The plastic cover on the bottom is to low to the ground. Multiple times over the years it’s been torn or stuck on parking space separators or when I have to get my car towed it falls down. By this point half of it is gone and all the things under the car are exposed. Right now I barely have any p

When accelerating into traffic (usually merging onto highway or changing lanes on the highway) my car will loose all power and fail to accelerate. This is extremely dangerous and has happened on several occasions. I have taken this into Acura as well as other automotive dealers and no one can seem t

I am submitting this complaint regarding my 2015 Dodge Dart SXT with an active safety recall that currently has no available remedy. My vehicle experienced failure related to the recall issue, resulting in the vehicle becoming inoperable and requiring repair. At the time of failure, I was informed
... that the official recall remedy was not yet available and that the necessary part was on backorder. Because no recall remedy was available, I was forced to pay out of pocket to repair the vehicle in order to restore operability. I also incurred significant rental vehicle expenses while my car remained in the repair shop awaiting parts. I contacted Stellantis/Dodge Customer Care multiple times requesting assistance or reimbursement related to: * the repair costs associated with the recall-related failure, and * rental expenses incurred while the vehicle was out of service. My requests were denied, and the matter was repeatedly categorized as an “out-of-warranty” issue despite my concern specifically involving an active safety recall with no available remedy at the time of failure. I am concerned about: * the lack of an available recall remedy, * the financial burden placed on consumers when a recall-related failure occurs before a remedy is available, * extended delays due to backordered parts, and * the absence of transportation assistance or reimbursement for affected consumers. I am requesting that this complaint be documented and reviewed as a recall-related consumer safety and financial hardship issue.Read more

The malfunction involves the windshield wiper control system, including the rain-sensing activation module and associated control software. The system activates windshield wipers without any driver input under dry and clear conditions. The vehicle remains available for inspection upon request. Safe
... ty was compromised due to sudden and uncommanded wiper activation occurring during active driving conditions, including highway and nighttime operation. This results in temporary loss of visibility as the windshield is smeared by debris, requiring driver distraction to manually control or disable the system while operating the vehicle in traffic. Initial complaints regarding this issue were made by [XXX] during dealership service visits; however, those concerns were not consistently documented in the official repair records at the time they were reported and were initially dismissed or not formally recorded by service personnel until later intervention ensured the issue was entered verbatim on the service order. The issue has been reported and addressed during multiple dealership service visits; however, the condition has not been reliably reproduced and permanently corrected by the dealer despite documented repair attempts, including sensor replacement and manufacturer-level software (ODIS) updates. The malfunction has persisted after these interventions. The vehicle has been inspected by an authorized Volkswagen dealership service department. No inspection has been conducted by police, insurance representatives, or independent third parties. No consistent warning lamps or dashboard messages were present prior to the failure. The activation occurs unpredictably and without prior warning indicators. The issue first appeared intermittently at lower mileage and has continued through recent operation. The defect persists after multiple repairs and manufacturer intervention, indicating an uncorrectable electronic system failure causing an unresolved safety risk. The vehicle has 57000 miles and i am reporting a commonly known issue with the transmission. Sometimes, at low speed or at a stoplight, the vehicle would disengage the drive train and roll in neutral while the RPMs revved when the throttle was pressed. The vehicle was not operating safely, would roughly bounce around gears, when reported to Ford, told that there was nothing wrong and now being told post warranty that the transmission is a total fail that will require replacement. The latest service appointment one month ago, where I was again told that an oil topoff was all that was needed. This is a well reported safety issue by numerous owners and Ford has failed to take any action or initiate a recall.Read more

During a routine oil change test drive, a code for Crankshaft Loss of Position appeared on my truck and as a result, the autoshop I took it to (a Kerry's Car Care) recommended that I do not drive it. There were no warning lamps, alerts, or abnormal sounds prior to the incident to indicate an imminent issue. No emergency crews responded, and there were no injuries. On April 1st 2026, our brand-new 2026 Tesla Model X, 6-seat configuration, experienced a dangerous third-row seat incident. Our dog slipped into a blind spot beneath/behind the third-row seat while the seat was folded down. After the dog became trapped, the powered third-row seat would not respond,
... and there was no usable manual release or mechanical way for us to raise the seat and free him. The dog was trapped under the seat and in immediate danger. We drove directly to Tesla Service for an emergency rescue. Tesla personnel informed us that the only way to free the trapped animal was to dismantle and destroy/remove the rear seat assembly. That is what they did. The dog was then taken for emergency veterinary treatment and remains traumatized, with possible physical injury. This vehicle had been delivered only 4 days earlier. In my view, this presents a serious safety/design issue involving the third-row power seat system and the lack of an accessible emergency release when a living occupant or animal is trapped under the seat. The fact that destructive disassembly was allegedly the only way to free the trapped animal raises a serious safety concern. I am requesting that NHTSA review this incident as a potential safety defect involving the third-row seat mechanism, emergency release/access, and occupant/animal entrapment risk. I have photos, veterinary records, and related documentation.Read more

