Incident date: Jul 13, 2026
Car was started on afternoon of XXX and the Low Coolant alert sound and dashboard light came on. Opened hood and car was completely out of coolant. Took to Royal Automotive, Vestavia Hills, AL on Wednesday XXX to be fixed. Dealer assessment was “PERFORMED COOLING SYSTEM LEAK TEST. THE VEHICLE
... PASSED TEST. CHECK FOR VISUAL LEAKS. FOUND NO LEAKS, BUT FOUND SEEPAGE AT THE HEAD GASKET. VOLKSWAGEN SAID THIS WAS NORMAL PER 00-26-01 - DIFFERENCE BETWEEN FLUID LEAKAGE AND SEEPAGE. TOP OFF COOLING SYSTEM.” The car was purchased new by me in October 2022 and my warranty expires in October 2022. It has just over 17,100 miles on it. Further, Volkswagen published a Technical Service Bulletin 15-23-01 - Head Gasket Leakage on XXX documenting known problems with head gaskets for my car’s engine. Despite dealership stating the head gaskets is leaking, Volkswagen stated to the dealership it doesn’t warrant repair. I believe Volkswagen should repair at their expense the defect of their vehicle. The dealership acknowledged the head gasket is clearly “seeping” coolant. Coolant shouldn’t be leaking at all from a closed loop system. This is causing a potential safety issue for my [XXX] daughter who drives the car, as when the head gasket fully fails as documented by Volkswagen, the car could be in traffic and put my daughter’s life and other peoples lives in jeopardy from a blown engine or an overheated engine. It’s unacceptable that Volkswagen’s direction is for us to carry around a gallon of coolant and constantly check the coolant levels because they don’t want to fix their known defect. I called Volkswagen customer care and complained. They refused to assist, stating they stand by the dealership’s assessment. I reminded them that the dealership was doing what Volkswagen directed, but that didn’t change their decision. I asked for escalation, was told it was escalated twice and that their decision is not changing. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6Read more

At only 24,000 miles, the rear brakes are already showing significant wear. When checked by the dealer, they were 2mm, while the front brakes were 12mm. The service representatives at the dealership have said this is a known problem with this model year, and they have already seen several cars with

I would like to report a potential safety issue regarding Tesla Full Self-Driving (FSD), specifically version 14.3.2. Following any driver intervention (such as braking or steering override), the system presents a prompt asking “Why did you intervene?” with selectable options (e.g., Navigation,
... Discomfort, Preference, Critical) or voice input. In my experience, this prompt does not automatically dismiss and requires manual interaction with the touchscreen to clear. This design creates a safety concern because: - It introduces a mandatory driver interaction immediately after a disengagement event. - It diverts driver attention away from the road during a potentially critical moment. - It may encourage rushed or inattentive responses just to clear the interface. Driver intervention moments are often associated with complex or unsafe driving situations, where full attention is essential. Requiring touchscreen interaction at that time may increase the risk of distraction-related incidents. I believe this behavior could represent a human-machine interface design issue that impacts driver safety. A 2021 Buick Encore GX with 66,828 miles experienced sudden transmission failure while driving. The vehicle lost forward gear engagement and began slipping, creating an immediate safety risk to the driver and other motorists due to unexpected loss of vehicle propulsion and control. The vehicle was
... diagnosed by an authorized GM dealership. Diagnostic codes P2714 (Transmission Control Solenoid Valve 4 Stuck Off) and P0700 (Transmission Control Module Request) were confirmed. The transmission fluid was found to have a burnt odor indicating severe internal damage. The check engine light was the first warning symptom, followed by slipping and loss of forward gear. General Motors previously issued Technical Service Bulletin 24-NA-009 in March 2024 specifically identifying DTC P2714 on the 2021 Buick Encore GX as a known internal defect affecting model years 2020 through 2024. This bulletin confirms GM was aware of this defect prior to this vehicle's failure. Despite issuing this bulletin, GM has not issued a recall, leaving owners of affected vehicles responsible for repairs caused by a known manufacturer defect.Read more

I am less than 8 months into a brand new vehicle that has had and has major safety recalls with no remedy in sight. I bought this car for safety and driving it while seatbelts and ejection mitigation have all failed NHTSA standards is horrifying for me and my family.
Incident date: Mar 21, 2026
The base failed - the mechanism holding the seat into the base is .5 inch plastic spinner latches and if they fail and don’t deploy fully (which the green check mark shows), then it would be so unsafe and not connected to the car at all. This is what’s happening with my son’s car seat. It does
... n’t indicate green and I have to jiggle the entire thing to get it to be green instead of red. Sometimes it sits half way between red and green. Also, the chest clip failed to connect after I had thought that I put my child in it properly, he touched it and un clipped himself from the seat, if I would not have noticed and kept driving, he could’ve been projected from the seat. If the seat is not attached to the base properly, it could eject from the seat in the event of an accident, which would be fatal. I am so unimpressed with this car seat, and I expect them to recall it. When trying to get the seat out to clean it, it was stuck and the detach key would not make the click sound and disconnect the seat from the base. Even with following the instructions exactly. The seat does not sit well in a Tesla; the seatbelt folds over itself. Also, while driving home today we noticed for the first time from the inside of the seat one indicator was red, and one indicator was green.Read more

