Defect Description
Vantage Mobility, LLC (VM) is recalling certain 2021-2026 Honda Odyssey, 2021-2025 Chrysler Pacifica, and Toyota Sienna vehicles equipped with QRT-Deluxe and QRT-Max wheelchair restraints. The retractors may not lock, preventing the wheelchair from being properly secured.
Consequence
An unsecured wheelchair can move during transit, increasing the risk of injury.
Remedy
Dealers will inspect and replace the retractors, free of charge. Owner notification letters was mailed January 27, 2026. Owners may contact Vantage Mobility customer service at 1-800-488-9082.

Defect Description
Honda (American Honda Motor Co.) is recalling certain 2023-2024 Odyssey and 2024 Acura RDX vehicles. The steering gear box assembly may not have been tightened properly, allowing water to enter the assembly and cause corrosion and binding gears.
Consequence
Corroded and binding steering gears can result in a loss of steering control and increase the risk of a crash.
Remedy
Dealers will replace the electric power steering rack, free of charge. Owner notification letters were mailed April 4, 2024. Owners may contact Honda customer service at 1-888-234-2138. Honda's numbers for this recall are LHU and FHT.

When in traffic at a stop, my car goes into idle mode, and when I take my foot off the brake to switch to the accelerator the car shuts down. It takes a few tries to get the car started back up again. This has been happening intermittently for over a year.

After turning off the A/C fan, rear entertainment system, and unplugging my phone, the vehicle eventually started. I took the vehicle to a Honda dealer (Honda of [XXX]) They inspected it but said they could not reproduce the problem, so no repair was made. And they refuse to use my warranty to fix it. I am guessing it is similar to the recall 23V-431, but this van VIN number does not have any recalls associated with it yet. Restarting the radio, resetting the radio, etc does not alleviate the issue. Also, the cabin camera is not working, says "problem with camera" and displays "loading" with the moving circular icon. The display remains dark for the cabin camera, and rear entertainment screen is also not accessible. Cannot recall any large bumps in the road, no collisions with anything. If the design of the cables has not changed, then the recall needs to cover all VINS manufactured with the same wire assemblies and procedures, including all future VINS, as this problem is clearly repeatable. The customer should not bear the burden to fix an issue that has been deemed an issue in prior year models, with no changes to the design or assembly procedure to the related components.Read more

... lowing, and did not respond to accelerator input. There were six occupants in the vehicle, including four young children. I was traveling on a busy six-lane highway with little to no shoulder. I had to maneuver across traffic without motor power to reach the side of the road, placing all six occupants at immediate risk. After turning the vehicle off for about five minutes, I restarted it. Only the check engine light remained illuminated, and the vehicle appeared to have normal power. We continued driving at 55–65 mph to reach a safer location. About 10 minutes later, the same warning appeared and the vehicle again lost all motor power. This time we were on a steep, busy exit ramp and had to immediately pull off as the vehicle rapidly lost speed, again placing all six occupants at risk. After waiting about five minutes, we restarted the vehicle, drove it to a safe location, and had it towed to a dealership. The dealership was closed, so the cause of the failure has not yet been determined. This incident created an extremely dangerous situation, and we narrowly avoided a collision on the interstate. A sudden loss of propulsion at highway speeds without warning is a serious safety hazard that warrants investigation and consideration for a recall.Read more

This also puts at risk others as it impedes the drivers field of view allowing the possibility of an accident of occurring. I like to drive with my sunroof open very often, so now that is a risk I will have all the time I open my sunroof where I will have my field of view blocked. I have called two ford dealership to try to get quotes to repair the issue. First I was shocked with the price for the repair, but I was also shocked when I was told that it is a very common issue with the panoramic sunroof where the shade drops down. I asked do people usually get it repaired or not and they told me that most people don’t fix it that they just decide to deal with it or not use the sunroof at all. One of the dealerships told me the cost of the repair was roughly above $3 thousand dollars and the other told me it would be around $2.8 thousand dollars. A lot of labor is required as they have to bring down the headliner and remove the sunroof glass panels to be able to get it repaired. They also were not able to guarantee that after the repair that it would not happen again as they told me it was a design flaw that it was just a matter of time before it happened again. I believe Ford should be hold accountable for this and provide a permanent fix to this issue that is very common with the panoramic sun roof that creates a safety hazard for vehicle occupants and others. Especially if after customers have to pay considerable amounts of money for repairs and the issue will happen again as it is a design flaw.Read more

