FIRST ISSUE-THIS REPORT - LOSS OF POWER / ELECTRICAL FAULT (4/10/26) On April 10, 2026, I backed out of a parking space into a drive lane in a commercial parking lot. As the vehicle was rolling, it suddenly lost power. I attempted to restart the vehicle, but it would not start. The vehicle continued
... rolling backward into the intersection. Two warning messages appeared on the dashboard -"Stop Safely/Electrical Fault Detected" "Stop Safely – Loss of Drive".Photos attached. Because the vehicle could not be restarted or moved, it remained disabled in the intersection for approx two hours. The vehicle had to be towed to the dealership. When the vehicle was taken to the dealership, I was later told that no problem could be found. The dealership reportedly ran a computer diagnostic test & stated that nothing was wrong with the vehicle. They performed routine maintenance, charged more than $900, & returned the vehicle to me without identifying or repairing the cause of the loss-of-power event. SECOND ISSUE – INTERMITTENT SENSOR FAILURES - The vehicle's front & rear sensors operate intermittently. I scheduled a service appointment for May 20, 2026, to address this issue, but I was forced to cancel a I did not have transportation available. THIRD ISSUE (reported to nhtsa.gov) BRAKE SLAMING/SUDDEN STOP (5/2/26) On 5/2/26, the vehicle experienced a brake slamming/sudden stop while I was driving through an intersection. Following this incident, I developed neck pain & headaches. The vehicle was towed to the dealership for a second time. During discussion with the dealership, I was informed that the April 10, 2026 service visit had been documented as relating to a "check engine" light. I want to make clear that the warning messages displayed during the April 10 incident were not a check-engine light. The messages displayed were: • "Stop Safely–Electrical Fault Detected"• "Stop Safely–Loss of Drive" My research found reports of other RR owners experiencing the same issue.Read more

I took it back to the shop and this time the spark plug in cylinder 4 was bad. I believe I am at the beginning of what many others have complained about with these same symptoms. This problem makes operation of my vehicle unsafe and uncertain, at any time this issue can kick in and now you are limping along at 20 mph while cars zoom all around you. While trying to get the vehicle to the shop I created traffic buildups and was almost hit multiple times due to the vehicles diminished acceleration.Read more

Following engine replacement per NHTSA for 6.2l GMC engine. I am submitting a safety complaint regarding my 2022 GMC Sierra 1500 Denali (refresh body style). The vehicle has experienced repeated failures of driver-assist features and safety system warnings, including: Dash messages: “Lane Keepin
... g Assist System Unavailable” and “Service Safety Restraint System.” When the fault occurs, driver-assist features including Adaptive Cruise Control become inoperative. The issue has recurred multiple times, including after repairs. After one event, the truck sat parked approximately 10 days and the battery discharged to failure and required replacement. A dealership visit documented DTC U3000 (“Internal Electronic Failure”) in the front view camera module. The dealer replaced/programmed/calibrated the front camera on 02/09/2026 at approximately 58,405 miles (invoice/RO #285795). The symptoms have recurred despite prior resets and this repair. I am concerned about loss of driver-assist functions while driving and the presence of a safety restraint system warning. Please investigate whether there is a defect related to the front camera/driver-assist systems and/or electrical module behavior that can cause repeat faults and battery drain on this model.Read more

