In addition, occupants may believe the doors are secured when they are not. The problem has occurred on both front doors, indicating this is not an isolated component failure. The issue developed under normal use and was not caused by an accident, abuse, or modification to the vehicle. Multiple Toyota owners have reported similar failures involving door lock actuators in the same generation of vehicles, and there is pending litigation alleging Toyota has long been aware of this defect but has not issued a recall. They said Forward Recognition Camera had 'INTERNAL MALFUNCTION' and needed to be replaced. It was going to be quite expensive, but the camera part is not even available in foreseeable future as there's back order for hundreds and they're still in production! It seems this too is a common safety problem~ We can't understand why there's not a RECALL on this. Toyota took no responsibility for this issue, which is APPALLING as, like the first 'side driver's side mirror failure' was also a safety issue! The whole reason we bought this vehicle was for its safety features! Without the front facing camera that comes with the car in same compartment as rear view mirror resides, we now have NO Pre-Collision Warnings, NO Headlight safety feature, NO RSA (Road Sign Assist System). When attempting to restart the car, the engine would start but then shut off showing a "low power steering" warning message

The panoramic moonroof on my 2022 Toyota RAV4 spontaneously shattered and exploded outward at approximately 2:09 PM on June 21, 2026 while my wife was driving and exiting the freeway. There was a loud bang, followed by glass shards ejecting primarily outward onto the roof with a pattern showing the
... glass failed/bent from the inside out. The outer glass panel broke into multiple large sections with a clean perimeter failure and no visible external impact point or road debris damage. This created an immediate safety hazard: loud distraction while driving at highway speeds, flying glass, and sudden exposure through the roof opening. No warning lights, messages, noises, or prior symptoms appeared. The moonroof had been used normally with no issues. The component (panoramic moonroof glass assembly) is available for inspection. I have a dealer appointment scheduled for tomorrow, June 22, 2026 at 9:45 AM for inspection. The vehicle has not yet been inspected by the dealer, manufacturer, insurance, or police. This matches numerous similar reports of spontaneous panoramic sunroof failures on RAV4 models.Read more

Loss of cellular connectivity simultaneously disables Tesla's roadside assistance and the vehicle's calling capability. The vehicle acknowledges this risk by displaying a yellow caution alert reading "Poor GPS location accuracy — Additional attention may be required" on the primary touchscreen while in motion, with no advance warning before the failure occurs. Reproduced or confirmed by a dealer or service center: Yes. On May 22, 2026, Tesla Service Columbia-Two Notch Rd, 6301 Two Notch Road, Columbia, SC 29223, confirmed this as a known issue. While attempting a slow right turn from a complete stop at an intersection, I heard a loud thunk sound followed by an immediate loss of power steering response. The steering wheel spun freely with little resistance while the front tires did not respond proportionally. I was able to safely pull over.
... Upon restarting the vehicle it produced an abnormal wheezing sound suggesting additional mechanical stress. At the time of the failure the vehicle had approximately 40,000 miles, which I believe represents a premature failure for a safety-critical steering component. The vehicle was brought to a Hyundai dealership for inspection. The dealer pulled diagnostic fault codes, found none, performed a 20-minute test drive, and returned the vehicle stating they could not find anything wrong. No physical inspection of the steering components was performed. The failure condition — a slow full-lock turn from a complete stop — was never specifically replicated during the dealer's test drive. The problem has not been confirmed or repaired by the dealer. The vehicle is available for inspection. My safety and the safety of other drivers was put at risk by the sudden and unexpected loss of steering control. No warning lights appeared prior to or during the failure.Read more

Cylinder head is cracked and leaking oil. The Cylinder head has an active warranty extension but Mazda only applies this to coolant leaks and not oil leaks. The root cause is a casting defect along with a exhaust manifold design issue that imparts undue stress onto the cylinder head causing cracks

The limitation of the warranty to the type of fluid that leaks rather than the failure due to design issues with the exhaust manifold and cylinder head does not make sense given that both components have been redesigned due to the prevalance of both coolant and oil leaks from cylinder head cracks. Problem has been confirmed by Quality Mazda Service departmetn on April 4, 2026 but no warranty coverage extended to repair the failure due to the fluid leak being oil and not coolant. No warnings or messages given prior to leaks. Leaks began at roughly 76,400 miles.Read more