The same failure occurred twice in one day this week. During one of those incidents, the vehicle lost propulsion and stopped in the middle of a busy street, creating a serious safety hazard because I was unable to move the vehicle out of traffic. Immediately before and during the failure, the vehicle displayed multiple warning messages, including: Error: Drive system. Please safely stop the vehicle. Electrical system not working correctly. Please service vehicle. Error: Electronic Stability Control (ESC). Front Assist is not available. Lane Assist is currently not available. Rear Traffic Alert is currently not available. Park Distance Control is not available. Error: Auto Hold. These warnings appeared at the same time the vehicle lost propulsion. The vehicle stalled without warning while driving and became undriveable. This is extremely concerning because it can occur suddenly in traffic, placing the driver, passengers, and surrounding motorists at risk of a crash. A software update was performed but did not resolve or improve the issues. The defects include: noticeable delay in throttle response when pressing the gas pedal (making acceleration unpredictable); the infotainment and driver information center screen going completely black while the vehicle is in motion; failure of the backup camera system; and sudden, unexpected jerking of the vehicle for no apparent reason. Additionally, the key fob and Volvo Cars app frequently become completely unresponsive. The app will show the current vehicle status (for example, “Locked” with battery/range info) but unlock and remote commands do not work. On XXX, the situation escalated severely. The car was locked and would not respond to the key fob buttons or the Volvo app. I had to use the manual detachable key blade from the fob to unlock the driver’s door. Once inside, placing the full key fob on the backup reader produced no response at all. The center screen remained completely black, there was no power-up of the systems, and the vehicle would not start. The car is now in a completely unresponsive state. These problems affect the keyless entry system, key fob detection module (likely related to the TCAM or similar electronics), infotainment and power management systems, electronic throttle control, and overall vehicle communications. The vehicle is readily available for inspection and testing. The issues have been consistently reproduced over time. The car is pretty much brand new with just over 10k miles. I was only an hour in and had just left Kinston when I got a warning to pull over and then another alert a few moments later as I was trying to get off the parkway that the rear motor was shutting down and I began to lose speed rapidly. I stopped the car which proceeded to make a ton of noise and called roadside assistance. I had never experienced this type of noise before and was worried about combustion. Tesla was excellent at responding, towing, and the car is currently being repaired under warranty but I wanted to report the incident as it is a 2026 model with only 10k miles on it and has not been in an accident. This should not be happening to a brand new car. The latest repair report shows the rear drive inverter inlet hose and the supermanifold coolant pumps required replacing.Read more

In order to go more than 20-25 mph, I have to floor the pedal after completing the turn. I have taken it back to the dealer twice now for power issues and this past time (June 2026), even though the techs could tell there was an issue, Jeep is saying that it's "normal" functioning after the recall update. ... a). Honda issued recall 26V227 on April 9, 2026, to address concerns of inadvertent side air bag deployment. In the recall filing, the company stated that “[a]s of April 2, 2026, Honda has had 130 warranty claims, 25 reports of an injury, and no reports of death related to this issue from January 24, 2017 – April 2, 2026.” Honda states that the Supplemental Restraint System (SRS) Electronic Control Unit (ECU) contains incorrect deployment parameters for the side thorax and side curtain air bags, which may cause inadvertent deployment when the vehicle encounters strong road impacts, such as driving over potholes, speed bumps, or road debris. Honda stated that the SRS control logic for the second and third rows of seating contained an insufficient deployment threshold margin, allowing signal inputs to be misinterpreted as side impacts and causing inadvertent deployment of the side thorax and side curtain air bags. Recall 26V227 states that dealers will reprogram or replace the SRS ECU with improved air bag deployment parameters. Consumers received notice of this recall in late May 2026. In view of the recall being conducted by Honda, ODI is closing this Preliminary Evaluation (PE). The Agency reserves the right to take additional action if warranted by future circumstances. ... 2006 through September 8, 2008 (NHTSA Recall 13V-500, Honda Recall JC5).The report describes a defect condition caused by a combination of VSA system components and software that is unique to the MY 2007-2008 Honda Odyssey vehicles.The condition may result in unexpected brake application by the VSA system with no brake lamp illumination.The braking forces resulting from the condition can approach the maximum braking capability of the vehicle.Honda's investigation determined that (1) if the yaw rate sensor off-set exceeds a certain value, (2) the sensor calibration is not completed before the vehicle starts moving after start-up, and (3) the vehicle is driven in a specific manner; the VSA system may build hydraulic pressure in the brake system resulting in heavy, unexpected braking without the driver applying the brake pedal.Honda estimates that the parts necessary to correct this condition will not be available until March 2014.Pending part availability, Honda will send interim letters to affected owners including instructions to avoid the defect condition using a procedure to ensure proper VSA system calibration after each vehicle start-up, as well as instructions about what to do if unexpected brake application occurs while driving.Once the remedy parts are available, Honda will send a second notice advising owners to bring their vehicle to their dealer for installation of a redesigned yaw rate sensor.This investigation is closed.Read more

