The vehicle locked into Park, and I could not get power to shift into neutral or restart it. The January 30 incident required towing. The vehicle was inspected by a mechanic, who was unable to replicate the problem. Since the second failure, I have observed that system voltage drops rapidly during normal driving on nearly every trip. To avoid another shutdown in traffic, I now monitor voltage and pull over to shut the engine off and restart it once it reaches 12.4V. This takes several efforts each trip to pull over and restart the system. It has produced anxiety in my family about the safety of this vehicle. This is a full-size SUV losing power in highway traffic without warning. It endangers my family and every driver around us. I am aware of ODI Preliminary Evaluation PE24018 covering this vehicle and system, and I am submitting this so my incidents are part of that record.Read more

Motors Ford in McPherson, KS, for failing to provide a free remedy under active federal safety recall 24S37 (NHTSA Campaign 24V444).My 2014 Ford F-150 was brought to the dealership to complete safety recall 24S37, which addresses severe, sudden transmission downshifts. The dealership performed a software update to the Powertrain Control Module (PCM). Immediately following this update, the truck's drivability severely deteriorated, and the transmission began violently slamming into 3rd gear, locking itself into a dangerous transmission fault "limp mode." The diagnostic scans post-update indicate a failure of the molded lead frame/speed sensor. According to Ford's official technical service bulletin for recall 24S37, physical replacement of the molded lead frame is an explicitly authorized remedy when diagnostic codes appear following the software flash. However, both the local servicing dealership (Midway Motors Ford) and Ford Corporate Customer Service are actively refusing to perform this physical safety repair free of charge. They are wrongfully claiming that because my vehicle has over 200,000 miles, it is disqualified from financial assistance. They are confusing a federal, non-expiring safety recall with an older, expired warranty extension program (19N01).By refusing to replace the defective lead frame that their own software update triggered into an active failure state, the manufacturer and dealer are leaving my vehicle in an unsafe, undrivable condition and violating federal recall mandates. I request that NHTSA intervene to compel Ford to complete the mandatory safety recall remedy at zero cost to me.Read more

The dash immediately displayed several warning lights and messages: Antilock Brake Fault, Antilock Brake System Fault, Powertrain Malfunction/Reduced Power Fault, Electronic Parking Brake Fault, Hill Start Assist Fault, Precollision Assist Fault, Service Advance Trac Warning, Advanced Front Lighting Features Fault. 5. The light turned green and I proceed to the next light to turn off the main road and park in a neighborhood to call a tow truck. 6. In the process of slowing down for the turn, I applied pressure to the brake pedal, and it went to the floor with no resistance. Brakes were 95% gone. 7. Luckily there were no cars or pedestrians in front of me and I was moving under 30 MPH. This could have been CATASTROPHIC! I was able to use the transmission to downshift to slow the truck down. I was also able to apply extreme pressure to the brake pedal to barely slow the truck. When pressing hard on the brake pedal, I could here one or both of the rear tires sporadically screech, as if they locked up a small amount. 8. Finally was able to pull into a neighborhood and call a tow truck to bring truck to dealership. 9. When turning vehicle on to load onto tow truck. The vehicle started as no normal, with no warning lights and normal brake operation. Awaiting further diagnosis from dealer. 10. I have lost all trust in this vehicle if the brake sporadically give out without warning. Luckily this did not happen while going 80MPH on the highway and needing to make an emergency stop. Had a pedestrian, child, or another moving vehicle been behind my car instead of a metal pillar, this software malfunction would have resulted in severe injury or a fatal collision. 3. Inspection Status: The vehicle has not yet been inspected by the manufacturer, police, or insurance representatives. I am intentionally securing the vehicle to preserve the crucial diagnostic logs and EDR data from being erased or reset during standard repair procedures, pending official data extraction. 4. Warnings/Symptoms: There were absolutely no warning lamps, error messages, or visual/audible symptoms prior to the failure. Furthermore, during the uncommanded acceleration, the system did not issue an FSD disengagement warning or chime. The exact root cause is UNKNOWN, but the affected systems appear to include the EPS/power steering system, steering/suspension components, battery/electrical system, and possibly related control modules. My safety and others’ safety were put at risk because the vehicle has lost steering assist and had electrical failure while being driven, including a prior failure in an intersection. When the steering assist drops or the wheel jerks, the vehicle becomes difficult to control. This is especially dangerous because I transport my family in this vehicle. The issue has been reported repeatedly since purchase. The vehicle was inspected/repaired through Carvana/SilverRock’s repair process at Brockman Automotive, including rack and pinion/steering-related work. After that repair, the vehicle continued to drift and handle improperly. A later independent inspection at Davidson Automotive identified a loose/not properly secured tie rod, alignment problems, and a failed auxiliary battery. The shop could not complete deeper diagnostics until those prerequisite issues were corrected. The vehicle has also suffered abnormal front tire wear after the steering/alignment problems. Warning/symptoms before or during failures include steering wheel jerking/pulling, clunking/clanking/grinding/groaning noises, vehicle drift, service start/stop warning, electrical malfunctions, intermittent steering wheel control/light issues, loss of backup camera guidelines, and loss of steering assist. The vehicle was repaired, but the failure persisted. The vehicle was then taken to another independent mechanic, but the cause of the failure was not determined. Fast forward to 2/2026 and the same thing happened. This time it was scarier as it shut off as a car was backing up from a parking spot. Fortunately, the driver saw the car and was able to stop before running into me. The car failed to start, and I had to get it towed. Could not tow it to the dealer since it was late in the evening. The next morning, the car did start, but all the warnings continued to flash on the dash. The car is being towed to the dealer where the "repair" was done. All this happening within a year of repairs. This should be a recall and Honda should refund my initial repairs as this is happening to other drivers as well.Read more