... the full rod bearing wear test report and they will not provide it. They have had my vehicle for 20 days now and I am out of work on disability. Hyundai service center is generally expected to provide you with a report following a rod bearing wear test (often referred to as a Bearing Clearance Test or by a related service campaign, such as T6G or TSB 21-EM-004H). Here is what you can expect regarding reports and documentation: Documentation of Results: The dealership should provide a copy of the results, specifically noting if the bearing clearance passes or fails. Proof of Inspection: If the test is performed under a recall or Service Campaign (e.g., Campaign 966, 982, or T6G), they are required to document the inspection, which you can request for your records. Evidence of Damage: If the test results in a failed diagnosis, technicians often take photos of the engine internals, such as metal flakes in the oil pan or excessive wear, which are used to support the warranty claim. If the Test Passes: If your vehicle passes, ask the technician to print a copy of the results. If the Test Fails: A formal diagnosis of rod bearing failure, confirmed by this test, is required for engine replacement under the extended 15-year/150,000-mile warranty. The vehicle has not been inspected by the manufacturer, law enforcement, or the insurance company. It is currently available for inspection but is not safely drivable. Loss of power while driving presents a risk of collision. Multiple attempts were made to schedule an appointment with Wilkes-Barre Honda regarding the outstanding recall; however, no response was received. The manufacturer was also contacted on several occasions, but no meaningful assistance was provided. Despite repeated requests, the dealership has not conducted a formal inspection or confirmed the cause of the failure. The vehicle has remained inoperable for several months. Under these circumstances, asking me to arrange towing is not reasonable, as delays in addressing recalls have created additional practical barriers to accessing necessary transportation and resources. *Note: I've included a phone number for this complaint but do not have one. Additionally, lack of vehicle access due to unresolved recalls had already created practical barriers in obtaining one.Read more

The open recall repair (NHTSA 23V-858) has not been completed. Additionally, there is a separate open recall, NHTSA Recall Number 25E-071 (Manufacturer Recall MMZ), involving 18-inch Honda Genuine Accessory wheels with improperly installed lug seat inserts. Both recalls remain unresolved. Beginning
... in approximately 2024, the vehicle exhibited intermittent jerking and stalling during acceleration, along with loss of smooth power delivery. No warning lamps were illuminated prior to failure. The vehicle was inspected in July 2025 after Wilkes-Barre Honda declined to provide assistance, and it passed inspection at that time. In October 2025, the engine failed to start, and the vehicle became fully non-operational. The insurance carrier was notified in October 2025 that the vehicle had become inoperable. On [XXX] I purchased a brand-new 2024 Nissan Altima SR from Vaden Nissan located at 10421 Abercorn Street, Savannah, GA 31419, Phone 912-920-5600; with approximately 25 miles on the odometer. On the day of purchase, the vehicle emitted a noticeable mold odor inside the cabin. The vehicle was return
... ed to the dealership on March 11, 2024, due to the mold odor. It was again returned on March 13, 2024, after water was discovered inside the trunk area of the vehicle. At the time of sale and shortly thereafter, water was present inside the trunk area, and the vehicle continued to exhibit water intrusion. The water intrusion appears to be associated with defective weatherstripping or sealing, which allowed moisture to enter the vehicle. On March 19, 2024, the dealership returned the vehicle to me with visible mold present inside the vehicle. The mold condition was observable at the time it was returned to consumer use. The vehicle was not professionally remediated for mold contaminated before being released back to consumer possession. A professional mold inspection later identified over 100,000 mold spores and at least ten (10) different types of mold inside the vehicle. Although the source of water intrusion was reportedly repaired on October 25, 2024, the vehicle remains inoperative and unsafe due to mold contamination. The mold condition persists despite repair attempts. I remain in possession of the vehicle, which cannot be safely driven or occupied. The manufacturer was notified of the water intrusion and mold contamination; however, the safety issue remains unresolved. I respectfully request that NHTSA review this matter to determine whether the water intrusion and resulting mold contamination present a broader safety concern and whether the handling and return of the vehicle to consumer use was appropriate given the visible mold condition.INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

