When I’m driving with the A/C on when I start to decelerate to a stop at a stop sign or light, my car lurches forward. It feels like I’m going to crash into the car in front of me, but I’m not sure how far it actually moves. It happens more in the summer months (more than once per week) when the A/C is on. On XXX my 2023 Toyota Tundra Limited’s check engine light came on and after consulting with my service center, Jones Toyota in Bel Air, MD, via phone I brought the truck into Jones to be assessed. My vehicle is also subject to active safety Recall Campaign 25TA14 for manufacturing debris in the V35A engine block, but the dealership and manufacturer are treating the turbo failure as a separate repair item and have not linked it to the recall in writing. Operating a twin-turbo V6 platform experiencing sudden propulsion and turbo loss—while an open, un-remedied main bearing engine recall remains pending on the same vehicle—presents a severe underlying safety and mechanical failure pattern putting anyone in the vehicle or on the road with the vehicle when its being operated at riskRead more

On September 1, 2026, my 2022 Toyota Tundra experienced a serious loss of accelerator response while I was leaving a gas station and entering traffic. I pressed the accelerator, but the truck provided effectively no acceleration, leaving me partially in the roadway. An approaching vehicle had to swerve into the opposing traffic lane to avoid a collision. I had experienced a similar low-speed loss
... of accelerator response within the prior two weeks. On September 2, while traveling on I-70 at approximately 70 mph, the truck experienced a separate loss-of-power event. Engine RPM dropped considerably and the vehicle felt as though it were coasting. I pressed the accelerator firmly, but it did not respond normally for approximately 5-10 seconds before power returned. Radar/adaptive cruise control was not engaged. I immediately took the truck to Western Slope Toyota. Technicians checked for codes, performed multiple road tests and recorded vehicle data but could not reproduce the intermittent failure. The issue was escalated to Toyota technical assistance under TAS Case 262460664. This problem has been documented by Western Slope Toyota since 2023. Toyota's applicable TSB repairs were performed on prior occasions in an attempt to correct the throttle/hesitation problem, but they did not resolve it. In 2025, the dealer again documented the condition after the earlier TSB had already been performed. According to the September 2026 repair documentation and my discussion with the Western Slope Toyota service manager, Toyota's specialist technical team is aware that the condition can remain despite the TSB work. The repair order states that Toyota identified a known hesitation concern that remains under investigation and that there was no repair to be made at that time. The condition is intermittent and unpredictable and can cause loss of accelerator response or motive power at low and highway speeds. I believe it creates a serious crash risk. The rear power liftgate hinges on my 2021 Toyota Sienna failed structurally under normal, everyday operation. The power liftgate motor and struts appear to exert uneven or excessive pressure on the rear hatch assembly, causing the metal hinges to crack. Before the failure, we noticed a loud popping/grinding sound when opening the hatch, which quickly progressed to the liftgate binding and misalign
... ing. This is a severe safety hazard because the structural failure prevents the heavy rear hatch from closing or latching securely, creating a risk that it could fly open while driving, dropping cargo onto the roadway or exposing passengers. It also creates a risk of the heavy door falling unexpectedly on someone loading the vehicle. The vehicle had to be taken to a dealership body shop for structural repairs due to the cracked metal, costing me over $1,500 out of pocket. Toyota has not issued a recall or good faith reimbursment for this known design defect.Read more

2012 Toyota Prius v with 89,662 miles. Multiple brake warning lights illuminated, and the vehicle stored diagnostic trouble code C1391, “Abnormal Leak in Accumulator.” The brake booster/accumulator pump is also cycling about every 13 seconds, even when the brake pedal is not being applied. The vehicle and components are available for inspection. My safety concern is that this involves a criti
... cal braking-system component. When the warning lights are on, regenerative braking is disabled, and braking behavior changes. I am concerned that continued deterioration could reduce braking performance or result in loss of brake assist. The issue has not yet been formally diagnosed by a Toyota dealer. I scanned the vehicle with a diagnostic scan tool, which currently shows C1391. I also contacted my independent repair shop, which recommended that Toyota diagnose the vehicle because this code is associated with the brake booster/booster pump failure previously addressed by Toyota Customer Support Program 20TE07. The vehicle has not yet been inspected by Toyota, police, insurance, or another manufacturer representative. A Toyota corporate case has been opened and escalated. There were no known brake warning lights before this incident. The problem first became apparent when multiple brake warning lights illuminated. Afterward, I noticed the brake booster pump cycling unusually often. Toyota previously identified C1391 as a qualifying code under 20TE07 for certain 2012–2014 Prius v brake booster/booster pump failures. My vehicle is outside the time-based coverage period despite relatively low mileage. I am reporting this because I am concerned this safety-related failure may continue occurring in similarly aged vehicles.Read more

