Consequence

Consequence

Remedy

Defect Description
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2024 RAV4 and 2025 Lexus NX vehicles. Some of the bolts that secure the front brake caliper and the wheel hub bearing may be loose, which can result in a damaged caliper brake hose or possible wheel detachment.
Consequence
A damaged brake line can cause a brake fluid leak, reducing brake performance and increasing the distance required to stop the vehicle. Wheel detachment can result in a loss of vehicle control. Either condition increases the risk of a crash.
Remedy
Dealers will inspect and tighten the front brake caliper mounting bracket and hub bearing bolts, as necessary, free of charge. In addition, any damaged components will be replaced, free of charge. Owner notification letters were mailed January 23, 2025. Owners may contact Toyota customer service

I was in a drive thru and my car had been idling for less than 5 minutes in the rain and started shuttering really bad with my roms dipping low. i put the vehicle in park and the the entire vehicle shut off and would not stay on without dying. for the next 20 minutes everytime i tried to start it it

... e a safety risk to myself, my child, and others on the road. The most concerning issue involves the braking system. I have had the brakes replaced and serviced multiple times, yet the brakes begin grinding again within a very short period, including an instance where the grinding returned approximately one month after replacement. Despite spending money on repairs, the problem continues to reoccur. I am concerned that there may be an underlying defect affecting the braking system that is not being properly identified or corrected through normal maintenance. In addition, the vehicle consumes an excessive amount of oil, requiring frequent monitoring and replacement between oil changes. I am concerned that this issue could potentially lead to engine failure or other dangerous mechanical problems while the vehicle is being operated. As a single mother, I rely on this vehicle daily for transportation to and from work and for caring for my family. I have experienced significant financial hardship attempting to keep the vehicle operational and safe. Despite my efforts to maintain the vehicle, these issues continue to occur. I respectfully request that the National Highway Traffic Safety Administration review this complaint and determine whether these issues may be related to known defects, recalls, investigations, or patterns reported by other owners of 2013 Hyundai Sonata vehicles. I am willing to provide maintenance records, repair invoices, mileage information, and any additional documentation that may assist in evaluating this matter. ... eems like this is a common occurrence with these Honda Odysseys. There were no warming lamps, messages, or lights that would have alerted me that something this serious would occur. We have taken our care to Honda for years for regular maintenance. We don't take it to any independent shops. Strictly to Honda for years. The most recent checkup/service was performed on February 11, 2026 where they performed the following services which they had recommended. 1) Oil Change 2)Engine coolant flush with conditioner 3)Transmission fluid change: 4) wheel alignment 5)Fuel tank additive, throttle body service, and injector. This is serious safety event and our family and children could have been killed in this accident. We have pictures, videos, police report, and the car has been taken to a salvage yard. URGENT SAFETY DEFECT - Repeated front hub overheating/fire risk (2025 Western Star 47x) I am submitting a formal safety complaint regarding my 2025 Western Star 47x The front wheel hubs have over heated to the point of failure on 6 separate occasions, resulting in a fire on two of those occasions.
... The issues have been documented with Freightliner and accompanied by photo/video evidence. Each incident required the vehicle to be towed to an authorized dealership, and repairs were performed under warranty. Despite these repeated repairs, the issue has not yet been resolved. The hubs are currently overheating again, indicating a persistent and unresolved defect. This is no longer a routine maintenance or warranty issue. This is an ongoing safety defect that presents a serious fire hazard and risk of injury, property damage, or loss of life. Key Concerns - - Six separate over heating and/or fire incidents involving the front hubs - Multiple warranty repairs with no permanent resolution - Recurrence of the same failure condition - Within days or weeks of warranty repair, the vehicles becomes unsafe to operate under normal working conditions due to over heating hubs, which break down the lubricating gear oil, furthering the problem, causing more heat with thinner oil, which eventually leaks onto the brakes, causing a fire. ... mpany for problems with their oil pans and oil drain plugs... consumers have filed complaints their engine’s oil drain plug falls out, causing all the oil to drain out of their engine while driving. In multiple tests the oil drain plug was tightened according to factory torque specifications and the tightening was well documented. Yet, testing shows in multiple cases that while driving, the oil drain plug falls within a few thousand miles after the service. In those cases, the crankcase drained completely. The Hyundai and Kia drain plug falls out due to inferior oil pan material and the drain plug gasket. The testing and investigations appear to show that the oil drain plans are made with THIN METAL AND THE DRAIN PLUG MOUNTING IS POORLY REINFORCED. In addition, the oil pan is coated with a thick paint coating the bonds the gasket to the pan, so the gasket does not come off when the drain plug is removed. That gasket issue alone is not why the oil drain plug falls out, but it is a factor in the chain of events. The oil pan is the biggest problem Apparently, the thin metal, along with engine vibrations and expansion/contraction tend to loosen the drain plug without any other intervention. NHTSA is investigating this as a factory defect ... The problem is two-fold. First, even if the repair shop properly torques the drain plug, the oil drain plug falls out on its own. The customer then blames the shop for shoddy workmanship. Second, Hyundai and Kia dealers are telling customers that servicing their vehicle at an aftermarket shop automatically voids their factory warranty. This is totally false & actually illegal under the terms of the Magnuson-Moss Warranty Act.Read more

