Toyota Motor Engineering & Manufacturing (Toyota) notified the agency on May 13, 2015 that they are recalling certain model year 2004 Sequoia vehicles manufactured August 18, 2003, to August 19, 2004, and 2003-2004 Tundra vehicles manufactured March 18, 2003, to August 25, 2004. On June 16, 2015, Toyota expanded the recall to include an additional 1,270,468 vehicles, including model year 2002-2003 and 2005-2007 Sequoia, 2005-2006 Tundra, 2003-2007 Corolla, 2003-2007 Corolla Matrix, 2003-2007 Pontiac Vibe and 2002-2007 Lexus SC vehicles. This recall addresses both the passenger side frontal air bags that were originally installed in the vehicles, as well as replacement air bags that may have been installed as replacement service parts. A replacement air bag may have been installed, as one example, if a vehicle had been in a crash necessitating the replacement of the passenger side frontal air bag.
Remedy: 2003-2004 Corolla and Corolla Matrix vehicles that are recalled under both 15V-043 and 15V-285 are under a Do Not Drive advisory. Owners of these vehicles are advised not to drive their vehicles until the recall remedy has been completed. Toyota will notify their owners. Pontiac Vibe owners will be notified by GM. Dealers will replace the front passenger air bag. The recall began on July 13, 2015. Toyota owners may contact Toyota customer service at 1-800-331-4331. Pontiac owners may call 1-800-762-2737. The contact's son was driving at 55 MPH, another vehicle ran a stop sign and pulled into his path, resulting in a frontal collision with the side of the other vehicle. The contact’s vehicle spun around and came to rest on the shoulder of the road. Despite the high-speed frontal impact, the air bags failed to deploy. A police report was filed, and the vehicle was towed to an auto body shop; it has not been repaired. The contact's son sustained a bruised knee, neck and back pain, and a headache, for which he was treated with Tylenol. The contact referenced NHTSA ID Number: 19V741000(AIR BAGS), recall repair was done a year before. The vehicle was not repaired. The manufacturer was not notified of the failure. There was a recall in 2015 and 2019 for airbags on my model of Toyota. I own a 2006 Toyota Corolla CE. I was sent a letter from Toyota to go into a local dealership to get this done. There's only one dealership in our tri-city area. I took it into our local dealership called Findlay Toyota in Prescott, Arizona. I will say ahead of time that they have over 50 - one star reviews on Yelp.com From the moment I left the dealership in December of 2019 my airbag light was on in the dash area and it’s still there. When i went in and told Brian at the time he said it will go away. It never has. This warning was never on my dash prior to this recall. I’ve filed a complaint with the Attorney general’s office of Arizona and Findlay have no recollection of me ever going into their dealership. I've sent documentation of my visit on to the Attorney Generals Office and I’ve never heard back from Findlay Toyota. I've spoken to a Mike in July at Findlay and he was willing to waive the $130 or $150 fee only though if nothing is found wrong. I don't trust dealerships and if there's anything wrong then it was more than likely due to their workmanship back in 2019 that created this issue to begin with. I’ve also filed a complaint with the Attorney general’s offic of Texas against the Toyota headquarters because they in my opinion have caused this issue because I never had this problem prior to the work that was done at Findlay Toyota. All Toyota USA did was give me a case number and send me right back to Findlay Toyota who don't want to help me in the slightest. It’s just a conundrum. The sad thing is I think Toyota is a great company but the way that I’ve been treated in this matter is beyond appalling. I've also written to the headquarters of Toyota in Japan, explaining he above story. They never replied either. I've also called the general manager, a Mr. McCoy at Findlay Toyota and he hasn't called me back.
Inflator Rupture - front passenger inflator. A vehicle collision at Alma School and 8th Avenue where one vehicle turned left in front of the oncoming involved 2006 Toyota Corolla resulting in the Corolla striking the right side of the other vehicle. During the collision the airbags were commanded to deploy and the passenger inflator had a significant rupture/failure which sent shrapnel out of the dash, striking the passenger. Our unit has investigated two confirmed Takata inflator fatalities and after a brief visual inspection it was confirmed the passenger inflator is in several pieces. The appearance of the inflator is consistent with prior observed passenger side Takata inflators. The car has been preserved in our parking lot and the passenger side airbag, with related tears and shrapnel holes, was obtained and preserved as well. The crash was filmed on intersection CCTV and the inflator can be seen expelling fragments outwards. A Dropbox file has been created to exchange photos and video upon request. Thank you. I am a Detective with the Mesa Police Department Vehicular Crimes Unit and was made aware of NHTSA's interest in identifying inflator related cases.

