The cover for the seat belt and airbag on the front passenger seat have become disconnected without any incident causing such. Possibly not allowing the seat belt or airbag to function properly. Also the airbag sensor & seatbelt sensor for this seat stopped working at the same time the cover became

The contact suffered a concussion, and his dog, who was sitting in the rear center seat, died as a result of his injuries. The fire department and police arrived on the scene.
