The internal mechanical failure of the Ford 10r80 transmission due to design fault causes unsafe driving conditions as the transmission stalls (fails to power at request) or lunges (powers forward without driver input) causing unsafe conditions for vehicle occupants and nearby vehicular & pedestrian
... traffic. My vehicle exhibits two well known and documented conditions; stalling and lunging forward. The most common failure is that the transmission fails to shift from 1st to 3rd gear in a low speed condition (e.g. navigating parking lots amongst pedestrians). Upon the failure to shift, the vehicles computer system senses this, overrides by raising the vehicle RPMs significantly without driver input to do so, the vehicle then hard shifts and jerks forward several feet in an uncontrollable manner. Anticipating the jerk and overriding with the brakes fails to stop the vehicle from lunging forward. The second most common failure is the transmission totally failing to provide drive. A common scenario would be sitting at a traffic light waiting to make a left turn. Upon the opportunity to make a safe left turn, the vehicle at times will roll slightly forward (in its first gear), it will then attempt a shift while the vehicle may potentially still be in cross-traffic, and the vehicle at times will fail to drive, without any inputs (brake, gas, manual shift) getting the vehicle out of the no-drive condition, and leaving the vehicle subject to a dangerous potential for collision or safety hazard. The issues with the 10r80 transmission failures are well documented, well understood by Ford Motor Company, and specific to my vehicle have been identified by a reputable dealer as requiring repair. My vehicle's transmission is exhibiting failure at 47,000 miles. No warning indications exist prior, during or after this repeatable incident. Ford has since fixed the design flaw, but refuses to address the safety issue with vehicles manufactured before the re-design.Read more

I purchased a used 2022 Honda HR-V with approximately 32,000 miles. Upon experiencing severe steering anomalies, a professional mechanical inspection revealed that a non-compliant, off-market/aftermarket steering rack had been installed on the vehicle. This was done without my knowledge or disclosur
... e at the time of sale. This unapproved component presents an immediate safety risk of sudden steering loss, binding, or unexpected mechanical failure at highway speeds. This generation of the Honda HR-V is currently subject to a massive 1.7-million-vehicle NHTSA steering recall (Campaign 24V744) for "sticky steering" and gear defects. While the recall officially targets 2023–2025 models, my 2022 model is experiencing identical low-mileage steering failures. It appears the previous owner installed a cheap, undocumented aftermarket rack to bypass these exact steering defects before trading it in, which the selling dealership failed to detect during their safety inspection.I am filing this report to document an immediate vehicle safety hazard and to urge the NHTSA to expand its current steering defect investigation to include the 2022 model year, as these vehicles are suffering from the same core steering system vulnerabilities.Read more

During the incident, the vehicle’s pre-collision and automatic braking system failed to engage despite an obstruction being present. Additionally, the airbags did not deploy at the time of impact. A minor child was present in the vehicle, properly secured in a child's safety seat. While no injuries occurred, the vehicle sustained extensive damage and was rendered inoperable. I would also like to note that this vehicle was subject to an airbag recall on November 29, 2025, which was serviced prior to this incident. Despite completing the recall service, the airbags failed to deploy during the accident. After the vehicle was towed, the airbags unexpectedly deployed while it was being lifted onto the tow truck using a crane, further raising concerns regarding the reliability and safety of the airbag system.Read more

Since the release of V14, the Full Self-Driving (FSD) system automatically defaults to operating approximately 7 MPH above the posted speed limit. This occurs on all road types, including city streets and highways. When FSD is engaged, the vehicle sets the cruising speed above the detected speed li
... mit without driver input. There is no setting that allows the driver to enforce strict compliance with the posted limit. Available speed settings only permit equal or greater deviation above the speed limit. The driver cannot manually lock or command the system to remain at or below the posted limit while FSD is active. The only way to prevent speeding is to fully disengage FSD. This is not a malfunction. The vehicle is operating as designed under V14. This creates a safety concern because: 1.The system is programmed to exceed legal speed limits by default. 2.The driver does not retain meaningful authority to ensure lawful operation while automation is engaged. 3.Automatic speeding increases crash risk in construction zones, school zones, and changing speed areas. 4.Drivers may receive citations despite not intentionally selecting an unlawful speed. Because this behavior was introduced via V14 software, it likely affects all Tesla vehicles running FSD V14, not just this vehicle.Read more

It has not been inspected by police or insurance representatives at this time. 5. Yes. Warning messages appeared on the dashboard, including system errors and power failure alerts. These issues first appeared around 20,000 miles, and have recurred intermittently since then. I have communicated with the dealership where the car was purchased and GM corporate requesting replacement with a safe and reliable vehicle. To date, I have not received a satisfactory resolution.
