No accident or damage occurred. I was able to drive the vehicle back to the dealership that installed the fluid mixture without needing the automatic 4-wheel drive feature to prevent damage. The vehicle is at the dealership to address the potentially incorrect fluid that were used and to install the
... Ford recommended fluid. The safety concern is that the Ford dealership followed the guidance of a well known company (BG products) and installed a mixture of fluids that have never been tested in a complex transfer case that has the Torque-on-demand, automatic 4A feature that controls the vehicle's traction during wet/icy conditions. This type of transfer case is not installed in all F-150 trucks, only the upper models like the Lariat or Platinum models. The dealership installed BG 792 plus they mixed in BG 328 additive per a universal guideline published by BG products. This mixture doubles the viscosity of the fluid recommended by Ford, which is Mercon LV (Low Viscosity). The guidance was intended for less complex or manual type transfer cases not the Borg Warner 44-67. The installed mix of BG 792 +328 could cause a slower engagement and inconsistent operation of the Traction-On-Demand system during wet or icy conditions. The Borg Warner 44-67 uses speed sensing and injects low viscosity fluid through portsinto a "wet clutch" pack, and the hydraulic pressure is modulated by a specifically programmed PID loop. The hydraulic system has not been tested with the BG mixtures, that doubles the viscosity of the fluid. The BG 328 additive is specifically designed for friction type clutching plates, and could cause damage to the "wet clutch" pack found in the Borg Warner 44-67. I have notified the dealership and BG products of the misinterpretation of the BG guidance but they have claimed that the fluid mix they used "should be ok". They can not, and/or will not provide the chemistry calculation or testing protocols for the mix used, especially with BG 328 added into BG 792.Read more

The contact owned a 2015 Ford F-150. While driving approximately 10 MPH inside a shopping center parking lot, the contact initiated a turn. Suddenly and without warning, the vehicle experienced a sensation of "gliding" and the contact was unable to maintain control. The vehicle then accelerated unex

The contact referenced NHTSA Campaigns Number: 26V237000 (Power Train) to be the cause of the incident. The front bumper and windshield of the vehicle were heavily damaged. The property damage include a building for a music academy. The contact stated the vehicle stopped immediately upon impact. The Air Bags were not deployed. The vehicle was towed to a insurance lot. The contact did report of any illuminated waring lights prior to the collision. A police report was filed. The contact was the only occupant in the vehicle. The contact was the only party involved in the collision. The contact stated she injured her shoulders, neck, left knee, and back. The contact sustained injuries to her shoulders, neck, left knee, and back and received medical treatment at a hospital and was placed on medication. The dealer was not notified of the failure. The vehicle was not repaired. The manufacturer was not notified of the failure. The failure mileage was 160,000.Read more

The UB80E (Front-Wheel Drive) transmission had a total failure at only 70,000 miles. The vehicle started to make unusual noises with harsh and delayed shifting. No warning lights or messages were displayed at any time. The mechanic who replaced the transmission stated my trans fluid was burned up p

My cost for a rebuilt transmission was $7,700 plus 667.00 for a rear trans mount, as well as the cost of a rental car. I have all repair receipts for inspection. This defect in the UB80E transmission warrants immediate investigation by the NTSB as is a legitimate safety issues for the passengers of these Toyota models.Read more

Toyota made a defective coolant valve in their 2019-2021 cars that leaked spontaneously in these vehicles. Over 950,000 cars have been affected to date, including my 2020 Rav4. They are aware of the issue and made a customer support program for this issue which led to engines overheating/electrical
... code problems, which can cause a severe safety issue if left unrepaired. The program however has a limitation until November 2025 or 100,000 miles whichever comes first. When I was alerted of this, I brought my car to the dealer for service, but they wouldn't replace the part because it wasn't leaking too much at the time to need repair. However, just 4 months after the primary coverage expired, my engine code went off and when I brought my car, the coolant valve turned out to be defective and leaked into the electrical wire harness which then also had to be replaced in addition to the coolant valve. The repair cost was $1200 and both were required to keep the car driving safely and reliably on the road. Because my car had 120,000 miles and the known issue presented after Nov2025, Toyota refuses to cover the service, although it is a known manufacturing defect. Toyota is committing fraud to avoid covering this safety issue. The repair is expensive and if left unrepaired, the cars can spontaneously catch on fire on highways and in parks leading to wild fires and endanger others on the road. The toyota technician specifically informed me the issue was similar to other cars and known and the defect led to the destruction of the wire harness. To avoid a safety recall, Toyota reached a private settlement against class action lawsuit and created a limited CSP to avoid covering the defect in cars like mine to maintain their reliability status in consumer reports so cars like mine with little higher mileage won't be reported. It is fraud. I believe they are skipping on consumer repair bills that consumers on entitled to for their manufacturing defect.Read more

My 2016 Jeep Cherokee AWD began showing a “Service 4WD” warning on the dashboard. At the same time the vehicle developed noticeable drivetrain vibration at highway speeds above approximately 55 mph and gears began taking longer than normal to change. A dealership diagnosed the vehicle and found
... diagnostic code C14A7-97. The technician determined the Power Transfer Unit (PTU) shift motor is binding due to internal wear and that the PTU must be replaced to restore proper four-wheel-drive function. The failure prevents the vehicle from engaging 4WD and causes vibration while driving at highway speeds. The dealer quoted approximately $3,176 to replace the PTU and stated the part is currently on national backorder. This appears similar to the PTU failure addressed in recall 20V343000 affecting other Jeep Cherokee vehicles, but my VIN is not included. Vehicle: 2022 Tesla Model S (HW 3.0) System in Question: Full Self-Driving (FSD) Capability, initially purchased (Level 1 equivalent) for $6,000, and then upgraded (Level 2 equivalent) for an additional $3,000 (Total $9,000). I. Initial FSD Purchase and Unsafe Performance (Level 1) Upon initial pur
... chase of the FSD package, the system (what Tesla later called FSD Beta, or its precursor) consistently demonstrated unsafe and aggressive behavior that necessitated immediate driver intervention to prevent a crash. Specific Safety Concern: The system would often execute turns, both in city and highway off-ramp scenarios, with excessive speed and aggressive lateral acceleration. This erratic behavior felt uncontrolled and risky, frequently causing the driver to feel unsafe and to rapidly override the system by taking manual control of the steering wheel. Resulting Risk: The system was virtually unusable for anything beyond basic highway cruising (a feature standard on many vehicles), yet its aggressive behavior posed a safety risk on the highway when attempting lane changes or taking exits. II. Upgrade and Deterioration of Practical Safety (Level 2) I upgraded to the top-tier FSD package with the hope of receiving a safer, more functional system that matched the "Full Self-Driving" name. Instead, the system introduced new safety and usability defects: Forced Disengagement/Driver Monitoring Defect: The system's driver monitoring is overly aggressive and interferes with safe, necessary driving actions. It delivers constant "nag" warnings and requires disengagement when the driver performs a brief but necessary safety check, such as looking at a side mirror, checking over the shoulder for a lane change, or briefly glancing at the car's screen to adjust climate/radio. Safety Implication: This defect forces the driver to prematurely disengage FSD in complex traffic situations to avoid a warning/lockout, which is counter-productive to safety. I am effectively penalized forRead more