Inadvertent Automatic Emergency Braking
On February 21, 2022, the Office of Defects Investigation (ODI) opened Preliminary Evaluation (PE22003) to assess reports of inadvertent activation of the Collision Mitigation Braking System (CMBS), an automatic emergency braking (AEB) system, in model year 2017-2019 Honda CR-V and 2018-2019 Honda A
... ccord vehicles. The reports allege that activation of the AEB system occurs while driving with no apparent obstruction in the vehicle's forward path, resulting in sudden vehicle deceleration. Honda indicated that they were aware of a total of X,XXX reports that may relate to the alleged defect. Honda provided analysis of the alleged defect and alleges that some customers possibly had an inadequate understanding of the CMBS and its limitations. However, many consumer complaints allege that Honda dealerships were unable to reproduce the condition or state that they were informed that this is considered normal CMBS operation. To date, ODI has received a total of 1,294 consumer complaints of inadvertent activation of CMBS in 2017-2022 Honda CR-V and 2018-2022 Honda Accord vehicles. A total of 31 complaints alleged a crash and 50 alleged an injury. The Total column in the Failure Report Summary removes duplicate reports and shows the total number of reports with unique VINs from all Manufacturer, ODI, and EWR data sources. In some cases, there were multiple reports associated with a particular vehicle in which recurring failures were alleged. In total, there were X,XXX reports, 93 injury incidents and 47 crashes involving vehicles with unique VINs that may relate to the alleged defect. PE22-003 has been upgraded to an Engineering Analysis to further assess the scope, frequency, and potential safety related consequences of the inadvertent AEB activations. The scope has been expanded to include assessment of model year 2020-2022 Honda CR-V and Accord vehicles. Front Seat Belt Assemblies may be Missing the Rivet that Secures the Quick Connector and Wire Plate
On April 4, 2024, the Office of Defects Investigations (ODI) opened RQ24006 to investigate American Honda Motor Co.’s (Honda’s) proposed remedy for recall 23V-782 as well as scope of the vehicles involved. Honda issued recall 23V-782 on November 21, 2023, which concerns certain model year (MY) 2
... 023-2024 Honda Accord, Honda Accord Hybrid, and Honda HR-V vehicles. Recall 23V-782 states that the driver and front passenger seat belt pretensioner assemblies may have been manufactured without a rivet that secures the pretensioner quick connector and wire plate. Recall documentation states that seat belt pretensioner assemblies that are missing the rivet will not properly restrain the occupant in the event of a collision, increasing the risk of injury.Subject defective seat belt assemblies are installed as original equipment for both the driver and front passenger positions for the HR-V, but only for the driver position for the Accord and Accord Hybrid. The recall remedy for 23V-782 instructs the technician to inspect both front seat belt pretensioner assemblies and, if necessary, replace them. The inspection procedure requires the technician to utilize a pull force gauge to determine whether the subject component is missing the subject rivet and requires replacement. Parts requiring replacement are determined by whether the pretensioner quick connector separates from the pretensioner assembly while the technician is pulling on the seat belt. If no separation occurs, then the part is determined to be not defective and not require replacement. There have been no reports alleging a failure of seat belt pretensioner assemblies after the inspect and replace remedy for recall 23V-782 was performed. Additionally, no comparable failures have been observed in field data for similar vehicles outside of the scope of the recall. As of the 4th quarter of 2024, the recall completion rate is at approximately 84% of the total recalled population. In view of the lack of failures after the remedy for recall 23V-782 was performed, and the lack of failures in similar vehicles outside of the scope of the recall, ODI is closing this Recall Query (RQ). The Agency reserves the right to take additional action if warranted by future circumstances.Read more

