Engine Failure at 35,000 miles due to well known and documented engine design flaw between cylinder 2 and 3. The gasket fails and leaks coolant into the cylinders causing engine failure. An upgraded 2020 engine that has fixed the issue is required.

The contact received notification of NHTSA Campaign Number: 24V957000 (FUEL SYSTEM, DIESEL); however, the SSSC (Special Service Center assistance request) was declined by Ford agency (SSSC), due to the issue not having metal shavings although the dealer technician stated there were shavings. The driver exited to inspect the vehicle and became aware that the front passenger’s side wheel was lying on the ground. The vehicle was towed to an independent mechanic, but was not diagnosed. The vehicle was then towed to another dealer, where it was diagnosed, and it was determined that the passenger’s side upper and lower control arms, ball joints, rack and pinion, brake rotor and pads, hub, outer tie rod, brake line, speed wheel sensor, wheel and tire, and suspension parts needed to be replaced. So I left the car got it towed the next day to another shop because any ford dealerships I called to see if I could tow for diagnostic test said they had extensive waiting period like one had weeks before they could look at the car. So I had it towed to another car lot who had a good business rating to be told that it seems to look like something had came loose from the transmission. That i may have to get it replaced, this car starts right up, didn't give me any indication of a problem and doesn't even have a hundred thousand miles on it. Where did ford drop the ball on this car? I am filing this complaint regarding repeated premature strut failure on my 2022 Subaru Forester. The original set of struts failed at approximately 36,000 miles. The issue was not covered under warranty and required replacement. At approximately 60,000 miles, the replacement struts are again faili
... ng. This means two full sets of struts have failed before 100,000 miles, which is highly abnormal for a vehicle of this age and mileage. Premature strut failure affects vehicle stability, ride control, and braking performance. This raises safety concerns, particularly at highway speeds and during emergency maneuvers. Suspension components such as struts should not require replacement this frequently under normal driving conditions. Given the repeated nature of this issue, I believe this may represent a broader defect rather than normal wear and tear. I have seen many other 2022 Subaru Foresters have this same issue but have not heard of a recall to date. While driving my 2025 Nissan Armada Platinum Reserve during morning rush hour traffic and moving with the speed of traffic, the accelerator (gas) pedal assembly physically broke off, causing an immediate loss of throttle control. The failure occurred in active traffic at a busy intersection located
... at the top of a hill, leaving the vehicle stranded partially in a turn lane and straight-through lane. I was unable to safely move the vehicle out of traffic due to the pedal detaching from its mounting point. There were no warning lights, no prior looseness, and no unusual resistance prior to the failure. This appears to be a mechanical/structural failure of the accelerator pedal assembly, not an electronic throttle or sensor malfunction. Due to the dangerous location and inability to move the vehicle, law enforcement (a sheriff) responded approximately 30 minutes later to assist with traffic safety. The vehicle remained immobilized in traffic for an extended period, and it took nearly two hours for a tow truck to arrive. This incident created a serious and prolonged safety hazard, exposing me and other motorists to a high risk of collision. The vehicle had approximately 9,700 miles and has been driven under normal conditions. The vehicle is currently not being driven due to safety concerns. The failed accelerator pedal assembly is available for inspection. I am reporting this as a potential manufacturing or material defect and believe it warrants investigation to prevent similar failures in other vehicles.Read more

