... and I attempted to stop but due to ice I kept sliding forward adn the ABS was going wild and while I had the brake held down the pedal was going wild from the ice and after the car has stopped the brake pedal arm had bent so drasticly my brake lights wouldn't turn off due to the pedal arm being bent into the sensor below the dash. Toyota then refused to cover any part of the repair cost under the warrenty due to outside influence. I did not and could not have expected that using the brake pedal in an emergency situation would brake the brake pedal and they refuse to cover it under manufacturer warranty.Read more

These false alerts distract the driver and could cause confusion or unsafe reactions. The issue undermines confidence in the reliability of the vehicle’s electrical and safety systems. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes. The Toyota dealership confirmed the problem and reported replacing the wireless charging unit, but the defect persists. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives, or others? Yes. The vehicle has been inspected by the Toyota dealership and reviewed by Toyota Customer Care. Toyota denied further assistance (Case #250811002888, letter dated August 21, 2025). Were there any warning lamps, messages, or other symptoms of the problem prior to the failure, and when did they first appear? Yes. Shortly after purchase, the vehicle displayed false “door open” messages when attempting to use the wireless charger. These warnings cause the system to stop charging and continue to appear despite the dealer’s attempted repair.Read more

... oise is high-frequency, sharp, and continuous, and appears to originate from within the dashboard area or HVAC components. I went to the dealership multiple times with this complaint and they claim it is normal operation. I did not hear the same issue in the cars they compared mine to, but after Toyota's refusal to address the issue i looked it up and there are other reports [XXX] ). When at the dealership, I am continually faced with dismissive responses from the service departments and technicians, who have failed to address the underlying cause of the sound. This sound creates a serious distraction while driving and has caused physical symptoms including headaches, nausea, ear discomfort, and dizziness. These symptoms impair my ability to concentrate and safely operate the vehicle. On longer drives, the effects intensify, increasing the risk of driver fatigue, disorientation, or even potential loss of control. I believe this presents a significant safety hazard, not only due to the physical impact on the driver, but also because it undermines focus and reaction time, especially in high-traffic or high-speed conditions. This issue should be investigated for potential defects in the HVAC design, acoustic insulation, or related components. Please investigate whether this condition is present in other vehicles of the same make/model and consider whether it warrants a safety recall or manufacturer intervention. The driver depressed the brake pedal with both feet, and the vehicle decelerated but failed to stop as needed, causing the front passenger's side of the vehicle to drive under the rear of the unoccupied truck and to crash into the truck. The air bags failed to deploy. The contact's daughter was able to exit the vehicle. The check engine warning light and other unknown warning lights were illuminated after the crash. The vehicle was reversed out of the rear of the truck and was driven to the driveway of the residence, where the engine failed. The contact stated that the vehicle had failed to start and was undrivable. The contact's daughter sustained whiplash, body soreness, lower back pain, a seat belt mark and bruises under the chest. Some drivers (~25%) related the feel to driving in a strong crosswind and others indicated that the effort and attention required to maintain heading increased driver fatigue on long trips.These complaints all relate to the system operating as designed and do not involve failure of any steering system components.The subject vehicles are equipped with an electronic power steering system that was introduced in the MY 2009 vehicles.The power assist, including on-center steering feel, is controlled electronically and is part of the system calibration.Toyota responded to steering feel concerns from some customers by issuing a technical service bulletin (TSB-0140-10 STEERING ON-CENTER FEEL) that released an optional EPS electronic control unit that has a re-calibrated on-center feel.Accordingly, this investigation is closed.The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist. Engine Stalling
On November 30, 2009, the Office of Defects Investigation (ODI) opened a preliminary evaluation (PE09-054) to investigate alleged engine stalling, while driving in model year (MY) 2006 Toyota Corolla and Corolla Matrix vehicles .In its response to ODI's information request submitted on March 2, 2010
... , Toyota indicated that it had identified two possible causes of production defects of the engine control units (ECU) used in the MY 2005 through 2007 Corolla and Matrix vehicles equipped with the 1ZZ-FE engine.Failure mode A1 - BGA ball failure caused by improperly cured conformal coating applied to ECU circuit boards. This can cause cracks to form in the soldered joints of some components. Failure mode A2 - Varistor over heating; a glass coating is created on the surface of the Varistor during its production process.In case of an insufficient coating, a crack can occur in the surface of the glass coating. When ions are charged in the plating electrolytes after the glass coating process, the plating electrolytes can penetrate into the Varistor through the glass crack then an electrical short occurs. With respect to these failures, if the condition arises, it can potentially lead to one or more of the following: MIL on, engine stall or engine no start, harsh shifting.Of the approximately 155,000 warranty and extended warranty claims Toyota has analyzedthe projected 5 year failure rate of the ECU associated with an engine stall is 0.8%.