On March 17, 2023 at approximately 4:12 PM on Hwy 288 northbound just south of Southmore Boulevard I was struck from behind by a Blue Jetta. Skidding into me I think he was still going about 30 miles per hour if not a little more based on the violence of the crash. My vehicle was pushed forward into the car ahead of me damaging the front of my vehicle as well. Despite the impact, my air bags did NOT deploy. I had taken the vehicle in to the Toyota shop here in Humble, TX and had the "final fix" done on the airbags. I have been physically injured as a result of this automobile accident. I have filed with my Progressive Claim Adjuster and they are talking about totaling the vehicle. Policy Number 46753320. My concern is, despite the vehicle air bags being replaced, they did not deploy. The force of the impact should have been sufficient to activate them. It could of course be a defective switch given her age, but I'd rather let science do the talking.

I purchased a Toyota three years ago, immediately I complained about the brakes scrubbing and squeaky sounds, the dealer denied anything was wrong with the brakes. After paying the last payment the other day, now the brake pedal is stiff as a board and takes alot of pressure to stop, which is a safety hazzard to all. Also there have been no warning lights come on notification of any problems. I've also noticed Electrical problems like the dash lights going off and on the clock works when it takes a notion.

The contact's mother owns a 2006 Toyota Corolla. The contact stated while reversing out of a parking space and as she shifted the vehicle into drive, the vehicle surged forward. The contact's mother lost control of the vehicle and crashed into several vehicles and then a curb and turned the vehicle ignition off. The contact's mother stated she had no brakes. The contact sustained bruises to her left and right legs by the knees and both wrists were bruised. Additionally, the contact had pain in her left and right shoulders. The contact did not seek medical attention. The other vehicles were not occupied. The contact stated she did not notice any warning lights were illuminated. The police were on the scene and filed a report. The contact's mother had the vehicle towed to her residence and then later towed to a dealer. The dealer did not inspect the vehicle, however, the manufacturer had an independent mechanic inspect the vehicle brakes. The mechanic determined that there were no mechanical problems with the brakes. The vehicle was not repaired. The manufacturer had been informed of the failure. The failure mileage was approximately 42,000.

My mother’s car suddenly went out of control in the parking lot of the local power company. The car was out of control like a wild horse and crashed several cars. This is not a driver error. The car mileage reading is ~40,000. I found that there are recalls on the Toyota cars (with problems such as cars went of control and accelerated uncontrollably), but not this particular car/VIN #. (The car’s airbag was on the recall list, but when we stopped at the local Toyota dealership’s service department, the assistant manager always tell us that they are busy, come back next time. It happened more than 3 times.) After contacting Toyota Corporate, Toyota sent out a third party to do an inspection on the car. The 3rd party inspector downloaded a whole bunch data. Based on the 3rd party’s findings, Toyota Corporate states that no evidence that the incident was the result of the defect in [this] vehicle. My mother maintains the car accordingly to the instructions of the local Toyota dealership’s service department. One strange thing was that the 3rd party inspector told Toyota Corporation that there were a lot fire ants presented in the car at the time of his inspection. This seems impossible. My family members and the tow truck driver did not find any fire ants before and after the 3rd party’s inspection. The vehicle’s parked at the local Toyota dealership’s parking lot at the time of inspection. There must be a problem with car when it went out of control. Please advise, thanks!

The car went to replace the air bag with a hood and everything seemed fine but the SRS light activated every so often. this is happening since they changed the air bag and it turns it on and off whenever it wants for seconds the car has barely 22,000 miles and has not been crashed this has been happening since they changed the car's air bag air bag in the authorized toyota dealer of santana puerto rico
Dealer repaired airbag recall, now SRS light is on and won’t turn off. Dealer now wants hundreds of dollars to check and or fix possible issue told that it may be something else and more work needed. This issue should be part of recall repairs. This car only has 63,200 miles on it well maintained should not have these issues.