Driving on the highway, I slowed down from 65mph to 45-50mph due to standing water ahead of me, immediately after hitting water, the engine stumbled. Soon after the engine stopped running. ... icle range dropping rapidly. The odor of unburnt gasoline was very strong and I had begun to get a headache and get dizzy, I had to open the windows while it was pouring heavy rain. Given the area had a lot of standing water, I wasn't able to stop safely. I was able to limp the vehicle in hybrid mode to a nearby uphill exit. Once stopped, I immediately left the interior of the vehicle for fresh air. If there was no shoulder and I wasn't literally next to a highway exit, I would be in deep sh**. After stopping, I checked the airbox to see if water made it into the intake. It was bone dry. Walking the right side of the vehicle I saw fuel on the ground. I called for a tow truck and they dropped me off at home. When I looked underneath the car and found the fuel line hanging free. The clip holding the tank and chassis fuel lines together was missing. I went to the local Toyota dealership and the technicians said it was not a warranty issue because it's impact damage and that I should talk to my insurance about fixing the car because it was going to be $3500 dollars to drop the prop shaft, transfer case, transmission, and other parts to replace the line because "toyota does not sell the clip alone". The line was $168 dollars. I probed further and they brought the price down to $118. Online, the fuel line is $56 dollars. I checked the Land Cruiser forums and multiple people have experienced this issue under identical conditions, others have found the clip loose during regular maintenance. This appears to be a dangerous design flaw. My 2012 Tacoma and 2016 Land Cruiser do not have these issues since they use common steel spring clips to hold fuel lines in place.Read more

This sudden failure created a serious safety risk due to the unexpected airbag deployment and loss of vehicle control while driving. As a result of the incident, I sustained physical injuries and sought medical attention. The vehicle has been inspected by the manufacturer. The manufacturer has stated they are unable to confirm whether the vehicle is safe to operate and the investigation remains ongoing. No warning lights or messages appeared prior to the failure. The event occurred suddenly and without warning. Law enforcement responded to assist with traffic safety and relocation of the disabled vehicle; no formal accident or crash report was generated.Read more

Kawasaki is advised all owners of these bikes to immediately stop riding their affected 2024 through 2026 Ninja ZX-6Rs until the repairs could be completed. ... ng the accelerator pedal did not result in normal response or acceleration. The throttle input was significantly reduced, making it difficult to maintain speed and safely control the vehicle. At highway speed and in heavy highway traffic, if we had not been able to safely exit the roadway, this loss of control could have put not only the occupants of this vehicle but also other drivers at serious risk. Fortunately, we were already in the exit lane, and a McDonald’s parking lot was located immediately off the exit. This allowed us to leave the highway quickly. Otherwise, this situation could have been extremely dangerous in active, heavy highway traffic. As the vehicle continued to shake, a strong smell of gasoline and a burning odor entered the cabin, raising concerns about a possible fire hazard. We safely stopped the vehicle in the McDonald’s parking lot and then shut the engine off due to the severity of the symptoms. After shutting the vehicle off, we attempted to restart it, but the engine would not start. The vehicle had to be towed to a Hyundai service center for inspection. The Hyundai service center later confirmed that the failure was caused by a faulty fuel injector, and they have changed all 4 injectors as I attached receipt from Hyundai This failure created a serious safety risk due to sudden loss of engine power, reduced accelerator response, severe shaking at highway speed, and fuel and burning odors inside the cabin. Inadvertent Automatic Emergency Braking
