On January 26, 2026, at approximately 6:50 AM, I experienced a serious safety-related malfunction in my 2023 Toyota Mirai while entering traffic on Inglewood Avenue in Lawndale, California. I was pulling out of a parking lot into active traffic at low speed. At the moment I was entering the roadway
... , the vehicle suddenly lost propulsion/power without warning. Simultaneously, the vehicle’s infotainment/center display went completely black, consistent with a system reboot or electrical interruption. After a brief pause, the display rebooted and turned back on, and immediately afterward the vehicle unexpectedly surged forward. At the time of the power loss, my foot was lightly on the accelerator pedal as I was merging. When the vehicle’s systems reinitialized, the car applied power consistent with the pedal position, resulting in a sudden forward movement. The accelerator was not fully depressed, and this was not a full-throttle event, but the surge occurred without warning while the vehicle was in active traffic. The sequence of events — loss of propulsion, apparent electronic system reset, followed by unintended propulsion upon re-engagement — created a hazardous situation while entering traffic and raises serious concerns about the vehicle’s electronic control and power management systems. The vehicle’s behavior was unpredictable at a critical moment when controlled acceleration was required, which could have resulted in a collision or injury. There were no warning lights or messages prior to the event. I am reporting this incident because it appears to involve a safety defect related to loss of propulsion and unintended surge while in motion. I am scheduling a service inspection and will request a full diagnostic scan, including history and freeze-frame data from all control modules. Vehicle information: Year/Make/Model: 2023 Toyota Mirai Mileage at incident: approximately 47,033 miles Location: Lawndale, California Speed: Low speed while entering trafficRead more

It has its good mornings but once I'm on the road for more than 10 minutes its back to the struggle. I've had my K4 serviced twice at Dtla service, they never once test drove the car because I was tracking the GPS almost all day for the three days they had it. Once I got my car back it tooks about 45 minutes the service advisor Jeffery Montoya was so lost on where my car was, I had to ask and ask and ask finally the guys who are out front welcoming you helped me and brought my car. That was my decision to never go back to DTLA service. Next took it to Van Nuys Kia waited to weeks from my appointment because they are moving. Finally they kept it for three days they still werent able to diagnose the issue and repair it. Modesto the advisor there said " we can't duplicate the issue''. Some how they made me pay $82 because they supposedly found plastic bags stuck to my exhuast. [XXX] I even told him I don't want it cleaned and he said oh well you authorized the diag so you have to. He reported that info to kia Corp which got my case closed and said the car works fine. NO it does not work fine. Never going. Now today [XXX] I was parking arriving somewhere the k4 said engine off shift to P while ringing the alarm from the dash. I have it on film. Second driving down [XXX] and boom car engine shuts off while car are behind me. I had to shift to p then turn the car off. ... ol warning Occasionally, the high beam headlights turn on by themselves These warnings appear randomly, about a few times a month, while driving under normal conditions. The issue usually resolves temporarily after restarting the vehicle, but this raises serious concerns about the reliability of key safety systems such as traction control and collision avoidance. On several occasions, the check engine light (CEL) accompanied these other warning lights during the "malfunction" and the CEL remained on even after a restart. A diagnostic scan revealed P0101 (Mass Air Flow Sensor Performance issue). However, when I had the vehicle inspected at a Nissan dealership, the technicians stated that the MAF sensor and related systems were operating correctly, and they could not identify a specific mechanical issue. Despite that, the CEL and other malfunctions continue to occur intermittently. They were also not able to recreate the situation I explained with the malfunction since it occurs at random times. This issue has happened multiple times and creates uncertainty about whether the vehicle is safe to operate. I have found numerous reports online from other Nissan Kicks owners experiencing the same combination of issues (P0101, random warning lights, and vehicle malfunctions that reset upon restart). This suggests it may be a systemic electrical or ECU problem, not isolated to my car. I urge the NHTSA to investigate whether this issue warrants a recall, software update, or mandatory fix. It undermines the safe and reliable operation of the vehicle.Read more

I am writing to report an incident that occurred on [XXX] [XXX] while my Cybertruck was operating in Full Self-Driving (FSD) mode. While driving on [XXX] Brooklyn NY with no other moving traffic present, my Cybertruck did not detect a vehicle that was parked improperly—sticking out too far into
... the lane. The FSD system failed to adjust its path accordingly and struck the side view mirror of my vehicle, causing significant damage to the mirror glass and housing. I understand that FSD is a driver-assist feature that requires supervision, and I was attentive during the drive. However, the clearance of side view mirrors from the driver’s perspective is extremely difficult to judge at street level, and I believe stationary obstacles of this nature should be reliably detected and avoided by the FSD system. This type of incident raises concerns about the performance of FSD in handling stationary objects, especially when the vehicle was otherwise properly supervised and operating in a straightforward environment.