Loss of Direction Control
On May 6, 2021, the Office of Defects Investigation (ODI) opened Engineering Analysis (EA21-001) to assess a sudden loss of steering control, in Model Year (MY) 2013-2015 Honda Accord vehicles. Consumers alleged while driving the steering wheel would pull to the left or the right independently. Stee
... ring pull may pose a risk of loss of directional control. Honda stated that they believed the inadvertent steering input was caused by a loose magnet located within the electronic power steering (EPS) system. Due to poor formulation of the adhesive, this magnet can become loose and move up to 2 degrees around the steering column. When the magnet moves, the torque sensor generates a signal that results in unwanted steering in either direction. Honda stated the steering force required to maintain directional control with a loose magnet is a maximum of 3-pound feet. The maximum force of the unwanted steering input can be overcome by the driver and is similar to driving on grooved pavement. The Vehicle Research Test Center (VRTC) sent over 4,000 questionnaires to 2013-2015 Honda Accord vehicle owners. Of the 637 responses, 58 consumers experienced unwanted steering. Consumers experiencing unwanted steering did not report difficulties maintaining directional control. There were no crashes or injuries reported to VRTC. Testing done by VRTC was not able to reproduce the allegations. Analysis of all relevant data indicates that the subject condition has a relatively low failure rate. The poor adhesive formulation was addressed by the supplier with three (3) counter measures in the manufacturing process. There has been a declining trend in consumer complaints to NHTSA which indicates possible exhaustion of parts prone to failure. In view of the low rate, declining complaint trend, the nature of failure, and resolution of the adhesive formulation, this Engineering Analysis is closed. The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist. The agency reserves the right to take additional action if warranted by new circumstances. The ODI complaints cited above can be viewed at www.NHTSA.gov under the following ODI identification numbers: 11538714, 11528538, 11528008, 11522953, 11496557, 11495837, 11494328, 11478680, 11477167, 11476758, 11476263, 11471198, 11470001, 11467026, 11466561, 11466016, 11465626, 11464143, 11463558, 11462759, 11461500, 11460777, 11460191, 11455540, 11454334, 11452198, 11448114, 11447666, 11444888, 11444823, 11444172, 11443324, 11443243, 11443105, 11441374, 11439520, 11438345, 11437261, 11436824, 11436350, 11435994, 11435079, 11434969, 11434545, 11434442, 11434381, 11433824, 11433745, 11433555, 11433368, 11433176, 11431206, 11430465, 11430253, 11430009, 11429253, 11428301, 11427449, 11426841, 11426340, 11425928, 11425261, 11425230, 11423611, 11423245, 11422445, 11422395, 11422351, 11422220, 11422138, 11422069, 11421683, 11421589, 11421540, 11420573, 11419338, 11419270, 11418147, 11418143, 11417768, 11417742, 11417570, 11417437, 11416804, 11416699, 11416401, 11416378, 11416375, 11416155, 11416117, 11416084, 11416051, 11416032, 11416028, 11416024, 11416000, 11415933, 11415915, 11414105, 11406220, 11398736 ,11387075, 11387049, 11375580, 11364698, 11361125, 11351660, 11349961, 11327909, 11327170, 11301957, 11289067, 11282525, 11277659, 11258017, 11256914, 11218752, 11208077, 11196050, 11194358, 11189901, 11165075, 11163481, 11154891Read more

SRS Control Module Failure
As discussed further below, and by letter dated January 22, 2016, Honda is recalling certain model year (MY) 2008 through 2010 Honda Accords to replace a defective air bag/SRS control module that can result in the Supplemental Restraint System (SRS) becoming fully inoperative (disabled).See NHTSA Sa
... fety Recall 16V056 at SaferCar.gov for further details.Honda's response to an agency Information Request letter indicated that the SRS module was manufactured by tier-one supplier Continental and contained a power supply ASIC manufactured by tier-two supplier Atmel. The original ASIC was susceptible to a moisture ingress issue due to inadequate ASIC passivation layer.Water ingress could result in moisture reaching the Titanium Nitride (TiN) layer in the ASIC causing corrosion and breakage of electrical circuits. The damaged ASIC then causes failures of the SRS module resulting in 1) incomplete boot-up when power is supplied to the module or 2) the inability of the module to establish either internal or external bus communications. In either case the air bag readiness indicator light on the instrument cluster is illuminated and all air bags and seat belt pretensioners are disabled.During the investigation, ODI obtained the SRS module from a complainant crash vehicle where the air bags did not deploy despite a significant frontal impact. An inspection of the module was performed at a Continental facility. The inspection confirmed that the module had the ASIC fault thus preventing the SRS module from completing the boot-up process. An attempt was made to recover stored EDR data from the module however none was present; this outcome is consistent with an Atmel ASIC failure.A second SRS module alleged to have the ASIC problem was inspected during the same visit. This second module was unable to communicate with the test equipment. The ASIC was replaced on this module and full function was restored to the module confirming the ASIC as the cause of the SRS module fault.According to Honda, tier-two Atmel implemented three different corrective actions in early 2008.Implementation of the corrective actions into module production by tier-one Continental, and into vehicle production by Honda resulted in three distinct vehicle populations. The first population consists of certain non-consecutively built MY 2008 to 2010 Accords with modules that did not contain any of the corrective actions. This population of 341,444 vehicles exhibited a significant level of module failure in service and is thus being recalled. The second population of 382,693 non-consecutively built MY 2008 to 2010 Accords contained two of the three corrective actions. This population exhibited a significantly lower level of SRS module failure, compared to the first population, but a marginally higher level compared to the third population.Honda is conducting a 15 year unlimited mileage extended warranty program for this second population.Under the program Honda will replace the SRS module free of charge for vehicles that experience an SRS module malfunction due to ASIC failure.Owners eligible for the extended warranty will be notified by Honda in writing. The third population consists of vehicles containing all three corrective actions, this population exhibits a normal level of SRS module failure.Based on the safety recall and extended warranty this investigation is closed. The VOQ reports cited above can be reviewed at SaferCar.gov under the following identification numbers: 10871612, 10869953, 10810554, 10788238, 10781994, 10778712, 10765025, 10761583, 10759019, 10758622, 10747850, 10735806, 10734761, 10733519, 10715339, 10703521, 10676517, 1065Read more

