... hen able to detach the molded electrical connector/plug from the back of the outlet, exposing the electrical terminals/conductors. The inverter/outlet was powered at the time, meaning my [XXX] had direct physical access to energized electrical components. He was about to touch the exposed terminals when his sibling saw what he was doing and stopped him. This prevented potential electrical contact and injury. I immediately stopped the vehicle, turned off the power, moved my child away from the area, and removed the applicable fuse to ensure the circuit was de-energized before securing/reassembling the connector and outlet. No injury occurred, but this was a serious near-miss. A [XXX] passenger was able to remove a factory AC electrical outlet and expose energized electrical terminals without tools or adult-level disassembly. The outlet's mounting/retention and connector design did not prevent a young child from accessing energized conductors within the passenger compartment. This creates a potential electrical shock, burn, electrocution, and electrical-short hazard, including the possibility of a child inserting a finger or conductive object into the exposed area. I request that NHTSA investigate the outlet assembly, retention method, connector, and protection from access to energized components in the 2016 Honda Odyssey, and determine whether other vehicles using the same components are affected. This design should not permit a young child to gain tool-free access to energized electrical terminals. I have repeatedly brought this issue to my local Honda dealership and specifically questioned the locking system. Unfortunately, the dealership has either been unable to duplicate the problem or has made changes to the locking mechanisms without resolving the issue. The vehicle now no longer under warranty, yet the problem continues. This week, however, the issue became a terrifying and extremely serious safety emergency. My one-month-old grandson was accidentally locked inside the vehicle with the key fob. We were approximately 30 minutes from home, and the outside temperature was around 90 degrees. We had no way to unlock the vehicle ourselves. We had to contact the local police, a tow company, and EMTs for assistance. My grandson was trapped inside the vehicle for approximately 10 very frightening minutes before they were able to get the doors unlocked. Thankfully, my grandson was safely removed from the vehicle, but I cannot stop thinking about how differently this situation could have ended. A child, particularly an infant, can be in extreme danger inside a hot vehicle within a very short period of time. I am extremely concerned that this locking problem has continued for years despite my repeatedly bringing it to the attention of the Honda dealership. A professional diagnostic scan confirmed active Diagnostic Trouble Code (DTC) P0741 alongside a flashing transmission "D" indicator on the dashboard.This mechanical component failure creates a severe, immediate safety hazard. Because the cracked piston allows the torque converter clutch to continuously slip under load, operating the vehicle at sustained highway speeds generates extreme internal friction and excessive hydraulic heat. This thermal buildup presents a high risk of sudden, catastrophic transmission failure, fluid vaporization, or powertrain lock-up while operating on public roadways.The manufacturer is actively exacerbating this safety hazard. Because there is no Acura dealership within a 90-mile radius, Acura Corporate Client Relations (Case #164446080) and the nearest authorized Acura dealership service manager have explicitly instructed me to drive the malfunctioning vehicle over 90 miles on the highway to reach their facility for service. The vehicle was driven in ambient temperatures exceeding 108 degrees Fahrenheit in Arizona with no cabin cooling and a compromised battery thermal management system, creating a safety risk to the driver. The dealer quoted repair including AC compressor replacement, battery chiller replacement, and inverter coolant flush. Kia Consumer Affairs denied warranty coverage under the federal emissions warranty despite the vehicle being within the 8 year/80,000 mile coverage window at approximately 58,309 miles. No written explanation for the denial was provided. This failure appears to be a known pattern on this platform based on owner reports. The battery chiller and inverter coolant system are integral to battery thermal management and should be covered under the federal emissions warranty. While traveling on the freeway, the vehicle will unpredictably and forcefully pull to either side without warning. This repeated and erratic movement creates a legitimate fear that the vehicle could sideswipe another car, resulting in a serious accident. This is not a minor steering issue — it is a dangerous driving condition that compromises the safety of myself, my passengers, and other motorists. I reported this unsafe driving behavior to the service department at Kendall Nissan. The service manager acknowledged that similar complaints have been reported on several other vehicles they have serviced and attributed the issue to the tires. My vehicle is equipped with Bridgestone Alenza Sport AS 255/50R20 105H tires. In an effort to confirm or eliminate the tires as the cause, I requested that they be replaced. That request was denied, and I was directed to contact Nissan Customer Care. I contacted Nissan Customer Care on two separate occasions seeking resolution. My initial complaint was dismissed. After clearly stating that we felt highly unsafe operating the vehicle, I was informed that a buyback request would be submitted. Later, I was contacted and told the buyback request had been denied. These bulletins show GM has long been aware of defective lifters in 5.3L engines. This is not just a drivability concern. A sudden engine failure and loss of propulsion while driving poses a clear safety risk, especially when merging, crossing intersections, or towing. I maintained the vehicle properly and have all service records. I request NHTSA investigate these widespread lifter failures in GM’s 5.3L V8 engines as a safety defect, since they can and do cause sudden loss of power in motion.Read more