On February 21, 2022, the Office of Defects Investigation (ODI) opened Preliminary Evaluation (PE22003) to assess reports of inadvertent activation of the Collision Mitigation Braking System (CMBS), an automatic emergency braking (AEB) system, in model year 2017-2019 Honda CR-V and 2018-2019 Honda A
... ccord vehicles. The reports allege that activation of the AEB system occurs while driving with no apparent obstruction in the vehicle's forward path, resulting in sudden vehicle deceleration. Honda indicated that they were aware of a total of X,XXX reports that may relate to the alleged defect. Honda provided analysis of the alleged defect and alleges that some customers possibly had an inadequate understanding of the CMBS and its limitations. However, many consumer complaints allege that Honda dealerships were unable to reproduce the condition or state that they were informed that this is considered normal CMBS operation. To date, ODI has received a total of 1,294 consumer complaints of inadvertent activation of CMBS in 2017-2022 Honda CR-V and 2018-2022 Honda Accord vehicles. A total of 31 complaints alleged a crash and 50 alleged an injury. The Total column in the Failure Report Summary removes duplicate reports and shows the total number of reports with unique VINs from all Manufacturer, ODI, and EWR data sources. In some cases, there were multiple reports associated with a particular vehicle in which recurring failures were alleged. In total, there were X,XXX reports, 93 injury incidents and 47 crashes involving vehicles with unique VINs that may relate to the alleged defect. PE22-003 has been upgraded to an Engineering Analysis to further assess the scope, frequency, and potential safety related consequences of the inadvertent AEB activations. The scope has been expanded to include assessment of model year 2020-2022 Honda CR-V and Accord vehicles. Front Seat Belt Assemblies may be Missing the Rivet that Secures the Quick Connector and Wire Plate
On April 4, 2024, the Office of Defects Investigations (ODI) opened RQ24006 to investigate American Honda Motor Co.’s (Honda’s) proposed remedy for recall 23V-782 as well as scope of the vehicles involved. Honda issued recall 23V-782 on November 21, 2023, which concerns certain model year (MY) 2
... 023-2024 Honda Accord, Honda Accord Hybrid, and Honda HR-V vehicles. Recall 23V-782 states that the driver and front passenger seat belt pretensioner assemblies may have been manufactured without a rivet that secures the pretensioner quick connector and wire plate. Recall documentation states that seat belt pretensioner assemblies that are missing the rivet will not properly restrain the occupant in the event of a collision, increasing the risk of injury.Subject defective seat belt assemblies are installed as original equipment for both the driver and front passenger positions for the HR-V, but only for the driver position for the Accord and Accord Hybrid. The recall remedy for 23V-782 instructs the technician to inspect both front seat belt pretensioner assemblies and, if necessary, replace them. The inspection procedure requires the technician to utilize a pull force gauge to determine whether the subject component is missing the subject rivet and requires replacement. Parts requiring replacement are determined by whether the pretensioner quick connector separates from the pretensioner assembly while the technician is pulling on the seat belt. If no separation occurs, then the part is determined to be not defective and not require replacement. There have been no reports alleging a failure of seat belt pretensioner assemblies after the inspect and replace remedy for recall 23V-782 was performed. Additionally, no comparable failures have been observed in field data for similar vehicles outside of the scope of the recall. As of the 4th quarter of 2024, the recall completion rate is at approximately 84% of the total recalled population. In view of the lack of failures after the remedy for recall 23V-782 was performed, and the lack of failures in similar vehicles outside of the scope of the recall, ODI is closing this Recall Query (RQ). The Agency reserves the right to take additional action if warranted by future circumstances.Read more