Electric Power Steering Failure
The Office of Defects Investigation (ODI) opened PE14-033 to investigate 24 complaints alleging loss of power steering while driving in model year (MY) 2013 Honda Accord vehicles equipped with electric power steering (EPS).Analysis of service data for the original 24 complaints and 33 additional com
... plaints received after PE14-033 was opened found that 85 percent of the vehicles with diagnostic trouble codes available to identify the faulty component had a code indicating torque sensor failure (DTC 53).In its response to ODI's information request letter for PE14-033, Honda identified two manufacturing process conditions that may have affected the quality of some early production torque sensors.Analysis of warranty returns and manufacturing processes determined that a relatively small number of torque sensors were potentially affected by the conditions, which were corrected by the supplier (Bourns) relatively early in production.Analysis of warranty data indicates that both conditions were early-life failure concerns and most of the affected sensors have already failed, resulting in a cumulative failure rate of less than 0.2 percent.Of the 20 VOQ's that appear to be related to torque sensor failure, 6 reported failure dates in 2013, 12 in 2014, and just 2 to date in 2015.ODI's analysis identified 2 minor crashes with no injuries that may have been related to torque sensor failure in the subject vehicles. This investigation is closed because the low failure rate and a declining trend indicate that failures are rare. The closing of this preliminary evaluation does not constitute a determination that no defect exists or that power steering failures do not present an unreasonable risk to safety. The VOQ's referenced in this resume are:10515618, 10515696, 10537336, 10542360, 10545177, 10563724, 10588425, 10595068, 10605909, 10627926, 10629629, 10630852, 10644762, 10654768, 10661240, 10664234, 10667364, 10672452, 10676256, and 10695191Read more

... art of model year 2008 production to June 25, 2008) as only about 500 early-built 2009 Accords (built from June 24 to June 30, 2008) are included in the recall.The Office of Defects Investigation (ODI) has identified 351 reported incidents of inadvertent side air bag deployments in the 2008 Accord sedans.This investigation found there is risk of serious injury to vehicle occupants or users who are positioned in the direct path of a deploying side curtain air bag.Based on the recall action taken by Honda, this Engineering Analysis is closed.Additional information on this closing resume and the investigation are in the EA14-004 public file available on-line at SaferCar.gov.The ODI reports cited above can be reviewed online at http://www-odi.nhtsa.dot.gov/owners/SearchNHTSAID under the following identification numbers:10232206 10235294 10255323 10268158 10268191 10269514 10276755 10279446 10280675 10308292 10319979 10342520 10353536 10408925 10436506 10447151 10458732 10468196 10468200 10493407 10494971 10522291 10524389 10543092 10544473 10545258 10547591 10554073 10563629 10565311 10565423 10576518 10579241 10584501 10585442 10606937 10608973 10627302 10638541 10640968 10650861 10654858 10679518 10680063 10683967 10712981 10717410 10717777 10761860Read more

