vehicles exhibited a significant level of module failure in service and is thus being recalled.The second population of 382,693 non-consecutively built MY 2008 to 2010 Accords contained two of the three corrective actions.This population exhibited a significantly lower level of SRS module failure, compared to the first population, but a marginally higher level compared to the third population.Honda is conducting a 15 year unlimited mileage extended warranty program for this second population.Under the program Honda will replace the SRS module free of charge for vehicles that experience an SRS module malfunction due to ASIC failure.Owners eligible for the extended warranty will be notified by Honda in writing.The third population consists of vehicles containing all three corrective actions, this population exhibits a normal level of SRS module failure.Based on the safety recall and extended warranty this investigation is closed.The VOQ reports cited above can be reviewed at SaferCar.gov under the following identification numbers: 10871612, 10869953, 10810554, 10788238, 10781994, 10778712, 10765025, 10761583, 10759019, 10758622, 10747850, 10735806, 10734761, 10733519, 10715339, 10703521, 10676517, 1065Read more

... plaints received after PE14-033 was opened found that 85 percent of the vehicles with diagnostic trouble codes available to identify the faulty component had a code indicating torque sensor failure (DTC 53).In its response to ODI's information request letter for PE14-033, Honda identified two manufacturing process conditions that may have affected the quality of some early production torque sensors.Analysis of warranty returns and manufacturing processes determined that a relatively small number of torque sensors were potentially affected by the conditions, which were corrected by the supplier (Bourns) relatively early in production.Analysis of warranty data indicates that both conditions were early-life failure concerns and most of the affected sensors have already failed, resulting in a cumulative failure rate of less than 0.2 percent.Of the 20 VOQ's that appear to be related to torque sensor failure, 6 reported failure dates in 2013, 12 in 2014, and just 2 to date in 2015.ODI's analysis identified 2 minor crashes with no injuries that may have been related to torque sensor failure in the subject vehicles.This investigation is closed because the low failure rate and a declining trend indicate that failures are rare.The closing of this preliminary evaluation does not constitute a determination that no defect exists or that power steering failures do not present an unreasonable risk to safety. Vehicle Owner Questionnaire (VOQ) 10329383 alleging ineffective braking performance of the Honda Accord Hybrid vehicle while driving over rumble strips along side of the highway and simultaneously attempting to the brake.The petitioner alleges this began a chain of events, where the vehicle accelerated uncontrollably, the passenger attempted to stop the vehicle by pulling the parking brake and eventually the vehicle crossing over into traffic flowing in the opposite direction.The result was a crash fatally injuring the passenger and severely injuring the petitioner and two occupants of another vehicle. ... ).In a September 22, 2010 supplemental DIR, Honda expanded the scope of the recall population based on manufacturing and complaint data.The total population for the complete recall is approximately 551,498 vehicles.The information listed in this closing report refers only to the subject vehicles, MY 2003 Accords with automatic transmissions.According to Honda's DIR, Under certain conditions the interlock lever of the ignition switch may unexpectedly deform, which can allow the interlock function of a vehicle with an automatic transmission to be defeated if the driver does not follow the typical shut-down procedure of shifting the gear selector to the Park position before rotating the ignition key to the off position. Removal of the ignition key when the gear selector of a vehicle with an automatic transmission has not been shifted to the Park position can allow the vehicle to roll away, increasing the risk of a crash.Honda's dealers will be instructed to inspect the ignition interlock switch and, if necessary, replace the original interlock pin and lever within the ignition switch and replace them with new, redesigned parts.This investigation is closed.Read more

ENGINE MALFUNCTIONS THAT MAY RESULT IN UNINTENDED SUDDENACCELERATION.THE AFFECTED VEHICLES ARE EQUIPPED WITH A CARBURETOR DESIGNWHICH, ACCORDING TO HONDA'S EVALUATION, WILL NOT PERMIT ENGINE RPM TOEXCEED A FAST IDLE CONDITION OF 2000 RPM WITHOUT DRIVER ACTUATION OF THETHROTTLE. TEST DATA WERE PROVIDED TO SHOW THAT AN ENGINE SPEED OF 2000 RPMWITH THE TRANSMISSION IN THE PARK POSITION WOULD, WHEN SHIFTED INTODRIVE, YIELD A VEHICLE SPEED OF 8 MPH AFTER APPROXIMATELY 10 SECONDS.OTHER TEST DATA WERE PROVIDED TO SUPPORT HONDA'S POSITION THAT FAILEDENGINE MOUNTS WOULD NOT CONTRIBUTE TO SUDDEN VEHICLE ACCELERATION ASALLEGED IN THIS PE.ANALYSIS:THE FOLLOWING ARE CONSIDERED SIGNIFICANT OBSERVATIONS AND/ORANALYTICAL ASSESSMENTS OF THE FACTS IN THIS PE FILE: OTHE ALLEGED INCIDENTS ARE SIMILAR TO THOSE FROM OTHER PE/EA FILESIN THAT THEY TEND TO EMPHASIZE OCCURRENCE OF THE PHENOMENON WHENTHE TRANSMISSION IS SHIFTED FROM PARK INTO ANOTHER GEAR.
