In August and September 2025, the vehicle was serviced for exhaust/turbo-related gaskets and clamps and a misfire condition. ... uently required. On July 28, 2026, an exhaust leak near the turbocharger was diagnosed. While operating the vehicle, I experienced headaches, dizziness, chest discomfort, and irritation of my eyes and nose. I was concerned that exhaust fumes could enter the passenger compartment and impair my ability to operate the vehicle safely. I sought medical care for a headache on July 29, 2026. I have not received medical confirmation of carbon-monoxide poisoning. The exhaust leak was repaired in August 2026, but on August 17, 2026, the leak appeared again in the same location. The condition has therefore been reproduced and confirmed through service inspections and has recurred after repair. The vehicle and its repair records are available for inspection upon request. Previous symptoms included a check-engine light, misfire, knocking, sluggish acceleration, and exhaust-related concerns. ... ed within the high-pressure fuel system. After notifying BMW Fort Walton of these findings, I was advised that my VIN is not included in NHTSA Recall 21V-586: High Pressure Fuel Pump. I am concerned because the metallic contamination found in my vehicle appears similar or identical to the high-pressure fuel pump failure conditions addressed by Recall 21V-586. The contamination may also have caused consequential damage to the fuel injectors and other associated fuel-system components. I respectfully request that NHTSA review whether my vehicle, and potentially other BMW X5 xDrive35d vehicles currently excluded from Recall 21V-586, may be experiencing the same or substantially similar high-pressure fuel pump defect. Replacing only the failed injectors without identifying and correcting the source of metallic contamination could leave debris in the fuel system and potentially result in additional fuel-system failure. BMW Fort Walton has advised me that my VIN is outside the current recall population. This indicates that my specific vehicle was excluded from the original manufacturing batch of 13,004 affected vehicles. However, given that multiple fuel pumps are being rapidly destroyed by localized fuel delivery pressure failure, it is clear that the scope of Campaign 22V673000 was drawn too narrowly by Volkswagen Group of America. I am requesting that the NHTSA review this complaint to investigate an expansion of the fuel tank recall to include this vehicle block before an unaddressed tank defect causes a catastrophic highway stall. Hyundai Techline was contacted and advised the dealer to not replace any parts, hold the vehicle, and place the customer in a loaner. Hyundai is aware of the problem and working on a fix. As of September 10, 2026, the dealer identified that a campaign was launched on August 28, 2026 to update the MCULDC module to correct code P0C381C. The dealer confirmed the ROM ID matches the TSB and the software update is ready to apply at the dealership. However, Hyundai Techline has not responded to the open tech case to authorize the update, leaving the vehicle stranded at the dealer. This issue is consistent with Hyundai Recall 301 (NHTSA Campaign 26V308), issued May 2026 for 54,337 Elantra Hybrid vehicles for the same HPCU MOSFET overheating defect producing identical warning messages and symptoms. The Palisade Hybrid uses the same Hyundai Mobis-supplied HPCU platform. The fix exists, the dealer is ready to apply it, and Hyundai Techline authorization is the only thing preventing resolution.Read more

On September 1, 2026, my 2022 Toyota Tundra experienced a serious loss of accelerator response while I was leaving a gas station and entering traffic. I pressed the accelerator, but the truck provided effectively no acceleration, leaving me partially in the roadway. An approaching vehicle had to swerve into the opposing traffic lane to avoid a collision. I had experienced a similar low-speed loss
... of accelerator response within the prior two weeks. On September 2, while traveling on I-70 at approximately 70 mph, the truck experienced a separate loss-of-power event. Engine RPM dropped considerably and the vehicle felt as though it were coasting. I pressed the accelerator firmly, but it did not respond normally for approximately 5-10 seconds before power returned. Radar/adaptive cruise control was not engaged. I immediately took the truck to Western Slope Toyota. Technicians checked for codes, performed multiple road tests and recorded vehicle data but could not reproduce the intermittent failure. The issue was escalated to Toyota technical assistance under TAS Case 262460664. This problem has been documented by Western Slope Toyota since 2023. Toyota's applicable TSB repairs were performed on prior occasions in an attempt to correct the throttle/hesitation problem, but they did not resolve it. In 2025, the dealer again documented the condition after the earlier TSB had already been performed. According to the September 2026 repair documentation and my discussion with the Western Slope Toyota service manager, Toyota's specialist technical team is aware that the condition can remain despite the TSB work. The repair order states that Toyota identified a known hesitation concern that remains under investigation and that there was no repair to be made at that time. The condition is intermittent and unpredictable and can cause loss of accelerator response or motive power at low and highway speeds. I believe it creates a serious crash risk. During the most recent incident, the vehicle again began idling very roughly and then stalled while being driven. The engine was restarted but stalled again. After another restart, shifting the vehicle into Drive caused the engine to stall again. A tractor-trailer was traveling behind the vehicle during the incident, creating a serious risk of a rear-end collision. The truck driver was able to recognize that the vehicle was having a problem and avoided a collision. There were no warning lights or messages on the instrument panel during the most recent incident. The vehicle is currently at a Kia dealership for inspection. The dealer has not identified the cause and is retaining and road-testing the vehicle in an attempt to reproduce the intermittent failure. I recently noticed that the metal lip and outer cap area of my fuel filler neck is rusting. This isn't just a cosmetic issue this is the exact surface where the gas cap needs to seal to keep the fuel system airtight. The vehicle was taken to an authorized Toyota dealership for inspection. The dealership was unable to identify a mechanical problem and reset the vehicle's computers. I was told the reduced fuel economy could be