Inadvertent Side Air Bag Deployment
ODI has identified 293 incidents of alleged inadvertent deployment of the side air bags in model year (MY) 2008 Honda Accord 4-door vehicles. This total represents all incidents reported to both ODI and Honda including those from Honda's warranty system (note: the total shown above eliminates all dup
... licative reports to ODI and Honda). This investigation has revealed that when the driver or front passenger door is shut, the driver-side or passenger-side side curtain air bag can deploy, and in some cases, the side seat-mounted torso air bag can deploy as well.Fourteen people allegedly suffered injuries caused by the deploying air bags.In June 2008, near the end of MY 2008 Accord 4-door production, Honda changed the crash parameter for door closing force, which is embedded in the electronic control unit's software code, in order to reduce the incidents of the inadvertent side air bag deployments.More specifically, the software design change was made on June 5, 2008 for the Accords built in the U.S. and on June 30, 2008 for those built in Japan.ODI found the number of the inadvertent deployment incidents declined significantly for the vehicles with the new crash parameter setting, i.e., the late-built MY 2008 vehicles and all MY 2009 vehicles.Honda reported the Accord 2-door model utilizes a different software/crash parameter than the subject 4-door model. This Preliminary Evaluation has been upgraded to an Engineering Analysis (EA14-004) to determine, among other things, the risk of air bag deployment injuries to vehicle users in vulnerable positions. The ODI reports cited above can be reviewed online at http://www-odi.nhtsa.dot.gov/owners/SearchNHTSAID under the following identification numbers: 10232206, 10235294, 10255323, 10268158, 10268191, 10269514, 10276755, 10279446, 10280675, 10308292, 10319979, 10342520, 10353536, 10408925, 10436506, 10447151, 10458732, 10468196, 10468200, 10493407, 10494971, 10522291, 10524389, 10543092, 10544473, 10545258, 10547591, 10554073, 10563629, 10565311, 10565423, 10576518, 10579241, 10584501, 10585442, 10606937, 10608973.Read more

Braking Performance
The Office of Defects Investigation (ODI) received a defect petition requesting that it open an investigation and recall MY 2005 Honda Accord Hybrid vehicles for a possible safety defect related to braking performance. The petitioner in a defect petition letter and in a complaint filed with ODI under
... Vehicle Owner Questionnaire (VOQ) 10329383 alleging ineffective braking performance of the Honda Accord Hybrid vehicle while driving over rumble strips along side of the highway and simultaneously attempting to the brake. The petitioner alleges this began a chain of events, where the vehicle accelerated uncontrollably, the passenger attempted to stop the vehicle by pulling the parking brake and eventually the vehicle crossing over into traffic flowing in the opposite direction. The result was a crash fatally injuring the passenger and severely injuring the petitioner and two occupants of another vehicle. During the defect petition review, ODI analyzed complaints involving similarly equipped Honda Accord Hybrid vehicles and first and second generation Honda Civic Hybrid vehicles, reviewed data provided to ODI by Honda in response to an information request letter and performed testing of braking performance of a 2005 Honda Accord Hybrid vehicle at NHTSA's Vehicle Research and Test Center in East Liberty, Ohio.Based on this review, ODI does not believe additional investigation is likely to result in a finding that a defect related to motor vehicle safety exists. Therefore, in view of the need to allocate and prioritize NHTSA?s limited resources to best accomplish the agency's safety mission, the petition is denied. This action does not constitute a finding by NHTSA that a safety-related defect does not exist. The agency will take further action if warranted by future circumstances. For more information, see the Federal Register notice in the DP10-004 file (www-odi.nhtsa.dot.gov/defects).Read more

Ignition Shift Interlock
On August 4, 2010, American Honda Motor Co. (Honda) submitted a Defect Information Report (DIR) to NHTSA regarding a ignition interlock defect in approximately 384,220 model year (MY) 2003 Honda Accord and Civic and MY 2003 and 2004 Honda Element vehicles with automatic transmissions (Recall 10V-364
... ).In a September 22, 2010 supplemental DIR, Honda expanded the scope of the recall population based on manufacturing and complaint data. The total population for the complete recall is approximately 551,498 vehicles. The information listed in this closing report refers only to the subject vehicles, MY 2003 Accords with automatic transmissions.According to Honda's DIR, Under certain conditions the interlock lever of the ignition switch may unexpectedly deform, which can allow the interlock function of a vehicle with an automatic transmission to be defeated if the driver does not follow the typical shut-down procedure of shifting the gear selector to the Park position before rotating the ignition key to the off position. Removal of the ignition key when the gear selector of a vehicle with an automatic transmission has not been shifted to the Park position can allow the vehicle to roll away, increasing the risk of a crash.Honda's dealers will be instructed to inspect the ignition interlock switch and, if necessary, replace the original interlock pin and lever within the ignition switch and replace them with new, redesigned parts. This investigation is closed.Read more