... nificantly and required full replacement on 08/30/2024. The vehicle has been operated under normal city and highway conditions, and Tesla-recommended tire pressure was maintained consistently. This level of tire wear is well below reasonable tire life expectations and raises safety concerns, including reduced traction, increased stopping distance, and the risk of tire failure. After replacing the prematurely worn tires at my own expense, I reported the issue to Tesla Service and requested that the vehicle be inspected to determine whether alignment, camber, or suspension issues contributed to the abnormal wear. Tesla indicated that an evaluation would only be performed if I agreed to pay for diagnostic services, rather than proactively inspecting the vehicle for a potential underlying defect. As a result, no documented root-cause analysis or corrective action was provided at that time. During a later Tesla service visit for a separate suspension noise concern, Tesla replaced multiple front suspension components (including control arms and lateral/compliance links) and performed a four-wheel alignment under warranty. These repairs occurred after the tires had already been replaced, which created doubt as to whether the suspension condition at the time may have contributed to the earlier premature tire wear. Because the suspension was not evaluated when the abnormal wear occurred, the underlying cause remains unresolved. Replacing tires alone without timely evaluation of possible contributing suspension or alignment issues does not adequately address the associated safety risk.Read more

... the Bluelink operator confirmed that multiple fault codes were visible in Hyundai’s system. I scheduled a Hyundai service appointment at the next available date. On [XXX], before the scheduled service appointment could occur, the vehicle experienced a sudden and total electrical failure while traveling at highway speed (approximately 65 mph). The vehicle lost propulsion and displayed brake system warnings. After reaching the shoulder, the vehicle also lost hazard lights while disabled in heavy rain. The vehicle was towed via Hyundai Bluelink to a Hyundai dealership and removed from service. The occupants at the time of the incident included my wife and my minor child. The loss of propulsion, braking warnings, power steering, and hazard lights created a serious risk of a rear-end collision and injury. Due to repeated and escalating safety-related failures involving electrical, braking, and steering systems, I no longer consider the vehicle safe to operate. The contact later became aware that the VIN was included in NHTSA Campaign Number: 25V099000 (Engine and Engine Cooling), and the vehicle was taken to the dealer for the recall repair. The dealer diagnosed that the engine oil pump needed to be replaced, and the contact was informed that the repair was not covered under the recall. The contact stated that the oil pump failure might also be related to the oil piston rings failure described in the recall. The manufacturer was notified of the failure but declined to cover the cost of the repair. The vehicle was not yet repaired. The failure mileage was approximately 62,000.Read more

This occurred intermittently from approximately 1,200 miles onward and continued for over a year despite multiple repair attempts by the manufacturer’s service centers. The malfunction directly affected safe operation, as the system could disengage without warning at highway speeds, forcing sudden driver intervention. The issue was reproduced and documented by the manufacturer’s technicians during at least one service visit, which resulted in replacement of the vehicle’s driver-assistance computer. The failure continued even after that replacement. The vehicle has been inspected and serviced numerous times by the manufacturer, with diagnostics performed on the cooling system, cameras, sensors, and ADAS module. Each repair attempt either failed to duplicate or failed to permanently correct the defect. The safety risk is loss of Adaptive Cruise Control and Highway Assist functions while driving, unexpected disengagement of automated steering/braking, and lack of reliable warning timing to the driver. Warning messages appeared frequently throughout 2024–2025, beginning within the first few months of ownership. The defect remains unresolved and appears systemic rather than isolated to this vehicle.Read more

We had not had strap loosening issues initially, but now are despite having followed the instructions as part of the recall which I was told would resolved the issue and prevent further concern over the strap mechanism failing. To my knowledge this is a known issue that is what caused the recall for the car seat in the first place. I have removed the car seat from use as it is no longer safe with the straps loosening. This creates a dangerous situation, especially in traffic or intersections. When I contacted the dealer, they confirmed that the issue is caused by a starter motor failure, and the replacement part will not be available until early November — meaning I must drive for over a month with this feature turned off. Many other owners have reported similar problems online, which strongly suggests a potential defect that should qualify for a safety recall. 2. Water Leak and Poor Quality Repair My vehicle also experienced a water leak from the roof area. The dealer attempted to fix it by simply applying sealing material on the roof molding (see attached photos). The repair looks extremely unprofessional, and I requested them to properly replace or reinstall the roof molding, but they refused. This type of repair is unacceptable for a new vehicle of this class and price. 3. Honda’s Unsatisfactory Response I contacted Honda Customer Service (1-800-999-1009) and spoke directly with a representative. However, I was told that: They cannot extend the warranty. They cannot offer any compensation or goodwill service. They cannot provide me with a record or copy of our phone conversation (even though they recorded it). Ford dealership staff acknowledged metal in the fluid but gave inconsistent explanations and then claimed the transmission had to be replaced. The vehicle also displayed a “Powertrain System Fault” warning. No such problems existed before the recall. This raises a safety concern because transmission malfunction after a recall could cause loss of control or failure to move the vehicle. The vehicle was returned to us on October 7, 2025; it remains unsafe to drive. Ford Corporate has been notified (Case CXH-05773791-T4G0R8), but no resolution has been provided. The failure occurred while my husband was driving our [XXX] son in the car, and the warning message immediately displayed on the dashboard was: “12V battery not charging. Stop engine.” This sudden loss of engine function and dashboard warning posed a serious safety risk, as it could have resulted in loss of power or control while driving. The problem has been confirmed and documented by an authorized VW service department, including detailed repair orders and technical diagnosis. Prior to failure, there were oil-related warning lights and symptoms of excessive oil pressure, which appeared intermittently and indicated the underlying PCV failure.