1. The 3.5L V6 Engine (2GR-FE) failed due to the subsequent loss of lubrication caused by the rupture of the Oil Cooler Pipe (Rubber component), a manufacturing defect known under TSB T-SB-0201-11 (ZE2 Program). The failed engine is currently available for inspection by request at the independent se
... rvice center before it is disposed of. 2. The defect causes a sudden and severe loss of engine oil. This creates a high probability of the engine seizing (losing all power) while driving at highway speeds, resulting in a complete loss of propulsion and placing occupants at severe risk of collision. 3. The failure was confirmed, not reproduced. The vehicle was initially diagnosed with misfire codes (P0300, P0301). New spark plugs and ignition coils were installed in an attempt to resolve the issue. As the misfire persisted, the mechanic performed a compression test, confirming catastrophic zero compression failure in cylinders 1 and 2. Professional diagnostics then revealed critical codes: P1603 (ECM Malfunction), P1604 (Startability Malfunction), and P1605 (Rough Idling). These codes confirm a severe internal engine issue, consistent with permanent damage sustained during the oil loss event caused by the ZE2 pipe defect in July 2024. 4. No, not physically inspected by the manufacturer. The manufacturer (Toyota Executive Office Case Manager) was provided with certified documentation of the failure, including the photo of the faulty pipe, maintenance records during our ownership, and the dealership purchase statement for the replacement part. Toyota refused to authorize a physical diagnostic inspection of the failed engine at a dealership before denying goodwill assistance. 5.Yes. The initial lubrication loss event on July 4, 2024, was preceded by a message displayed on the dashboard (followed by a second failure event in October 2025 indicated by a flashing Check Engine light and the Traction Control light).Read more

... r, moved forward. I released the brake slightly but my foot was still on the brake pedal. At this moment, my car suddenly accelerated and quickly approached the Explorer. I immediately pressed the brake pedal hard but was unable to stop my car and hit the back of the Explorer. The Explorer was forced out in front of the stop sign but the driver was able to turn right to park at the roadside. With my foot still firmly on the brake pedal, my car kept on accelerating into [XXX] , crossing 5 lanes of traffic until we hit the roadside curb of westbound [XXX] . The car then swerved left and hit a roadside tree, turning sideways into [XXX] middle turn lane. During this entire time, I did not press the accelerator and the car was violently shaking and jerking right and left. As the car slowed down, I was able to steer the car to the right eastbound lane of [XXX] , next to the parking lot near Bank of America’s ATM sign. After I got out of the car, I noticed the front and right sides of the body and tires were damaged including crushing, dents, and significant scratching. The problem I believe is as follows: 1.Sudden Unintended Acceleration and Control Circuit Malfunction. 2.Airbags did not deploy even though we hit a car, curb and tree. 3.Onboard Data Recorder should have the information needed. UNKNOWN while driving, the electrical system seemed to malfunction causing the lights to flash on/off, the rear windshield wiper to activate, the horn was blaring (quick repeated beeps, with occasional blaring), the dash indicated that the parking brake could not be used, indicated that the airbag s

The car indicated that it needed to be put into park, and allowed for the turning of the dial; however, the dash indicated that the vehicle was moving too quickly to shift into park. We were able to make a turn onto a street with an uphill incline from the road we were driving on; and the brakes stopped the car - using a lot of force as the ABS braking system was still malfunctioning. The vehicle was put into park and the parking brake indicated that it was active; but of course could not be trusted as the indicators on the dash had been so haywire just moments before. While the vehicle was “off” the horn was still blaring (both short beeps and intermittently long blares), the rear wiper was still active (while in the off position), and the interior lights were flickering. This continued until the red connector was disconnected from the battery. The black connector seemed to make no change, connected or not. When the red connector was reconnected, the electrical issues seemed to be replicated with the horn and rear wiper. The vehicle was towed and we have no current answers. This was extremely dangerous as we lost all power and control of the car. We were driving, on what is oftentimes, a very busy street.Read more

The car started shaking while driving down the road and when coming to a stop. The gears were not shifting properly. I took it to a mechanic shop and they told me several different things "could" be wrong, supposedly fixed it, $700 for that. Started shaking and shift gear problems again, took it in

I have seen this be reported online, but apparently it only covers the KIA Model, not the hyundai model. But the ODS Mat/Bladder defect in our passenger seat in our Hyundai Santa Fe 2016. Drops Codes B149000 and B1764 - this causes our air bag light to come on. This flipped on around 70k miles bu
... t because we are the second owner, we have to foot the bill out of pocket. Its asinine to think that we have to replace the entire seat and that Hyundai doesnt see this as a problem for them. Hyundai looked at our car, and again they stated we have to fix it ourselves - or pay them. They confirmed it. The problem is, it shuts our air bags off meaning if we dont foot the $2000-$3000 to replace it, our air bags just wont deploy. Thats a massive security risk. This seems to be a problem with various Hyundai and Kia models, but apparently only SOME models with the same codes get to be replaced. Ridiculous. Attached are various issues by Hyundai, Kia, with the same issue across various models. Here is the link to the same issues in a different model of Kia/Hyundai Cars [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

I got into a car accident today. The electrical wiring is constantly giving out and I had no idea my airbags wouldn't work until today when they did not deploy. My car hit the other car enough to move it several feet and no airbags.