We had it towed to the closet Chevy dealership who had it for 39 days replacing the motor that had blown up, the dealership said that the #1 piston or rod broke and destroyed the engine. The car had only 28k miles on it and there were no motors to be found, they did after 39 days replace the motor and turbo in the car. Mine and her concern is with this immediate loss of power on these types of roads she could've been in an accident or even worse, there has to be answers as to why this happened and what is the long term solution to r his issue. When we were able to safely get out and call 911, there was significant drivers side front end damage around tire area and inside near axle. Looked like something broke inside car, sheered off bottom of tire. Police arrived and had to push vehicle on to shoulder of road. He assessed scene and thought it had to be a mechanicla failure as there was no other vehicle or debris on site. We did contact our insurance company and subsequently it was totaled. We also have Toyota investigating what the cause was if it was a faulty part or mechanical failure. We have not heard back but did receive the "recording of time of crash". The condition developed during normal vehicle use and exposure to rain and environmental moisture. No collision, impact, or external damage occurred prior to the failure. No dashboard warning messages or alerts were observed prior to discovering the issue. The vehicle was inspected by an authorized dealership, which confirmed the taillight failure. The dealership stated the light source is not serviceable and that the entire taillight assembly must be replaced. The manufacturer acknowledged the issue but stated the vehicle was outside the warranty period and offered only a partial goodwill assistance of approximately 20%, leaving an out-of-pocket repair cost exceeding $800. Because these recalls involve potential fire risk and high-voltage system faults, I do not feel the vehicle is safe to operate, especially with children riding in it. The vehicle has remained at the dealership for almost 2 weeks with limited communication. I am currently paying for a rental vehicle out of pocket while awaiting reimbursement from Stellantis, with no clear timeline for repayment or confirmation of continued coverage. Attempts to obtain updates from both the dealership and Stellantis case management have often gone unanswered. I am submitting this complaint to report what I believe to be a manufacturing or design defect involving the sunroof of my Jeep Grand Cherokee. On or around December 1, 2025, I discovered a severe water leak originating from the sunroof area during normal rainfall. Water entered the vehicle interio

The amount of water intrusion was significant and far beyond what would be expected from normal wear. The vehicle has only around 45,000 miles and is regularly maintained. The leak occurred without any visible damage to the sunroof glass and without prior indication of a problem. The sunroof was fully closed at the time. This suggests a defect related to the sunroof sealing system, drainage system, or overall design rather than misuse or external damage. This defect presents a serious safety concern. Water intrusion inside the vehicle creates the risk of: Electrical system damage or failure, Mold and health hazards, Impaired visibility due to water dripping near the windshield, Distraction to the driver while operating the vehicle. In heavy rain conditions, this defect could reasonably occur while driving, creating a dangerous situation for the driver and passengers. As a result of this defect, my vehicle sustained significant interior damage, requiring repairs. I believe this issue reflects a broader defect in the sunroof design or drainage system rather than an isolated incident. I am concerned that other Jeep Grand Cherokee owners may be experiencing similar failures. I am requesting that NHTSA investigate this issue for potential safety-related defects involving sunroof leakage in Jeep Grand Cherokee vehicles. There was only one cryptic dashboard message that "acceleration may be reduced", that briefly appeared and then disappeared, three days before the incident. With no check engine light, the mysterious message was chalked up to possibly a DEF low warning. DEF was added. The vehicle displayed no problems during those three days -handling normally. Then while at interstate speeds approximately 90 miles later, the warning that "acceleration is limited" flashed and the vehicle went into limp mode, shortly thereafter the check engine light illuminated. OnStar read the code - P06DD. The email from them stated the recommended action: "Service within 7 days." The following morning the vehicle was presented to a GM dealership service department. Loud "knocking" noises were originating from the rear of the engine compartment at that point in time. The service department reported the engine needed replacement and did so.Read more

... got to the front of the semi, we reached an exit off the right lane. The Q7 slammed on the brakes - not slowed down, it literally slammed on the brakes. Luckily I reacted to hit the gas pedal to override the cruise control and the pickup truck driver reacted by slamming on his brakes. But a simple pass could have resulted in a disastrous accident solely because of the vehicle. I had some of these issues earlier in my trip, but not to the degree of the above incident. It was most common when passing near any intersection, including exit ramps and emergency vehicle cross-overs. An intersection icon showed up and the vehicle slowed down - sometimes gently, sometimes more forcefully. It appears to be operating within its design parameters. I reported the issue to the dealer, but I am currently across the country. I am awaiting a response. I found the settings to turn off predictive control and am testing the system to see if this works. But if the system is operating as designed, there appears to be a flaw if predictive control is active.Read more