Loss of Direction Control

... ring pull may pose a risk of loss of directional control. Honda stated that they believed the inadvertent steering input was caused by a loose magnet located within the electronic power steering (EPS) system. Due to poor formulation of the adhesive, this magnet can become loose and move up to 2 degrees around the steering column. When the magnet moves, the torque sensor generates a signal that results in unwanted steering in either direction. Honda stated the steering force required to maintain directional control with a loose magnet is a maximum of 3-pound feet. The maximum force of the unwanted steering input can be overcome by the driver and is similar to driving on grooved pavement.The Vehicle Research Test Center (VRTC) sent over 4,000 questionnaires to 2013-2015 Honda Accord vehicle owners. Of the 637 responses, 58 consumers experienced unwanted steering. Consumers experiencing unwanted steering did not report difficulties maintaining directional control. There were no crashes or injuries reported to VRTC. Testing done by VRTC was not able to reproduce the allegations. Analysis of all relevant data indicates that the subject condition has a relatively low failure rate. The poor adhesive formulation was addressed by the supplier with three (3) counter measures in the manufacturing process.There has been a declining trend in consumer complaints to NHTSA which indicates possible exhaustion of parts prone to failure. In view of the low rate, declining complaint trend, the nature of failure, and resolution of the adhesive formulation, this Engineering Analysis is closed. The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist. The petitioner further alleges that this deviation from the intended path of travel occurs without warning or driver input while operating the vehicle under normal driving conditions. The petition itself can be reviewed at NHTSA.gov under ODI Number 11349961.On November 23, 2020, ODI opened a Defect Petition (DP20-005) to evaluate whether to grant or deny the petition. ODI sent two Information Request (IR) letters to Honda requesting information on steering anomalies in MY2013 Honda Accord vehicles and later MY2014-2015 Honda Accord vehicles.ODI performed extensive analysis on the information obtained from Honda as well as information in NHTSA's database.Based on ODI's review of the applicable materials, including responses to the IR letters sent to Honda, Vehicle Owner Questionnaires (VOQ) and Early Warning Reporting data, NHTSA has decided to grant the petition. The original ASIC was susceptible to a moisture ingress issue due to inadequate ASIC passivation layer.Water ingress could result in moisture reaching the Titanium Nitride (TiN) layer in the ASIC causing corrosion and breakage of electrical circuits.The damaged ASIC then causes failures of the SRS module resulting in 1) incomplete boot-up when power is supplied to the module or 2) the inability of the module to establish either internal or external bus communications.In either case the air bag readiness indicator light on the instrument cluster is illuminated and all air bags and seat belt pretensioners are disabled.During the investigation, ODI obtained the SRS module from a complainant crash vehicle where the air bags did not deploy despite a significant frontal impact.An inspection of the module was performed at a Continental facility.The inspection confirmed that the module had the ASIC fault thus preventing the SRS module from completing the boot-up process.An attempt was made to recover stored EDR data from the module however none was present; this outcome is consistent with an Atmel ASIC failure.A second SRS module alleged to have the ASIC problem was inspected during the same visit.This second module was unable to communicate with the test equipment.The ASIC was replaced on this module and full function was restored to the module confirming the ASIC as the cause of the SRS module fault.According to Honda, tier-two Atmel implemented three different corrective actions in early 2008.Implementation of the corrective actions into module production by tier-one Continental, and into vehicle production by Honda resulted in three distinct vehicle populations.The first population consists of certain non-consecutively built MY 2008 to 2010 Accords with modules that did not contain any of the corrective actions.This population of 341,444 vehicles exhibited a significant level of module failure in service and is thus being recalled.The second population of 382,693 non-consecutively built MY 2008 to 2010 Accords contained two of the three corrective actions.This population exhibited a significantly lower level of SRS module failure, compared to the first population, but a marginally higher level compared to the third population.Honda is conducting a 15 year unlimited mileage extended warranty program for this second population.Under the program Honda will replace the SRS module free of charge for vehicles that experience an SRS module malfunction due to ASIC failure.Owners eligible for the extended warranty will be notified by Honda in writing.The third population consists of vehicles containing all three corrective actions, this population exhibits a normal level of SRS module failure.Based on the safety recall and extended warranty this investigation is closed.The VOQ reports cited above can be reviewed at SaferCar.gov under the following identification numbers: 10871612, 10869953, 10810554, 10788238, 10781994, 10778712, 10765025, 10761583, 10759019, 10758622, 10747850, 10735806, 10734761, 10733519, 10715339, 10703521, 10676517, 1065Read more

... plaints received after PE14-033 was opened found that 85 percent of the vehicles with diagnostic trouble codes available to identify the faulty component had a code indicating torque sensor failure (DTC 53).In its response to ODI's information request letter for PE14-033, Honda identified two manufacturing process conditions that may have affected the quality of some early production torque sensors.Analysis of warranty returns and manufacturing processes determined that a relatively small number of torque sensors were potentially affected by the conditions, which were corrected by the supplier (Bourns) relatively early in production.Analysis of warranty data indicates that both conditions were early-life failure concerns and most of the affected sensors have already failed, resulting in a cumulative failure rate of less than 0.2 percent.Of the 20 VOQ's that appear to be related to torque sensor failure, 6 reported failure dates in 2013, 12 in 2014, and just 2 to date in 2015.ODI's analysis identified 2 minor crashes with no injuries that may have been related to torque sensor failure in the subject vehicles.This investigation is closed because the low failure rate and a declining trend indicate that failures are rare.The closing of this preliminary evaluation does not constitute a determination that no defect exists or that power steering failures do not present an unreasonable risk to safety. Vehicle Owner Questionnaire (VOQ) 10329383 alleging ineffective braking performance of the Honda Accord Hybrid vehicle while driving over rumble strips along side of the highway and simultaneously attempting to the brake.The petitioner alleges this began a chain of events, where the vehicle accelerated uncontrollably, the passenger attempted to stop the vehicle by pulling the parking brake and eventually the vehicle crossing over into traffic flowing in the opposite direction.The result was a crash fatally injuring the passenger and severely injuring the petitioner and two occupants of another vehicle. During the defect petition review, ODI analyzed complaints involving similarly equipped Honda Accord Hybrid vehicles and first and second generation Honda Civic Hybrid vehicles, reviewed data provided to ODI by Honda in response to an information request letter and performed testing of braking performance of a 2005 Honda Accord Hybrid vehicle at NHTSA's Vehicle Research and Test Center in East Liberty, Ohio.Based on this review, ODI does not believe additional investigation is likely to result in a finding that a defect related to motor vehicle safety exists.Therefore, in view of the need to allocate and prioritize NHTSA?s limited resources to best accomplish the agency's safety mission, the petition is denied.This action does not constitute a finding by NHTSA that a safety-related defect does not exist.The agency will take further action if warranted by future circumstances.For more information, see the Federal Register notice in the DP10-004 file (www-odi.nhtsa.dot.gov/defects).Read more