We had recall work done for Airbag, sensor light came on after repairs. One dealer quoted $150 to look at the sensor another wanted $450+. The light was not on before recall work was done. It is clear that whatever they did for the recall caused this to happen, don't care if it's not under warranty the dealer caused the issue electrical whatever and should fix it. Obviously this is a known issue with the 2006 Toyota and should also be part of the recall.

I HAVE AN AFTERMARKET WEBASTO HOLLANDIA 700 SUNROOF THAT GOT RECALLED FOR "IN THE EVENT OF A SEPERATION, THE FALLING GLASS COULD CREATE A ROAD HAZARD OR STRIKE A TRAILING VEHICLE AND CAUSE A CRASH." THE GLASS FOR THE MOONROOF FELL APART FROM THE VEHICLE AND IS GONE UNFORTUNATELY. I WAS TRYING TO BUY THE GLASS FROM TOYOTA WITH THE HELP FROM MY AUTO BODY SHOP "KURODA AUTO BODY INC." WHEN THEY WERE BUYING THE OEM GLASS FORM TOYOTA IN CALIFORNIA. THUS THEY CANT FIX THE CAR BECAUSE THEY CANT GET THE GLASS AND OEM DOESNT FIT EXCEPT WEBASTO. IT SAYS THAT WEBASTO WILL BE PROVIDING THE RECALLS BUT THE ONLY PROBLEM IS THAT I LIVE IN HAWAII AND THERE ARE NO WEBASTO DEALERS IN HAWAII, CLOSEST IS IN CALIFORNIA WITH LIKE 8 LOCATIONS, THATS THE CLOSEST FOR US, PAY FOR LIVING IN PARADISE I SUPPOSE. IM OKAY WITH THE PARTS BEING SHIPPED TO ME OR TO MY AUTOBODY SHOP THAT CAN DO IT AND OF COURSE WEBASTO PAYING FOR THE LABOR FEES FOR THE RECALL. IM OPEN TO PAYING FOR SHIPPING COSTS IF THEY AGREE BECAUSE I DONT KNOW IF A RECALL PAYS FOR SHIPPING. I NEED A NEW GLASS, THATS THE ONLY THING MISSING, MECHANISM WORKS AND MOVES BUT ITS CORRODED AND I WOULDNT MIND FOR A WHOLE NEW SET IF POSSIBLE. IN THE PICTURES IT IS BETWEEN THE VIN NUMBER OF THE SUNROOF MODEL UNDER THE RECALL. I THINK IT WAS FIX BEFORE FROM ONE OF THE PICTURES SAYING IT WAS CLAIMED ALREADY FROM MY TOYOTA VIN NUMBER, BUT APPARENTLY IF IT WAS FIX THIS WOULDNT HAVE HAPPENED AGAIN SO ANOTHER RECALL FIX I THINK THIS MAY BE.

DURING MY SCHEDULED VISIT TO ROGERS AND ROGERS NISSAN IN THE CITY OF IMPERIAL CA. I FELT THAT I WAS TREATED UNFAIRLY BECAUSE I WAS THE LAST PERSON TO BE ATTENDED OUT OF SEVERAL VEHICLES THAT CAME AFTER ME. I WAS INFORMED THAT REPAIRING THE DEFECTIVE AIRBAG WOULD TAKE OVER THREE HOURS. I WAS NOT OFFERED A RIDE HOME NOR ANY EFFORTS WERE MADE BY ROGERS AND ROGERS PERSONNEL TO ADDRESS MY CONCERNS WHEN I INFORMED THEM THAT I NEEDED MY VEHICLE AND COULD NOT WAIT THREE HOURS. I CONTINUE TO RECEIVE LETTERS AND EMAILS INFORMING ME OF THE URGENCY OF REPAIRING THE AIRBAGS, BUT I FEEL THAT ROGERS AND ROGERS IS NOT MAKING IT A PRIORITY. PLEASE ASSIST ME.
DRIVING ON THE FREEWAY, MY CAR ENGINE SHUT-OFF AND BREAKS LOCKED. CAR WILL NOT START ON THE MOVE SO I HAVE TO BRING IT TO A STOP. THE PROCESS OF TRANSITIONING LANES WITHOUT BREAKS WAS VERY SCARY. AFTER THE CAR WAS BROUGHT TO A FULL STOP THE CAR WAS ABLE TO START.