SRS READINESS INDICATOR ILLUMINATION
THE SUBJECT VEHICLES ARE DESIGNED WITH AN OCCUPANT POSITION DETECTION SYSTEM (OPDS) FOR THE PASSENGER SEAT TO SUPPRESS THE SIDE AIRBAG IN THE EVENT OF AN OUT-OF-POSITION OCCUPANT.ON SOME VEHICLES, THE OPDS UNIT AND SENSOR GRID MAY BE SUSCEPTIBLE TO HIGH MOISTURE CONTENT AND/OR TO SPORADIC ELECTRICAL
... INTERFERENCE THAT WOULD RESULT IN THE ILLUMINATION OF THE SRS READINESS INDICATOR.IN THIS CONDITION, OPDS SUPPRESSION WILL NOT FUNCTION, BUT THE SIDE AIRBAG WILL STILL DEPLOY IN THE EVENT OF A SIDE CRASH. HONDA ISSUED A TECHNICAL SERVICE BULLETIN IN APRIL 2004 TO ADDRESS THIS PROBLEM. HOWEVER, THE PROBLEM PERSISTS EVEN AFTER THE WARRANTY PERIOD EXPIRES.AS A RESULT, SOME OWNERS MAY NOT BE OBTAINING THE NECESSARY REMEDIAL SERVICE WORK.CONTINUED ILLUMINATION OF THE SRS INDICATOR CAN MASK THE DETECTION OF OTHER AIRBAG COMPONENT FAULTS IF THEY OCCUR. IN MARCH 2006, HONDA ISSUED A SERVICE BULLETIN (06-009) TO EXTEND THE WARRANTY ON SRS ILLUMINATION AND OPDS ISSUES TO 10 YEARS OR 150,000 MILES (WHICHEVER OCCURS FIRST).HONDA WILL SEND NOTIFICATION LETTERS TO OWNERS OF THE SUBJECT VEHICLES AND WILL PROVIDE A NO COST REPLACEMENT OF THE OPDS UNIT AS NEEDED.IN ADDITION, OTHER MY 1999-04 HONDA AND ACURA VEHICLES ARE ALSO COVERED BY THE BULLETIN.THIS ACTION TAKEN BY THE MANUFACTURER IS SUFFICIENT TO RESOLVE THE ISSUE RAISED BY THIS INVESTIGATION.THE CLOSING OF THIS INVESTIGATION DOES NOT CONSTITUTE A FINDING BY NHTSA THAT NO SAFETY-RELATED DEFECT EXISTS.THE AGENCY RESERVES THE RIGHT TO TAKE FURTHER ACTION IF WARRANTED BY THE CIRCUMSTANCES.Read more

... MS FROM COMPLAINT DESCRIPTIONS THAT CONTENDED THE INTERLOCK SYSTEM FAILED TO OPERATE PROPERLY.HONDA PRESENTED THE RESULTS OF THEIR INVESTIGATION INTO INTERLOCK SYSTEM FAILURES AT AN OCTOBER 16, 2003 MEETING WITH ODI IN WHICH HONDA IDENTIFIED A SUPPLIER AND AN ENGINEERING SPECIFICATION CHANGE FOR THE IGNITION CYLINDER BODY (CB), A SUBCOMPONENT OF THE IGNITION LOCK ASSEMBLY.RESULTS FROM HONDA?S ANALYSIS DEMONSTRATED A STRONG CORRELATION BETWEEN THE PRODUCTION DATE RANGE OF VEHICLES THAT EXPERIENCED HIGHER INTERLOCK WARRANTY CLAIM RATES AND THE IMPLEMENTATION DATES OF THE CHANGES.AN ENGINEERING SPECIFICATION FOR A SURFACE TREATMENT PROCESS FOR THE CB WAS ELIMINATED AT THE BEGINNING OF MY 1998.THE SUPPLIER OF THE CB WAS CHANGED IN FEBRUARY 1999.VEHICLES BUILT WITHIN THIS TIME PERIOD CONTAIN A CB WITH POOR SURFACE HARDNESS THAT CAN WEAR PREMATURELY.AS THE WEAR PROGRESSES, THE INTERLOCK SYSTEM BECOMES INCAPABLE OF PREVENTING KEY REMOVAL WHEN THE SHIFTER IS IN A NON-PARK POSITION.INTERLOCK FAILURE INCREASES THE RISK OF A VEHICLE ROLLAWAY AND SUBSEQUENT CRASH OR INJURY WHEN VEHICLES ARE PARKED UNSECURED.HONDA ACCORD VEHICLES SUBJECT TO THIS CHANGE INCLUDE MY 1998 AND MY 1999 THROUGH FEBRUARY 1999 PRODUCTION.VEHICLES PRODUCED AFTER THIS DATE ARE NOT AFFECTED BY THIS CONDITION AND THUS ARE NOT CONSIDERED AT RISK FOR FAILURE.ODI NOTES THAT 12 OF THE 13 VOQ REPORTS FALL WITHIN THIS PRODUCTION RANGE (THE 13TH DOES NOT HAVE A VIN AND CANNOT BE VERIFIED).THE ABOVE POPULATION REPRESENTS THE NUMBER OF THE ACCORD VEHICLES TO BE RECALLED.HONDA ALSO REPORTED THAT CERTAIN ODYSSEY AND ACURA CL AND TL MODELS WERE AFFECTED AND WOULD ALSO BE RECALLED.A TOTAL OF 570,120 VEHICLES WILL BE RECALLED.UNDER RECALL 03V-423, HONDA WILL INSPECT VEHICLES AND REPLACE COMPONENTS (INCLUDING THE CB) ON VEHICLES WITH EXCESSIVE WEAR.VEHICLES NOT SHOWING WEAR WILL HAVE A REDESIGNED PART INSTALLED WHICH PREVENTS WEAR.Read more