Removal of the ignition key when the gear selector of a vehicle with an automatic transmission has not been shifted to the Park position can allow the vehicle to roll away, increasing the risk of a crash.Honda's dealers will be instructed to inspect the ignition interlock switch and, if necessary, replace the original interlock pin and lever within the ignition switch and replace them with new, redesigned parts.This investigation is closed.Read more

THIS PE WAS OPENED BASED ON 10 CONSUMER COMPLAINTS IN THE ODI DATABASE ALLEGING THAT THE POWER ASSISTED SEATS WERE FAILING IN CERTAIN 1998-2002 HONDA ACCORD VEHICLES.FAILURES ARE CONCENTRATED IN THE 1998-99 MODEL YEAR VEHICLES.THE BRACKET THAT ATTACHES THE SEAT BOTTOM TO THE SEAT BASE IS FRACTURING

DUE TO: 1-THE WELD CONDITION OF BRACKET, 2-THE DRIVERS WEIGHT, AND 3-THE IMPACT ON THE SEAT WHEN THE DRIVER ENTERS THE VEHICLE AND SITS DOWN.OVER TIME THE BRACKET THAT ATTACHES THE SEAT MAY FAIL AT THE LEFT FRONT CORNER OF THE SEAT BOTTOM.THE OTHER 3 POINTS OF ATTACHMENT OF THE SEAT BOTTOM TO THE SEAT BASE STAY ATTACHED AND THE SEAT MOVEMENT IS LIMITED.THE ATTACHMENT OF THE SEAT BOTTOM TO THE SEAT BACK DOES NOT FAIL.THE CONDITION IS MOST APPARENT DURING RAPID ACCELERATION CAUSING THE SEAT TO TILT REARWARD.ALTHOUGH A FEW COMPLAINTS REPORT CONCERN OVER LOSS OF VEHICLE CONTROL, NO LOSS OF CONTROL INCIDENTS, CRASHES OR INJURIES ARE REPORTED.IN RESPONSE TO CUSTOMER COMPLAINTS, HONDA WILL PROVIDE OWNERS OF 1998 AND SOME 1999 ACCORDS A NO-COST REINFORCEMENT FOR THE SEAT AT THE LEFT FRONT CORNER.A SAFETY-RELATED DEFECT HAS NOT BEEN IDENTIFIED AT THIS TIME AND FURTHER USE OF AGENCY RESOURCES DOES NOT APPEAR TO BE WARRANTED.ACCORDINGLY, THIS INVESTIGATION IS CLOSED.THE CLOSING OF THIS INVESTIGATION DOES NOT CONSTITUTE A FINDING BY NHTSA THAT A SAFETY-RELATED DEFECT DOES NOT EXIST.THE AGENCY WILL TAKE FURTHER ACTION IF WARRANTED BY THE CIRCUMSTANCES.Read more