TL* TAKATA RECALL. THE CONTACT OWNS A 2006 TOYOTA COROLLA. THE CONTACT RECEIVED NOTIFICATION OF NHTSA CAMPAIGN NUMBER: 19V741000 (AIR BAGS), HOWEVER, THE PART TO DO THE RECALL REPAIR WAS NOT YET AVAILABLE. THE CONTACT STATED THAT THE MANUFACTURER HAD EXCEEDED A REASONABLE AMOUNT OF TIME FOR THE RECALL REPAIR. FRED HAAS TOYOTA COUNTRY, 22435 SH 249, HOUSTON, TX 77070, CONFIRMED PARTS WERE NOT AVAILABLE FOR THE RECALL REPAIR. THE MANUFACTURER WAS NOT NOTIFIED OF THE ISSUE. THE CONTACT HAD NOT EXPERIENCED A FAILURE. PARTS DISTRIBUTION DISCONNECT.
RECEIVED IMPORTANT SAFETY RECALL IN MAIL (NHTSA RECALL NO. 19V-741). CALLED LOCAL TOYOTA DEALER TO SCHEDULE SERVICE. DEALERSHIP WAS UNABLE TO SCHEDULE SERVICE DATE DUE TO NOT HAVING PARTS ON HAND. I ASKED IF OTHER DEALERSHIPS ON ISLAND HAD PARTS AND WAS TOLD NO. DEALERSHIP SAID THEY WILL CALL ME TO SCHEDULE APPOINTMENT WHEN PARTS ARE AVAILABLE; BUT, THEY DO NOT HAVE AN ESTIMATED TIME ON WHEN THAT WOULD BE. DEALERSHIP TOLD ME IT COULD BE DAYS, WEEKS, OR MONTHS. THIS IS NOT ACCEPTABLE FOR A RECALL THAT COULD RESULT IN SERIOUS INJURY OR DEATH. TOYOTA DEALERS SHOULD PROVIDE A LOANER CAR IN THE MEANTIME OR OTHER COMPENSATION.
WAS NOTIFIED RE RECALL. IMMEDIATELY MADE APPOINTMENT WITH BERLIN CITY TOYOTA, 191 RIVERSIDE ST;, PORTLAND ME 04103. APPOINTMENT CONFIRMED. ARRIVED EARLY THIS MORNING FOR REPLACEMENT. TOLD THE WORK COULD NOT BE DONE. NO PARTS! HOW COULD THEY CO0NFIRM AN APPOINTMENT WEEKS AGO AND NOT ORDER THE PARTS? NOW I SHALL IGNORE THE RECALL NOTICE. SHOULDN'T GOVERNMENT GO AFTER DEALERSHIP FAILURES TO DO THE REQUIRED RECALL?????
THE AIR BAG LIGHT CAME ON IN MY CAR. I BROUGHT IT TO THE DEALERSHIP WHERE THEY IDENTIFIED THE PROBLEM AS THE PLS P&A CENTER AIRBAG SENSOR AND EXPLAINED THAT THE MALFUNCTION WAS THE SAME AS IN OTHER CARS THAT HAD BEEN RECALLED. BECAUSE IT WAS THE SAME MALFUNCTION, I BELIEVE THAT MINE AND OTHER SIMILAR VEHICLES SHOULD BE COVERED BY THE RECALL TO PREVENT HARM TO OTHER DRIVERS.
TL* THE CONTACT OWNED A 2006 TOYOTA COROLLA. WHILE DRIVING APPROXIMATELY 35 MPH, A SECOND VEHICLE CRASHED INTO THE CONTACT'S VEHICLE. DURING THE CRASH, THE AIR BAGS DID NOT INFLATE AND IMPROPERLY DEPLOYED. THE VEHICLE WAS DESTROYED AND TOWED AWAY. THE DRIVER SUSTAINED NECK, BACK, AND LEFT SHOULDER INJURIES THAT REQUIRED MEDICAL TREATMENT. POLICE REPORT NUMBER: 2019000010196 WAS FILED. THE CAUSE OF THE FAILURE WAS NOT DETERMINED. THE MANUFACTURER AND LOCAL DEALER WERE NOT NOTIFIED OF THE FAILURE. THE FAILURE MILEAGE WAS 170,000.