ENGINE MALFUNCTIONS THAT MAY RESULT IN UNINTENDED SUDDENACCELERATION.THE AFFECTED VEHICLES ARE EQUIPPED WITH A CARBURETOR DESIGNWHICH, ACCORDING TO HONDA'S EVALUATION, WILL NOT PERMIT ENGINE RPM TOEXCEED A FAST IDLE CONDITION OF 2000 RPM WITHOUT DRIVER ACTUATION OF THETHROTTLE. TEST DATA WERE PROVIDED TO SHOW THAT AN ENGINE SPEED OF 2000 RPMWITH THE TRANSMISSION IN THE PARK POSITION WOULD, WHEN SHIFTED INTODRIVE, YIELD A VEHICLE SPEED OF 8 MPH AFTER APPROXIMATELY 10 SECONDS.OTHER TEST DATA WERE PROVIDED TO SUPPORT HONDA'S POSITION THAT FAILEDENGINE MOUNTS WOULD NOT CONTRIBUTE TO SUDDEN VEHICLE ACCELERATION ASALLEGED IN THIS PE.ANALYSIS:THE FOLLOWING ARE CONSIDERED SIGNIFICANT OBSERVATIONS AND/ORANALYTICAL ASSESSMENTS OF THE FACTS IN THIS PE FILE: OTHE ALLEGED INCIDENTS ARE SIMILAR TO THOSE FROM OTHER PE/EA FILESIN THAT THEY TEND TO EMPHASIZE OCCURRENCE OF THE PHENOMENON WHENTHE TRANSMISSION IS SHIFTED FROM PARK INTO ANOTHER GEAR. OA HIGH PERCENTAGE OF THE REPORTED OCCURRENCES ALSO INVOLVEACCIDENTS,BUT THE RELATIVELY LOW PERCENTAGE OF INJURY ACCIDENTSSUGGESTS THAT MOST ARE LOW SPEED COLLISIONS. OHONDA HAS NOT IDENTIFIED, AND ODI'S ANALYSES HAVE NOT DISCOVERED,A TREND OF MECHANICAL OR ELECTRICAL COMPONENT FAILURE WITH ACAUSAL RELATIONSHIP TO THE ALLEGED OCCURRENCES.IN ADDITION TO THE ABOVE, ANALYSIS OF THE RATES OF ALLEGED INCIDENTS,ACCIDENTS, AND INJURY ACCIDENTS WITH RESPECT TO THE TOTAL VEHICLEPOPULATION OF 746,000, SHOWS THAT THE HONDA ACCORD COMPARES FAVORABLYTO OTHER VEHICLES REPORTED TO HAVE EXPERIENCED SIMILAR OCCURRENCES.THIS ANALYSIS, TOGETHER WITH OTHER INFORMATION ACQUIRED, SHOWS THAT NODEFECT TREND EXISTS TO JUSTIFY CONTINUATION OF INVESTIGATIVE ACTIVITY.PROPOSED ACTION:CLOSE THIS PRELIMINARY EVALUATION.===Read more