Everything working properly and just fine until Monday 3/2/26 when I started the vehicle to go to work, the main radio multimedia screen was black and did not display anything. ... verything was pitch black. I then proceeded to drive the car to work and then decided to take it to the Toyota dealership near my house at the earliest convenience which was today (3/4/26). In the mean time until I brought the car to the dealership, I researched information and videos on how to diagnose it myself and try troubleshooting my self. Also, when car is running, I got a message stating malfunction with AEB/FCW. Tried the fuse route and tried disconnecting the negative battery terminal for 15 minutes and tried holding the main power knob for 30 plus seconds. All of those did not work. I then took into dealership and once I got there, the service technician saw the screen working and then also saw the screen acting up and turned on but had lines in the middle of the screen. The technician had me turn vehicle off and once we turned back on it went back to the black screen. So once I went the route of having them diagnose it, they said they could not duplicate concern that we saw with the lines in the screen etc., therefore the technician gave the vehicle back with the screen not working still until we do have it working with the lines. I was about to leave and then the issue with the screen and lines cam up on the screen and I was able to have the technician look at it and diagnose. That is when advisor came to me and said the radio head is completely not working and cannot be fixed or saved. Repair is $2,769 with labor for a new radio head per the advisor. Car is not under warranty because of how many miles are currently on vehicle so I cannot get this fixed for free under warranty. Would have to be out of pocket and I denied that. When I mentioned this to the lady handling the paperwork, she went and asked someone about it. She came back and pointed to a front wheel that was never in question or out of tolerance and said see they did fix something. My other concern is the fact that when I got a text at 11:23 am like my vehicle was finished, and I saw it in the finished area, I sat a little longer before I went to see if it was ready. When I asked the young lady that checked me in about my car being ready, she said, I was getting your paperwork ready, you just kinda came over here. Well I got a text saying thank you, and my car was parked in the finished area. Then she asks me to sign paperwork that had my name on it, but had a completely different service done, and it was for a Subaru Accent, my car is an Impreza. When I asked about that, she said don’t worry I’m going to reprint everything out, but she was still trying to get me to sign incorrect paperwork. She never reprinted the paperwork, and she just came back and said I just need a signature to release your vehicle back to you. So you want me to sign paperwork that releases a vehicle back to me that doesn’t even have my vehicle on it. Apparently, this was a major known issue and had warranty up to 150k miles. At the time of the incident, my car had about 165k miles. I had the car for years, and no one at the dealer ever mentioned anything about this or even tried to make me aware of it! I called the Toyota dealer and corporate number several times and they declined providing me with any help at all! This caused me fear and traume because I never thought this was possible! It could’ve ended my life and the life of people around me if there were any cars in sight! The problem has been confirmed by a reputable mechanic! He did the repairs! He also called the Toyota dealer and corporate and attempted collecting anything since he found out about the warranty and common issue. They also denied help. No lights were on at all before this incident happened! It all came at once!
