Defect Description
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2025 RAV 4 vehicles. The front seat brackets for the seat rails may be welded improperly.
Consequence
Improperly welded seat brackets can prevent the seat from adequately restraining an occupant during a crash, increasing the risk of injury.
Remedy
Dealers will inspect and repair the seat bracket welds as necessary, free of charge. Owner notification letters were mailed June 19, 2026. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 26TB09 and 26TA09.

Consequence

... ng any pedal, and shot uphill and over a speed bump, throwing the car off course toward parked cars. I stepped hard on brake, and assisted braking probably also kicked in. Car slammed to a stop. No damage or injuries. I took the car to Toyota dealer service immediately to report and address self-acceleration. Was told Vehicle Control History confirmed an event but not its nature or cause; didn’t find problem. Dealer claimed they reported incident to Toyota national center as UA (Unintended Acceleration), as we requested. Returned the car to me without repair or further diagnostic steps. There was never any follow up by Toyota, local or national. One year later, a similar self-acceleration by this car after a hard left turn at low speed caused life-threatening event and damage. Both incidents occurred while the vehicle was operating with Full Self-Driving (FSD) engaged. During the most recent occurrence, while parking at less than 3 mph, the vehicle’s braking assistance appeared reduced and the brake pedal required significantly more effort than normal. As a result, I nearly struck a curb because the vehicle did not slow as expected. The warnings indicated that increased steering and braking effort may be required. Because the vehicle is brand new and these warnings involve steering, braking, and collision-avoidance systems, I believe this creates a significant safety risk. It sounded like my motor was starving for oil. I decided to go ahead and change my oil to see if that could have been the issue. I had just changed the oil approximately 2000 miles before this. I changed the oil and filter. Went to start it up and still same very loud grinding sound, shut it back off and have not started it since cause I’m afraid to damage it further. I checked the oil that I drained out of it and looks like small metal flakes in the oil. No warning prior to this happening. The next day, Monday the 23rd, I took the car over to James River Ford near my house and had them look at it. They stated that they had never seen this before. All but one of the panels around the sunroof had come off! They looked up recalls for me, but none were on the sunroof panels. Again, I was in shock. How could a car have been designed such that there was the possibility that the ROOF of the car could come off randomly while driving down the highway? I looked online and there was a lot of talk on how these panels are NOTORIOUS for coming off! There are multiple stories of issues with these panels. I purchased this car used and would have preferred to NOT have a sunroof, but I really liked the rest of the car. In fact, the sunroof did not work properly when I first purchased the car, so I chose not to even use it. Now, to have the panels rip off and shatter on the road is extremely alarming and frustrating! Most importantly, I need to know that Ford understands the dangers of these poorly designed panels around their panoramic sunroof! No “special” sunroof design is worth the dangers that these can cause if they come off. I do not see other vehicles around with these types of panels. I reached out to Ford online on March 23rd and April 2nd. Of course, nothing really came of that, which is why I am reached out and sent them a certified letter. The contact's husband assisted the contact in driving, and the vehicle was able to respond after several attempts; however, the contact's husband stated that the vehicle felt like the parking brake was engaged unintendedly. The vehicle was taken to the dealer, where the failure could not be duplicated; however, an unknown part was replaced. The contact stated that while driving 35-45 MPH, the vehicle in front of the contact's vehicle slammed on the brakes. The contact then depressed the brake pedal, but the pedal travelled down and rested on the floorboard. Additionally, the front end of the vehicle lowered while the rear end of the vehicle rose, and the vehicle crashed underneath the rear-end of the truck. There was no warning light illuminated. The air bags failed to deploy. Toyoya Safety Connect system automatically connected the contact to a representative. The contact was transported to the hospital by emergency units. The contact sustained a fractured sternum, three fractures on the back, and the contact was hospitalized for 3 days. The contact's husband later arrived at the hospital and received medical assistance. The contact's husband sustained headaches, neck pain, bruised stomach as the seat belt fractured. The other driver did not sustain injuries. A police report was filed. The