We took delivery of our 2024 Prius XLE (Gen 5) on 09-05-2024. It came with Toyo 19" Low Profile tires (195/50R19). On 04-17-2025 we had to replace a tire at 10,848 miles due to sidewall damage. On 05-05-2025 we had to replace a second tire at 11,749 miles due to side wall damage. Our dealer stat
... ed both side wall failures were most likely due to potholes. We did not remember hitting any major potholes. However, we live in Wisconsin were potholes of all sizes are prevalent. Both tire failures happen at night. Toyota does not provide a spare tire for the Prius. On the second tire failure, my wife had to drive a few miles to get to a safe spot to stop. On 11-25-2025 I replaced these Toyo tires with 19" Michelin tires. At that time I was told one of our wheels was damaged. I expect that happen when my wife had to drive on the flat to get to a safe location. In researching these tire problems, I found many other Gen 5 Prius owners had similar problems with the 19" Toyo tires. Interestingly, most owners reporting these tire problems lived in cold climate areas as we do. My Toyota dealer refuses to cover the wheel damage under warranty. In addition, Toyota refuses to cover the wheel replacement under warranty. These are the expenses I have encountered addressing this problem. First tire: $216.81, Second tire: $233.48, Replacement set of tires: $952.92. The cost for a new wheel is estimated at $781.93. In my opinion, the 19" Toyo tires are Unsafe At Any Speed on cold climate roads. Can you provide me with assistance in getting Toyota to cover my replacement wheel under warranty? In addition, I feel Toyota should face a RECALL for the 19" Toyo tires on the Gen 5 Prius. I have all the documentation for these charges. In addition, if you want more data on other owners that have faced this problem, I can reach out to them on social media and get them to contact you. Thanks for your assistance. I am submitting this complaint to escalate a safety-related concern involving premature and abnormal tire wear on my Tesla Model Y, which has not been adequately addressed by Tesla. I purchased my Tesla Model Y in [Dec, 2023]. Within less than one year of normal driving, all four tires wore out sig
... nificantly and required full replacement on 08/30/2024. The vehicle has been operated under normal city and highway conditions, and Tesla-recommended tire pressure was maintained consistently. This level of tire wear is well below reasonable tire life expectations and raises safety concerns, including reduced traction, increased stopping distance, and the risk of tire failure. After replacing the prematurely worn tires at my own expense, I reported the issue to Tesla Service and requested that the vehicle be inspected to determine whether alignment, camber, or suspension issues contributed to the abnormal wear. Tesla indicated that an evaluation would only be performed if I agreed to pay for diagnostic services, rather than proactively inspecting the vehicle for a potential underlying defect. As a result, no documented root-cause analysis or corrective action was provided at that time. During a later Tesla service visit for a separate suspension noise concern, Tesla replaced multiple front suspension components (including control arms and lateral/compliance links) and performed a four-wheel alignment under warranty. These repairs occurred after the tires had already been replaced, which created doubt as to whether the suspension condition at the time may have contributed to the earlier premature tire wear. Because the suspension was not evaluated when the abnormal wear occurred, the underlying cause remains unresolved. Replacing tires alone without timely evaluation of possible contributing suspension or alignment issues does not adequately address the associated safety risk.Read more

On [XXX], my 2025 Hyundai Tucson Hybrid displayed a red dashboard alert instructing me to “stop vehicle and check brake system.” The vehicle experienced brake system warnings and a loss of power steering, requiring me to pull over. I immediately contacted Hyundai through the Bluelink system, and
... the Bluelink operator confirmed that multiple fault codes were visible in Hyundai’s system. I scheduled a Hyundai service appointment at the next available date. On [XXX], before the scheduled service appointment could occur, the vehicle experienced a sudden and total electrical failure while traveling at highway speed (approximately 65 mph). The vehicle lost propulsion and displayed brake system warnings. After reaching the shoulder, the vehicle also lost hazard lights while disabled in heavy rain. The vehicle was towed via Hyundai Bluelink to a Hyundai dealership and removed from service. The occupants at the time of the incident included my wife and my minor child. The loss of propulsion, braking warnings, power steering, and hazard lights created a serious risk of a rear-end collision and injury. This occurred intermittently from approximately 1,200 miles onward and continued for over a year despite multiple repair attempts by the manufacturer’s service centers. The malfunction directly affected safe operation, as the system could disengage without warning at highway speeds, forcing sudden driver intervention. The issue was reproduced and documented by the manufacturer’s technicians during at least one service visit, which resulted in replacement of the vehicle’s driver-assistance computer. The failure continued even after that replacement. The vehicle has been inspected and serviced numerous times by the manufacturer, with diagnostics performed on the cooling system, cameras, sensors, and ADAS module. Each repair attempt either failed to duplicate or failed to permanently correct the defect. The safety risk is loss of Adaptive Cruise Control and Highway Assist functions while driving, unexpected disengagement of automated steering/braking, and lack of reliable warning timing to the driver. Warning messages appeared frequently throughout 2024–2025, beginning within the first few months of ownership. The defect remains unresolved and appears systemic rather than isolated to this vehicle.Read more

When I contacted the dealer, they confirmed that the issue is caused by a starter motor failure, and the replacement part will not be available until early November — meaning I must drive for over a month with this feature turned off. Many other owners have reported similar problems online, which strongly suggests a potential defect that should qualify for a safety recall. 2. Took it back to the dealership, ran another diagnostic and determined the fuel injectors needed to be replaced. These repairs cost over $5,000. 2022 GMC Sierra has a P1098 code. This code has to do with a faulty coolant control valve. This causes the engine fan to run constantly and displays on the screen "Reduced Acceleration, Drive with Caution". The truck does not have any power when this happens, which is 99% of the time. This makes usi

Apparently GM knows about this and is not getting the part to correct the problem to the dealerships in a timely manner.