transmission has failed. Toyota states: Toyota Technical Service Bulletin T-SB-0008-21 addresses a whining or grinding noise in select 2021 Toyota vehicles equipped with UA80E orUA80F automatic transmissions caused by a defect in the front carrier assembly pinion shafts. Yes it can be available for inspection. This can be repaired with a $12,000 new transmission. * Sudden loss of acceleration: Y
... ou may press the gas and the vehicle doesn’t accelerate normally. This is particularly dangerous when merging onto a highway, crossing an intersection, or passing. How this toyota highlander is putting safety at risk: * Unexpected shifting or jerking: A failing transmission can slam into gear, hesitate, or shift unpredictably, potentially causing loss of control. * Loss of drive: The transmission may suddenly stop transferring power to the wheels. You could be unable to move forward even though the engine is running. * Getting stuck in a gear: If it goes into a “limp” or failsafe mode, the vehicle may be restricted to a single gear and have very limited acceleration. * Loss of power while already moving: If the transmission disengages unexpectedly, you can lose the ability to accelerate out of a dangerous situation. * Difficulty maintaining speed: On a highway or uphill, a failing transmission may suddenly stop providing adequate power. * Transmission fluid leaks: A significant leak can cause the transmission to lose lubrication and worsen rapidly. Depending on where fluid goes, it can also create other mechanical problems. * Unexpected engine RPM increases: The engine can rev very high without the Highlander gaining speed (“slipping”), which can leave you unexpectedly slow in traffic. * Being stranded in traffic: A complete failure can leave the vehicle immobile, creating a collision hazard if you’re stopped in a travel lane or on a shoulder. unknown if it has been inspected by anyone. There is no warning lights. Then only way you know is by a high whining sound.Read more

I am experiencing a strong gasoline odor & what appear to be gas vapors coming from the gas cap/filler area of my vehicle. A daily problem. Fumes are consistently present & enter the cabin, when the air conditioning is not on recirculation mode or when doors are opened. I can see and smell it when stopped & idling & I can visually see it when driving. Toyota has already performed multiple inspecti
... ons & safety checks but has been unable to identify the source. A vehicle health check found no diagnostic trouble codes. The EVAP system was pressure tested & passed w/ no leaks found. Fuel lines, fittings, injectors & the high-pressure fuel pump were also inspected & all reportedly passed. The inability to identify the source does not mean the condition does not exist—it demonstrates that the current testing has not identified the source. I should not be expected to continue transporting my family in a vehicle w/ an ongoing gas vapor concern simply because standard diagnostic testing has not located the problem. My greatest fear is that an unidentified source of gas vapors could create a serious fire or explosion hazard. Gas vapors are highly volatile & when present in sufficient concentrations & exposed to an ignition source, can ignite. I continue to smell gasoline & detect what appear to be vapors around & inside the vehicle every day, I am afraid to drive it & constantly worry about the possibility of a fire or explosion while my children are inside. Breathing in these fumes are extremely toxic. My greatest concern is for my three children. Two are under the age of [XXX] & cannot communicate whether they are experiencing headaches, dizziness, nausea, respiratory irritation, or other symptoms. My [XXX] rides in the third row, closest to the rear area where I am detecting these vapors. I purchased this vehicle to safely transport my children. Instead, I no longer feel safe transporting my family in it. This is an ongoing, unresolved safety concern. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIRead more