related to the computers relearning my driving habits. After the vehicle was returned to me, the loss-of-power/acceleration condition occurred again, so the issue remains unresolved. There have been no warning lights or dashboard messages associated with the loss of power. The specific component or cause of the malfunction is UNKNOWN. Kia's recall documentation identifies illumination of the oil pressure warning lamp as one of the potential symptoms/consequences of this defect. Despite the continuing symptoms, including the oil pressure warning light and sputtering at approximately 3,000 RPM, the problem has not been repaired. The dealership has now stated that Kia is requesting oil-change records and removal of the valve cover for further diagnosis. We perform our own oil changes. We were told that without service records the engine claim may be denied and that we could be responsible for approximately $878 in diagnostic charges if Kia denies coverage. I am concerned about operating the vehicle because the engine continues to malfunction and display the oil pressure warning despite being evaluated by an authorized Kia dealership. Rivian later stated it could not reproduce the concern. In March 2026, Rivian documented Limited Performance while driving, the vehicle coming to a stop, and multiple restarts before it would move. On September 4, 2026, the vehicle again entered Limited Performance and would not exceed approximately 20 mph. After a restart, it operated normally. Rivian received the vehicle for service on September 7. On September 9, Rivian stated that initial testing did not duplicate the Limited Performance telltale, that it had no confirmed repair, and that it proposed an extended approximately 100-mile test drive to try to reproduce the issue. I retained photos and videos of additional events involving reduced speed, warnings, restarts, and inability to move. The intermittent loss or reduction of propulsion while driving can affect my ability to maintain safe speed and creates a potential crash risk. In [XXX], the check engine light illuminated and the truck began malfunctioning. The vehicle subsequently stalled again. The problem continued after returning to [XXX]. A Chevrolet dealership in [XXX] inspected the vehicle and reproduced/confirmed a problem involving the camshaft position sensor wiring/connector. The dealership demonstrated the problem by manipulating the connector/wiring and recorded a video of the diagnostic procedure. The vehicle is currently at the Chevrolet dealership and is available for inspection. The affected component should also be available for inspection because it has not yet been repaired. The [XXX] dealership has characterized the wiring/connector condition as normal wear. However, the first stall occurred immediately after the GM recall engine replacement, while the vehicle was still on the repairing Chevrolet dealership's property. Unexpected engine stalling creates a serious safety risk because the vehicle can lose propulsion without warning while driving, including at highway speeds or in traffic. I am concerned that the camshaft position circuit/wiring/connector problem may be associated with the recall engine replacement procedure or replacement engine. Because of this contamination, the shop determined that the entire fuel system required replacement, including replacement of all eight fuel injectors, cleaning of the fuel tank and fuel lines, replacement of the fuel filter, and installation of an upgraded diesel injection pump. The estimated repair cost exceeded $11,000, resulting in a significant financial burden. I understand that Ford Safety Recall 24S78 addresses a high-pressure fuel pump issue on certain 2020–2022 Ford Super Duty trucks equipped with the same 6.7L Power Stroke diesel engine. Although my 2019 model is not included in the recall, my vehicle experienced what appears to be a very similar failure that resulted in sudden loss of engine power while driving and complete contamination of the fuel system. I am not asserting that my vehicle is covered by the current recall. Rather, I respectfully request that NHTSA investigate whether the same or a substantially similar defect may also affect certain 2019 Ford F-250 Super Duty vehicles equipped with the 6.7L Power Stroke diesel engine. The overwhelming gasoline odor and visible vapors occurred again. On 9/8, at approximately 108°F and with 152 miles to empty, I drove 46 miles, idled 27 minutes, then drove another 10 miles. The overwhelming raw gasoline odor and visible vapors again came from the fuel-filler area. A strong gasoline odor was also present underneath/around the driver's-side rear wheel well, but not around the passenger-side rear wheel. This recurring release of gasoline vapors presents a potential fire and occupant/public safety concern because gasoline vapors are flammable. I have kept the vehicle outside after incidents rather than parking it in an enclosed garage near an ignition source. The vehicle is available for inspection. The vehicle was previously inspected by an authorized Lexus dealer for this same concern, and Lexus Brand Engagement opened a case and technical-assistance ticket. The dealer reported no fuel leak and deemed the vehicle safe to drive but did not reproduce the condition. The vehicle was driven only approximately 3 miles at the dealership under different conditions. The problem has since recurred multiple times. There have been NO warning lights or dashboard messages. These vehicles are dangerous because there is no warning when they stop working. They just stop - sometimes in traffic or the middle of the street. With 68C, we were led to believe that only some batteries would have issues and that the installed software would monitor battery health. What we began to see was the batteries were structurally deficient and ALL batteries would fail at some point. The software was designed to shut the vehicle down if it detected a fault. Post 68C recall, many owners experienced a decrease in the performance of their Jeeps as the internal computer sought to balance battery load. This meant we weren't getting the MPG advertised and promised. We also received a warranty extension of unlimited miles and unlimited years. BUT, the fine print says the battery will be replaced ONLY if the value of the vehicle is more than the cost to replace the battery. My jeep has lost $14,000 in value in less than a year while I put less than 10,000 miles on it. The dealer has had my vehicle for a month and confirmed the hybrid battery is the problem. They have ordered a new one, but the new ones have the same defect. Owners are also seeing additional electrical faults after a hybrid battery replacement.Read more