THIS ISSUE HAS BEEN ON GOING FOR OVER 2 YEARS; THE ENGINE REVES UP WHEN STARTING THE CAR, RPM GOES INTO THE RED ON THE GAUGE & CONTINUES TO GO UP & DOWN FOR THE FIRST 30 MINUTES OR SO, WHILE DRIVING .THE CODES SAY IT IS AN EMISSIONS ISSUE/ CHARCOAL CANISTER, ACCORDING TO WHAT I'VE RESEARCHED. I TOOK THE CAR IN FOR THE ECU RECALL AS INSTRUCTED BY THE LETTER RECEIVED. I WAS NOT EXPERIENCING ANY OF THESE ISSUES PRIOR TO THE RECALL REPLACEMENT?? I TOOK THE CAR TO THE DEALERSHIP FOR A SCHEDULED OIL CHANGE AND EXPLAINED TO THE SERVICES PERSON, I WAS TOLD THE EMISSIONS WAS FINE, BUT THEY WOULD BE HAPPY TO RUN A DIAGNOSTIC ON IT FOR $200, I HAVE NOTICED THAT MANY OTHER ARE HAVING ISSUES WITH THE SAME CODES/ EMISSIONS ISSUES. COULD YOU PLEASE HELP ME WITH THIS, I DON'T KNOW WHERE OR WHO TO TURN TO FROM HERE. THANK YOU AND I LOOK FORWARD TO HEARING FROM YOU SOON. I WOULD LIKE TO SEND A VIDEO OF WHAT IS HAPPENING, BUT THERE ISN'T AN OPTION FOR VIDEOS. IF YOU NEED TO SEE THE ISSUE LIVE ON VIDEO, I WOULD BE HAPPY TO SEND IT TO YOU ANOTHER WAY. YOU CAN EACH ME AT [XXX] OR [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6). *TT
THE AIRBAG LIGHT IS ON ON THE DASHBOARD.
VEHICLE'S CHECK AIR BAG LIGHT CAME ON. TOYOTA DEALER SAYS THE SPIRAL CABLE HAS FAILED AND MUST BE REPLACED. TOYOTA HAS ISSUED A VOLUNTARY RECALL FOR OTHER MODELS FOR SAME ISSUE, BUT NOT FOR 2006 COROLLA. NO ACCIDENT OR DAMAGE TO VEHICLE. VEHICLE IS IN FL AND DEALER VERBALLY STATED THIS IS A COMMON PROBLEM IN HOT CLIMATE. VEHICLE AIR BAG IS INOPERABLE.
HAD PASSENGER AIRBAG REPLACED VIA THE FIRST RECALL AT THE LOCAL TOYOTA DEALER (I-10 TOYOTA IN INDIO). WHEN THE CAR WAS RETURNED TO ME, THE AIRBAG LIGHT WAS ON. I BROUGHT IT TO THE ATTENTION OF THE REPAIR PERSON AND I WAS TOLD THE LIGHT WAS FOR A PROBLEM WITH THE OTHER AIRBAG AND I WOULD HAVE TO SPEND $150 TO GET IT FIXED. SCAM! I DID NOT GET IT FIXED SO NOW I'M DRIVING AROUND WITH THE AIRBAG LIGHT ON, NOT KNOWING IF OR WHEN IT WILL EXPLODE. THIS SHOULD BE TAKATA'S EXPENSE, NOT MINE.

THE MANUFACTURER WAS NOT CONTACTED. THE CONTACT HAD NOT EXPERIENCED A FAILURE. PARTS DISTRIBUTION DISCONNECT.
I WAS SLOWLY PULLING UP THE DRIVEWAY TO PARK BEHIND ANOTHER VEHICLE WHEN THE COROLLA SUDDENLY SURGED FORWARD. IT ACCELERATED QUICKLY AND I COULDN'T STOP IT. IT CRASHED INTO THE BACK OF THE OTHER VEHICLEWAAQ. BOTH VEHICLES SUSTAINED DAMAGE BUT THE COROLLA'S IS SIGNIFICANT.
