Vehicle primarily failed while parked at owner residence [XXX] and remained inoperable for approximately three weeks prior to tow to Cadillac dealership. The ignition key became trapped and could not be removed. A manual override release beneath the driver-side steering column was required to free
... the key — confirming the lock was held electronically, not mechanically jammed. Fault confirmed by authorized Cadillac dealer: intermittent electrical fault within the shifter assembly lock mechanism. A similar intermittent concern was previously documented on this vehicle in 2011 (logged as "Customer Concern Not Duplicated"), establishing a recurring pattern. GM Technical Service Bulletin 07-07-30-032D (2006–2011 Cadillac CTS, automatic transmission) explicitly documents this exact condition: key sticking, Shift to Park DIC message, shifter binding, and battery drain. OEM replacement part is now discontinued. Manufacturer declined goodwill or recall coverage. Dealer refuses repair unless owner assumes full financial responsibility for a used part with no warranty and no guarantee against recurrence. Safety concern: ignition/shift interlock failure causing vehicle immobilization, key entrapment, and loss of transportation for an elderly owner dependent on the vehicle for medical care. Defect evades shop diagnosis due to intermittency. No safe OEM remedy exists. Prior recall: NHTSA 14V394000 / GM Campaign 14172 addressed same interlock system. Current failure represents recurrence beyond that recall's scope. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

The issue appears to involve the electric oil pump and related components. My 2021 Kia Sorento had a recall for the electric oil pump that was marked “completed” in January 2023; however, the dealership only performed a software update and did not replace any mechanical components because no fau
... lt code was present at that time. Beginning around October 2025, the vehicle started experiencing drivability issues including hesitation and bogging down when accelerating from a stop. A dealership representative later stated the electric oil pump and housing likely need replacement and that the transmission may also be affected. The vehicle is available for inspection upon request. This issue creates a safety risk because the vehicle frequently hesitates or bogs down when attempting to accelerate, especially when pulling into highway traffic. When pressing the accelerator, the vehicle sometimes struggles to engage and move forward, which increases the risk of a collision. I brought the vehicle to Kia of RBC and spoke with Service Advisor Justin Hawkins. At the time of inspection, the check engine light had turned off and I was told they could not diagnose or repair the issue unless the warning light was active. I was instructed to continue driving the vehicle until the light returns. During a later conversation, dealership staff indicated the electric oil pump and housing likely need replacement and that the transmission may also require replacement, but they could not confirm this without the warning light being present.Read more

I am submitting a safety complaint regarding a vehicle restraint system failure. I was involved in a collision with a deer. During the incident, the seat belt pretensioner activated; however, the seat belt did not properly restrain me. I experienced forward movement despite the pretensioner activat
... ion. The airbags did not deploy. Because pretensioner activation is intended to lock and tighten the seat belt during a collision, I am concerned that the restraint system failed to function as designed. This raises concerns about occupant safety and the ability of the seat belt system to provide protection in future crashes. I am submitting this complaint to document a potential safety defect and request review of this restraint system failure.“After reporting the seat belt restraint failure to General Motors, I was informed that because I was not injured, no action would be taken and no further review would occur.” Thank you for your attention to this matter.Read more

Intermittent failures of both front turn signals and parking lamps due to wiring failure. The turn signals and parking lights on both sides of the front end of the vehicle sometimes failed to illuminate correctly. Upon investigation, failed wiring was found on the jumpers between the main engine
... bay harness and the lamp sockets. The wire insulation had become brittle and was flaking away, causing electrical shorts. The same failures were present on both sides. Only that short jumper segment experienced the issue, all other wiring in the engine bay was fine. The faulty wiring jumpers are available for inspection upon request. Safety was put at risk because the turn signals would not reliably signal driver intent. I am an independent mechanic, and discovered the issue while performing other work in May 2025. The vehicle is a 2024 Toyota Tacoma SR5 that was later traded for a 2025 Tacoma SR5 due to a serious safety defect involving the rear brake lines. The problem is the same one described in Toyota’s recall: the rear brake hoses can be damaged by debris, resulting in brake fluid loss and potential br
... ake failure. The safety risk was significant. The vehicle was used to transport a young child, and because the brake lines could fail without warning, the truck could not be safely operated for daily driving. Multiple franchised Toyota dealerships confirmed that they had no remedy parts available for several months and that the repair could not be performed. This left the vehicle without a functional safety repair for an extended period. The manufacturer’s public statements indicated that a fix existed, but dealerships consistently stated the opposite. No alternate transportation, loaner vehicle, or temporary replacement was provided during this period. The defect was not reproducible by driving because of the risk involved, but the safety issue was acknowledged by dealerships who stated that the recall was active and unresolved. The vehicle was effectively unusable for normal safe operation. No warning lights appeared on the dashboard; the hazard is related to the physical brake line design and exposure to debris rather than an electronic sensor. The vehicle was inspected by multiple Toyota dealerships, all of whom confirmed that no repair could be performed at the time. The component was not replaced, repaired, or made safe before the vehicle was traded out of due to the unresolved safety defect. The situation caused substantial safety concerns and financial hardship and appears to reflect a wider problem in remedy availability for this recall.Read more

The contact owns a 2021 Grand Design Momentum RV. The contact stated that while inspecting the RV, the contact became aware that the outrigger brackets were bent and fractured. The contact stated that the failure was visible over the axles. In addition, the welding on the cross-member beams was frac
... tured, causing the floor to crack, and the exterior fiberglass was cracked by the bedroom slide-out. Additionally, there was a four-inch-long crack on the frame of the upper deck. Most recently, the contact found movement of the cabinets in the upper deck, and a portion of the fiberglass on the front cap had separated from the roof. The RV was taken to a welding shop to be diagnosed, and it was determined that the metal used to support the walls in the unit was not strong enough. The contact was informed that the weight on top caused it to bend and crack. The outrigger brackets were replaced, the cross members were replaced, and the other cracks were repaired. The manufacturer was notified of the failure and denied the warranty claim. However, the manufacturer failed to acknowledge the failure and advised the contact to call the insurance company since the manufacturer believed that the RV had been involved in an accident. The contact denied that the RV had been in an accident. The contact noticed the issues since the RV was purchased. The failure mileage was unknown.Read more