TOTAL COMPLAINT COUNT OF 656.SOME COMPLAINTS ALLEGE THAT THE CONDITION RESULTED IN EXTENDED STOPPING DISTANCES, INCLUDING SOME REPORTS OF ROLLING PAST STOP SIGNS OR TRAFFIC LIGHTS.THERE ARE 14 ALLEGED CRASHES IN THE SUBJECT VEHICLE POPULATION RELATED TO LOW/SOFT PEDALS. IN RESPONSE TO ODI'S INFORMATION REQUEST FOR PRELIMINARY EVALUATION PE09-024, HONDA INDICATED THAT SMALL AMOUNTS OF AIR MAY ENTER THE BRAKE SYSTEM DURING INITIAL DIAGNOSTICS OF THE VEHICLE STABILITY ASSIST (VSA) MODULATOR WHILE THE PUMP IS BRIEFLY ACTUATED FOR SYSTEM DIAGNOSTICS AFTER THE ENGINE IS STARTED.THE AMOUNT OF AIR GRADUALLY INCREASES WITH EACH SUCCESSIVE ENGINE START CYCLE, RESULTING IN PROGRESSIVELY INCREASING BRAKE PEDAL STROKE.ACCORDING TO HONDA, THE CONDITION DOES NOT PRESENT A SIGNIFICANT RISK TO MOTOR VEHICLE SAFETY BECAUSE THE PEDAL STROKE GRADUALLY INCREASES, PROVIDING THE DRIVER WITH INFORMATION THAT THE BRAKE PEDAL HEIGHT IS CHANGING AND REQUIRES ATTENTION.HONDA HAS FURTHER INDICATED THAT THERE IS NO DETERIORATION OF BRAKING PERFORMANCE CAUSED BY THE SUBJECT CONDITION, EVEN THOUGH THE PEDAL HEIGHT AND FEEL ARE DIFFERENT, UNTIL PEDAL STROKE HAS REACHED THE END OF TRAVEL AND THE DRIVER CANNOT PRODUCE ANY ADDITIONAL BRAKE LINE PRESSURE.HONDA STATES THAT IT IS UNAWARE OF ANY VEHICLES THAT HAVE REACHED SUCH A CONDITION AND HONDA TESTING USING A LEAKING FIELD RETURN MODULATOR INDICATES THAT IT WOULD TAKE OVER A YEAR BEFORE THE PEDAL TRAVEL WOULD REACH ITS LIMIT DURING HARD STOPS WITH 200 N (45 LBF) OF PEDAL EFFORT - THE NOMINAL FORCE REQUIRED TO REACH ABS BRAKING MODE. ODI'S ANALYSIS OF WARRANTY DATA INDICATES THAT THE MY 2007-08 ODYSSEY VEHICLES HAVE EXPERIENCED THE ALLEGED DEFECT AT A RATE OF 0.67%.THE MY 2006 ODYSSEY VEHICLES HAVE EXPERIENCED THE ALLEGED DEFECT AT A SIGNIFICANTLY LOWER RATE (0.04%) THAN THE MY 2007-08 ODYSSEY.HONDA IMPLEMENTED A DESIGN CHANGE IN THE MY 2007 VEHICLES TO CHANGE THE VSA DIAGNOSTIC PROGRAM.