Crash
ENGINE CUTS OUT UNDER ACCELERATION. ON HOTTER THAT 80 DEGREE DAYS IT WILL NOT RUN UP OVER 3000 RPMS. CUTS OUT AND WILL NOT ACCELERATE TO SPEED UNLESS VERY SLOWLY ACCELERATING. HAVE HAD TO DEALER AND THEY TELL ME IT'S THE ECM, BUT YEARS AGO THAT WAS REPLACED UNDER A RECALL FOR THE SAME REASON. 'SUPPOSEDLY'. I'M THINKING EITHER THE WORK WAS NOT PERFORMED OR THE REPLACEMENT UNITS ARE FAULTY AS WELL.
STEERING WHEEL AIR BAG SPIRAL CABLE BROKE. THE DASHBOARD LIGHT WENT ON DIAGNOSTICS INDICATED DETACHED CABLE.
TAKATA RECALL. THE LIGHT IS COMING ON SAYING THAT THE AIRBAG IS MALFUNCTIONING. IT HAS BEEN ON FOR OVER A WEEK
TL* THE CONTACT OWNS A 2006 TOYOTA COROLLA. THE CONTACT RECEIVED NOTIFICATION OF NHTSA CAMPAIGN NUMBER: 15V286000 (AIR BAGS). THE CONTACT STATED THAT THE MANUFACTURER EXCEEDED A REASONABLE AMOUNT OF TIME FOR THE RECALL REPAIR. NORTH PARK TOYOTA OF SAN ANTONIO (10703 SW LOOP 410, SAN ANTONIO, TX 78211, (210) 600-5677) WAS CONTACTED AND CONFIRMED THAT THE PARTS WERE NOT AVAILABLE FOR THE RECALL REMEDY. THE MANUFACTURER WAS MADE AWARE OF THE ISSUE. THE CONTACT HAD NOT EXPERIENCED A FAILURE. PARTS DISTRIBUTION DISCONNECT.
TAKATA RECALL. CAR WAS BROUGHT IN FOR THE RECALL IN AUGUST 2018. TOLD BY THE DEALERSHIP THAT THE ENTIRE MODULE NEEDS TO BE REPLACED (BECAUSE THE STICKER ON THE AIRBAG WAS MISSING FROM A CAR THAT IS OVER 12 YEARS OLD) AND THE PARTS NECESSARY WERE ON BACK ORDER & IT WOULD TAKE 6 MONTHS TO COME IN. IN DECEMBER, I WAS TOLD THERE WERE AROUND 205 PEOPLE IN FRONT OF ME ON THE LIST FOR THE SAME PART. I'VE CONTACTED TOYOTA'S CORPORATE OFFICE MULTIPLE TIMES WITH NO ETA, AND VERY LITTLE CUSTOMER SERVICE IN TRYING TO GET THIS RESOLVED. AS OF FEBRUARY 2019, I STILL DO NOT HAVE MY CAR FIXED AND IT'S SITTING AT THE DEALERSHIP WHILE I"M FORCED TO DRIVE A LOANER THAT I HAVE TO RENEW EVERY 30 DAYS. I CALLED TOYOTA'S REGIONAL OFFICE A FEW WEEKS AGO AND WAS TOLD THAT THIS MAY TAKE YEARS BEFORE THE PARTS ARE IN AND READY BECAUSE THE PARTS HAVE TO GO THROUGH TOYOTA TESTING AND THE GOVERNMENT. I WAS ORIGINALLY TOLD THIS WOULD BE A 3 HOUR FIX, NOW IT'S BEEN OVER 6 MONTHS AND THERE'S STILL NO END IN SIGHT WHEN THE PARTS NECESSARY WILL BE RECEIVED WHILE MY CAR ROTS FROM DISUSE ON THE TOYOTA DEALERSHIP GROUNDS. THE ONLY REASON I BROUGHT MY CAR IN WAS BECAUSE TOYOTA WAS HASSLING ME ABOUT THE RECALL AND NOW THEY MAY NEVER HAVE THE PARTS IN TO FIX IT (AT LEAST WHERE I CAN STILL DRIVE MY CAR). THIS IS ABSOLUTELY UNACCEPTABLE THAT PARTS ARE NOT BEING PRODUCED TO FIX THESE ISSUES THAT TOYOTA KNOWS ABOUT IN A TIMELY MANNER.
