The incidents happened at all different levels of SOC - from 98% charge to 1% charge. All happened at different times of day. Temperatures between 73-85 degrees. All in daylight with different sun/clouds. Yes it is available for inspection upon request. - Safety is an obvious issue when the vehicle comes to a compete and sudden stop in traffic unexpectedly. - Yes the problem has been reproduced and confirmed my an independent automotive engineer. I am an experienced professional automotive test driver/engineer for multiple OEM's. - Not as of this time. It is due for service on Thursday July 2nd for its 78C recall completion. At that time, I will have the dealership inspect it. - There were no warnings of any kind prior to each failure. I have only owned the vehicle for 14 days, and in that time, the issue has repeated 6 times. All when in full electric/battery mode, or, when in hybrid mode but the vehicle was operating solely on electric propulsion and the ICE was not on or engaged.Read more

The contact was informed that the clock spring had failed and needed to be replaced. The clock spring and brake light switch were replaced, but the failures persisted. The vehicle was taken to a third dealer, Red Rock Honda (748 N 1st St, Grand Junction, CO 81501); and it was diagnosed and determined that the clock spring and brake light switch needed to be replaced another time. The clock spring and the brake light switch were scheduled to be replaced. The manufacturer was made aware of the issues, and a case was opened. The failure mileage was approximately 1,200.Read more

Also, the broken glass might have fallen in onto passengers. We slowed and exited the highway, and inspected the glass. There did seem to be one location where some object might have hit first, causing the shatter. But the evidence of a single impact point is not strong. We will never know exactly why the Moon roof exploded. I tapped the glass moon roof extensively and we drove home. There was no warning. ... ying to pull over, the vehicles lane keeping assist appeared to resist my steering input, making it difficult to move fully onto the shoulder. I was focused on safely exiting the roadway die to what I believed was failing tire. Trying to reach the shoulder area, the tire suddenly blew out. The vehicle lost control, spun, and we were involved in a head-on collision with a semi-truck. My family and I are fortunate to be alive. I sustained whiplash and ongoing neck pain; my husband suffered a hand injury that is not healing. My daughter had abrasion on her chin but recently has faded away. Since the accident, I have experienced significant anxiety while driving, especially related to vehicle noises and highway conditions. My children faced a traumatic event and still continue to bring up the day of the accident or show signs of it affecting them. My concern is that the lane assist system may have interfered with my ability to safely pull over during an emergency situation involving a tire failure. We are uncertain what caused the initial tire failure. The situation I was in could pose a safety risk to other drivers if the system resists drivers input during critical moments.Read more

Despite this known issue, the repair is not being covered and the cost quoted is several thousand dollars. A failure of the brake booster/accumulator system significantly affects braking performance and should not occur at this mileage. Shortly after the warning lights appeared, the vehicle began emitting continuous warning beeps. Audible buzzing or pumping noises were also present from the brake system during the malfunction. During this event, brake performance was reduced and braking felt inconsistent. The brake pedal became hard at times and spongy at other times, requiring increased effort to slow the vehicle and resulting in unpredictable braking response. This created a safety concern while the vehicle was in motion. The driver was forced to alter driving behavior and exercise extreme caution due to reduced braking confidence and unpredictable pedal feel. The malfunction has occurred intermittently during normal driving. The condition temporarily resolves on its own and later reoccurs without corrective action. The suspected failed component is the brake actuator / ABS actuator assembly. A diagnostic scan retrieved fault codes C1252 (Brake Booster Pump Motor ON Time Abnormally Long), C1253 (Pump Motor Relay Malfunction), and C1256 (Accumulator Low Pressure), which are consistent with brake actuator malfunction. The failure occurred at approximately 154,000 miles. This vehicle model and generation have a documented history of premature brake actuator failures with similar safety-related symptoms reported by other owners. The condition places the driver and others at risk due to unpredictable braking behavior, reduced braking effectiveness, and repeated warning alarms while driving. Warning lamps and audible alerts continue intermittently.Read more

The sudden braking issue, often referred to as "phantom braking," occurs when the vehicle's Collision Mitigation Braking System (CMBS) activates without any obstacle or danger present. The car abruptly comes to a full stop, without warning, on a clear road. This is extremely dangerous and has nearly caused multiple rear-end collisions. In addition to the braking problem, the steering has also become intermittently stuck. This happens without warning and makes it difficult to turn the wheel, which could lead to a loss of control, especially at higher speeds or on curves. I have already reported these issues to two authorized Honda dealerships: Curry Honda in Atlanta, Georgia Honda AutoNation in Columbus, Georgia On each occasion, the dealerships have been unable to replicate the problem and have told me they cannot find anything wrong. Despite their inability to diagnose the issue, the problems are very real and continue to happen. These defects are not just an inconvenience; they are serious safety hazards that could lead to a crash. I am concerned that if this problem is happening to my vehicle, it may be affecting other 2018 Honda CR-V owners as well. This happened with 20,300 miles on the vehicle, which is less than 8000 miles later. BMW is refusing to replace under the recall or a warranty the fuel pump which means I am stuck with the bill. I believe that BMW should be replacing a defective part with a new design and better part not an old design faulty part. I would encourage you to issue another recall on the vehicles in question under bulletin 160114 in the interest of motorist safety.Read more

Some drivers (~25%) related the feel to driving in a strong crosswind and others indicated that the effort and attention required to maintain heading increased driver fatigue on long trips.These complaints all relate to the system operating as designed and do not involve failure of any steering system components.The subject vehicles are equipped with an electronic power steering system that was introduced in the MY 2009 vehicles.The power assist, including on-center steering feel, is controlled electronically and is part of the system calibration.Toyota responded to steering feel concerns from some customers by issuing a technical service bulletin (TSB-0140-10 STEERING ON-CENTER FEEL) that released an optional EPS electronic control unit that has a re-calibrated on-center feel.Accordingly, this investigation is closed.The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist. ... , Toyota indicated that it had identified two possible causes of production defects of the engine control units (ECU) used in the MY 2005 through 2007 Corolla and Matrix vehicles equipped with the 1ZZ-FE engine.Failure mode A1 - BGA ball failure caused by improperly cured conformal coating applied to ECU circuit boards. This can cause cracks to form in the soldered joints of some components. Failure mode A2 - Varistor over heating; a glass coating is created on the surface of the Varistor during its production process.In case of an insufficient coating, a crack can occur in the surface of the glass coating. When ions are charged in the plating electrolytes after the glass coating process, the plating electrolytes can penetrate into the Varistor through the glass crack then an electrical short occurs. With respect to these failures, if the condition arises, it can potentially lead to one or more of the following: MIL on, engine stall or engine no start, harsh shifting.Of the approximately 155,000 warranty and extended warranty claims Toyota has analyzedthe projected 5 year failure rate of the ECU associated with an engine stall is 0.8%. This investigation has been upgraded to an Engineering Analysis (EA10-006) to further assess the scope, frequency and safety risks associated with the identified defects.Read more