2020 Toyota Camry XSE, VIN [XXX], 2.5L 4-cylinder A25A-FKS engine, built at Toyota Motor Manufacturing Kentucky (TMMK) during the production window covered by NHTSA Recall 20V-064 (Toyota campaign 20TA04), which addresses porosity in engine castings that can cause internal and external coolant leaks, engine overheating, and possible engine failure. Toyota’s VIN lookup returns this vehicle as not
... affected by the campaign, but the failure pattern matches the recall defect exactly. Timeline of failures and repair attempts: XXX, at approximately 92,832 miles, Camelback Toyota in Phoenix, AZ (RO XXX). Check engine light active. Diagnosed DTC P268115 (Engine Coolant Bypass Valve Circuit, Short to Battery or Open). Battery also failed load test. Overheating symptom noted on the same repair order. Coolant bypass valve replacement recommended. Following that visit, water pump and thermostat were replaced at Berge Toyota (Mesa/Gilbert, AZ area). Camelback Toyota stated the vehicle was fixed. Vehicle overheated again the next day after being returned as repaired. XXX, at 93,034 miles, Camelback Toyota (RO XXX). Oil and filter change performed under good faith goodwill service, transmission fluid exchange completed. Overheating condition persisted. Vehicle continued to overheat through the summer and into September 2026. Current dealer now recommends head gasket replacement and states that if the head gasket does not resolve the overheating, the engine block is cracked and the engine will need replacement. Dealer describes the engine as “sucking air,” indicating combustion gases entering the cooling system and/or a compromised block-to-head seal. This failure sequence — persistent overheating, multiple failed repair attempts across two Toyota dealers, replacement of water pump, thermostat, and coolant bypass valve without resolution, and dealer conclusion pointing to a cracked engine block — matches the failure mode described in NHTSA Rec INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)Read more

I am requesting that NHTSA investigate Toyota's compliance with recall remedy timelines and follow up on the status of parts/software availability for this recall.Read more

The vehicle is experiencing a critical failure of the Brake Actuator and Booster Pump assembly. The dashboard currently displays multiple warning indicators: ABS light, Traction Control light, and the Brake System Warning light (Master Warning "!"). Symptoms: Mechanical Failure: The internal high-pressure accumulator is failing to hold pressure. The electric brake booster pump is heard cycling e
... very 10-20 seconds even when the vehicle is stationary and the brake pedal is not depressed. Loss of Braking Safety Features: The system has disabled regenerative braking and Anti-lock Braking (ABS), causing significantly increased stopping distances and a “spongy" brake pedal feel with diminished braking response. Predictability of Failure: This failure matches the exact symptoms described in Toyota's Customer Support Program 20TE07 (and previously ZJB), which acknowledges an internal defect in the brake actuator. Safetv Risk: The sudden transition from power-assisted braking to manual hydraulic braking during a drive creates an immediate safety hazard. As the pump motor continues to cycle excessively to compensate for the internal leak, it is at risk of total thermal failure, which would lead to a near-total loss of braking assist while the vehicle is in motion. Despite the vehicle having only 75,689 miles (well under the 150,000 mile limit acknowledged in Toyota's support programs), Toyota has declined to provide assistance because the vehicle has surpassed the 10-year age mark. Toyota is wrong in not providing support for vehicles whose critical components such as brakes shouldn’t experience catastrophic internal failure at 75,689 miles, regardless of the vehicle's age.Read more

Over the past two weeks the steering effort on my 2nd generation Tacoma has become progressively harder to turn the wheel past certain points in both directions. I initially thought it was a steering pump problem, but based on reporting on the TacomaWorld site, it appears very likely this is a problem with the universal joints at the bottom of the Steering Intermediate Shaft. Their site shows th

The vehicle intermittently experiences a loss of engine power or significant hesitation when accelerating from a stop or from a slow speed. This has occurred multiple times, including when accelerating from a complete stop and after making a low-speed turn. This creates a safety risk when pulling into traffic, crossing an intersection, turning into traffic, or accelerating from a stop or slow spe

My wife is scared to take the car after a close call when the car would not accelerate. The vehicle has been taken to the dealership multiple times for evaluation. However, the dealership has been unable to reproduce the condition and has returned the vehicle without repair. The issue remains unresolved and continues to occur intermittently. The dealer said they are seeing this in other Toyota Crowns and they don't have fix unless they can reproduce the problem. Multiple people have been in the car and experience the problem but the dealer simply say "we can't repeat it".Read more