vehicle was towed and totaled by the insurance company. The dealer and the manufacturer were contacted. For a year I have had my car to the shop multiple times due to a musty smell inside my car. The car had approximately 40,000 mi on it when this started about a year ago, Maybe 8 months. Recently I have noticed that the coolant in my car is disappearing, I have kept a constant check on it and about once a week have had to add fluids to keep it from getting too low. I have had the coolant system checked out. I've had it flushed. They pressure tested the coolant tank and all tests passed and they could find no issues. However, when my father came to town he checked it out and there is coolant fluid leaking inside my car under the dash and it is dripping slowly on to the underpad of the carpeting and I assume drying up before it gets to a point where it is making a wet spot in the floor. Obviously after more research my father has told me that this has been a known issue with this model of car, however, there is not a recall that I can find with my VIN number. Obviously being stuck in a hot car in Florida, breathing in the fumes from a poisonous liquid is not healthy for me or anyone in my car. This is a safety issue not only for the fumes that I have been breathing, but foggy residue that collects on my windshield creates visual issues especially at night. This could cause me to have a major accident and possibly lose my life. I should not be struggling with a car that I treat like my baby. I have read that this is a design flaw due to the plastic connectors on the hoses, I would like Chevy to stand behind their vehicle and fix this issue for me. The contact later experienced the same failure while driving approximately 60 MPH on the freeway, and the steering wheel erroneously turned to the right, causing the contact to lose control of the steering wheel and causing the vehicle to drive to the right and off the road, nearly crashing into a wall. The contact regained control of the steering wheel and continued driving to the residence. During the drive to the residence, the steering wheel continued to malfunction and erroneously turned to the right. The contact forcibly held the steering wheel while driving. The vehicle was taken back to the dealer, who was unable to identify a failure; however, the dealer deactivated the Lane Departure feature. ... ch 25–26, 2026, while operating the vehicle at approximately 35 mph on a surface street, the following failure sequence occurred: the vehicle began producing a repetitive thudding sensation (thud-thud-thud), followed by violent shaking of the steering wheel, followed by a complete loss of steering response. The vehicle drifted uncontrollably into an adjacent lane of active traffic under clear, dry weather conditions — a loss of control consistent with brake hose failure causing uneven braking force, compounded by the known transmission and PCM defects. Collision with another vehicle was narrowly avoided. The vehicle currently has five open, incomplete recalls: (1) front brake flexible hose leak, (2) rear brake flexible hose leak, (3) transmission shifter cable bushing, (4) rear-view camera inspection, (5) powertrain control module reprogram. None have been completed. The described failure is consistent with and likely caused by these unresolved manufacturing defects. But the Owner has attempted multiple times to bring the vehicle to Ford but was turned away due to dealer capacity. No loaner vehicle was offered.Read more

... e transmission began showing signs of failure at approximately 52,000 miles. Early symptoms included hesitation, harsh or delayed shifting, inconsistent engagement, and warning signs related to drivetrain or transmission performance. I sought service and diagnosis, but despite many similar failures being reported by other owners, no corrective action or goodwill repair was offered by the manufacturer or dealership. I was quoted approximately $9,000 by the dealership for a full transmission replacement. Due to the high cost, I continued operating the vehicle while monitoring the issue. Over time, the condition worsened significantly. As the transmission continued to fail, the vehicle would sometimes move forward and then suddenly stall or lose drive power while in traffic. This occurred while my family was in the vehicle and created a major safety concern, as the truck could unexpectedly lose movement in active traffic lanes, putting us at risk of a collision and leaving us unable to safely accelerate or clear intersections. On January 30, 2026, the transmission failed completely. The vehicle no longer moved when placed in gear, leaving it inoperable and unsafe to drive. Because assistance was denied and no recall or corrective action was available, I was forced to repair the vehicle at an independent repair shop at my own expense. The repair cost is approximately $7,000 out of pocket. This failure occurred at relatively low mileage for a major drivetrain component and appears consistent with numerous reports from other owners experiencing similar transmission issues.
